Abstract Introduction Individuals living in low-income neighborhoods have disproportionately high rates of obesity, Type-2 diabetes, and cardiometabolic conditions. Perceived safety in one’s neighborhood may influence stress and physical activity, with cascading effects on cardiometabolic health. Methods In this study, we examined relationships among feelings of safety while walking during the day and mental health [perceived stress (PSS), depression score], moderate-to-vigorous physical activity (PA), Body Mass Index (BMI), and hemoglobin A1C (A1C) in low-income, high-vacancy neighborhoods in Detroit, Michigan. We recruited 69 adults who wore accelerometers for one week and completed a survey on demographics, mental health, and neighborhood perceptions. Anthropometrics were collected and A1C was measured using A1CNow test strips. We compiled spatial data on vacant buildings and lots across the city. We fitted conventional and multilevel regression models to predict each outcome, using perceived safety during daytime walking as the independent variable of interest and individual or both individual and neighborhood-level covariates (e.g., number of vacant lots). Last, we examined trends in neighborhood features according to perceived safety. Results In this predominantly African American sample (91%), 47% felt unsafe during daytime walking. Feelings of perceived safety significantly predicted PSS (β = − 2.34, p = 0.017), depression scores (β = − 4.22, p = 0.006), and BMI (β = − 2.87, p = 0.01), after full adjustment. For PA, we detected a significant association for sex only. For A1C we detected significant associations with blighted lots near the home. Those feeling unsafe lived in neighborhoods with higher park area and number of blighted lots. Conclusion Future research is needed to assess a critical pathway through which neighborhood features, including vacant or poor-quality green spaces, may affect obesity—via stress reduction and concomitant effects on cardiometabolic health.

(1) Background: Review-based studies are required to consider the different designs and data that describe the association between physical activity and mental health for underprivileged children and adolescents. There is a particular need to identify mechanisms which could explain the association, and factors which influence the association in this population group. (2) Methods: An integrative review with a systematic search was conducted in three stages: (i) a systematic literature search on four databases from inception until May 2020. Studies were eligible if they examined underprivileged children, involved moderate to vigorous physical activity (MVPA) and had a focus on at least one outcome measure related to mental health. (ii) Two critical appraisal tools were used to assess quality across different study designs. (iii) Synthesis was undertaken in four stages, examining social, internal and physical outcomes and mechanisms. (3) Results: A total of 16 studies were included (2 qualitative; 12 quantitative and 2 mixed methods). Fifteen (15/16; 93.8%) showed an association between MVPA and a significant improvement in at least one mental health outcome. The most studied outcomes included: internal aspects (n = 14), significant effects were identified in 12/14 studies; social aspects (n = 6), significant effects were identified across all studies and finally physical aspects (n = 5), significant effects were found in 3/5 studies. Autonomy support was identified as a mechanism to explain the change, whilst age and gender influenced the strength of the association. (4) Conclusions: The findings provide confirmation of the link between MVPA and the improved mental health and wellbeing of disadvantaged children and adolescents. Future research must consider the long-term effect through longitudinal studies, along with determining whether any specific types of PA are more impactful than others.

The study aim was to implement and evaluate the feasibility of a culturally informed (“BeFAB”) app for African American/Black women to address postpartum weight. Women (n = 136; mean age = 27.8 ± 5.4; mean BMI = 32.5 ± 4.3) were recruited from postpartum units, and randomly assigned to receive BeFAB (n = 65) or usual care (n = 71) for 12 weeks. App content included didactic lessons delivered via a virtual coach, app-based messages, goal setting and tracking, and edutainment videos. Feasibility outcomes included recruitment, retention and engagement, and self-reported acceptability. Behavioral (i.e., diet, physical activity), psychosocial (i.e., stress, coping, support, self-efficacy) and weight outcomes were also examined. Recruitment goals were met, but attrition was high, with 56% retention at 12 weeks. Approximately half of participants accessed the app and set a goal ≥one time, but <10% reported achieving a nutrition or activity goal. Among study completers, ≥60% found the app content at least somewhat helpful. Within-group changes for BeFAB among completers were found for increased moderate-to-vigorous physical activity and decreased fruit/vegetable intake and weight. Findings indicate initial feasibility of recruiting postpartum women to participate in a digital healthy body weight program but limited use, reflecting low acceptability and challenges in engagement and retention. Future research is needed on strategies to engage and retain participants in postpartum interventions.

