There are a variety of barriers to eye-care service access in rural Northern First Nations communities. Semi-structured, opened-ended key informant interviews were conducted on the topic of eye care, with eight First Nations individuals employed by the health office in a small Northern British Columbian First Nations community. Data analysis comprised identifying themes by analyzing similarities and dissimilarities in participants’ narratives, including comparing and contrasting viewpoints of participants and placing themes within broader sociocultural and historic contexts. Themes identified in the data included the current state of community eye care, facilitators and barriers to accessing eye care, and community needs and preferences. The theme of “facilitators and barriers” was further analyzed, resulting in subthemes of awareness, attitudes, social, economic, and service related. Better understanding of the barriers and their interactions would provide a foundation upon which innovative eye-care programs might be developed.

ObjectiveThe purpose of this study was to evaluate the stratification of follow-up and referral pathways after implementation of a systematic cloud-based electronic-referral teleophthalmological service for optometry-initiated ocular posterior segment disease referrals to the Danish national eye care system. Methods and AnalysisA retrospective cohort study was conducted in the period from 1 August 2018 to 31 July 2019. Patients with suspected ocular posterior segment disease reviewed by the telemedical ophthalmology service were included. The service stratified patients into the categories: no need for follow-up, follow-up by optometrist, follow-up by the telemedical service and referral to the national Danish eye care service.ResultsFrom a pool of 386 361 customers, 9938 patients were enrolled into this study. 19.5% of all patients were referred to the Danish national eye care system, while 80.5% of the patients in the telemedical service were not, in the period from 1 August 2018 to 31 July 2019. 14.4% of the optometrist referrals did not need any follow-up, while a majority of 66.1% needed some follow-up either by the optometrist themselves or within the telemedical service.ConclusionOptometrist posterior segment disease referrals can be considerably reduced with a risk stratified approach and optimal use of technology. New models can improve and streamline the healthcare system.

Most people prefer to age in place. However, there is a growing body of literature to suggest a reluctance to accept supportive services in the home due to concerns about trust, privacy, cost, and fear of being a burden. The purpose of this study was to examine potential facilitators and barriers to accepting home care services in the website content of Licensed Home Care Service Agencies (LHSCA). In this linguistic analysis study, the written content from 88 randomly selected LHCSA websites was examined. We used LIWC2015 and Microsoft® Word software to analyze websites for relevant word categories that reflect older adult identified facilitators and barriers to the acceptance of home care services. Results revealed that the summary score for clout (i.e., confidence and leadership reflected in the writing) was high. Some of the most commonly used word categories were positive emotions, present focused, and affiliation. The word category money was included, but to a lesser degree. However, Burden and related words were highly prevalent in the writing sample. In summary, LHCSA website content contains both facilitators and barriers to the acceptance of home care services. Given the importance of home care services in promoting the ability of older adults to age in place, greater attention may be needed regarding the way services are presented and advertised to consumers.

The Platygastroidea Planetary Biodiversity Inventory is a large-scale, multinational effort to significantly advance the taxonomy and systematics of one group of parasitoid wasps. Based on this effort, there are some clear steps that should be taken to increase the efficiency and throughput of the taxonomic process. Increased collaboration among taxonomic specialists can significantly shorten the timeline and add increased rigor to the development of hypotheses of characters and taxa. Species delimitations should make use of multiple data sources, thus providing more nearly independent tests of these hypotheses. Taxonomy should fully embrace electronic media and informatics tools. Particularly, this step requires the development and widespread implementation of community data standards. The barriers to progress in these areas are not technological, but are primarily social. The community needs to see clear evidence of the value added through these changes in procedures and insist upon their use as standard practice.
