Parasuicidal poisoning treated in a Greek medical ward: epidemiology and clinical experience

Objective: To study the epidemiology of acute poisoning patients presenting to an acute medical service ward in a Greek hospital between January 1998 and December 2000. Design: Prospective case series. Results: A total of 273 patients with self-poisoning were included in the study. This represented 3.8% of the overall admissions to the unit. The mean age of patients was 33, the most frequent age group being that aged 20–30 years (36.2% of total) with a male-to-female ratio of 1:1.97. Sixty per cent of patients was admitted within 4 h. Those from urban areas comprised 76.2% and 23.8% from rural areas. The most frequently ingested agents were psychopharmaceuticals (37.4%) and analgesics/anti-rheumatics (32.6%). Pesticides (7.7% of total) were most frequently used by patients coming from rural areas (32.3% of patients from rural areas). Alcohol was included in the overdose in 8.4%. Of the patients, 16.2% had a previous history of overdose. In this case series, psychiatric assessment suggested that 52% of the patients had a formal psychotic diagnosis, 21% had personality disorder and 27% had taken an overdose in response to stress. The most frequently documented precipitating factors were family problems and disputes (37%). Unusually, the seasonal distribution in these patients suggested a peak in summer (37.5% of presentations) with lower numbers in spring (30.2%), autumn (17.7%) and winter (14.6%). Of the patients, 23.7% presented in July. A total of 73.5% of patients was conscious, 16.4% was somnolent, 4.5% was in precoma and 5.6% was in coma (GCS <8). Patients who received antidotal therapy comprised 17.9%. Evidence of hepatic dysfunction was observed in 8.9% of patients and renal dysfunction in 3.6%. Extracorporeal techniques for drug removal (hemodialysis and hemoperfusion) were used in 2.2% of patients. Intensive care therapy was required in 11.4% of patients. The mean overall hospitalization time was 3.3 days. The mortality rate was 2.9%. Conclusions: This study shows that the epidemiology of self-harm by overdose in Greece is significantly different in terms of the seasonal presentation from other parts of Europe. The agents ingested and other features are similar to northern Europe. Psychiatric diagnoses are more common in our group than in those reported from northern Europe.

The patterns of poisoning usually differ from country to country and from province to another within the same country.The purpose of this study was to clarify the characteristics of acute chemical and drug poisoning.cases admitted to the emergency department (ED)ofmaternity and children,teaching hospital in Al- Diwaniyah city and to compare poisoning characteristics between different age groupsin the region. A review of medical records of 125 poisoningcases under 7 years of age, except for cases of food poisoning,snake bite,scorpion stings and cases with incomplete data were not included in this study,presenting to the Emergency Department (ED) during 2016 were determined.The mean age of all poisoned patients (mean ± standard deviation) was 2.26 ± 0.750.One hundred and twenty five children,forming 2.2% (125/5494) of total emergency unit admissions of all patients,were under seven years of age. Slightly more boys 71 (56.8%) than girls 54 (43.2%) were intoxicated with the male to female ratio of 1.3:1. The majority of all cases were due to accidental poisoning. In patients younger than one year of age 23 (18.4%),of all poisonings were due to therapeutically error,1 – 3 year 47 (37.6%),4 – 7 year 55 (44%). Chemical agents (58.4%) mostly kerosene (30.1%),agriculturalpesticides/insecticides (19.1%),were the most frequent offending agent,followed by drugs comprised (41.6%),mostly due to Cold and cough medication (15.3%),The vast majority of these materials was stored in non-original containers,These drugs were available at home,being used either by the child or the parents and grandparents.The most common route of poisoning was ingestion of the poison (118 patients; 94.4%) and most were ingested inside the house (73.6%). No deaths were reported.There were obvious seasonal variations and most cases were admitted during in summer (42.4%) and spring (29.6%). Patients from rural areas (57.6%) were more than the patients from the urban areas (42.4%). Effective health promotional programs for parents regarding poisoning hazards are needed to increase the awareness and reduce the incidence of poisoning among children in our region in addition correct dealing with the chemical materials. A relationship exists between the parents’ level of education and the occurrence of pediatric poisoning particularly mothers. Since the level of education in this study was lower in the mothers and for mothers are those who take primary care of children at home; they should be educated regarding care and prevention of lethal injuries to their children at home.

