Assessment of the Therapeutic Effect of Physiotherapy on Minimizing Imbalances of Geriatric Patients

Introduction: The progressing aging process and comorbidities worsen the efficiency of the balance system in the elderly, which leads to a weakening of stability and, as a consequence, to falls and injuries. The first ones lead to: worsening of functioning, reduced mobility, an increased risk of disease and mortality, therefore systematic physical activity and shaping the balance using physiotherapy, which can prevent dangerous falls is very important. Aim: Assessment of the therapeutic effect of physiotherapy on minimizing imbalances in geriatric patients. M aterial and Methods: The study group consisted of 46 people, including 32 women (69.6%) and 14 men (30.4%); average age of respondents – 72.5 years. They were patients of the Department of Rehabilitation of Poddębice Health Center, Ltd. The researchers used a self-made questionnaire, body mass to height index (BMI) and the Tinetti Test. Results: Patients with an elevated BMI (89.1%), as well as those taking more than 4 medication (78.3%), have had more falls over the past year (respectively 91.4% and 81.4%). After the use of comprehensive therapy, none of the patients achieved a worse result than before the physiotherapy while 91.3% of the respondents had an increase in the number of points scored in the Tinetti Test. The percentage of patients at high risk of falling reduced from 67.4% to 37%. There was also a decrease in the fear of walking (in 58.7%), falling (in 57.7%) and climbing stairs (in 47.9%). According to 78.3% of respondents, physiotherapy positively affected their independence and quality of life. Conclusions: The use of comprehensive physiotherapy reduces the fear of walking, climbing stairs and falling, which can be a good predictor of prevention. Both polypragmasia and an elevated body mass index (BMI) increase the risk of falling. Comprehensive physiotherapy of geriatric patients helps to improve balance and gait stereotype. Physiotherapy for the elderly helps improve the quality of life, independence, minimize imbalances, and thus reduce the risk of falls. Balance exercises play an important role in preventing falls.

Multimedication related to multimorbidity is common in the elderly with asthma. We aimed at comprehensively characterising medications used by elderly women and assessing how multimedication impacts on asthma prognosis.We performed network-based analyses on drug administrative databases to visualise the prevalence of drug classes and their interconnections among 17 458 elderly women from the Asthma-E3N study, including 4328 women with asthma. Asthma groups sharing similar medication profiles were identified by a clustering method relying on all medications and were studied in association with adverse asthma events (uncontrolled asthma, attacks/exacerbations and poor asthma-related quality of life).The network-based analysis showed more multimedication in women with asthma than in those without asthma. The clustering method identified three multimedication profiles in asthma: “Few multimorbidity-related medications” (43.5%), “Predominantly allergic multimorbidity-related medications” (32.8%) and “Predominantly metabolic multimorbidity-related medications” (23.7%). Compared with women belonging to the “Few multimorbidity-related medications” profile, women belonging to the two other profiles had an increased risk of uncontrolled asthma and asthma attacks/exacerbations, and had lower asthma-related quality of life.The integrative data-driven approach on drug administrative databases identified specific multimorbidity-related medication profiles that were associated with poor asthma prognosis. These findings support the importance of multimorbidity in the unmet needs in asthma management.

Geriatrics help prevent and promote actions that help to minimize the impacts on the quality of life of the elderly. Being the nurse together with a multidisciplinary team, of vital importance in the development of these actions, The Nurse seeks to adapt the reality and the environment to which the elderly person is, thus enabling a better quality of life in this environment. The audiologist audiologist aims at a concern in hearing health, seeking to assist in the prevention and promotion of actions that help patients to have a more effective social interaction and avoid isolation in the face of difficulties that they may present. In short, the natural aging process causes damage to various functions of the human body, and one of them is auditory. The presence of hearing loss has several negative consequences for the quality of life of individuals, especially social isolation and in these cases hearing aids end up being strong allies to minimize the impacts that hearing loss causes on people's lives. Thus, this work aims to relate the issue of social isolation caused by hearing loss, increased risk of depression in the selection process and adaptation of hearing aids for the elderly and the role of the geriatric nurse aiming at improving the quality of life and preventing problems that may arise. In this study, a bibliographic search was made in the databases and an interface between two areas of health: Nursing and Speech Therapy, and its search to improve the quality of life of the elderly population as a social being.

