Are Diabetic Metabolic Compensation and CA19.9 Really Correlated?

Diabetes has been claimed to be a risk factor for pancreatic carcinoma, but it is probably a consequence of gland invasion from the neoplastic tissue. A link between diabetes and pancreatic carcinoma was suggested by means of biochemical markers of the diseases, namely glycated hemoglobin and CA19.9. Moreover, CA19.9 was proposed as a sensitive and useful marker of the severity of exocrine damage in diabetes, since the mucin decreased when metabolic compensation improved. We examined 64 diabetic patients (36 insulin dependent, 16 non insulin dependent, 12 treated with diet) by measuring CA19.9 using two different immunometric methods and glycemia and glycated hemoglobin. We observed that a correlation between CA19.9 and biochemical markers of metabolic compensation of diabetes was inexistent and no differences between insulin dependent and non insulin dependent patients were found. A high concentration of CA19.9 in a diabetic patient should be interpreted and evaluated in the same manner as for a non diabetic patient.

Biochemical changes occurring in two types of diabetic patients have been investigated to identify the correlations between these changes and type 1 and type 11 of diabetic patients, in comparison with that of normal healthy, sera of 68 diabetic patient with type 1 and type 11were used to estimate the L-fucose level and other related parameters. These parameters measured throughout this project are: TF, PBF, TP, TFYTP ratio and PBF\TP. As final approach, the author concluded that fucose level together with its related parameters could be a biochemical markers for early assessment of re­sponse to the therapy served.

Abstract Background People with diabetes mellitus, especially insulin-dependent diabetic patients, are a risk group for staphylococcal infections. Asymptomatic infection with Staphylococcus aureus is common and favors dissemination of the microorganism, rendering these individuals a source of infection. This study aimed to characterize the resistance profile, clonal profile and sequence type, as well as to analyze the prevalence and risk factors for nasal and oropharyngeal carriage of methicillin-susceptible (MSSA) and methicillin-resistant S. aureus (MRSA) isolated from insulin-dependent diabetic individuals in the city of Botucatu, SP, Brazil. Methods Staphylococcus aureus was collected from the nasopharynx and oropharynx of 312 community-dwelling insulin-dependent diabetic individuals over a period of 3 years (October 2015 to December 2018). The isolates were characterized by susceptibility profiling, detection of the mecA gene, SCCmec typing, and molecular typing by PFGE and MLST. The risk factors associated with S. aureus and MRSA carriage were determined by logistic regression analysis. Results The overall prevalence of colonization with S. aureus and MRSA was 30.4% and 4.8%, respectively. Fifteen of the 112 S. aureus isolates carried the mecA gene; SCCmec type IV was identified in 10 isolates, SCCmec type I in three, and SCCmec type II in two. Among the 15 resistant isolates (MRSA), four were susceptible to oxacillin/cefoxitin by the disc diffusion method and one MSSA isolate was resistant to sulfamethoxazole/trimethoprim. The analysis of risk factors revealed a protective effect of age and lung disease, while lower-extremity ulcers were a risk factor for S. aureus. For MRSA, only male gender was significantly associated as a risk factor in multivariate analysis. Clonal profile analysis demonstrated the formation of clusters among MRSA isolates from different patients, with the identification of ST5-IV, ST5-I, and ST8-IV. Isolates carrying ST398 were identified among MSSA and MRSA (ST398-IV). Conclusion Our findings reinforce the importance of epidemiological studies of S. aureus carriage, especially in populations at high risk of infections such as diabetics. The data suggest widespread dissemination of MRSA in the population of insulin-dependent diabetic patients studied, as well as the emergence of important lineages among these individuals.

Abstract Glucose molecules are joined to protein molecules to form stable ketoamines, or fructosamines, through glycation, a nonenzymatic mechanism involving a labile Schiff base intermediate and the Amadori rearrangement. The amount of fructosamine in serum is increased in diabetes mellitus owing to the abnormally high concentration of sugar in blood. The concentration of fructosamine in serum thus reflects the degree of glycemic control attained by the diabetic patient and is useful in monitoring the effectiveness of therapy in diabetes over a period of several weeks, in a manner analogous to the determination of glycated hemoglobin. Of the analytical approaches used to measure fructosamine, affinity chromatography with m-aminophenylboronic acid and the nitroblue tetrazolium reduction method appear to be the most practical means for clinical chemists to assay fructosamine quickly, economically, and accurately. Fructosamine values can readily distinguish normal individuals and diabetic patients in good glycemic control from diabetics in poor control. Unlike glycated hemoglobin, which reflects the average blood sugar concentration over the past six to eight weeks, fructosamine reflects the average blood sugar concentration over the past two to three weeks. Thus a clinical advantage is that fructosamine responds more quickly to changes in therapy, thereby allowing for improved glycemic control. Used in conjunction with determinations of blood sugar and (or) of glycated hemoglobin, or by itself, the fructosamine assay can provide clinically useful information for the detection and control of diabetes.
