Background Thalassemia and hemoglobinopathies are themost common inherited disorders in many areas of the world,including South East Asia. The siblings of thalassemia major is agroup of high risk to carry the gene of thalassemia. Determiningthe carrier is useful for early treatment planning and preventionto the next child.Objective To determine carrier status among siblings ofthalassemia patients using a capillary electrophoresis system.Methods A cross-sectional study on the siblings of thalassemiamajor patients was performed from January 2011 to February2012 at Dr. Moewardi Hospital. Complete blood counts wereperformed in the siblings. Subjects with mean corpuscular volume(MCV) <80 fl and mean corpuscular hemoglobin (MCH) <27pg were subjected to analize hemoglobin fraction by capillaryelectrophoresis.Results Of the 26 subjects, there were 12 males and 14 females.The mean age was 9.38 (SD 6.8) years (range 1 to 29 years). Fromthe siblings, 10 were identified as normal, 5 were identified as ßthalassemia carriers and 5 were hemoglobin E (HbE) carriers. Sixsiblings were diagnosed with ß thalassemia/ HbE.Conclusion There are high occurrence of the two common typesof thalassemia carriers (ß and HbE) in our small group of subjectswho had a family history of thalassemia. Most of the siblingsof thalassemia had low MCV and MCH.

Objective: To evaluate endocrine profile in transfusion dependent beta-thalassemia major patients. Study Design: Descriptive cross sectional study. Setting: Department of Pathology Liaquat University of Medical and Health Sciences, Jamshoro /Hyderabad and Thalassemia Centre Hyderabad. Period: 6 months from January 2018 to June 2018. Material & Methods: Cases already diagnosed with β- thalassemia major with recurrent blood transfusions (>20 transfusions or transfusion period over 2 years) both genders were included. All the patients went through routine laboratory checkups including Testosterone hormone, Luteinizing Hormone, and Follicle Stimulating Hormone. Data was recorded in preplanned proforma. Results: Overall 114 subjects were studied and their mean age was found to be 12.38+5.71 years. Females remained in majority as 56.1%. Mean of follicle stimulate hormone (FSH) and luteinizing hormone (LH) level were 4.60+4.88 and 5.39+8.39 respectively. Most of the patients, 35.1% had >300 of transfusion rate. Ferritin levels were significantly correlated with patients with history of transfusion rate >300; p-value 0.003. Testosterone level’s mean was considerably high among patients presented with transfusion rate of 201 to 300 and >300 transfusions; p-value 0.006. Likewise, mean of FSH and LH level was as well significantly high among the female subjects with history of transfusion rate of 201 to 300 and >300 transfusions; P-Values 0.019 and 0.026 respectively. Conclusion: Frequent hypogonadism was noted among thalassemia patients. Very low level average of FSH, LH and testosterone was seen.

Abstract Background Data on peripheral blood cell values in older subjects are rare. While hemoglobin (Hb) values are supposed to change with rising age, little is known about reference values for other erythrocytic blood cell counts. This cross-sectional study was initiated to analyze hematologic laboratory parameters among subjects aged ≥60 years. Methods This was a retrospective cross-sectional study of outpatient laboratory data between January 1st and December 31st, 2015 originating from a German countrywide laboratory group; inclusion criteria: age ≥60 years, normal C-reactive protein (CRP), transferrin saturation, reticulocytes, lactate dehydrogenase, haptoglobin and soluble transferrin receptor; exclusion criteria: glomerular filtration rate (GFR)<60 mL/min, lack of inclusion criteria; primary objective: assessment of the mean Hb value; secondary objective: assessment of mean values of red blood cell (RBC) counts. Results Of 30,611 subjects ≥60 years, 4641 met the inclusion criteria and were thus considered hematologically healthy; the following age groups were formed: 60–69 years (2094), 70–79 years (2171), 80–89 years (360), >90 years (16); median values for male/female subjects were: Hb 15.2/14.0 g/dL, RBC 5.0/4.6/μL, mean cellular volume (MCV) 89/89/fl, mean corpuscular hemoglobin (MCH) 31/30 pg/RBC, mean corpuscular hemoglobin concentration (MCHC) 34/34 g/dL, hematocrit (hct) 44/41%. Statistical evaluation revealed a slight but significant decrease in values over age decades for all parameters except for MCH. However, all values remained within the recommended German Society of Hematology and Oncology (DGHO) reference ranges. Hb values remained above the recommended World Health Organization (WHO) cut-offs for definition of anemia. Conclusions The results confirm the WHO reference values and are in accordance with the recommended DGHO reference values and previous results of other study cohorts outside Germany. There seems to be no need for establishing age-specific RBC or erythrocytic reference ranges for subjects >60 years.