Abstract This study investigated the role of objectively measured moderate–vigorous physical activity (MVPA) and sedentary behavior on cardiometabolic risk factors of young Latino children. We hypothesized that MVPA would be associated with lower cardiometabolic risk when sedentary behavior is low. We studied 86 primarily low-income, Latino children using a cross-sectional study design. The study sample consisted of 51 girls and 35 boys, with mean age 5.6 (SD = .53) years. Physical activity was measured by accelerometry, anthropometric measures obtained, and fasting blood samples were used to measure cardiometabolic risk factors. Greater levels of sedentary behavior were associated with increased waist circumference (rs = .24, p &lt; .05) and metabolic risks. MVPA, however, had significant beneficial associations with all cardiometabolic risk factors (rs-range = −.20 to −.45, p &lt; .05) with the exception of plasma insulin. MVPA predicted latent variables representing anthropometric risk (β = −.57, p &lt; .01), cardiac risk (β = −.74, p &lt; .01), and metabolic risk (β = −.88, p &lt; .01). Sedentary behavior significantly moderated the effect of MVPA on anthropometric (β-interaction = .49, p &lt; .01), cardiac (β-interaction = .45, p &lt; .01), and metabolic risk (β-interaction = .77, p &lt; .01), such that more MVPA was associated with better health outcomes under conditions of lower sedentary behavior. The model explained 13%, 22%, and 45% variance in anthropometric, cardiac, and metabolic risk factors, respectively. Increased MVPA is associated with decreased cardiometabolic risk in young Latino children, particularly when sedentary behavior is low.

Purpose: The authors examined the relationship between mother and child activity. Methods: The authors compared moderate–vigorous physical activity (MVPA) and sedentary time of low-income mothers with obesity and their 6- to 12-year-old children on week (WD) and weekend (WE) days. A total of 196 mother–child pairs wore accelerometers simultaneously for a week. Mothers completed questionnaires. Spearman correlation and multivariate regression were used. Results: WE MVPA (accelerometry) was significantly correlated between mothers with children aged 6–7 (rs = .35) and daughters (rs = .27). Self-reported maternal PA time spent with one of their children was significantly correlated with the WE MVPA of all children (rs = .21) and children aged 8–10 (rs = .22) and with the WD MVPA of all children (rs = .15), children aged 8–10 (rs = .23), aged 11–12 (rs = .52), and daughters (rs = .37), and inversely correlated to the WD sedentary time of all children (rs = −.21), children aged 8–10 (rs = −.30), aged 11–12 (rs = −.34), daughters (rs = −.26), and sons (rs = −.22). In multivariate regression, significant associations were identified between reported child–mother PA time together and child MVPA and sedentary time (accelerometry). Conclusions: Mothers may influence the PA levels of their children with the strongest associations found in children aged 6–7 and daughters. Mother–child coparticipation in PA may lead to increased child MVPA and decreased sedentary behavior.

Introduction: Although it is known that urban design and land use at the community level contributes to active living, there remains a paucity of such information among low income and diverse populations affected by such infrastructure. Reconstruction of an inner city community in Chattanooga, Tennessee afforded the opportunity to assess the impact of new urbanist construction on active living among African-American children/youth living in the inner city. Hypothesis: Transportation and recreational physical activity levels among children/youth residing in a new urbanist community was hypothesized to be higher compared to demographically similar children/youth who reside in a more traditional inner city community. Methods: Using the System for Observing Physical Activity and Recreation in Communities (SOPARC) we examined an urban trail and recreational park areas of two distinct communities to assess physical activity, sports, and recreational of children/youth. SOPARC data were collected in each of 4 settings in each community (East, a traditional and South, a new urbanist) during four 1-h observation periods during 7 days of clement weather. Observations were made during each day of the week in each setting (i.e., Sunday through Saturday). Results: The SOPARC assessments of the urban trail and pedestrian/bicycle routes in the South and East yielded a total of 672 and 436 children/youth observations, respectively. South children/youth were over three times more likely to engage in vigorous physical activity compared to their East counterparts (Mantel-Haenszel Chi Square = 19.67; P< 0.00001). Conclusions: The present findings support the hypothesis that enhanced environmental supports, such as those found within the South community, increase the likelihood of greater levels of activity among children/youth compared to children/youth that were not exposed to these environmental enhancements. This comparative difference was most remarkable when examining the SOPARC assessment differences along the transport path/sidewalk routes, with South children/youth being significantly more likely than the East children/youth to engage in more vigorous forms of physical activity along this transport/recreational corridor, providing evidence that access to these types of ‘urban’ trails appears to translate into increased opportunities for physical activity among children/youth.

