235 Background: Part of Dana-Farber Cancer Institute’s mission is to educate the community and raise awareness about the importance of cancer prevention, outreach, screening, early detection, clinical trials and survivorship. Development of a logic model and evaluation plan enables us to assess program quality and identify areas for improvement and future outreach. Methods: A forward logic, iterative approach was used to develop logic models for our community outreach programs. Models were reviewed by key stakeholders including program staff and management. These were used as the basis for a comprehensive logic model encompassing the common elements across all programs. Results: Seven logic models were developed to capture work in tobacco, sun safety, breast health, obesity prevention, community education programs, as well as comprehensive community benefits program model. We designed a developmental evaluation to allow for an assessment that could be adapted in a dynamic, ever-changing environment. This approach is also responsive to program growth and changes. It measures demographic information about our community participants, satisfaction with our programs, and strength of our community partnerships and compares these to internal benchmarks and state information. We can re-contact participants for on-going assessments and provide cross-sectional and longitudinal reports. RedCap freeware was used to create online versions of all tracking measures. Since (date) we have captured information from 119 community participants, most of whom are Black and Latino/a, and are representative of traditionally underserved minorities. Conclusions: Logic models and evaluation plans are integral components of community outreach programming. They ensure that effective, high-quality community outreach programming is being delivered, allow for the assessment of impact and assessment against program benchmarks. More importantly, they provide continuous information about areas for program growth and improvement.

A well-designed logic model shows inter-connections within components of outreach programs, community projects, grant proposals, and even the design of a graduate internship. The logic model is a systematic display of inputs, outputs, outcomes, and impact on a community. One may define community as any group of people with a common vision, purpose, or problem. Relationships among people are the focal point for a community and the logic model can be used as a tool for fostering open communication within the community. The logic model is typically one of three approaches in design: 1) theoretical, 2) outcomes-based, or 3) activities approach. Use of a logic model ensures accountability for stakeholders, a tool for generating a common vision, and a method for reporting far-reaching impact as a result of activities identified for the model.

Abstract IntroductionWe describe the VMMC uptake across in Rural Non-circumcising provinces of Luapula, Northern and Muchinga 2018 to 2020. Method This was cross sectional analysis of routine programme data targeting males 10 years older (from October 2018 to April 2020), 15 years (from May 2020) and above VMMC was undertaken using both static and outreach activities. A multi-pronged strategy was adopted that included procurement of VMMC kits to supplement the government’s shortfall, training of VMMC providers (doctors, clinical officers and nurses) dedicated space for circumcision in each facility, logistics support ( fuel and allowances) for outreach programs, mapping of catchment areas for community mobilization and demand creation. All circumcisions performed were recorded in MOH VMMC registers. Data collected was verified by Provincial Health Office. Daily Situation Room (DSR) reports were used to monitor performance. Poor performing districts were followed up and challenges addressed to improve performance. ResultsA total of 2,130 focused outreach activities were carried out between 2018 and 2020 across the three provinces and 486,750 participants were reached. Of the 486,750 participants reached, 151, 428 were circumcised; 56136 (37%) from Northern, 49297 (33%) from Muchinga, and 45995 (30%) from Luapula. There was a three-fold increase in circumcision between 2018 and 2019 (14,746 circumcisions in 2018 vs 54,287 circumcisions in 2019) and a 14% increase from 2019 (38%) to 2020 (52%; 58,287 circumcisions). Most (76%) of the circumcision were done in the age group 15 to 29 years. HIV testing was undertaken among 6,319 participants giving a positivity rate of 2.4% (149). Improved results were associated with logistical support such as transport, VMMC commodity supplies, and increased dedicated VMMC providers. ConclusionHealth systems support including training, logistical support, dedicated space and supply of commodities assisted in increase of VMMC uptake in these rural non-traditional circumcision provinces. even amidst the outbreak of COVID19. Health systems strengthening and community outreach programmes are recommended for building health programs resilience in era of the COVID pandemic.

Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.