Abstract Objective: to determine the prevalence of body image satisfaction in elderly subjects, as well as its relationship with nutritional status, anthropometric indicators and quality of life. Method: a cross-sectional, descriptive and analytical study was carried out with elderly individuals from Primary Care units (Family Health Strategy) from all the health districts of Porto Alegre, in the state of Rio Grande do Sul. The variables studied were: sociodemographic (gender; age; marital status and schooling), anthropometric (weight; height; body mass index; arm, calf, waist and hip circumference; waist-hip ratio and triceps skinfold), nutritional (Mini Nutritional Assessment), and clinical (quality of life; body image satisfaction). The Stunkard Silhouettes Scale, the Mini Nutritional Assessment and the Flanagan quality of life scale were applied. For data analysis, logistic regression (five steps) was performed with the Conditional Backward method. Results: a total of 532 elderly users (68.96 (7.14) years) were evaluated, mostly women (64.8%) who were dissatisfied with their body image (71.7%). The predictors of body image satisfaction were: eutrophic/underweight body mass index, eutrophy/insufficient arm circumference, and higher mean quality of life scores. Conclusion: in general, a low prevalence of body image satisfaction was identified among the elderly. Subjects with lower body mass index, smaller arm circumference measures and better quality of life had a greater chance of being satisfied with their body image.

Abstract: Elderly have higher risk for fractures due to aging process which causes decreased bone density and quality. Inferior trunk fractures are the most common fractures in the elderly group, namely fractures of the hip, pelvis, lower vertebrae, and ankle. Geriatric fractures can be caused by high and low impact mechanisms. Low impact fractures most often occur due to osteoporosis associated with a mechanism of fall. Changes in musculoskeletal system are decreased muscle mass as well as bone density and quality that lead to osteoporosis. The diagnosis of fracture is based on history, physical examination, and supporting investigations. Treatment of geriatric fractures needs to be carried out by a team of doctors consisting of orthopedic doctors and geriatric doctors. Good communication and appropriate therapy plans need to be prepared thoroughly to achieve proper treatment in handling geriatric patients, therefore, the quality of life can be improved and disabilities can be prevented.Keywords: geriatric fracture Abstrak: Kelompok lanjut usia (lansia) memiliki risiko tinggi untuk terjadinya fraktur akibat proses penuaan yang menyebabkan penurunan kepadatan dan kualitas tulang. Fraktur trunkus inferior merupakan fraktur paling umum pada kelompok lansia yaitu fraktur pinggul, panggul, vertebra bagian bawah, dan pergelangan kaki. Fraktur geriatrik dapat disebabkan oleh mekanisme high impact maupun low impact. Fraktur low impact paling sering terjadi disebabkan oleh karena keadaan osteoporosis disertai dengan mekanisme jatuh. Perubahan yang dapat terjadi pada muskuloskeletal yaitu penurunan massa otot serta penurunan kepadatan dan kualitas tulang yang menyebabkan terjadinya osteoporosis. Diagnosis fraktur ditegakkan berdasarkan anamnesis, pemeriksaan fisik, dan pemeriksaan penunjang. Penanganan fraktur pada lansia perlu dilakukan oleh tim dokter yang terdiri dari dokter ortopedik dan dokter geriatrik. Komunikasi yang baik dan rencana terapi yang tepat perlu dipersiapkan agar pasien lansia dapat ditangani dengan baik sehingga dapat memperbaiki quality of life dan mencegah disabilitas.Kata kunci: fraktur geriatrik

