Introduction Compassionate and respectful care is a pillar for improving health-seeking behaviour. It has given much attention globally, following the concerns that healthcare often falls significantly; however, less research emphasis was paid in the last decade. Therefore, this study aims to assess compassionate and respectful care and associated factors among outpatient clients at public health facilities in Northwest Ethiopia, 2020. Methods A facility-based quantitative cross-sectional study supplemented with the qualitative findings was conducted from 30 February to 30 March 2020. A semi-structured interviewer-administered questionnaire was used to collect the quantitative data among 593 participants. Systematic and purposive sampling techniques were used to select the quantitative and qualitative participants, respectively. A semi-structured interview guide was also employed for the qualitative data collection. Epi-Data version 4.6 and SPSS version 25 were used for data entry and analysis, respectively. The bi-variable and multivariable logistic regression model was fitted to identify the factors associated with each outcome variable (compassionate and respectful care separately). Adjusted odds ratio (AOR) with 95% confidence interval (CI) and p-value less than 0.05 were used to declare the strength and factors statistically associated with the outcome variables. Results Overall, 72.8% and 82.6% of the respondents experienced compassionate and respectful care, respectively. Patients attending primary or above education (AOR: 0.35; 95% CI: 0.21–0.59), attending at the hospital (AOR: 0.59; 95% CI: 0.39–0.88), new clients (AOR: 0.33; 95% CI: 0.16–0.70) and service users who had three or more visits (AOR: 0.34; 95% CI: 0.17–0.71) were negatively associated with compassionate care. On the other hand, female patients (AOR = 0.53; 95% CI: 0.32–0.87), aged over 36 years (AOR = 0.43; 95% CI: 0.20–0.90), primary or above school attended clients (AOR = 0.18; 95% CI: 0.09–0.36), waiting two or more hours (AOR = 0.28; 95% CI: 0.13–0.62), and use public or private transport access (AOR: 0.49; 95% CI: 0.29–0.83) were negatively associated with a respectful care. Conclusion Compassionate and respectful care provided to the outpatient clients in public health facilities of northwest Ethiopia was high. However, our result suggests that immediate actions are necessary to address respectful and compassionate care at hospitals, and hospital management should adopt mitigation measures. Consideration should be given to foster patient-centric services and educate the health care workers about compassionate and respectful care.

Abstract Background: Ante natal care is a key entry point for a pregnant woman to receive abroad range of promotion and preventive health services. Objective: was to assess the quality of antenatal care services at public health facilities of western Ethiopia. Methods: Facility-based cross-sectional study was conducted from May 30th to June 30th, 2016. All public health facilities in the zone were audited, 316 medical records were reviewed, and 316 pregnant women were interviewed. The data was entered using EPI Data version 3.1 and analyzed using SPSS version 20.0. Descriptive statistics, binary, and multiple logistic regressions were conducted. Variables with a P-value of <0.05 were considered as statistically significant predictors of the outcome variable.Results: A total of 316 pregnant women were enrolled in the study. All facilities were categorized as “good” by the possession of necessary equipment, 3/4 by basic amenities and 87.34% by general and gynecologic examination. The information was provided for 222(86.21%), which is categorized as poor.252 (79.7%) of the women were satisfied with ANC. A urine sample taken during ANC visit AOR 3.36(1.70, 6.61) and counseling on food eaten during pregnancy AOR 2.27(1.16, 4.45) were predictors of client satisfaction on ANC. Conclusions: The majority of pregnant women were satisfied with the ANC they received. A urine sample taken during the ANC visits and being counseled on the types of food eaten during pregnancy were predictors for client satisfaction on ANC. Concerned bodies need to improve laboratory tests and information provision.

