The technical and interpersonal aspects of telemedicine: effects on doctor–patient communication

The influence of telemedicine on the nature and content of doctor–patient communication stems from both its technical and its interpersonal aspects. While the technical aspects are concerned with the communication technologies used and the clinical processes enabled by those technologies, the interpersonal aspects are concerned with relationships between system personnel, providers and patients, and the way in which those relationships are organized. On the one hand, this paper posits that the influence of the technical environment stems from depersonalization of the doctor–patient relationship, participatory enhancements and impediments, and sensory and non-verbal limitations. On the other hand, it posits that the influence of the interpersonal environment stems from third-party participation, social and professional distancing, and underdeveloped norms and standards. A combined positivist and interpretivist evaluation strategy would enable researchers to make better-informed connections between telemedicine, medical encounter behaviour and health outcomes.

This paper questions the nature of the communicative event that takes place in online contexts between doctors and web-users, showing computer-mediated linguistic norms and discussing the nature of the participants’ roles. Based on an analysis of 1005 posts occurring between doctors and the users of health service websites, I analyse how doctor–patient communication is affected by the medium and how health professionals overcome issues concerning the virtual medical visit. Results suggest that (a) online medical answers offer a different service from that expected by users, as doctors cannot always fulfill patient requests, and (b) net consultations use aspects of traditional doctor–patient exchange and yet present a language and a style that are affected by the computer-mediated environment. Additionally, it seems that this new form leads to a different model of doctor–patient relationship. The findings are intended to provide new insights into web-based discourse in doctor–patient communication and to demonstrate the emergence of a new style in medical communication.

The article deals with issues of technologies in the environment of creative economy and creative society, mostly focusing on the following topics: 1) invasion of technologies, which is accompanied by technical illiteracy or simplification of intellection presupposed by a certain technique (e.g. computers); 2) new technologies emerge in the environment dominated by consumption in order to boost consumption; 3) political, media and communication technologies are intertwined to the extent that allows us to speak about the technologized society; 4) technologies are inseparable from creative activities: on the one hand, development of technologies needs creativity, on the other hand, every branch of creative industries needs certain technologies; 5) technologic development is conditioned by their syncretism, i.e. their ability to serve the art (technē) of life and creative intentions; 6) in the creative society, happiness does not depend on constantly upgraded (i.e. consumed) technologies but is rather possible in spite of them; 7) unlimitedness is the greatest limitation of global technologies: unconnected with any existential region, they billow in the wind of ever newer technologies.

This paper analyses the recent phenomenon of private/public partnerships (PPPs) in the ICT sector of the developing world. The partners may come to these projects with divergent motivations: profit on the one hand and the provision of public services on the other, but at the end of the day, the interests of the partners that are symbiotic can – and indeed should – be aligned to ensure successful long-term projects. To investigate what can be done to promote successful and sustainable PPPs, this paper extends the traditional two-actor analysis to include both a third-party non-profit-oriented facilitating organization and the technology recipients that are the targets of these projects. Following an overview of the current state of PPPs in the developing world, the paper provides two case studies, based in Vietnam, where all four of the above-mentioned stakeholders were involved. The cases reveal important success factors that can be applied to future PPPs in the ICT sector.

The formulation of Charts for research ethics and Codes of conduct has been growing in the last few decades, on the one hand due to a renewed awareness of the ethical dimensions of research governance and the relationship between regulators and researchers, and on the other hand for the expansion of possibilities achieved by innovation in information and communication technologies. The voluntary involvement of research participants, risk management and prevention, data protection, community engagement, reflexivity of researchers are some of the centres of gravity of a debate that involves researchers, institutions, and citizens.

Abstract Background The doctor-patient relationship in China has deteriorated in recent years, and poor doctor-patient communication is one of the main reasons. How to effectively carry out doctor-patient communication training originated from the West among Chinese medical students still to be studied. In the past decade, Peking Union Medical College has adopted clinical scenario drama to teach doctor-patient relationship and clinical communication skills. The aim of this study was to introduce clinical scenario dramas and evaluate its effectiveness in promoting doctor-patient relationships and clinical communication skills through students’ self-perceptions in Chinese medical students. Methods This study was a retrospective, self-controlled study and conducted from March 2009 to October 2018. Doctor-patient relationship and communication skills training were administered to all sixth-year medical students, which involved lectures and various clinical scenario dramas. The program totaled 24 h, of which each class session was 3 h, with 8 sessions in total. All students were requested to complete an anonymous 5 likert self-rating survey including self-confidence in using communication skills and self-perceived learning attitude and ability before and at the end of the course. In addition, they were requested to evaluate the curriculum after completion of the course. Results Clinical scenario dramas helped students improve their self-confidence in clinical communication skills except for psychosomatic history taking (p < 0.05). The interests for participation in clinical scenario dramas were higher compared to attending lectures (4.39 ± 0.610 Vs 4.07 ± 0.831, p<0.01). Study participants were highly satisfied in the course setting, teaching instructors and content (4.61 ± 0.546, 4.65 ± 0.535, 4.63 ± 0.534). The self-evaluation results demonstrated that clinical scenario dramas improved the learning ability of medical students (p < 0.05). Conclusion The use of clinical scenario dramas was helpful in teaching doctor-patient communication skills.

