Comprehensive and epidemiological characterization of urachal adenocarcinoma: A pan-Canadian collaboration.

516 Background: Urachal cancer is a rare form of urothelial cancer representing 0,1-0,2% of bladder cancers. The exact underlying cause of urachal cancer is still widely unknown and there is much speculation on multiple factors such as genetic and environmental ones that may play a role. Moreover, given the lack of treatment consensus, we proposed the first study that assesses the clinical, genetic and molecular features of this disease in Canada. Methods: This ongoing study is recruiting in Canadian centers from the year 2005 onwards. The clinical database is constructed using the hospital electronic medical file. In addition, pathological tissues are centralized for molecular and genetic analysis. Results: To this date, 15 patients were included, 12 males and 3 females. Median age was of 49 years old with a disease distribution of Stage II (4), Stage III (3) and Stage IV (4). 8/15 patients developed metastasis after initial cystectomy in the lungs (n = 7), peritoneum (n = 2), lymph nodes (n = 4), and bone (1). Median survival of all patients was 23 months. Metastatic patients after cystectomy had a shorter overall survival (OS) of 21 months compared to non-metastatic patients (OS – not reached). The majority of metastatic patients (6/8) died of their underlying malignancy. Conclusions: This study is an ongoing country-wide effort to expand knowledge of this poorly studied disease. With ongoing accrual, the clinical features will be better defined and further molecular characterization will be performed to better predict response to therapy.

Urachal cancer is a rare and aggressive cancer that often presents in advanced stages. Given the rarity of this malignancy, medical case studies provide one of the few sources of literature available through which clinicians can guide medical management. Surgery is widely considered to be the mainstay of therapy when disease is localised and surgically resectable, therefore most current case studies on urachal cancer focus on surgical management, occasionally with adjuvant chemotherapy. However, few case studies discuss chemotherapy alone in the treatment of metastatic disease. Most studies indicate a median overall survival between 12 and 24 months for metastatic urachal adenocarcinoma. Bone marrow metastasis of solid tumours, when considered alone, portends a poor prognosis. The patient in this case study represents a rare case of stage IV urachal adenocarcinoma metastatic to the bone marrow without progression of disease after 6 months of treatment.

Abstract This report describes the clinical and immunoglobulin features of a patient with gamma heavy chain disease (HCD), who presented with a clinical picture suggestive of an underlying malignancy rather than the usual picture of lymphoma or granulomatous disease. A unique clinical feature was the nearly total replacement of the submaxillary glands by plasma cells. The patient's serum and urine contained a paraprotein, gammaHCD protein BAZ, which belongs to the gamma1 subclass and forms noncovalently linked dimers with a molecular weight of approximately 60,000 daltons. This mutant protein exhibited a deletion which encompassed most of the variable (V) region, the first constant domain (CH 1), and the hinge region. In addition, preliminary structural analyses demonstrated the replacement of alanine by glycine in position 431 of the carboxyterminal octadecapeptide. This substitution may possibly represent another allotypic marker on IgG1 proteins.

Blackberry cane diseases with the symptoms of necrosis, canker, and wilting are caused by several fungi worldwide. Surveys conducted from 2013 to 2016 in Serbia revealed the occurrence of Gnomoniopsis idaeicola, the causal agent of cane canker and wilting, which was found to be distributed in almost half of the surveyed orchards, in three blackberry cultivars, and with disease incidence of up to 80%. Wide distribution and high disease incidence suggest that G. idaeicola has been present in Serbia for some time. Out of 427 samples, a total of 65 G. idaeicola isolates were obtained (isolation rate of 34.19%). Reference isolates, originating from different localities, were conventionally and molecularly identified and characterized. G. idaeicola was detected in single and mixed infections with fungi from genera Paraconiothyrium, Colletotrichum, Diaporthe, Botryosphaeria, Botrytis, Septoria, Neofusicoccum, and Discostroma, and no diagnostically specific symptoms could be related directly to the G. idaeicola infection. In orchards solely infected with G. idaeicola, blackberry plant mortality was up to 40%, and yield loses were estimated at 50%. G. idaeicola isolates included in this study demonstrated intraspecies diversity in morphological, biological, pathogenic, and molecular features, which indicates that population in Serbia may be of different origin. This is the first record of a massive outbreak of G. idaeicola infection, illustrating its capability of harmful influence on blackberry production. This study represents the initial step in studying G. idaeicola as a new blackberry pathogen in Serbia, aiming at developing efficient control measures.

