Aim. The paper aims to evaluate the effectiveness of respiratory training as part of a comprehensive spa and medical rehabilitation. Materials and methods. Studies were performed in 90 patients with bronchial asthma and 60 patients with chronic cerebral ischemia. During physical training, New Breath breathing simulators were used. The effectiveness of medical rehabilitation was evaluated according to the severity of impaired body functions related to the corresponding domains of the International Classification of Functioning, Disability and Health. Results. Under rehabilitation, the condition of patients in both groups improved significantly, systemic rehabilitation effects in the form of improved indicators of the blood system and its oxygen transport functions, as well as increased reserves for handling stress and other psychological demands were notified. Conclusion. The study suggests the inclusion of physical exercises with the New Breath simulator in the comprehensive rehabilitation of patients with chronic cerebral ischemia and obstructive diseases of the respiratory system.

Objective. The aim of the study was to analyze cognitive functions, emotional disorders, the quality of life with the application of the International Classification of Functioning (ICF) in patients with chronic cerebrovascular diseases, dyscirculatory (vascular) encephalopathy II stage in the Komi Republic. Design and methods. In the clinics of the Komi Republic, we examined 126 people (mean age — 65,8 ± 10,1 years; women comprised 76 participants, 60,3 %, p ≤ 0,05) with dyscirculatory (vascular) encephalopathy. Complaints and anamnesis were analyzed, somatic and neurological status, cognitive functions, emotional status, and sleep were assessed. Results. Out of 126 patients, 44,5 % patients are employed (56 people). Among non-working patients with an established group of disability, 35,7 % (in 14,3 % disability 2nd degree (10 people), in 21,4 % — disability 3rd degree (15 people), p ≤ 0,05). Among the workers, 3 people (5,3%) had disability 3rd degree. Hypertension (HTN) I stage was detected in 10,6 %, HTN II stage — in 44,4 %, HTN III stage — in 45 % (p ≤ 0,05); 27,8 % had history of ischemic heart disease, 7,2 % — myocardial infarction, 19,1% — cerebrovascular accident, 8,6% — heart rhythm disturbances, 15,8 % — significant stenosis of the brachiocephalic arteries. Based on memory function assessment by ICF, none demonstrated normal results. Mild cognitive impairment was usually found, and there was no case of dementia. Dysfunction in the domain of blood pressure function was found in all patients. The dysfunction of blood vessels was found in all patients with chronic cerebral ischemia II stage. Daily activities were reduced due to the physical dysfunction. According to the total assessment of health status by the SF-36 scale, the physical and mental components of health were similarly decreased. Conclusions. The SF-36 (questionnaire for assessing the quality of life) and ICF (International Classification of Functioning, Disabilities and Health) complement each other. Cognitive scales are the basic tools for examination of patients with chronic cerebral ischemia, making diagnosis according to ICF, and treating cognitive, physical and emotional disorders in dyscirculatory (vascular) encephalopathy.

The objective was to determine the most significant categories of ICF that allow us to provide objective information about the biopsychosocial state of patients who have suffered a new coronavirus infection COVID-19 and to choose the most effective methods of rehabilitation based on the rehabilitation diagnosis of each group of patients.Methods and materials. On the basis of the clinic of the Pavlov University, a course of medical rehabilitation was conducted for 2 weeks for patients who had suffered a new coronavirus infection in severe and moderate degree of course. 2 groups were formed: EG1-25 people participating in the rehabilitation program with the use of physical exercises and EG2 — 39 people with the use of physical exercises and physiotherapy. A categorical rehabilitation profile of patients by ICF domains was compiled.Results. As a result of the application of the medical rehabilitation program, a statistically significant improvement in the indicators in EG 2 was revealed. The respiratory function (the Barbell test) improved by 14.6%, the functional state of the cardiovascular system according to the DBP indicator by 3.9%. The analysis of the results of the ICF category «activity and participation» revealed a statistically significant improvement in the motor test parameters (6MSHT) in both groups: by 11.9 % in EG2 and by 20.6 % in EG1.Conclusions. The results of the study indicate the positive impact of the proposed program of medical rehabilitation and confirm the relevance of the problem, unsolved problems and prospects for scientific research on this topic.

The purpose of the study. To study clinical and neuroimaging features of chronic cerebral ischemia (CCI) in polycythemia vera (PV).Materials and methods. 66 patients with PV were examined – the main group (43 men, 23 women; mean age 62.0 ± 3.4 years), of which 64 (97.0%) patients were diagnosed with CCI. The comparison group consisted of 85 patients with CCI (34 men, 51 women; mean age 67.7 ± 4.6 years), who developed against the background of cerebral vascular atherosclerosis and arterial hypertension. To identify cognitive disorders, we used Mini Mental State Examination (MMSE). Insomnia was studied in accordance with the criteria of the International Classification of Sleep ICDS‑22005. The quality of sleep was determined using a questionnaire from the Federal Somnological Center. Neuroimaging (MRI of the brain) was performed on Siemens Symphony 1.5 T and GE Signa 1.5 T tomographs.Results. Subjective symptoms CCI are characterized by a greater representation of asthenic and insomniac disorders. Transient ischemic attacks in patients with PV are significantly more common than in the comparison group, their frequency depends on the duration of PV. The revealed changes in MRI of the brain in the majority of PV patients with CCI are characteristic of multiinfarction vascular encephalopathy; in the comparison group, changes that characteristic for subcortical arteriosclerotic encephalopathy were more often recorded.

