Telecardiology: supporting the decision making process in general practice

To assess the initial phase of a telecardiology diagnostic service for general practitioners GPs , we provided 93 GPs in 26 health centres with direct telephone access to a cardiologist, and equipped them with hand-held, automatic standard 12-lead electrocardiogram ECG transmitters for on-line cardiac consultations and ECG interpretation in their daily practice. Clinical details, reason for consultation and the ECG signal were transmitted from the GPs' practices or the patients' homes. A consultation followed and a full report, including ECG print-out, was then sent to the GP. During an 18-month study period, 2563 consultations were carried out. The system allowed the identification of 479 patients 19 with urgent cardiac problems and the remaining 2084 81 in whom admission or outpatient investigation was unnecessary. Following the study, we distributed a questionnaire asking the GPs to rate the quality, define the use and consider the benefit of the service to their daily practice. We conclude that a telecardiology diagnosis and ECG interpretation service is simple, reliable and efficacious in routine primary care. It offers instant access to cardiac assessment and supports the decision-making process of GPs. A preliminary cost comparison with a conventional referral indicated that a teleconsultation was substantially cheaper. We expect that the future incorporation of teleechocardiography would expand the scope of telecardiology even further and allow comprehensive cardiology consultations.

In clinical practice and in particular in the diagnostic process, the assessment of cardiac and respiratory functions is supported by electrocardiogram and auscultation. These exams are non-invasive, quick and inexpensive to perform and easy to interpret. For these reasons, this type of assessment is a constant in the daily life of a clinician and the information obtained is central to the decision making process. Therefore, it is essential that during their training, students of health-related subjects acquire skills in the acquisition and evaluation of the referred physiological signals. Simulation, considering the technological possibilities of today, is an excellent preparation tool since it exposes trainees to near real contexts but without the associated risks. Hence, the simulation of physiological signals plays an important role in the education of healthcare professionals, bioengineering professionals and also in the development and calibration of medical devices. This paper describes a project to develop synchronized electrocardiogram (ECG), phonocardiogram (PCG) and breathing sounds simulators that aims to improve an existing phantom simulator. The developed system allows, in an integrated way, to generate normal and pathological signals, being contemplated several distinct pathologies. For engineering education, it is also possible to simulate the introduction of signal disturbances or hardware malfunctions.

Abstract This review provides guidance in the decision-making process regarding when to choose a janus kinase [JAK] inhibitor as medical treatment strategy. The focus will be on ulcerative colitis, because the only yet available JAK inhibitor, tofacitinib, has approval for use in ulcerative colitis. The guidance path will include consideration of disease activity, previous treatment, comorbidities, family planning, patient preferences, pharmacology as well as concurrent chronic inflammatory diseases or extraintestinal manifestations. The suggested guidance path illustrates our daily difficulties in the decision-making process regarding best choice for the individual patient. However if predictive biomarkers are lacking, the named criteria can be applied to any other strategy and hence provide support in daily practice.

Over the past few centuries, the process of decision-making has become more complicated in different respects. Since the initial phase of Multiple Criteria Decision Making (MCDM) around fifty years ago, Multiple Attribute Decision Making (MADM) has continued developing over the years as a sub-concept of MCDM. Noticeably, the importance of the decision-making process is increasingly expanding to such an extent that it necessarily blends into the undeniable processes of MADM actual models. Novel methods with different perspectives have been introduced considering the dynamic MADM concepts of time and future in classical frameworks; however, they do not overcome challenges in practice. Recently, Prospective MADM (PMADM) as a specific approach has presented future-oriented models using already known approaches of MCDM, and it has innovative items which show barriers of classic model of MADM. However, PMADM practically needs more conceptual bases to illustrate and plan the future of real decision-making problems. The Multi-Aspect Criterion is a new concept in mapping the future of the PMADM outline. In this regard, two examples of sustainability will be analyzed, and different requirements and aspects associated with PMADM will be discussed in this study. This new approach can support the PMADM outline in more detail and deal with a decision-making structure that can be considered as novel to industry experts.

The purpose of this article is to analyze the influence of the key features of agritourism companies' websites on customers purchasing decision-making process, by measuring the level of perceived relevance and satisfaction of customers about the website's features. Empirical research was conducted based on a four-stage website adoption model, adapted from previous literature. A web-based questionnaire was addressed to a sample of Italian tourists who experienced a stay at an agritourism after selecting it through the website. By combining the perceived satisfaction and importance attributed to a series of website features, the priorities to take into account to design a website and to establish its main on-line services have been identified. In order to identify which variables have more influence in defining the overall evaluation of a website, a regression analysis was conducted. This study provides useful guidance on the main features of a website on which investments for improvement may be addressed, and which on-line services should be activated.

O objetivo deste artigo é discutir os conceitos e abordagens metodológicas sobre avaliação e sua importância para o enfermeiro no exercício da gerência do cuidado. O enfermeiro em sua prática profissional cotidiana de gerenciamento do cuidado necessita de habilidades e conhecimentos que possibilitem o desenvolvimento de ações administrativas. A avaliação é um instrumento administrativo utilizado sistematicamente pelo ser humano e tem sido apontada também como ferramenta necessária para o profissional enfermeiro na sua prática de cuidado. A importância da avaliação na área de saúde deve-se ao seu caráter processual, multidisciplinar e social, e situa-se na possibilidade de extrair subsídios para a tomada de decisão, de verificação da eficácia, da efetividade e da eficiência das ações de saúde desenvolvidas e na possibilidade de redirecionamento das mesmas para as reais necessidades da comunidade. Concluiu-se, portanto, que a avaliação possibilita um processo reflexivo da prática realizada e gera conseqüentemente um movimento de transformação da realidade e crescimento dos sujeitos envolvidos no processo na ação avaliada.Exploring assessment: a basic tool to care management in public healthAbstractThe objective of this article is to discuss methodological concepts and approaches related to assessment in the exercise of nursing care management. The importance of evaluation in the field of health is due to its process, multifaceted, and social character. Evaluation makes possible to obtain grounded information for the decision making process, for the assessment of efficacy, effectiveness, and efficiency of health actions as well as for reorientation of these actions towards the actual needs of the communities. Thus, evaluation allows for a reflective process of praxis and generates both changes in the real world and growth of the individuals involved in the action that was evaluated.

The purpose of this article is to outline a decision-making process and highlight which portions of the augmentative and alternative communication (AAC) evaluation process deserve special attention when deciding which features are required for a communication system in order to provide optimal benefit for the user. The clinician then will be able to use a feature-match approach as part of the decision-making process to determine whether mobile technology or a dedicated device is the best choice for communication. The term mobile technology will be used to describe off-the-shelf, commercially available, tablet-style devices like an iPhone®, iPod Touch®, iPad®, and Android® or Windows® tablet.