Soft tissue sarcomas (STSs) are a family of rare malignant tumors encompassing more than 80 histologies. Current therapies for metastatic STS, a condition that affects roughly half of patients, have limited efficacy, making innovative therapeutic strategies urgently needed. From a molecular point of view, STSs can be classified as translocation-related and those with a heavily rearranged genotype. Although only the latter display an increased mutational burden, molecular profiles suggestive of an “immune hot” tumor microenvironment are observed across STS histologies, and response to immunotherapy has been reported in both translocation-related and genetic complex STSs. These data reinforce the notion that immunity in STSs is multifaceted and influenced by both genetic and epigenetic determinants. Cumulative evidence indicates that a fine characterization of STSs at different levels is required to identify biomarkers predictive of immunotherapy response and to discover targetable pathways to switch on the immune sensitivity of “immune cold” tumors. In this review, we will summarize recent findings on the interplay between genetic landscape, molecular profiling and immunity in STSs. Immunological and molecular features will be discussed for their prognostic value in selected STS histologies. Finally, the local and systemic immunomodulatory effects of the targeted drugs imatinib and sunitinib will be discussed.

Background Colorectal carcinoma (CRC) during the peri-partum period is challenging to diagnose due to the overlapping symptoms of CRC and pregnancy. This is the first case series to investigate clinicopathologic, hormonal and molecular features of CRC diagnosed during the peri-partum period. We hypothesized that advanced presentations of CRC could possibly be mitigated by pregnancy-related hormonal factors. Methods We conducted a retrospective review of five women diagnosed with CRC during the peri-partum period and studied the clinical and molecular features of their cancer. Results All patients presented with stage IV CRC at diagnosis; three had primary tumors in the rectum and two had primary tumors in the sigmoid colon. The liver was the most common metastatic site (three of five women). Immunohistochemistry stains were negative for estrogen receptors alpha (ERα) and beta (ERβ), and one tumor demonstrated low-level positivity for PR (1%). Formalin-fixed and paraffin-embedded (FFPE) biopsies from each case were tested with next-generation sequencing and found that all tumors were mismatch repair (MMR) proficient, and three harbored a KRAS mutation. Germline testing showed no predisposition to CRC; however, several somatic variants of undetermined significance (VUS) were identified. Discussion CRC in the peri-partum period poses significant risk factors for presentations with advanced disease due to diagnostic challenges. While our study provides no evidence that pathogenesis of CRC during pregnancy is driven by elevated estrogen and/or progesterone levels during pregnancy, additional putative etiologic factors, including placental growth factors, the immunosuppressive state of pregnancy and other physiologic processes during pregnancy, warrant future study.

In this paper, we introduce the Clinical Transformer - a recasting of the widely used transformer architecture as a method for precision medicine to model relations between molecular and clinical measurements, and the survival of cancer patients. Although the emergence of immunotherapy offers a new hope for cancer patients with dramatic and durable responses having been reported, only a subset of patients demonstrate benefit. Such treatments do not directly target the tumor but recruit the patient immune system to fight the disease. Therefore, the response to therapy is more complicated to understand as it is affected by the patients physical condition, immune system fitness and the tumor. As in text, where the semantics of a word is dependent on the context of the sentence it belongs to, in immuno-therapy a biomarker may have limited meaning if measured independent of other clinical or molecular features. Hence, we hypothesize that the transformer-inspired model may potentially enable effective modelling of the semantics of different biomarkers with respect to patient survival time. Herein, we demonstrate that this approach can offer an attractive alternative to the survival models utilized incurrent practices as follows: (1) We formulate an embedding strategy applied to molecular and clinical data obtained from the patients. (2) We propose a customized objective function to predict patient survival. (3) We show the applicability of our proposed method to bioinformatics and precision medicine. Applying the clinical transformer to several immuno-oncology clinical studies, we demonstrate how the clinical transformer outperforms other linear and non-linear methods used in current practice for survival prediction. We also show that when initializing the weights of a domain-specific transformer by the weights of a cross-domain transformer, we further improve the predictions. Lastly, we show how the attention mechanism successfully captures some of the known biology behind these therapies

