Purpose: Aphasia in a dextral after right hemisphere injury is called crossed aphasia (CA). We are reporting a first case of transformation of motor aphasia to conduction aphasia after right hemisphere intracerebral hemorrhage (ICH) associated with arteriovenous malformation (AVM) with literature reviews.Methods: A case of a man in his 30s with CA following right hemisphere ICH in the temporal-parietal lobe associated with AVM was reviewed. We analyzed his brain images, initial linguistic characteristics, and changes in aphasia for 8 weeks of follow-up.Results: The initial Paradise Korean Western Aphasia Battery Revised (PK-WAB-R) was evaluated as aphasia quotient (AQ) 72, 64%ile; post evaluation was evaluated AQ 95, 98.9%ile after 8 weeks. The post-test repetition score was the patient score range, which can be attributed to impairment in phonological short-term memory. The patient is diagnosed anomalous CA based on Alexander et al., and we could predict that the language ability originates from both hemispheres based on Nagaraja et al. Considering the appearance of Gerstmann syndrome at the beginning of the onset, we could expect that the function of the dominant parietal lobe is partially crossed as well.Conclusion: Changes in aphasia were reported throughout the initial stage to the end of speech therapy. It is also important to note that literature review of Korean studies was analyzed in this study. It will be necessary to conduct a cognitive test in the early stage of onset to understand the language problems of crossed conduction aphasia to know the characteristics of the cognitive process.

Conduction aphasia is a syndrome characterized by impaired repetition in the context of relatively preserved auditory comprehension and fluent speech. The classical conceptualization of conduction aphasia as a disconnection syndrome has been undermined in recent years. Nevertheless, this diagnosis delineates a small subset of individuals with aphasia who have many common cognitive and anatomical characteristics. Conduction aphasia is associated with damage to a relatively narrow and well-defined group of left hemisphere brain structures, which may include the posterior superior temporal lobe, the inferior parietal lobe, and the insula. According to current cognitive neuropsychological frameworks, an impairment in phonological planning for speech production is the common underlying cognitive dysfunction in the majority of cases, which may sometimes be accompanied by an analogous impairment in receptive phonology. Other common features, such as sentence repetition problems and reduced short-term memory span, may be a secondary consequence of the primary phonological impairment. Current approaches to the treatment of conduction aphasia target the underlying impairment in phonological planning. It is argued that the diagnosis of conduction aphasia can be a useful first step toward understanding a person’s language difficulties and planning effective treatment interventions.

Thailand has been affected by COVID-19, like other countries in the Asian region at an early stage, and the first case was reported as early as mid-January 2020. Thailand’s response to the COVID-19 pandemic has been guided by the “Integrated Plan for Multilateral Cooperation for Safety and Mitigation of COVID-19”. This paper analyses the health resources in the country and focuses on the response through community-level public health system and legislative measures. The paper draws some lessons on future preparedness, especially with respect to the four priorities of Sendai Framework for Disaster Risk Reduction. At the end, the paper puts some key learning for future preparedness. While Thailand’s response to COVID-19 has been effective in limiting the spread of the disease, it falls short at being able to address the multiple dimensions of the crisis such as the economic and social impacts. The socioeconomic sectors have been hardest hit, with significant impact on tourism sectors. Sociopolitical system also plays an important role in governance and decision-making for pandemic responses. The analysis suggests that one opportunity for enhancing resilience in Thailand is to strive for more multilevel governance that engages with various stakeholders and to support grassroots and community-level networks. The COVID-19 pandemic recovery is a chance to recover better while leaving no one behind. An inclusive long-term recovery plan for the various impacted countries needs to take a holistic approach to address existing gaps and work towards a sustainable society. Furthering the Health Emergency Disaster Risk Management (HEDRM) Framework may support a coordinated response across various linked sectors rather than straining one particular sector.