Abstract A new logic-model derivation method for leak faults observed by light-emission microscopy (LEM) or in liquid-crystal analysis (LCA) has been developed to verify those faults by comparing them with failures observed on an LSI tester. Since CMOS devices display various kinds of faulty behavior depending on leak resistance, it is essential to include the effects of this resistance in logic models. Considering that the resistance of leaks observed in LEM and LCA ranges from 10 to 10,000 ohm, the new logic models have been derived so that the leak fault could be easily incorporated into logic simulators without SPICE simulation. The feasibility of the proposed method has been demonstrated by using it to diagnose LEM and LCA faults causing logic failure in a 20k-gate logic LSI circuit.

Background: Pigmented tumour of the skin is one of the common tumour in human including the benign pigmented tumours (more common) called Nevi tumours and the malignant one called melanoma which was less frequent but the most poor in prognosis. In addition, the others not belonging to these group had the same clinical appearance, so the application of histopathology and immunohistochemistry for the definitive diagnosis was indespensible. Objectives: 1. To describe the macroscopic features of the pigmented tumoral-like lesions; 2. To classify the histopathologic types of the pigmented cell tumours and the other pigmented tumours of the skin. Materials and Method: Cross-sectional research on 55 patients diagnosed as pigmented tumoral lesions by clinician, then all definitively diagnosed by histopathology combining the immunohistochemistry in difficult cases. Results: There was no difference in gender, the disease was discovered most common in adult, especially with the age over 51 years old (58.1%). the most region located was in the face accounting for 60%, following the trunk and limbs (14.6%, 12.8% respectively). All 3 malignant melanomas happened in foot. The most common color of the lesions was black (65.4%), the other ones were rose, grey and blue. Histopathology and immunohisthochemistry showed that the true pigmented cell tumours were 52.6% encompassing benign ones (Nevi tumour) (41.8%), melanoma (5.4%) and lentigo (5.4%). 47.4% was not the true pigmented cell tumour including pigmented basocellular carcinoma (36.4%) and the others less common as histiofibromas, acanthoma and papilloma. Conclusion: the pigmented tumoral-like lesions of the skin could be the true pigmented cell tumours and the others, so the application of the histopathology and the immunohistochemistry after the clinical discovery helps to determine and classify the disease definitely and for the best orientation of treatment as well. Key words: skin tumour, benign pigmented tumour (Nevi), malignant pigmented tumour (melanoma), pigmented basocellular carcinoma

Background: Studies have previously shown greater arterial and venous extracranial vascular changes in persons with multiple sclerosis (PwMS) when compared to healthy controls (HCs). Objectives: To determine the change in the number and size of secondary neck vessels in PwMS and HCs over a 5-year follow-up period. Methods: Both at baseline and follow-up, 83 PwMS and 25 HCs underwent magnetic resonance angiography (MRA) imaging and analysis. The number and cross-sectional area (CSA) of all secondary neck vessels (excluding the common/internal carotid, vertebral artery, and internal jugular vein) measured at levels from C2-T1 were determined by semi-automated edge detection/ contouring software. The longitudinal change in the number and CSA of the secondary neck vessels from the PwMS and HCs were analyzed by non-parametric Wilcoxon repeated measure. Benjamini-Hochberg procedure adjusted for false discovery rate (FDR). Results: For over 5 years, PwMS demonstrated a consistent longitudinal decrease in both the number of secondary neck vessels (Z-change between -3.3 and -5.4, q=0.001) and their CSA (Zchange between -2.9 and -5.2, q=0.004). On the contrary, the HCs did not demonstrate a significant longitudinal change in secondary neck vessels over the follow-up period. Due to the longitudinal decrease, the PwMS showed a lower number of secondary neck vessels when compared to HCs measured at follow-up (p<0.029, except for C4 with trending p=0.071). The PwMS changes were also corroborated within each MS phenotype. Conclusion: PwMS demonstrate a significant mid-term decrease in the number and the size of the secondary neck vessels. The clinical relevance of these findings and the effect on intracranial blood flow are currently unknown.
