The epidemics of obesity and type 2 diabetes mellitus (T2DM) globally are paralleling an increase in the number of women with T2DM becoming pregnant. Because T2DM is frequently undiagnosed before pregnancy, the risk of major malformations in the developing fetus is increased due to uncontrolled hyperglycemia. The lack of preconception care and the increase in complications of pregnancy due to the coexistence of obesity and T2DM are of concern from both an individual and a public health standpoint. Rapid achievement of normoglycemia with limited weight gain is critical to optimize maternal and fetal outcomes in all women with diabetes during pregnancy, regardless of the type of diabetes. This article will focus on T2DM preceding pregnancy due to its increasing prevalence and potentially dire fetal and maternal consequences. Euglycemia before, during, and after all pregnancies complicated by diabetes results in the best opportunity for optimal outcomes for mother and infant.

Rapid urbanization, increasing population and increased industrialization to cater to demands of the growing population has imposed upon us a huge environmental cost. The significantly deteriorated air quality across the globe is associated with a direct and indirect impact on public health. While associated disorders such as chronic obstructive pulmonary diseases, heart failures are well documented, less is known about the biological basis of the process. We hypothesize that the worsening air quality may likely impact common systemic inflammatory processes, thus driving communicable and non-communicable diseases alike.Receptor mediated entry of particulate matter (PM2.5) results in activation of signaling cascades which culminate in production of inflammatory chemokine and cytokine responses, traversing through the blood mediating impacting not only on other organs but also dysbiosis of microflora. For the purpose of the review we choose tuberculosis (TB) as a model for communicable infectious disease and type 2 diabetes mellitus (T2DM) as a marker for non-communicable disorder.  The increasing prevalence of these co-morbidities and the burdening of public health systems justifies this example. However the hypothesis may be applicable to other inflammation driven disorders also.

Diabetes merupakan peyakit kronis yang disebabkan karena kekurangan atau tidak efektifnya insulin yang dihasilkan. WHO memperkirakan jumlah penduduk dunia yang menderita diabetes pada tahun 2030 akan meningkat paling sedikit menjadi 366 juta. Indonesia menempati urutan keempat terbanyak dengan prevalensi 8,6% dari seluruh penduduk. Diabetes akan menjadi masalah yang serius apabila telah terjadi komplikasi, salah satu komplikasi adalah luka kaki diabetes (diabetic foot). Komplikasi luka kaki diabetes menjadi penyebab lamanya hospitalisasi dan amputasi lebih dari 90% ekstremitas bawah pada penderita diabetes. Tujuan penelitian adalah untuk menganalisis hubungan perilakuperawatan kaki dengan terjadinya komplikasi luka kaki diabetes pada pasienDM tipe 2di Puskesmas Tuntungan Medan. Disain penelitian adalah analitik dengan pendekatan cross sectional. Populasi penelitian adalah seluruh penderita DM tipe 2 yang datang ke Puskesmas Tuntungan Medan dengan sampel sebanyak 83 orang (consecutive sampling). Instrumen penilaian perilaku perawatan kaki yang digunakan adalah Questions determining the knowledge and practice about foot careyang terdiri dari 15 item pertanyaan. Analisis data menggunakan uji statistik chi square. Hasil penelitian menunjukkansebanyak 48 orang pasien diabetes (57,8%) memiliki perilaku yang buruk terhadap perawatan kaki, sebanyak 29 orang (349%) mempunyai riwayat komplikasi luka kaki dibetes (ulkus). Hasil analisis chi square menunjukkan terdapat hubungan antara perilaku perawatan kaki diabetes dengan kejadian komplikasi luka kaki diabetes pada pasien DM tipe 2 di Puskesmas Tuntungan Medan (p<0.05). Perilaku perawatan kaki sangat berperan terhadap terjadinya luka kaki diabetes. Untuk membentuk perilaku yang baik dibutuhkan edukasi oleh dokter maupun oleh petugas kesehatan kepada pasien diabetes. Diabetes is a chronic disease caused by lack or ineffectiveness of insulin. WHO estimates that the world population with diabetes in 2030 will increase to at least 366 million people. Indonesia is the world’s fourth most populated country and its prevalence of diabetes is 8.6% of the entire population. Diabetes will be a serious problem once complications occurrs. One of the complications is diabetic foot.  diabetic foot  caused a longer length of hospital stay and more than 90% leads to the amputation of the lower limb . The objective of this study was to analyze the relationship between diabetic foot care behaviors and the diabetic foot complications in patients with type 2 diabetes mellitus at Tuntungan Public Health Center in Medan. This study employed an analytic study  with cross sectional approach. The population of this study was all patients with type 2 diabetes mellitus who came to Tuntungan Public Health Center in Medan  and 83 patients were recruited as samples (consecutive sampling). The research used a questionnaires with 15 questions   to collect data of knowledge and foot care behaviour. Data were analyzed using chi square test. The results showed that 48 diabetic patients (57.8%) demonstrated poor behavior towards foot care, 29 patients (349%) had a history of diabetic foot ulcer complications. The results of the chi square analysis showed that there was a significant relationship between diabetic foot care behavior and diabetic foot complications in patients with type 2 Diabetes Mellitus at Tuntungan Public Health Center in Medan (p <0.05). It was concluded that foot care behavior had a high correlation with the incidence of diabetic foot. It is recommended that doctors and health staffs educate diabetic patients to perform good foot care behaviors.

