The persistence of lung parenchymal changes Post COVID-19 is increasingly recognized as a vital outcome observed on some patients recovering from SARS-CoV2 infection with limited evidence-based management so far. Here we present a 64-year-old male developed extensive interstitial lung changes post SARS-CoV2 infection and was successfully managed with oral prednisone, pirfenidone, and azithromycin maintenance regimen with significant improvement in his clinical and radiographic parameters noted within a relatively short time. The role of a combined therapeutic approach with anti-inflammatory and anti-fibrotic drugs might be provocative action in patients with COVID-19 related interstitial lung disease, especially those at risk for persistent long-term abnormalities and pulmonary fibrosis development.

Diagnosis of tinea capitis requires direct microscopy with potassium hydroxide (KOH) staining and culture examination as the gold standard. Dermoscopy for the diagnosis of tinea capitis needs to be appraised through evidence-based case report method. A 22 years old woman came with systemic lupus erythematosus which was treated with methylprednisolone and azathioprine for a year. She had wide patchy alopecia, dry scaling, dull gray hair and minimal inflammation. KOH 20% showed septate hyphae and spores surrounding her hair shafts. Dermoscopy examination showed “comma hairs”, “corkscrew”, “perifollicular scaling” and “broken hairs”. Three articles from Pubmed and Gale fulfilled the inclusion and exclusion criteria. Articles were critically appraised and concluded that comma hairs sensitivity rate varies between 60-66% and specificity of 100%, while corkscrew had sensitivity rate varies 20-80% and specificity of 83-100%. Combination of dermoscopy with clinical performance and direct microscopic examinations amplify definitive diagnosis. Further investigations are needed to validate the role of dermoscopy as a diagnostic tool per se for tinea capitis.

This chapter explores and examines the high incidence of malnutrition linked to a cancer diagnosis, and possible strategies to reduce or mitigate the effects of this phenomenon are also discussed. The complex nature of cancer cachexia is considered with regard to the metabolic and immune causes of cancer-related weight loss. The effect of cachexia on the patient and their treatment is also scrutinized, alongside ways to assess the severity of the problem. The important role of dietary support in counteracting cancer-related malnutrition is examined in detail, with the use of oral dietary supplements, feeding via nasogastric or gastrostomy tubes, as well as the place of parenteral nutrition forming part of this examination. The vital contribution of the dietician in cancer care is raised, as well as the importance of nurses in giving evidence-based nutritional advice.

AbstractHydatid disease is a parasitic infestation in humans caused by Echinococcus granulosus. Here, it develops in liver, lung, brain or other organs but liver is the most commonly affected organ. Intensity of morbidity depends upon location, expansion and activity of cyst. In modern medicine, surgery is still the treatment of choice but the chance of relapse and other complications is always there. Homoeopathy provides a better response in such conditions. One such case report is presented below. A young female patient attended outdoor patient department chiefly for complaints of jaundice, vomiting and right hypochondriac pain for the last 1 month and her diagnosis was hydatid cyst in right lobe of liver sized 11 × 15mm (by ultrasonography). She was successfully treated with the single and individualised homoeopathic medicine Crotalus horridus in 50 millesimal potencies in respect of both symptomatically and pathologically as evident by ultrasonography reports. Possible causal attribution of changes was explicitly depicted by Modified Naranjo Criteria. It shows positive role of homoeopathic treatment in regression of hydatid cyst in a young female.

AbstractObesity is a well-known risk factor for multiple diseases including multiple sclerosis (MS). Polymorphisms in the fat-mass obesity (FTO) gene have been consistently found to be associated with obesity, and recently found to increase the risk of developing MS. We therefore assessed the common FTO gene polymorphism (rs9939609) in relation to obesity, risk of developing MS and its disability in a cohort of MS patients. A cohort of 200 MS patients (135 females and 65 males) were genotyped for the FTO rs9939609 polymorphism. Using both logistic and linear regression we assessed the relationship between the variant and the selected phenotypes under both an additive and recessive genetic models. The A-allele was found to be associated with being overweight/obese in MS patients (OR = 2.48 (95% CI 1.17–5.29); p = 0.01). In addition, The A-allele was also found to be associated with increased MS disability (β = 0.48 (95% CI 0.03–0.92); p = 0.03). However, no association was found with risk of developing MS (p > 0.05). Moreover, our association with obesity is consistent with previous reports, whereas the association with disability is novel and warrants further investigation on the role of FTO in disease progression.

Purpose Misophonia is a sound tolerance disorder condition in certain sounds that trigger intense emotional or physiological responses. While some persons may experience misophonia, a few patients suffer from misophonia. However, there is a dearth of literature on audiological assessment and management of persons with misophonia. The purpose of this report is to discuss the assessment of misophonia and highlight the management option that helped a patient with misophonia. Method A case study of a 26-year-old woman with the complaint of decreased tolerance to specific sounds affecting quality of life is reported. Audiological assessment differentiated misophonia from hyperacusis. Management included retraining counseling as well as desensitization and habituation therapy based on the principles described by P. J. Jastreboff and Jastreboff (2014). A misophonia questionnaire was administered at regular intervals to monitor the effectiveness of therapy. Results A detailed case history and audiological evaluations including pure-tone audiogram and Johnson Hyperacusis Index revealed the presence of misophonia. The patient benefitted from intervention, and the scores of the misophonia questionnaire indicated a decrease in the severity of the problem. Conclusions It is important to differentially diagnose misophonia and hyperacusis in persons with sound tolerance disorders. Retraining counseling as well as desensitization and habituation therapy can help patients who suffer from misophonia.

Abstract. Two experiments investigate the role of self-regulatory resources in bullshitting behavior (i.e., communicating with little to no regard for evidence, established knowledge, or truth; Frankfurt, 1986 ; Petrocelli, 2018a ), and receptivity and sensitivity to bullshit. It is hypothesized that evidence-based communication and bullshit detection require motivation and considerably greater self-regulatory resources relative to bullshitting and insensitivity to bullshit. In Experiment 1 ( N = 210) and Experiment 2 ( N = 214), participants refrained from bullshitting only when they possessed adequate self-regulatory resources and expected to be held accountable for their communicative contributions. Results of both experiments also suggest that people are more receptive to bullshit, and less sensitive to detecting bullshit, under conditions in which they possess relatively few self-regulatory resources.