  Objective: Workers in the silver jewelry manufacturing units are exposed to heavy metals and toxic compounds during manufacturing. Hence, the present study aimed to assess the hematological indices among the workers in silver jewelry manufacturing units.Methods: A cross-sectional study was conducted in various silver jewelry manufacturing units located in Coimbatore, Tamil Nadu. One hundred and forty eight exposed and 35 unexposed (control) participants were included in this study. Their hematological indices were assessed.Results: There was significant (p<0.05) increase of serum silver (Ag) and hematological profile such as red blood cell (RBC), hematocrit, and the hemoglobin levels of exposure groups (B, C, D, and E) were found to be significantly decreased (p<0.05) compared to control group (A). There was also significant increase (p<0.05) in differential count and mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), platelet (PLT), platelet distribution width (PDW), and mean platelet volume (MPV) levels in exposed groups (B, C, D, and E) than controls (A).Conclusion: Decrease in RBC indices and the increased differential count of MCH, MCHC, PLT, PDW, and MPV levels might be due to the exposure to Ag in the jewelry units. In view of these results, it appears that ionized Ag induces hematological disturbances and the present study clearly establish that there were minimal negative effects of Ag exposure on hematological indices.

Introduction Thalassemia is an inherited hemoglobinopathy with high prevalence and incidence in Indonesia. It leads to health, psychosocial, and economic burdens that affect patients, caregivers, and the country. As treatments for thalassemia in Indonesia remain expensive, not readily available, or associated with poor compliance, thalassemia prevention through screening programs is highly recommended to reduce the number of new cases. It is best to target thalassemia prevention and education to youth, but baseline data regarding their knowledge, attitude, and practice must first be assessed to measure their current awareness level as well as behavioral patterns regarding thalassemia. Currently, there has been limited research on public perception towards thalassemia in Indonesia. Methods and analysis This observational, cross-sectional study will recruit at least 500 participants between the age of 15 to 24 across all provinces in Indonesia. This is the first nationwide thalassemia study that explores knowledge, attitude, and practice among Indonesian youth (15 to 24 years old), including thalassemia major patients, carriers, unaffected individuals, and individuals with unknown carrier status. A questionnaire will be disseminated online through social media. The questionnaire will consist of 28 questions to assess knowledge, attitude, and practice from the general population and 38 questions to assess knowledge, attitude, and practice specifically from thalassemia major patients. Questions about whether thalassemia is perceived as a curse, role of consanguinity on the mode of inheritance, and willingness to undergo screening are incorporated to specifically suit Indonesian sociocultural settings. Ethics and dissemination This study has been approved by the Ethical Committee of Faculty of Medicine, Universitas Indonesia, and Cipto Mangunkusumo Hospital. Informed consent will be obtained from all participants before completing the online questionnaire. Results will be published in a relevant journal and scientific meetings as well as shared with local stakeholders and policymakers. Study registration This study has been prospectively registered at ClinicalTrials.gov (13 January 2021; NCT04706585; https://clinicaltrials.gov/ct2/show/NCT04706585).

Abstract. Background: Medical students are at high risk of suicidal ideation. Aim: We aimed to obtain information on suicidal ideation among medical students in Dammam located in the Eastern Province of Saudi Arabia. Method: This cross-sectional study was conducted at the College of Medicine affiliated with Imam Abdulrahman Bin Faisal University in the Eastern Province of Saudi Arabia. Suicidal ideation in the past 12 months was assessed based on responses to four questions in the depression subscale of the General Health Questionnaire 28 (GHQ-28). In addition, data were collected to examine the association of suicidal ideation with various factors. Results: We found that 1 in 3 medical students in the study had suicidal ideation in the past 12 months, while around 40% had lifetime suicidal ideation. Suicidal ideation was associated with feelings of parental neglect, history of physical abuse, and dissatisfaction with academic performance. Limitations: The cross-sectional nature of this study limits its ability to determine causality regarding suicidal ideation. Conclusion: These rates are considerably high when compared with rates from studies in other countries around the world. This study provides a reference in the field of suicidology for this region of Saudi Arabia.

