AbstractAtherosclerosis and chemokines are strongly related, but the role of the chemokine CXCL17 in atherogenesis is still poorly understood. We aim to investigate the serum CXCL17 levels in different stages of patients with coronary heart disease and explore whether these differences contribute to atherosclerosis. In the current prospective study, we enrolled 48 patients with unstable angina (UA), 51 patients with stable angina (SA) and 41 patients for the control group (CG). All subjects were diagnosed by coronary angiography and Gensini score was used to evaluate the severity of coronary artery disease. The CXCL17 levels were determined using ELISA, while lipid metabolism indicators and high sensitivity C-reactive protein (hs-CRP) were detected by automatic biochemical analyzer. We observed that the unstable angina group had higher CXCL17 levels compared with the stable angina and the control group. The logistic regression analysis showed that CXCL17 was an independent risk factor for unstable angina. Our results showed that CXCL17 was also statistically correlated with hs-CRP, while it was irrelevant with Gensini score. CXCL17 levels were associated with activity of inflammatory response and the instability of atherosclerotic plaques. These results suggest that CXCL17 elevation may be a potential new biomarker of unstable angina.

Objectives. There are limited clinical data revealing the relationship between mitral annular calcification (MAC) and systemic inflammation. The goal of the present study was to compare high-sensitivity C-reactive protein (hs-CRP) levels in patients with and without MAC and investigate the relationship between MAC and hs-CRP.Methods. One hundred patients with MAC who underwent transthoracic echocardiography (TTE) and 100 age-matched controls without MAC who underwent TTE were included in our study. Hs-CRP levels were compared between groups.Results. Prevalence of female gender, hypertension, and coronary artery disease were significantly higher in the MAC group than in the control group (64% versus 45%,P=0.007, 42% versus 28%,P=0.03and 37% versus 18%,P=0.003, resp.). On multivariate analysis, age, gender, and coronary artery disease were the only independent predictors of MAC. The levels of hs-CRP were higher in the MAC group than in the control group (2.02±0.35versus1.43±0.47 mg/dl,P<0.001). This increase in hs-CRP levels in the MAC group persisted in patients without hypertension, coronary artery disease, and in male patients when compared to the control group.Conclusions. Our study demonstrated that hs-CRP, which is a sensitive marker of systemic inflammation, increased in patients with MAC.