This perspective article provides an example of a study planned using guidelines for comprehensive rehabilitation outcomes research, an approach that is believed to give service providers meaningful evidence to support practice. This line of investigation has been guided by the World Health Organization's International Classification of Functioning, Disability and Health. The short title of a study under way is Move & PLAY (Movement and Participation in Life Activities of Young Children). The article briefly describes the conceptual model, provides guidelines on how indicators and measures are selected, alludes to the details of selected measures, and describes processes of preparing for data collection, including obtaining ethics approval, preparing data collection booklets, training assessors and interviewers, and sampling. The aim of this investigation is to gain a better understanding of the multiple child, family, and service factors associated with changes in mobility, self-care, and play of preschool children with cerebral palsy as a result of using this research method. Comprehensive rehabilitation outcomes research holds promise in providing evidence that supports the complexities of planning rehabilitation services with clients with chronic conditions, such as children with cerebral palsy.

The aim of the study was to evaluate the effectiveness of respiratory muscles training in patients with chronic cerebral ischemia at thehealth resort stage of medical rehabilitation. Materials and methods. An open randomized controlled clinical trial was conducted in 60 patients with chronic cerebral ischemia,who underwent a course of sanatorium-resort rehabilitation treatment on the southern coast of Crimea (Yalta, Republic of Crimea). Men - 11.0%, women - 89.0%, age - 62.3 ± 2.3 years, disease duration - 4.5 ± 1.2 years. In control group 1 (n = 30), climatotherapy, physical training (morning exercises, remedial gymnastics, health path), massage, hardware physiotherapy and medications were carried out. In the main group 2 (n = 30), a similar basic complex was used, but therapeutic exercises were carried out with a breathing simulator and included walking, exercises for diaphragm training, general motor skills, coordination, dexterity, mobility, and joint flexibility. Number 10–12. Before and after the treatment, clinical, laboratory, biochemical, functional (spirography, electrocardiography, fitness test), psychological (Reeder and Back tests) studies were carried out; the indicators of the quality of life were studied; the state of 23 selected domains of the “International classification of functioning, disabilities and health”, included in the section “Body functions”, was assessed. Results. During the sanatorium-resort stage of medical rehabilitation, significant systemic effects were noted in the form of a decrease in clinical manifestations of the disease, an improvement in physical performance indicators and an increase in reserves for overcoming psychological stress. In patients, there was an improvement in static-coordination and psycho emotional state, reduction of dizziness and cephalgia, fatigue, increase in exercise tolerance, general physical endurance, normalization of carbohydrate metabolism indicators and blood lipids.Conclusion. A method of physical rehabilitation based on the principle of external respiration modification by using a breathing trainer with vibration function has been developed. The study allows us to recommend the inclusion in the complex of medical rehabilitation of patients with chronic cerebral ischemia, physical exercises with a breathing simulator, focused on increasing the functional body reserves.

The purpose of the work is to identify the dependence of the effectiveness of medical rehabilitation (MR) of patients with cerebral atherosclerosis (CA) on the seasons at the Southern coast of Crimea (SCC). Materials and methods. A prospective, single-center clinical trial in 216 patients with CA. Assessment of the effectiveness of MR was carried out in accordance with the "International Classification of Functioning, Disability and Health" (ICF). Results. As a result of MR, the level of violations of all controlled functions (on average for all ICF domains) was reduced by 28-35% in different seasons of the year, which indicates the high efficiency of the year-round integrated health resort MR at the SCC. Findings. The effectiveness of MR in patients with CA is quite high in all seasons of the year, which testifies to the validity of indications for referring patients to the SCC health resorts throughout the year.

The purpose of study was to elaborate methodology of assessment of effectiveness of a medical rehabilitation in patients with the bronchial asthma with use of the International Classification of Functioning, Disability and Health (ICF). Under observation there were 420 patients with bronchial asthma who received sanatorium treatment on the Southern coast of the Crimea. All patients received the clinical, laboratory and functional trial. For assessment of a condition of patients according to provisions of ICF, a set of domains was developed for patients with bronchial asthma. The health resort medical rehabilitation in the climatic resort leads to reliable increase of asthma control. Changes of continuants of the offered domains before carrying out sanatorium medical rehabilitation had reliable positive dynamics. A proposed methodology can be used for assessment of functional condition of patients with bronchial asthma under health resort medical rehabilitation.

The creation of a unified system, which will collect various tools for assessing the patient's condition, will provide serious assistance in the work of members of a multidisciplinary team in the process of medical rehabilitation. This will help in the formulation of rehabilitation diagnosis and in more accurate determination of rehabilitation potential. The dimension of a number of generally accepted scales to describe disorders of the musculoskeletal system does not coincide with the categories of ICF. The paper attempts to eliminate this contradiction. When describing a number of disorders that affect the function of the musculoskeletal system and are traditionally used in orthopedics, the category other or not described can be used. In the presented system of evaluation are given equal-weighted scale with the dimension adopted in the ICF. This system can be used as a basis for the preparation of medical rehabilitation programs and evaluation of their effectiveness.