Abstract Abstract Background: Implanon discontinuation is unacceptably high in developing countries, including Ethiopia; furthermore there is an observed problem of high unintended pregnancy rate after method discontinuation this might stride to program failure. Therefore, the purpose of this study was to assess the level and determinants of Implanon discontinuation among women who used Implanon in Adabet district, public health facilities, North-West Ethiopia, 2017. Methods: Facility-based cross-sectional study was conducted among 537 women, from Feb.03 to April 28, 2017, by face to face interview. Systematic random sampling technique was used to select the study subjects. The collected data were entered into Epi Info- version 7 then exported to SPSS version 20 for analysis. Both descriptive and analytical statistical analysis was computed. On multi-variable binary logistics regression, p-value and odds ratio (AOR) with 95%Ci was used to showing statistical association with the outcome variable. Results: In this study, 36.9% of Implanon users were discontinued the method before the intended time period. Among those women who discontinue the method 85.9% of them were discontinued before two years of Implanon insertion. Women who had no live child at the time of Implanon insertion[AOR=2.17,95%CI:1.25-3.77], didn’t received pre-insertion counseling on potential side effects [AOR=1.85,95%CI: 1.15-2.97], developed side effect secondary to Implanon insertion [AOR=5.17,95%CI:3.18-8.40], received appointment follow-up [AOR=0.23,95%CI:0.13-0.41], and not satisfied by the service provided [AOR=5.40,95%CI:3.04-9.57] were statistically associated with Implanon discontinuation. Conclusions: level of Implanon discontinuation before its intended period was high. Hence, to increase Implanon continuation rate; provide pre-insertion counseling including its possible side effects, improve client’s service satisfaction and strength appointment follow-up for Implanon users should be made.

Abstract Background Institutional delivery is one of the important strategies to reduce the maternal related risk at the time of delivery. Satisfaction of women on labor and delivery care services has good influence on their health and results in subsequent utilization of health services. This study was focused to assess women’s satisfaction and its’ associated factors with institution delivery services in public health institutions.Methods Cross- sectional study was conducted among 169 participants from June 2018 to November 2018 among women (having under one year children) residing in Tanahun district. Simple random sampling, face to face interview and semi structured interview schedule was used for the collection of data. Data were entered in Epi-Data and analyzed by SPSS. Ethical approval was obtained from Institutional Review Committee at Pokhara University and maintained during the process of research.Results The age of the participants were between 16 to 40 years with median age 25 years (IQR = 7 Years). Most (93.5%) of the participants were Hindu. Majority of the participants (57.4%) and their husbands (60.4%) had secondary level education. Almost all pregnancies (99.4%) are planned and normal delivery was most common (77.5%). The study shows that 55% of the women performed delivery in public health facilities. The study states high satisfaction score in health status of women after delivery (4.8) and lowest in availability of visitor's bed (2.99). Women's satisfaction and religion was seen to be significantly associated.Conclusion Majority of the participants were satisfied with the services provided by public health facilities. Although, number of birthing centers were increasing, facilities in the health institution is not sufficient as required. Different factors such as cleanliness of delivery room, availability of staffs, medicine, visitor's bed and behavior of staff are some issues to be improved.

ObjectivesThe study was aimed: (1) to describe the quality of antenatal care (ANC) at public health facilities in Northwest Ethiopia, including dimensions of the structure, process and outcome; and (2) to assess the relationship between ANC satisfaction and structure and process dimension of ANC quality.DesignCross sectional.SettingHealthcare facilities providing ANC services in Northwest Ethiopia.Participants795 pregnant women attending the antenatal clinics at 15 public health facilities and 41 health workers working for the surveyed facilities.Outcome measuresThe outcome variable, women’s satisfaction with ANC, was constructed from multiple satisfaction items using principal component analysis on an ordered, categorical and three-point Likert scale. The key hypothesised factors considered were structural and process aspects of care. Data were analysed using the partial proportional odds model with 95% CI.ResultsThe result revealed that only 30.3% of the pregnant women were highly satisfied, whereas 31.7% had a lower satisfaction level. The findings showed that process quality indicators better predicted client satisfaction. In relation to this, better scores in history taking (aOR1=aOR2; 1.81 (95% CI 1.25 to 2.60)), counselling (aOR1 = aOR2; 1.89 (95% CI 1.33 to 2.69)) and screening (aOR1= aOR2; 18.10 (95% CI 11.52 to 28.39)) were associated with achieving higher satisfaction. We also observed a significant but lower satisfaction among women in the late trimester of pregnancy (aOR1 = aOR2; 0.87 (95% CI 0.78 to 0.97)). However, we did not see any significant relationship between structural variables and client satisfaction.ConclusionsThe study demonstrated that women’s satisfaction with ANC was low. The contents of ANC services covered during client–provider interaction were the main factors affecting client satisfaction. This suggests that efforts are required to improve the competencies of health professionals to make them more effective while dealing with clients.