Lateropulsion (pusher syndrome) is an important barrier to standing and gait after stroke. Although several studies have attempted to elucidate the relationship between brain lesions and lateropulsion, the effects of specific brain lesions on the development of lateropulsion remain unclear. Thus, the present study investigated the effects of stroke lesion location and size on lateropulsion in right hemisphere stroke patients. The present retrospective cross-sectional observational study assessed 50 right hemisphere stroke patients. Lateropulsion was diagnosed and evaluated using the Scale for Contraversive Pushing (SCP). Voxel-based lesion symptom mapping (VLSM) analysis with 3T-MRI was used to identify the culprit lesion for SCP. We also performed VLSM controlling for lesion volume as a nuisance covariate, in a multivariate model that also controlled for other factors contributing to pusher behavior. VLSM, combined with statistical non-parametric mapping (SnPM), identified the specific region with SCP. Lesion size was associated with lateropulsion. The precentral gyrus, postcentral gyrus, inferior frontal gyrus, insula and subgyral parietal lobe of the right hemisphere seemed to be associated with the lateropulsion; however, after adjusting for lesion volume as a nuisance covariate, no lesion areas were associated with the SCP scores. The size of the right hemisphere lesion was the only factor most strongly associated with lateropulsion in patients with stroke. These results may be useful for planning rehabilitation strategies of restoring vertical posture and understanding the pathophysiology of lateropulsion in stroke patients.

Abstract Objective Longitudinal assessment of cognitive and emotional functioning in patients with Parkinson’s disease (PD) is helpful in tracking progression of the disease, developing treatment plans, evaluating outcomes, and educating patients and families. Determining whether change over time is meaningful in neurodegenerative conditions, such as PD, can be difficult as repeat assessment of neuropsychological functioning is impacted by factors outside of cognitive change. Regression-based prediction formulas are one method by which clinicians and researchers can determine whether an observed change is meaningful. The purpose of the current study was to develop and validate regression-based prediction models of cognitive and emotional test scores for participants with early-stage idiopathic PD and healthy controls (HC) enrolled in the Parkinson’s Progression Markers Initiative (PPMI). Methods Participants with de novo PD and HC were identified retrospectively from the PPMI archival database. Data from baseline testing and 12-month follow-up were utilized in this study. In total, 688 total participants were included in the present study (NPD = 508; NHC = 185). Subjects from both groups were randomly divided into development (70%) and validation (30%) subsets. Results Early-stage idiopathic PD patients and healthy controls were similar at baseline. Regression-based models were developed for all cognitive and self-report mood measures within both populations. Within the validation subset, the predicted and observed cognitive test scores did not significantly differ, except for semantic fluency. Conclusions The prediction models can serve as useful tools for researchers and clinicians to study clinically meaningful cognitive and mood change over time in PD.

Recently, subways have become an important part of public transportation and have developed rapidly in China. In the subway station setting, pedestrians mainly rely on visual short-term memory to obtain information on how to travel. This research aimed to explore the short-term memory capacities and the difference in short-term memory for different information for Chinese passengers regarding subway signs. Previous research has shown that people’s general short-term memory capacity is approximately four objects and that, the more complex the information, the lower people’s memory capacity. However, research on the short-term memory characteristics of pedestrians for subway signs is scarce. Hence, based on the STM theory and using 32 subway signs as stimuli, we recruited 120 subjects to conduct a cognitive test. The results showed that passengers had a different memory accuracy for different types of information in the signs. They were more accurate regarding line number and arrow, followed by location/text information, logos, and orientation. Meanwhile, information type, quantity, and complexity had significant effects on pedestrians’ short-term memory capacity. Finally, according to our results that outline the characteristics of short-term memory for subway signs, we put forward some suggestions for subway signs. The findings will be effective in helping designers and managers improve the quality of subway station services as well as promoting the development of pedestrian traffic in such a setting.

