Telemedicine allows healthcare professionals to assess, diagnose and treat patients remotely using information and telecommunications technologies. Telemedicine is an evolutionary discovery that is increasingly becoming an important part of medical science and practice. Objectives: To study and analyze the opinion of physician assistants for telemedicine as a predictor for overcoming the barrier in the process of treatment of elderly people living in small and remote settlements. Methods: We conducted a survey of the opinion of 80 physician assistants on telemedicine as a predictor for overcoming barriers in the treatment of elderly and senile people living in small and remote settlements. Results: According to the analysis of the data on the benefits of the introduction of telemedicine and because it is a predictor for overcoming the barrier in the treatment of the elderly and elderly, the following was found: Medical professionals providing medical care to geriatric patients face a number of challenges that require a timely and adequate response, and telemedicine is a predictor of overcoming the barrier in the process of their treatment. In hard-to-reach and remote settlements, there is often a lack of general practitioners, enough outpatient clinics and places to perform activities for the treatment of geriatric patients. In these cases, the application of telemedicine would be adequate. This will increase the possibility of access to adequate medical care. Patients will receive precise and clear instructions. This will reduce the need for transportation or time for patients to access hospitals. Conclusion: The study allows us to draw the following inferences: Telemedicine is an opportunity to improve and enhance the professional activity of physician assistants working with geriatric patients and the implementation of telemedicine will increase the quality of medical care and health care for the elderly population in remote areas and small settlements.

Objective: We defined the internal and external motivation features, performed a differential assessment of the states of reduced working capacity in medical workers of the Vinnytsia O.I. Yushchenko Regional Clinical Psychoneurological Hospital, Vinnytsia Regional Council (Communal Non-Profit Enterprise) and developed the preventive measures. Materials and methods: 224 medical workers (15.2% of males and 84.8% of females) of the Clinical Psychoneurologic Hospital (Communal Non-Profit Enterprise) took part in the study. 38.8% of the doctors, 61.2% of the nurses took part in the survey. The length of service in the professional activity was (19.7+12.3) years among doctors and (15.5+11.1) years among nurses. The average age of the doctors made up (44.6+12.2) years, nurses – (37.2+11.4) years. We carried out a survey on the motivation of the professional activity of doctors and nurses by the methodology of K. Zamfir in the modification of A. Rean and a study based on the technique of A. Leonov, S. Velichkovskaya regarding the differential assessment of the states of reduced working capacity (DOSR) of workers of health care institutions. To calculate the arithmetic mean, standard arithmetic mean error, we processed the results with the help of the Statistica 6.1 for Windows licensed standardized package. In the study, we used the analysis of the work of domestic and foreign scientific sources, biblio-semantic, analytical, and statistical research methods. Results: The data of the survey of the psychoneurologic hospital medical workers regarding doctors' internal and external motivation and nurses' professional activity are presented in the article. Three types of motivation and the motivation complexes in healthcare workers were determined, the following classification of motivational factors were used: «motivators» and «context factors», or «hygienic factors», which provoke negative stress affecting the quality of medical care. Analysis of the survey data shows that the external negative motivation exceeds the external positive one in male doctors and female nurses – (3.2+0.8) and (3.0+1.2) points, respectively. That’s why these medical workers have a decrease in their interest in work, lack of striving for career growth, lack of self-perception, and an indifference to work, to patients, which can negatively affect the quality of medical care and interpersonal relationships the team and at home. The questionnaire results regarding the differential assessment of the states of the reduced working capacity of the psychoneurologic hospital workers have been analyzed. The results demonstrate a high level of oversaturation index in female doctors - 3.2% and female nurses - 1.6% and a high degree of fatigue - 4.8% and 3.9%, respectively, which can be assessed as a pre-disease state in women. The preventive measures to overcome these states and to improve the psychosocial environment for the quality performance of the professional duties and satisfaction with the work are proposed.

Despite the ongoing reforms, the staff shortage in the healthcare system and the loss of medical personnel in the field still negatively affect the quality of medical care. The article presents the results of monitoring the opinions of future resident physicians of the current organization and the quality of medical care and their readiness for professional activity, as well as their views to the healthcare system needs. It has been established that every third graduate was not ready for the profession before entering the residency, 36.7% considered their future profession to be nonprestigious, only half of the residents (50.3%) considered the medical profession to be highly paid, and personal aptitude (53%) and abilities (40%) were considered the leading factors in choosing the profession. The residents evaluated the quality of medical care at a high level only in a clinical setting (69.7%), and they do not plan to work in regional health care but prefer to stay in Moscow (60.3%). To preserve the healthcare system human resources, it is advisable to develop a program for managing risk factors for the professional environment, starting with training of personnel at the institute of higher education.

Introduction: The progressing aging process and comorbidities worsen the efficiency of the balance system in the elderly, which leads to a weakening of stability and, as a consequence, to falls and injuries. The first ones lead to: worsening of functioning, reduced mobility, an increased risk of disease and mortality, therefore systematic physical activity and shaping the balance using physiotherapy, which can prevent dangerous falls is very important. Aim: Assessment of the therapeutic effect of physiotherapy on minimizing imbalances in geriatric patients. M aterial and Methods: The study group consisted of 46 people, including 32 women (69.6%) and 14 men (30.4%); average age of respondents – 72.5 years. They were patients of the Department of Rehabilitation of Poddębice Health Center, Ltd. The researchers used a self-made questionnaire, body mass to height index (BMI) and the Tinetti Test. Results: Patients with an elevated BMI (89.1%), as well as those taking more than 4 medication (78.3%), have had more falls over the past year (respectively 91.4% and 81.4%). After the use of comprehensive therapy, none of the patients achieved a worse result than before the physiotherapy while 91.3% of the respondents had an increase in the number of points scored in the Tinetti Test. The percentage of patients at high risk of falling reduced from 67.4% to 37%. There was also a decrease in the fear of walking (in 58.7%), falling (in 57.7%) and climbing stairs (in 47.9%). According to 78.3% of respondents, physiotherapy positively affected their independence and quality of life. Conclusions: The use of comprehensive physiotherapy reduces the fear of walking, climbing stairs and falling, which can be a good predictor of prevention. Both polypragmasia and an elevated body mass index (BMI) increase the risk of falling. Comprehensive physiotherapy of geriatric patients helps to improve balance and gait stereotype. Physiotherapy for the elderly helps improve the quality of life, independence, minimize imbalances, and thus reduce the risk of falls. Balance exercises play an important role in preventing falls.