Recent study trends on elderlies range from improving their quality of life to reducing the risk of falling. This paper investigates current concerns on the risk of any untoward fall or injury of elderly residents in public housing due to the rising number of the elderly population. This study aims to analyze published literature with a focus on finding a non-semantic relation on the built environment with the elderly quality of life and risk of falling. Introduction: Old age brings several factors that alter the physiological system, which results in loss of functionality in the elderly. Falls are a leading cause of death in this population or activity restrictions, social isolation, decline in health and increased risk of institutionalization. The quality of life in old age is a major challenge, it is necessary for improvement in all aspects of daily. Analyze physical exercise on quality of life of older physically active drives to see how exercise influences the improvement of the basic aspects to a healthier life, referring to physical and mental aspects of the elderly, prevention of the consequences arising from the process natural aging, such as falls. Objective: The objective of this study is to analyze quality of life in elderly practitioners of physical activity and relationship with risk of falls. Method: The sample was 84 seniors, recruited from the community of Londrina PR, physical activity practitioners, aged over 60 years who roam with or without assistive device, of both sexes . The evaluation was through structured questionnaires that focused on socioeconomic - demographic questionnaire falls which was used the Timed Up and Go (TUGT), analysis of quality of life (SF-36) and analysis of the risk of falls. Results: In relationship the anthropometrics characteristics of elderly subjects analyzed, the same presentation mean age 68 years, weight 63.5 kg, height 1.52, and body mass index 27.10. Was regarding correlation between TUGT and falls in the last year (P=0,048) e correlation between TUGT and Physicals Components of SF-36 (P=0,001), and was regarding that elderly with better scores in TUGT were better scores of Physical Components of SF-36 (P=0,003). Conclusion: The quality of life is directly linked to the physical condition of the elderly, elderly practicing physical exercises have a better quality of life and less prone to falls.

It is known that falls in the daily life of the elderly occur frequently during walking and rising up and greatly affect their quality of life. In the previous study we obtained the COG-heel horizontal distance and the COG horizontal velocity when the subjects performed rising up under the “fastest” or “slowest” velocity task. These seat-off parameters which were an indicator of the stability in rising from a chair were defined based on the relationship between them. In this experiment with healthy adults as subjects the seat-off parameters were found to become a different distribution because of the rising up strategies of each subject. Moreover, based on the relationship between the seat-off parameters and motor function, they could become an index expressing current fall risk and predicting future fall risk.

Abstract Background PASSI d'Argento (PdA) is an Italian population-based surveillance system, coordinated by the Istituto Superiore di Sanità (ISS) and carried out by the Local Health Units (LHUs). PdA monitors wide-ranged health related-behaviours in the elderly (65+) living in Italy as per the three pillars of the “Active Healthy Ageing” framework: Health, Participation, Security. Methods The PdA sample is randomly selected from the LHU lists, stratified by sex and age. 22,811 records have been collected in 2016-2017 on a representative sample of residents aged over65, not being hospitalised, in long-term care, in a nursing home or prison. Results 3,019 respondents resulted cancer-diagnosed elderly (CDE) reporting cancer diagnosis or confirmation, leukaemia and lymphomas included (annual average prevalence of 12.8%, Confidence Intervals 95%: 12.1-13.4%; one million and 729 thousand people). CDE refer: poorer health conditions (22%) than Chronic Elderly Patients (CEP; 18%) and Healthy Elderly (HE; 5%), sight impairment (12%), hearing loss (15%) and chewing problems (14%). CDE (10%) and CEP (11%) are more prone to fall than HE do (6%). Disability accounts for 21% in CDE and CEP vs. 12% in HE. A multivariate analysis confirms a compromised psychophysical health profile in CDE vs. HE, and vs. CEP. CDE behave still unhealthily, are insufficiently advised by health professionals, do not comply with the seasonal flu vaccination [Health]. CDE face increased risk for isolation or cognitive decline: they experience lower social connectedness (17%) than CEP (20%) and HE (28%) [Participation], and perceive higher neighbourhood insecurity (19%) than how CEP (17%) and HE (13%) do [Security]. Conclusions PdA data on the poor biopsychosocial health profile of CDE highlight to what extent healthy behaviours and prevention can ameliorate their quality of life PdA calls for global action strategies in Italy, which aim at taking complete charge of CDE and also CEP. Key messages In Italy, elderly cancer survivors show poor biopsychosocial health profile and overall quality of life. By modifying behaviours, elderly suffering from any cancer or chronic disease can age healthily.