Various investigations into the effects of brain injury on psychological test performance (Weisenburg and McBride, 1935; Patterson and Zangwill, 1944; Anderson, 1951; McFie and Piercy, 1952; Bauer and Becka, 1954; Milner, 1954) suggest the overall conclusion that patients with left hemisphere lesions are relatively poor at verbal tasks, while those with right-sided lesions do worst at practical tasks, particularly the manipulation of spatial or spatio-temporal relationships. Heilbfun's (1956) study confirmed that verbal deficits result from left-sided lesions but his left and right hemisphere groups produced almost identical scores on spatial tests. In so far as these workers paid attention to the specific sites of the lesions, their findings indicate that the pattern of test performance is a function of the hemisphere in which the lesion occurs rather than of its specific locus.

Alzheimer’s disease (AD) is the most common cause of dementia in the population. Late onset AD has a classic clinical picture with short-term memory deficit, apraxia and agnosia. Patients with early-onset AD may have an atypical clinical picture which complicates diagnosis. Atypical AD variants include the logopenic variant of primary progressive aphasia, posterior cortical atrophy, behavioral, biparietal, and cortico-basal variants. These variants have pathomorphological signs similar to classical AD, but at an early stage they are characterized by focal atrophy which explains their clinical polymorphism. This article provides a review of the current literature on atypical types of AD and presents a clinical case of a 62-year-old patient in whom the disease debuted with prosopagnosia due to focal atrophy of the temporo-occipital regions of the non-dominant hemisphere.

The goal of this project was to investigate the genetic heritability of hippocampal volume using twin pairs and assess the neuroanatomical structures of the hippocampus and how these properties relate to memory in humans. Data for this project was obtained from the Human Connectome Project: a data bank established to provide neural images to the public. MRI scans were used to obtain brain images of each of the participants and basic cognitive tasks were used to obtain memory ability. To date, 506 subjects have been analyzed: 66 monozygotic twin pairs, 44 dizygotic twin pairs, and 47 sibling pairs. The data collection for of this project was three-fold. First, segmentations were performed to calculate the volume of the anterior and posterior regions of the hippocampus. Secondly, the magnitudes of hippocampus dentations were recorded within the three segments – the head, body, and tail – of the hippocampus. Lastly, visual inspection was used to asses incomplete inversions, which was defined as an atypical anatomical pattern in the hippocampus. The results of this project showed a strong heritability observed on the right anterior hippocampus (hb2=1.365) and right amygdala (hb2=1.315), moderate heritability observed on the left posterior hippocampus (hb2=0.765), and weak heritability observed on the right posterior hippocampus (hb2=0.2654). This indicates that hippocampal volumetric heritability showed strong genetic control for the right hemisphere and strong environmental control for the left hemisphere. The project is still in the process of correlating the anatomical structures to the memory capabilities of the participants.

Background: Due to an ageing demographic and rapid increase of cognitive impairment and dementia, combined with potential disease-modifying drugs and other interventions in the pipeline, there is a need for the development of accurate, accessible and efficient cognitive screening instruments, focused on early-stage detection of neurodegenerative disorders. Objective: In this proof of concept report, we examine the validity of a newly developed digital cognitive test, the Geras Solutions Cognitive Test (GCST) and compare its accuracy against the Montreal Cognitive Assessment (MoCA). Methods: 106 patients, referred to the memory clinic, Karolinska University Hospital, due to memory complaints were included. All patients were assessed for presence of neurodegenerative disorder in accordance with standard investigative procedures. 66% were diagnosed with subjective cognitive impairment (SCI), 25% with mild cognitive impairment (MCI) and 9% fulfilled criteria for dementia. All patients were administered both MoCA and GSCT. Descriptive statistics and specificity, sensitivity and ROC curves were established for both test. Results: Mean score differed significantly between all diagnostic subgroups for both GSCT and MoCA (p<0.05). GSCT total test time differed significantly between all diagnostic subgroups (p<0.05). Overall, MoCA showed a sensitivity of 0.88 and specificity of 0.54 at a cut-off of <=26 while GSCT displayed 0.91 and 0.55 in sensitivity and specificity respectively at a cut-off of <=45. Conclusion: This report suggests that GSCT is a viable cognitive screening instrument for both MCI and dementia.