Abstract Background: Approximately 5 million Americans are living with metastatic cancer. Metastatic cancer survivors (MCS) are at risk for poor health behaviors, which may negatively influence wellbeing.Methods: Using a modified Dillman’s method, 542 MCS were mailed a survey querying physical and mental health (PROMIS® measures), health behaviors, and supportive care interest. Returned surveys were double-key entered into REDCap®. Data were analyzed using SPSS.Results: Two hundred and seventy-seven surveys were returned (51% response). Respondents (51% female; 88% Caucasian; 12% African-American; Mage=65 years; Msurvivorship=38 months; 23% female cancers, 23% melanoma, 21% gastrointestinal, 15% genitourinary, 12% pulmonary, and 6% other) reported low daily fruit and vegetable (F&V) intake (M=4.1) and weekly minutes of moderate-to-vigorous physical activity (PA) (M=41.9), with 66% of respondents having overweight or obesity. While mean scores for physical (M=43.6) and mental (M=47.7) health were considered “good,” scores in the “fair” to “poor” ranges were observed (40% physical; 23% mental). MCS meeting PA (≥150 min per week) and dietary (≥5 daily servings of F&V) guidelines reported better physical (p=.003; p=.056) and mental (p=.033, p=.549) health respectively, compared to MCS who were not. While current supportive care use was low (12%), future interest was high (57%), with greatest interest for nutrition (46%), MCS support group (38%), and gardening (31%).Conclusions: Our findings suggest that engaging in regular PA and consuming more F&Vs may enhance physical and mental health among MCS. Future research may explore supportive care approaches with high interest, such as gardening, to aid MCS in improving key health behaviors.

Objectives: To explore the preliminary effects and acceptability of a co-produced physical activity referral intervention. Study Design: Longitudinal design with data collected at baseline and post a 12-week physical activity referral intervention. Setting: Community leisure centre. Methods: In all, 32 adults with controlled lifestyle-related health conditions took part in a physical activity referral intervention (co-produced by a multidisciplinary stakeholder group) comprising 12 weeks subsidised fitness centre access plus four behaviour change consultations. A complete case analysis ( t-tests and magnitude-based inferences) was conducted to assess baseline-to-12-week change in physical activity, cardiometabolic, and psychological measures. Semi-structured interviews were conducted ( n = 12) to explore experiences of the intervention. Results: Mean improvements were observed in cardiorespiratory fitness (3.6 mL kg-1 min-1 [95% confidence interval: 1.9–5.4], p < .001) and moderate-to-vigorous physical activity (12.6 minutes per day [95% confidence interval: 4.3–29.6], p = .013). Participants were positive about the support received from exercise referral practitioners, but experienced some challenges in a busy and under staffed gym environment. Conclusions: A co-produced physical activity referral intervention elicited short-term improvements in physical activity and cardiometabolic health. Further refinements may be required, via ongoing feedback between stakeholders, researchers and service users, to achieve the intended holistic physical activity focus of the intervention, prior to a definitive trial.

Background:The purpose of this study was to examine the effect of a 36-week Comprehensive School Physical Activity Program (CSPAP) on cardiometabolic health markers in children from low-income schools.Methods:Participants were 217 school-aged children (mean age = 10.1 ± 1.1 years; 114 girls, 103 boys) recruited from 5 low-income elementary schools. Cardiometabolic health markers were collected in a fasted state at 2 time-points, before commencement of the CSPAP for classroom and school level clustering and the modifying effects of grade level and sex, there were statistically significant improvements in HDL cholesterol (Δ = 3.6 mg/dL, 95% CI: 1.4 mg/dL to 5.8 mg/dL, P = .039), triglycerides (Δ = –14.1 mg/dL, 95% CI: –21.4 mg/dL to –6.8 mg/dL, P = .022), and mean arterial pressure (Δ = –4.3 mmHg, 95% CI: –8.5 mmHg to –0.1 mmHg, P = .041) following the 36-week CSPAP intervention. Sixth-grade children showed decreases in LDL cholesterol (Δ = –15.3 mg/dL, 95% CI: –30.5 mg/dL to –0.1 mg/dL, P = .033).Conclusions:Improvements in specific cardiometabolic health markers were found following a 36-week CSPAP in children from low-income schools.