Abstract Background It has been reported that coronary artery disease (CAD) is characterized by inflammation and non-obstructive CAD (NOCAD) increases the risk of cardiovascular events (CVEs) compared with ones with normal or near-normal coronary arteries (NNCA), even is similar to obstructive CAD (OCAD). We hypothesized that elevated high-sensitivity C-reactive protein (hs-CRP) may be linked to CVEs in those patients with NOCAD. Purpose To investigate the predictive role of hs-CRP in patients with NOCAD. Methods Of 7,746 consecutive patients with angina-like chest pain admissions, 4,662 eligible patients were enrolled who received coronary artery angiography (CAG) and followed up for the CVEs comprising all-cause mortality, myocardial infarction, stroke and late revascularization. According to the results of CAG, the patients were classified as NNCA group (<20% stenosis, n=698, 15.0%), NOCAD group (20–49% stenosis, n=639, 14.3%), and OCAD group (≥50% stenosis, n=3325, 70.7%). They were further subdivided into 3 groups according to baseline hs-CRP levels (<1, 1–3 and >3 mg/L). Proportional hazards models were used to assess the risk of CVEs in all patients enrolled. Results A total of 338 patients (7.3%) experienced CVEs during an average of 13403 person-years follow-up. Patients with NOCAD and OCAD had higher rates of CVEs compared to those with NNCA (p<0.05, respectively). In Cox's models after adjustment of confounders, the risk of CVEs elevated with the increasing degrees of CAD with hazard ratio of 2.01 [95% confidence interval (95% CI): 1.07–3.79, p=0.03] for patients with NOCAD and 2.81 (95% CI: 1.60–4.93, p<0.001) for patients with OCAD compared with the NNCA group. Moreover, elevated hs-CRP levels were associated with the severity of coronary lesions and an elevated increased risk of CVEs in patients with NOCAD and OCAD compared those with NNCA (p<0.05, respectively). Conclusions Patients with NOCAD had indeed worse outcomes and hs-CRP levels were positively in relation to the CVEs in those with NOCAD, which may help to the risk assessment in ones with NOCAD. Although advanced diagnostic and treatment methods are available, congenital heart disease (CHD) holds an important place among the causes of death within the first year of age. Therefore, several prognostic factors are needed for diagnosis and monitoring of these patients. In this study, which includes 66 CHD patients and 38 healthy control children, serum cardiac troponin-I (cTnI), high-sensitivity C-reactive protein (Hs-CRP), and N-terminal prohormone brain-type natriuretic peptide (NT-proBNP) levels were analyzed for their prognostics values. The patient groups were categorized and then evaluated as cyanotic ( n = 16), acyanotic ( n = 50), symptomatic ( n = 23), asymptomatic ( n = 43), and isolated ventricular septal defect (VSD)-isolated atrial septal defect (ASD) groups. Cyanotic group was statistically compared with acyanotic group, symptomatic group with asymptomatic group, and VSD group with ASD group. Between the cyanotic, acyanotic, and control groups; between symptomatic and asymptomatic groups; and between the VSD and ASD groups, significant difference was not showed for age ( p > 0.05). NT-proBNP was found to be significantly higher in the cyanotic group than acyanotic and control group, in the symptomatic group than asymptomatic group; and in the patient group than healthy control group ( p < 0.05). Between the groups of VSD and ASD, significant difference was not showed ( p > 0.05). The same comparison results for TnI and Hs-CRP were not significant ( p > 0.05). TnI and Hs-CRP were only found significantly higher in the patient group than healthy control group ( p < 0.05). Eventually, we think that NT-proBNP, Hs-CRP, and TnI might be used for clinical management and estimation of outcome of these disorders in the future and these also might be able to modify existing strategies, but much more studies are needed.

BACKGROUND & OBJECTIVES: Inflammatory processes play an important role in intra-articular fractures. The present study aimed to examine the relationship between chemerin, high-sensitivity C-reactive protein (hs-CRP), and Interleukin 17 (IL-17) serum and synovial fluid levels of osteoarthritis patients and individuals with intra-articular fractures.METHOD: In this case-control study, all osteoarthritis patients and individuals with intra-articular fractures who visited the Imam Khomeini Orthopedic Clinic of Ahvaz were examined. Blood samples (5 cc) were collected prior to surgery to measure chemerin Interleukin 17, and hs-CRP serum levels. Synovial fluid samples (2 cc) were collected during the surgery.RESULTS: Measuring the levels of IL-17, chemerin and hs-CRP indicated a significant statistical difference between the serum and synovial fluids of osteoarthritis patients, individuals with intra-articular fractures, and the control group (p < .001). Post-hoc analyses showed statistically significant differences in all conditions except for hs-CRP levels between osteoarthritis patients and individuals with intra-articular fractures.CONCLUSION: Discovering ways to stop or slow down osteoarthritis is a matter of great concern. The findings on osteoarthritis indicate diverse, complex, and multidimensional processes involving cytokines. Information on cytokines that effect diseases can help develop efficient therapy methods.