Diabetes and its complications are main causes of morbidity and mortality among adults in the USA. An increase in the number of individuals with diabetes is primarily attributed to changes in dietary patterns including increased consumption of obesogenic foods and beverages. Many individuals who are overweight and obese show signs of insulin resistance and are at increased risk of Type 2 diabetes mellitus (T2DM) and cardiovascular disease. Lifestyle interventions (i.e., physical activity and nutrition) are the cornerstone of T2DM management and prevention. Prior research attests to the health benefits of consuming nuts, which have a substantial amount of mono- and polyunsaturated fatty acids, for individuals at risk for or with T2DM, and walnuts appear to be particularly promising. Walnuts are rich in nutrients, minerals, antioxidants, and vitamins that can contribute to improved cardio-metabolic risk factors in individuals at risk for or with T2DM. This review assesses the cardio-metabolic benefits of walnuts in T2DM. The authors’ review indicates that the reported effects of walnuts on glycaemic control have been inconclusive, with several studies showing association with improved glycaemic control while others show no effect. Despite their high energy density and potential to contribute to weight gain, the authors’ review suggests that walnuts can contribute to satiety without association with weight gain. This review also suggests that walnut consumption has been associated with improved low-density lipoprotein cholesterol levels and endothelial function but has not been associated with blood pressure improvement. Meta-analyses are warranted to quantitatively assess impact of walnut consumption on these cardio-metabolic risk factors in T2DM.

Type 2 diabetes mellitus (T2DM) is a noteworthy worldwide public health problem. It represents a complex metabolic disorder, mainly characterized as hyperglycemia and lipid dysfunction. The gut microbiota dysbiosis has been proposed to play a role in the development of diabetes. Recently, there has been considerable interest in the use of medicine food homology (MFH) and functional food herbs (FF) to ameliorate diabetes and lead to a natural and healthy life. Hence, this review compiles some reports and findings to demonstrate that the practical use of the MFH/FF can modulate the homoeostasis of gut microbiota, thereby ameliorating the development of T2DM. The results provided useful data to support further investigation of the functional basis and application of MFH/FF to treat T2DM through maintaining intestinal homeostasis.

Objective: The objective of this study was to determine the effect of Self-Efficacy Enhancing Intervention Program (SEEIP) with online method on Self-Efficacy in Self-Management in Type 2 Diabetes Mellitus (T2DM) at Juli I Public Health CentreMethods: This study was a pre-experimental pre-test and post-test with only one group design. The population wasT2DM at Juli I Public Health Center in 2020. The sample size was 22 participants, selected by purposive sampling technique. Diabetes Management Self-Efficacy Scale (DMSES) was used to measure the Self-Efficacy in Self- Management of T2DM patients. Data were analyzed using paired t-testResults: The results of the study showed that the means score of Self- Management of T2DM before receiving SEEIP was 39,00 (SD: 15,455) and 90,23 (SD: 7,571)after receiving the SEEIP. Paired t-test showed that there was a significant difference on Self- Efficacy in Self-Management in T2DM at Juli I Public Health Centre in 2020Conclusion:  SEEIP could improve self-efficacy in self-management for Type 2 diabetes mellitus at Juli I Public Health Centre.

BACKGROUND: Type 2 Diabetes Mellitus (T2DM) kills more than 4.9 million adults yearly, and it is one of the major threats to global public health for low- and middle-income countries that mostly affects the adult population. Kiribati is currently facing the increasing prevalence of morbidity and mortality from T2DM. OBJECTIVE: To find out the level of Knowledge, Attitude and Practice (KAP) towards T2DM among healthy adults in South Tarawa, Kiribati. METHODS: This cross sectional study was conducted on South Tarawa, Kiribati at three randomly selected public health clinics from September 25 to November 20, 2017. Non-diabetic patients from both sexes who aged 18 years and above were selected by a simple random sampling technique to participate in this study. A pretested structured questionnaire was used to collect data and SPSS (version 22) was used for data analysis. Descriptive statistics was used to study the characteristics of the population and level of KAP. RESULTS: 405 person participated in this study. Majority of the participants were in the age range of 18-24 years (30.4%), were females (66.2%) and had ever married (68.6%). The study revealed that the mean knowledge score was 20.47 (&plusmn;3.49) which shows that participants had moderate level of knowledge towards T2DM. The mean score for attitude score was 61.06 (&plusmn;5.48) which shows that participants had high level of attitude towards T2DM. The mean practice score was 4.57 (&plusmn;2.01) which shows that participants had a low level of practice towards T2DM. CONCLUSION: A great emphasis on health education regarding symptoms, risk factors modification and prevention is T2DM are necessary.
