This is the second of two articles exploring assessment and clinical reasoning of conditions relating to the heart and cardiovascular system in the context of emergency care. In the last article, the structure and function of the heart was reviewed, and reference made to many of the conditions that may affect the heart. In addition, the common presenting complaints of cardiac conditions were highlighted, together with important aspects of the history for each symptom. The full cardiac examination was outlined. In this article, some of the common cardiac conditions will be discussed. These will be linked to common findings in the history, examination, and investigations.

This article aims to increase knowledge of cardiac assessment. Anatomy and physiology of the heart are briefly reviewed and reference is made to pathology that can cause cardiac dysfunction. The main features to look for when taking a cardiac history are discussed, with suggestions for questions to elicit this information, and the signs to look for when undertaking a cardiac examination. There is also an introduction to the main investigations to aid differential diagnosis and clinical reasoning. A follow-up article will look in more detail at some common cardiac conditions presenting to emergency care, with an emphasis on critical thinking and diagnostic reasoning. These articles are written from an emergency care perspective, and therefore do not focus in great detail on invasive investigation of cardiology conditions, but more so on picking up these possibilities in undifferentiated patients presenting to emergency care.

Community detection is important for understanding the structure and function of networks. Resistance distance is a kind of distance function inherent in the network itself, which has important applications in many fields. In this paper, we propose a novel community detection algorithm based on resistance distance and similarity. First, we propose the node similarity, which is based on the common nodes and resistance distance. Then, we define the distance function between nodes by similarity. Furthermore, we calculate the distance between communities by using the distance between nodes. Finally, we detect the community structure in the network according to the nearest-neighbor nodes being in the same community. Experimental results on artificial networks and real-world networks show that the proposed algorithm can effectively detect the community structures in complex networks.

This study aims to determine the implementation of inquiry methods that can improve students' critical thinking skills on the concept of structure and function of plant tissues at the Sekolah Keberbakatan Olah Raga Negeri Aceh (Aceh State Sports School). The population of this research is the many 112 students of class XI of the Sekolah Keberbakatan Olah Raga Negeri Aceh. The sample in this study were 26 students in class XI-1 as an experimental class and 26 students in class XI-2 as a control class. The method used is a descriptive method with a structural approach. Data collection is done by test techniques and data processing using the t-test formula. The results of the data analysis showed that the average ability or average value of class XI-1 students of the Sekolah Keberbakatan Olah Raga Negeri Aceh in taking the test received 80.7. While the average value of class XI-2 students of the Sekolah Keberbakatan Olah Raga Negeri Aceh in taking the test gets 70. Based on the price of t-counts and t-tables at a significant level of 0.05 with db: 50 of the sample class XI, then t- the count is 6.40 and the t-table is 1.66. So that the hypothesis proposed the implementation of inquiry methods can improve students' critical thinking skills on the concept of the structure and function of plant tissues in the Sekolah Keberbakatan Olah Raga Negeri Aceh is Accepted. It is recommended that this research can increase knowledge through the use of inquiry methods in all biological science subject matter.

In the field of education, the presence of technology has helped teachers a lot in carrying out learning. Future education cannot only focus on cognitive aspects. To face various problems and challenges of the times in the future, students are expected to have much better attitudes, skills, and knowledge competencies, to be more creative, innovative, and productive. The purpose of this research is to analyze the feasibility and effectiveness of the development e-module Biology based on Contextual Teaching and Learning (CTL) on the material structure and function of plant and animal tissues to improve the critical thinking skills of students in class XI MIPA SMA that are valid, practical, and effective. The results showed that the e-module Biology based on Contextual Teaching and Learning (CTL) on the structure and function of plant and animal tissues to improve critical thinking skills of students in class XI MIPA SMA has a very valid category with a validator value of 89.33%, very practical 93.33% by Biology teachers and students with a score of 87.52% and the results of the effectiveness test of the CTL - based e-module showed that it was very effective, there was a significant difference between classes that used CTL - based e-modules and classes that did not use CTL - based e-modules . So, it can be concluded that the CTL-based Biology e-module that has been developed is very valid, very practical, and very effective.

Objective. Identification of the features of the cardiovascular system damage in chronic obstructive pulmonary disease (COPD) combined with arterial hypertension (AH) in workers exposed to toxic gases (TG). Materials and methods. 132 house painters and paint production workers of the Novosibirsk aviation enterprise were included in 3 strata and examined: I-COPD and TG; II-COPD in combination with AH when exposed to TG; III COPD in combination with AH without exposure to TG. Employees of all strata were divided by length of service less than 10 and more than 10 years. The selection of workers in groups was carried out by a continuous method. Results. The most pronounced changes on the ECG, regardless of the length of service, were observed in workers with COPD in combination with hypertension and contact with TG (levogram and metabolic changes in the myocardium). When exposed to TG for more than 10 years, hypertrophy of the left parts of the heart was also detected. Among workers with COPD in combination with hypertension exposed to TG, the indicators of average pulmonary artery pressure (APAP) and pulmonary vascular resistance (PVR) elevated with increasing work experience; changes in the structure and function of the right and left parts of the heart and signs of remodeling of large vessels were also revealed. The same workers showed more pronounced changes in the daily blood pressure profile (DBPP) of AP with the prevalence of Non-dippers and Night-pickers. Conclusions. Among workers suffering from comorbid pathology, with increasing duration of TG exposure over 10 years, there were observed the most pronounced ECG changes (deviation of electrical axis to the left, metabolic changes in the myocardium, hypertrophy of the left heart); increased APAP and PVR 1.21 and 1.10 times; changes in the structure and function of the right and left chambers of the heart, remodeling of large vessels; increase in the frequency of variants of the DBPP of Non-dippers and Night-pickers.