Background: Thyroid hormones influence nearly all major metabolic pathways. Their most obvious and well-known action is the increase in basal energy expenditure obtained by acting on protein, carbohydrate and lipid metabolism. The lipid metabolism is more influenced by the thyroid hormone. Methods: A cross-sectional study was conducted on 100 patients with suspicion of thyroid disorders were taken as cases. One hundred patients with normal thyroid profile and no history of other chronic diseases were taken as control group. Results: The serum TC, TG and LDL levels in hypothyroid individuals (both overt and subclinical) were significantly higher than euthyroid subjects but the levels were comparable between hyperthyroid and euthyroid group. Conclusion: Dyslipidemias are associated with thyroid disorders, so biochemical screening for thyroid dysfunction in all dyslipidemic patients. Therefore, patients presenting with dyslipidemia are recommended for investigation to explore thyroid dysfunction. Keywords: Thyroid profile, Total cholesterol, Triglycerides and LDL

Introduction: The global prevalence of metabolic syndrome (MS) among people living with HIV/AIDS varies from 20% to 33%. Objective: to estimate the prevalence of metabolic syndrome and associated factors in a group of HIV-infected patients on antiretroviral therapy. Methods: This is a cross-sectional study with HIV-infected patients from a reference center in Bahia, Brazil. We evaluated clinical, socio-demographic and anthropometric data. MS was defined according to the guidelines of International Diabetes Federation. Results: We evaluated 152 patients with mean age of 47.3±11.6 years, 59.2% male. The main comorbidities detected were diabetes (3.3%) hypertriglyceridemia (9.3%) and metabolic syndrome (MS,38.2%). Patients with MS were predominantly women (55.2% vs 31.9%; p=0.005), older [52.1 (10.4) vs 44.3 (11.3); p<0.001], and had overweight (74.1% vs 23.4%; p<0.001). After multivariate analysis MS remained associated with age (OR = 1.076; 95% CI: 1.030 – 1.125), female sex (OR = 2.452; 95% CI: 1.114 – 5.374) and family history of hypertension (OR = 3.678; 95% CI: 1.431 – 9.395). Conclusion: Almost half of the HIV-infected patients in Bahia presents with MS which seems to be driven by classical risk factors.

Background: According to the new classification criteria developed by The Assessment of SpondyloArthritis International Society, patients with axial spondyloarthritis (axSpA) can be classified in 2 subgroups: Patients with radiographic axial spondyloarthritis: ankylosing spondylitis patients (AS) and those with non-radiographic axial spondyloarthritis (nr-axSpA). Objective: The aim of the present study is to describe and discuss the differences and similarities between the two subgroups. Patients and Methods: A cross-sectional study was conducted in a single rheumatology hospital in Morocco. These included patients diagnosed as having axial spondyloarthritis according to ASAS criteria 2010, during a period of 6 years. The AS and the nr-axSpA subgroups were compared for the various axSpA-related variables. Results: Of the 277 patients with a diagnosis of axial SpA who were included in this study, 160 had AS and 117 had nr-axSpA. AS and nr-ax-SpA shared a similar age at diagnosis, similar prevalence of low back pain, lumbar stiffness, extra-articular manifestations, BASDAI and BASFI. In the multivariate analysis, AS patients were mainly male with cervical stiffness, enthesitis, coxitis and high level of ESR (erythrocyte sedimentation rate). The females generally had a family history of SpA and arthritis and were associated to the nr-axSpA form in the univariate analysis. Conclusion: This was the first study to characterise patients with AS and nr-axSpA in Morocco. Consistent with other studies published, this study showed that patients with nr-axSpA and patients with AS shared a comparable degree of disease burden.

Abstract Background The rising incidence of infections caused by MDR organisms (MDROs) poses a significant public health threat. However, little has been reported regarding community MDRO carriage in low- and middle-income countries. Methods We conducted a cross-sectional study in Siem Reap, Cambodia comparing hospital-associated households, in which an index child (age: 2–14 years) had been hospitalized for at least 48 h in the preceding 2–4 weeks, with matched community households on the same street, in which no other child had a recent history of hospitalization. Participants were interviewed using a survey questionnaire and tested for carriage of MRSA, ESBL-producing Enterobacterales (ESBL-E) and carbapenemase-producing Enterobacterales (CPE) by culture followed by antibiotic susceptibility testing. We used logistic regression analysis to analyse associations between collected variables and MDRO carriage. Results Forty-two pairs of households including 376 participants with 376 nasal swabs and 290 stool specimens were included in final analysis. MRSA was isolated from 26 specimens (6.9%). ESBL-producing Escherichia coli was detected in 269 specimens (92.8%) whereas ESBL-producing Klebsiella pneumoniae was isolated from 128 specimens (44.1%), of which 123 (42.4%) were co-colonized with ESBL-producing E. coli. Six (2.1%) specimens tested positive for CPE (4 E. coli and 2 K. pneumoniae). The prevalence ratios for MRSA, ESBL-producing E. coli and ESBL-producing K. pneumoniae carriage did not differ significantly in hospital-associated households and hospitalized children compared with their counterparts. Conclusions The high prevalence of ESBL-E across both household types suggests that MDRO reservoirs are common in the community. Ongoing genomic analyses will help to understand the epidemiology and course of MDRO spread.