Background and objectives: The term social phobia was first coined by Janet (1903) to describe the patient who feared being observed while speaking, playing, or writing, symptoms of shyness, social anxiety and social avoidance had been describe as early as the time of Hippocrates (1969). Aim: Against this background the following study aim of to determine the Prevalence of Social Phobia and Depression among Adolescent Girls. Materials and methods: This study is a Non - Experimental study. Sample size of was 120 adolescent girls. Sample was selected by using Non- Probability Convenience technique. Social phobia inventory (SPIN) and Beck depression inventory II (BDI II) on social phobia and depression among adolescent girls. Data analysis was done with the help by the mean of descriptive and inferential statistics. Results: The study finding reveals that majority of the subjects 77(64.2%) had very mild / no social phobia. Majority of the subjects in this study had moderate depression 54(45%). The chi – square value of social phobia and previous history of psychiatric illness was 6.386. The ‘P’ value is 0.041. which shows a statistically significant association. The correlation coefficient value between social phobia and depression was 0.264 which shows a mild positive correlation. Conclusion: The study concluded adolescent girls had depression more when compare with social phobia.

Objectives: To determine the frequency of hepatitis b & c viral infection insurgical patients. Study Design: Descriptive case series. Place and Duration of Study: Thisstudy was conducted at surgical department of multiple hospitals and compares the results,JPMC Karachi, Civil Hospital Naushahro Feroze and Jamshoro, Pakistan from August 2014 toDecember 2015. Methodology: All 2645 patients were admitted for emergency and electivesurgery. All patients taken detail history regarding demographic parameter and risk factorslike previous surgery, blood transfusion, barbar, Road Traffic accident, haemodialysis, Tattoos/body piercing, injecting drug user, family history of hepatitis, previous history of jaundiceand Hospitalization. Hospital laboratory was used for screened HBsAg and Anti HCV usingimmunochromatography (ICT method). Patients excluded who were those did not need thesurgery or known case of HBsAg and Anti HCV. Data collected was entered into and analyzedby using statistical package for the social science – 20. Results: Out of 2645 patients, maleto female ratio were 1.9:1. The mean age was 40.2+6.12years (20 to 60 years). Out of 2645patients, Anti HCV was positive in 288(10.88%) cases followed by HBsAg was in 152(5.74%)cases. While both positive in 36 (1.36%) cases. We observed Previous surgery was main riskfactor in the reactive 156(32.77%) cases followed by Barbar, Blood transfusion were 74(15.54%)and 47(9.87%) respectively. Conclusion: We conclude that preoperatively screening of hepatitisB and C should be performed.

Abstract Background Inflammatory adverse events following COVID-19 vaccination are being reported amidst the growing concerns regarding vaccine’s immunogenicity and safety, especially in patients with pre-existing inflammatory conditions. Methods Multinational case series of patients diagnosed with an ocular inflammatory event within 14 days following COVID-19 vaccination collected from 40 centres over a 3 month period in 2021. Results Seventy patients presented with ocular inflammatory events within 14 days following COVID-19 vaccination. The mean age was 51 years (range, 19-84 years). The most common events were anterior uveitis (n = 41, 58.6%), followed by posterior uveitis (n = 9, 12.9%) and scleritis (n = 7, 10.0%). The mean time to event was 5 days and 6 days (range, 1-14 days) after the first and second dose of vaccine, respectively. Among all patients, 36 (54.1%) had a previous history of ocular inflammatory event. Most patients (n = 48, 68.6%) were managed with topical corticosteroids. Final vision was not affected in 65 (92.9%), whereas 2 (2.9%) and 3 (4.3%) had reduction in visual acuity reduced by ≤ 3 lines and > 3 lines, respectively. Reported complications included nummular corneal lesions (n = 1, 1.4%), cystoid macular oedema (n = 2, 2.9%) and macular scarring (n = 2, 2.9%). Conclusion Ocular inflammatory events may occur after COVID-19 vaccination. The findings are based on a temporal association that does not prove causality. Even in the possibility of a causal association, most of the events were mild and had a good visual outcome.

Imported malaria has increased in Colombia since 2015 and has been attributed to migrants coming from Venezuela. We present a series of malaria cases, nested in a retrospective cross-sectional study between 2017 and 2018, aimed at calculating the prevalence of medical diseases among immigrants in a University Hospital in Colombia. Among 154 immigrants admitted for medical causes between 2017 and 2018, 8 were diagnosed with malaria, all due to Plasmodium vivax. Of these, seven had uncomplicated malaria, five had a previous history of malaria, one was critically ill, but none died. We highlight that, similar to other case series of imported malaria, Latin American migrants were young, with similar clinical profiles, having a low proportion of severe cases, and P. vivax was the most frequent cause.