Under criminal law, euthanasia can have two distinct qualifications: that of homicide in the event that the act of directly killing another person is characterized, or that of assisting a third party in the suicide. These two qualifications are applicable on the condition that the agent — the author of the act of causing death — is not the one who went through it. In fact, selfeuthanasia is nothing more than suicide.In addition to euthanasia imposed to a third party (such as in the case of Malevre, nurse from Mantes-la-Jolie, tried in 2003), the euthanasia requested and subscribed constitutes a complex legal question. Answering this question first involves specifying the position of contemporary criminal law in the face of suicide.In the event that suicide is only decriminalized, in fact, the author of the act — regardless of the outcome of his actions, who is himself the victim, cannot be prosecuted. Nor ultimately receive any condemnation.However, this lack of prosecution and conviction is by no means an endorsement of the act — suicide — by the law.Moreover, in the event that suicide is a right, it would then be necessary to agree that any candidate for this act can request assistance in the accomplishment of his death. Given these two opposing approaches, imposed on us the question of whether there is a right to die.Although the euthanasia imposed is unequivocally under ordinary criminal law, the euthanasia requested and granted is not based on any rights. To date, there is no right to approve a death request, but on the other hand, it does allow it to be respected and to some extent promotes its approach with dignity. This work will focus on two central points which are the possibility that euthanasia is a homicide under common law (I) and the attitude of French law concerning the right to death (II).

Abstract Sponsorships on YouTube – i.e., video creators on YouTube promoting a third-party product or service to their audience – have attracted considerable research interest recently in various disciplines. This multidisciplinary study analyzes it from the perspective of argumentation theory, specifically pragma-dialectics, which offers valuable new insights into the discursive tensions inherent to this type of promotion. These tensions arise between the creator’s relationship with their audience on the one hand, which is built upon ‘parasocial’ evaluations of authenticity and community, and the commercial third party brand on the other. The insights provided by the pragma-dialectic analysis are demonstrated by means of a case study examining a sponsorship segment by YouTuber PewDiePie, which shows that creators can employ specific types of presentational choices and audience adaptation strategically to undercut commitment to the sponsor while furthering the relationship with their viewers.

Abstract Objective To explore the relationship among patient perception of doctors' empathy, doctor-patient communication, and doctor-patient relationship.Methods From November 2019 to April 2020, we used the CARE scale, the SEGUE framework and the PDRQ-15 scale to survey 3,039 patients from 14 provinces in China, ranging in age from 18 to 92 years old.Results 1. The age of the patient, the level and type of hospital, the length of consultation, and the presence or absence of verbal and physical conflict have significant differences in the scores of perceived empathy; 2. There was a significantly positive correlation among patient's perceived empathy, doctor-patient communication, and the doctor-patient relationship (r = 0.65 ~ 0.75, p < 0.001). 3. The patient's perception of doctor empathy can not only directly predict doctor-patient relationship (β = 0.75, p < 0.001), but also influence doctor-patient relationship through the mediating effect of doctor-patient communication (β = 0.56, p < 0.001). Besides, the scores of CARE can effectively explain the variation of each stage of doctor-patient communication skills (R2 = 0.28 ~ 0.37) and the satisfaction (52%) and approachability (54%) of the doctor-patient relationship scale.Conclusion The age of the patient, the type and level of the hospital, and the consultation time affected perceived empathy. Doctor-patient communication plays a partial mediating effect in the influence of empathy on the doctor-patient relationship.Practice implications: Medical institutes should improve the medical environment and carry out humanistic education and patient-centered communication skills training for doctors. These approaches will enhance patients' perceived empathy and therefore improve the doctor-patient relationship.