Background: BIA-ALCL is a rare subtype of T cell lymphomas associated with textured breast implants which has recently been recognized. The pathogenesis of this entity remains elusive even if mutations in the JAK/STAT pathway have been identified. Little is known about the causes, prognostic factors of this disease, and treatment outcome. Methods: since 2016, a WebEx national multidisciplinary meeting has been implemented by the French Cancer Agency in order to better define therapeutic strategies for newly diagnosed cases after histologic confirmation. In the same time, BIA-ALCL registry was funded by LYSA in order to collect ambispectively, in France and Belgium, patient clinical data including reasons for breast implantation (breast augmentation, reconstruction), implant manufacturer, treatments and outcome. A biological program aiming molecular characterization of this T-cell lymphoma subtype has been set in coordination with the registry. Results: Fifty-eight patients (pts) have been analyzed so far among the 88 (67 in France and 21 in Belgium) identified from 2009 to 2019. Median age was 58 years (range 29-82) at diagnosis. In 29 out of the 58 pts (50%) the first implant followed a mastectomy for breast cancer. In this analysis, only implants in the breast(s) where the lymphoma occurred have been considered. Four pts (6.9%) had bilateral lymphoma and 54 pts had unilateral lymphoma (50% left side and 50% right side), 25 pts were implanted once (43.1%), 24 twice (41.4%) and 9 pts (15.5%) 3 times or more. The median delay between first implant and BIA-ALCL diagnosis was 11.9 years (range 4.1-37), and median delay from last implant to diagnosis was 6.5 years (range 0.2-25.4). The two clinical presentations i.e. seroma (n = 43, 74.1%) and breast tumor mass with or without seroma (n = 12, 20.7%) were most often correlated with the two distinct histological subtypes (in situ /mixed (n=41) or infiltrative (n=17). Three pts were diagnosed without any mass or seroma (1 lymph node involvement, 2 in the context of systematic implant removal). The majority of pts were stage I-II (n=45, 77.6%), and 13 (22.4%) pts were stage IV. One hundred and five implants have been used on lymphoma associated breast for these 58 patients. Considering available information regarding the type of implants, almost all patients had at least one silicone-filled (n=51) and at least one textured implant (n=49) with Biocell texturation (n=40, 69%). No patient had only smooth implant. Implant removal with total capsulectomy was performed in 49 patients and 17 underwent chemotherapy based mostly on CHOP or CHOP-like chemotherapy regimens (n=12) and brentuximab vedotin CHP (n=3). After 21 months of median follow-up, 52 pts are alive and free of evolutive disease and one was lost to follow up. Five pts have died, either from lymphoma progression alone (n=2), or associated with concomitant active breast cancer (n=2) and one due to another disease. All had an infiltrative histology, and the 2 patients who died from lymphoma were stage IV. All but one received systemic chemotherapy and one received palliative care only due to concomitant active breast cancer. One of these patients early relapsed after a first complete remission. After the BIA-ALCL diagnosis, breast reconstruction was performed in in 23 pts (39.7%), 17 with a new implant, lipofilling in 4 pts, with a flap in 4 pts, and one benefit from combined approaches. Whole exome sequencing and/or targeted deep sequencing was performed in 29 of these patients. Recurrent mutation of epigenetic modifiers were seen in 22 pts (76%) involving notably KMT2C (28%), CM2D (14%) and CREBBP (14%). Eighteen pts (62%) showed mutations in at least one member of JAK STAT signaling pathway including STAT3 (38%) and JAK1(21%). Conclusions: We here confirm that in situ BIA-ALCLs have an indolent clinical course and remain in complete remission mainly after implant removal. Infiltrative histological subtype which have a more aggressive clinical course should be precisely identified at baseline. In our series, most BIA-ALCL cases were associated with macrotextured implants with Biocell texturation observed in 69% of the cases. The molecular characterization of these cases highlights the key role of the JAK/STAT pathway, and the importance of epigenomics. Such observation provide basis to develop novel targeted therapies for patients with aggressive disease.