Abstract Background Previous studies have reported a high prevalence of hypertension in Iranian students, especially in rural areas. The aim of this study was to investigate the daily intake of salt in students and its association with high blood pressure. Methods A random sub-sample was selected from the participants of the second phase of Shahroud schoolchildren eye cohort study and then a random urine sample was tested for sodium, potassium and creatinine. Urine electrolyte esexcretion and daily salt intake were calculated by Tanaka et al.’s formula. Results Among 1455 participants (including 230 participants from rural area and 472 girls), the mean age was 12.9 ± 1.7 year and the mean daily salt intake was 9.7 ± 2.6 g (95% CI 9.5–9.8). The mean salt consumption in rural areas [10.8 (95% CI 10.4–11.2)] was higher than urban areas [9.4 (95% CI 9.3–9.6)], in people with hypertension [10.8 (95% CI 10.3–11.3)] was more than people with normal blood pressure [9.4 (95% CI 9.3–9.6)], and in boys [9.8 (95% CI 9.7–10.0)] was more than girls [9.3 (95% CI 9.1–9.6)]. Higher age, BMI z-score, male sex and rural life, were associated with increased daily salt intake. Increased salt intake was associated with increased systolic and diastolic blood pressure. Conclusion Daily salt intake in Iranian adolescents was about 2 times the recommended amount of the World Health Organization, was higher in rural areas and was associated with blood pressure. Reducing salt intake should be considered as an important intervention, especially in rural areas.

AbstractChina is a populous country but lacks epidemiological data on keratoconus (KC). The present study aimed to investigate the clinical data, demographic data, and visual function (VF) data of KC patients in Central China. A total of 524 KC eyes in 307 KC patients (217 bilateral and 90 unilateral) from Henan Eye Hospital were included in the current study. Demographic and VF data were assessed with questionnaires administered by well-trained staff during face-to-face interviews. Visual acuity value was examined by a qualified optometrist, and the clinical data were measured by professional clinicians. The distributions of sex, residence and education level of KC patients were compared by Chi-square tests, and the ratios of people wearing glasses and rigid gas permeable (RGP) lenses were compared by McNemar tests. General linear models/Chi-squared tests were used to compare the clinical and demographic data according to KC severity. Spearman’s correlation analysis was used to test the associations between the data and KC severity. The mean age at diagnosis was 20.98 ± 6.06 years, and males had a higher ratio of KC than females (P < 0.001). Patients in rural areas had a higher rate of KC than those in urban areas (P = 0.039), and the proportion of KC patients with a higher education level (above high school) was high (P < 0.001). A total of 68.40% of the patients reported eye rubbing and 3.52% had a positive family history. The percentage of people wearing glasses was higher than that of patients wearing RGP lenses (P < 0.001). The total VF score of KC patients was 69.35 ± 15.25. The thinnest corneal thickness (TCT) and stiffness parameter at the first applanation (SP-A1) values were inversely correlated with KC severity (P < 0.05). The mean, steep, and max keratometry (Km, Ks and Kmax) values, the RGP lens use and keratoplasty were positively correlated with KC severity (all P < 0.05). The total VF score of the eye with better VA decreased as the severity increased (r = − 0.21, P = 0.002). The present study comprehensively describes various associated features of KC patients from a tertiary hospital in Central China, providing a reference for understanding the characteristics of KC patients in China.

Sialendoscopy has emerged as a safe, effective and minimally invasive technique for management of obstructive and inflammatory salivary gland disease. The aim of our study was to analyze outcomes of sialendoscopy and steroid irrigation in patients with sialadenitis without sialoliths. We performed a retrospective analysis of patients who underwent interventional sialendoscopy with steroid irrigation from 2013 to 2016, for the treatment of sialadenitis without sialolithiasis. Twenty-two patients underwent interventional sialendoscopy with ductal dilation and steroid irrigation for the treatment of sialadenitis without any evidence of sialolithiasis. Conservative measures had failed in all. Eleven patients had symptoms arising from the parotid gland, 4 patients had symptoms arising from the submandibular gland, while 6 patients had symptoms in both parotid and submandibular glands. One patient complained of only xerostomia without glandular symptoms. The mean age of the study group which included 1 male and 21 females was 44.6 years (range: 3-86 years). Four patients had autoimmune disease, while 7 patients had a history of radioactive iodine therapy. No identifiable cause for sialadenitis was found in the remaining 11 patients. The mean follow-up period was 378.9 days (range: 16-1143 days). All patients underwent sialendoscopy with ductal dilation and steroid irrigation. Twelve patients showed a complete response and 9 patients had a partial response, while 1 patient reported no response. Only 3 patients required repeat sialendoscopy. The combination of sialendoscopy with ductal dilation and steroid irrigation is a safe and effective treatment option for patients with sialadenitis without sialoliths refractory to conservative measures. Prospective studies with a larger case series are needed to establish its role as a definitive treatment option.