AimsHigh-sensitivity troponin T (hs-TnT) and high-sensitivity C reactive protein (hs-CRP) may convey prognostic information in patients with aortic stenosis (AS). This study evaluated if hs-TnT and hs-CRP associate with myocardial mass, and risk of future surgery for AS.MethodsIn total, 336 patients (48% women) with surgery for AS with previous participation in large population surveys were identified. Preoperatively, myocardial mass and the presence of coronary artery disease (CAD) were assessed. Two matched referents were allocated for each case, and hs-TnT and hs-CRP were determined in stored plasma from the baseline survey. Conditional logistic regression analysis was used to estimate the risk (OR (95% CI)) related to one (natural logarithm) SD increase in hs-TnT and hs-CRP. Kaplan-Mayer and Cox regression analyses were used to evaluate time to surgery.ResultsMedian age (IQR) was 59.8 (10.3) years at survey, and median time between survey and surgery was 10.9 (9.3) years. Hs-TnT was independently associated with surgery for AS (1.24 (1.06–1.44)) irrespective of CAD, whereas Hs-CRP was not (1.05 (0.90–1.22)). Elevated hs-TnT levels at survey associated with shorter time to surgery (p<0.001), and with increased myocardial mass (p=0.002). Hs-CRP did not associate with time to surgery or with myocardial mass.ConclusionsHs-TnT—but not hs-CRP—was associated with increased risk of—and shorter time to—future surgery for AS. Hs-TnT associated with myocardial mass at surgery which indicates that hs-TnT could be a potential biomarker for determining intervention.

Introduction The correlation between inflammation and vascular disease is widely accepted. High levels of C-reactive protein (CRP) have been shown to play a role in the process of endothelial dysfunction. Hypertension is described as an inflammatory vascular disease, and is 1 of the most commonly encountered diseases in the outpatient setting. We studied the association between the elevated high sensitivity-CRP (hs-CRP) level and hypertension, as well as other comorbid conditions. Methods Electronic medical records of 169 adult patients in our internal medicine office were reviewed for hs-CRP levels, and divided into 2 groups: elevated hs-CRP (≥2 mg/L; n = 110) and normal hs-CRP (<2 mg/L; n = 59). Independent T-Test was used to compare the means of continuous variables between the groups if they were normally distributed. Mann Whitney U-Test was used to compare the continuous variables that were non-parametric. Logistic regression was used to compare the dependent and independent variables. Results Among subjects with elevated hs-CRP, 58.2% had hypertension while 47.5% of subjects with normal hs-CRP levels had hypertension ( P = .182). There were higher frequencies of association of coronary artery disease (CAD), cerebrovascular disease and hypothyroidism in elevated hs-CRP group but the differences were not statistically significant. Mean white blood cell count was statistically higher in elevated hs-CRP group ( P < .05), while alcohol use was significantly higher ( P < .05) and statin use was higher in the normal hs-CRP group. There was an inverse relationship between HDL-C and hs-CRP. Conclusions There was no statistically significant correlation between hs-CRP level and hypertension. Hs-CRP has statistically significant associations between alcohol use, dementia, white blood cell count, and HDL levels. Promising but not statistically significant correlations were observed between hs-CRP and statin therapy, hypothyroidism, coronary artery disease, and cerebrovascular disease.

Aim. To conduct the prospective study assessing the biochemical laboratory parameters in patients with stable angina and hemodynamically significant coronary artery stenosis before and 12 months after transluminal balloon-catheter angioplasty. Methods. 40 patients (males and females), mean age 57,3±9,6 years with stable angina and chronic coronary artery disease were examined. Patients were divided into 2 groups based on the absence (1st group, 14 patients) or presence (2st group, 26 patients) of hemodynamically significant coronary artery stenosis on selective coronary angiography. Patients with hemodynamically significant coronary artery stenosis underwent percutaneous transluminal coronary angioplasty (PTCA) with stenting (drug-coated stents). Blood lipid profile, inflammatory markers and endothelial dysfunction markers were measured before the surgery and 12 months after treatment. Results. In patients with hemodynamically significant coronary artery stenosis statistically significant association of hemodynamically significant coronary artery stenosis presence with atherogenic lipid fractions levels, inflammatory markers (high-sensitivity C-reactive protein, tumor necrosis factor-alpha) and an endothelial function parameter (endothelin-1) were revealed. Long-term results of PTCA with stenting included the significant decrease of atherogenic and increase of anti-atherogenic fractions in lipid profile. There also was a prolonged inflammatory response characterized by high-sensitivity C-reactive protein (hs-CRP) level elevation, and the endothelin system dysfunction retaining. Conclusion. Persistent hs-CRP level increase as well as endothelin-1 level increase suggest a high risk of late stent thrombosis, it should be taken into account while following-up patients at the late stages after PTCA.

Abstract BackgroundPrevious studies have shown that leukocyte cell-derived chemotaxin 2 (LECT2), a hepatokine, is associated with obesity and non-alcoholic fatty liver disease (NAFLD). It is well known that hepatokines play important roles in mediating interactions among bone, adipose tissue, and liver. We sought to examine serum LECT2 levels in subjects with osteoporosis (OP) to confirm its association with OP.MethodsFrom March 2019 to March 2020, a total of 96 adult subjects (52 OP patients and 44 controls) visiting the 2nd Spine Department of the Affiliated Hospital of School of Medicine of Ningbo University were recruited. The bone mineral density (BMD) of all subjects were assessed by dual-energy X-ray (DXA). Blood samples were collected for measurements of high-sensitivity C-reactive protein (hs-CRP), plasma glucose (PG), alanine aminotransferase (ALT), aspartate aminotransferase (AST), total cholesterol (TC), triglycerides (TG), creatinine and uric acid. Serum LECT2 levels of total 96 participants were measured by enzyme linked immunesorbent assay (ELISA). The relationships between serum LECT2 levels and biomedical parameters were analyzed using the Spearman correlation coefficient.ResultsSerum LECT2 levels in OP patients were significantly higher than that of healthy controls (29.57 ng/mL VS 19.82 ng/mL, P < 0.01). To confirm the role LECT2 played in OP, we found a significantly negative correlation in all subjects between serum levels of LECT2 and lumbar BMD, as well as femoralneck BMD. A significantly positive correlation in all was observed between serum levels of LECT2 and TC, whereas there was a significantly negative correlation between serum levels of LECT2 and creatinine. Meanwhile, serum LECT2 levels were measured to diagnose OP patient by plotting receiver-operating characteristic (ROC) curve, the area under the ROC curve was 0.729(P < 0.01). The optimal cutoff point for LECT2 concentration to diagnose OP patient was 16.44 ng/mL.ConclusionsWe showed that serum LECT2 levels were significantly up-regulated in OP patients, and LECT2 levels were significant positively associated with total cholesterol and negatively associated with creatinine. It could be a potential biomarker for OP diagnosis.

C-Reactive Protein is a marker of inflammation and vascular disease. The high serum of high-sensitivity C-Reactive Protein (hs-CRP) concentration was associated with interruption the integrity of the frontal-subcortical circuit. To determine the correlation between high serum concentration of hs-CRP and executive function disorder in elderly patients. Thirty six elderly patients was examined by using a case control study. The subjects were enrolled from the out-patient at Neurology Department Dr. Soetomo General Hospital, Surabaya, Indonesia. The time period was 4 months (April to July 2015). Executive function was measured with the Trail Making Test B (TMT B) and divided into two groups, case and control. Complete blood count and serum hs-CRP concentration were measured from nonfasting blood samples. Chi-square test was used for data analysis. Eighteen subjects were enrolled in the case group (3 men, 15 women) and the other 18 subjects were enrolled in the control group (4 men, 14 women). Executive function disorder was found in 17 (94,4%) subjects of case group and 11 (61,1%) subjects of control group. High levels of hs-CRP was not associated with executive function, with p = 0,149 and adjusted OR 5,629 (CI 95% 0, 538-58, 860). These data suggested that high levels of hs-CRP was not associated with executive function in elderly patients.
