An attempt was made to assess the women contribution towards agriculture in Vavuniya district, Sri Lanka. 60 farm family households' women were randomly selected from rural and urban area of Kovilkulam AI region of Vavuniya district in Sri Lanka and the data were collected by constructed questionnaire. The objectives of this study are to identify the factors contributing women participation in agriculture, to identify the constraints faced by the women in participating agriculture and to evaluate the women participation in decision making activity in agriculture. Minitab 15 and MS excel were used for data analysis. The level of women participation in agricultural activities was found out using chi-square test and the factors contributing for women participation in agricultural activities were identified using multiple regression analysis in urban and rural areas separately (α=0.05). The results revealed that 90% of the rural women respondents and 50% of the urban respondents participated in the agricultural activities which is a significant difference. The mean values of women participation in agricultural activities in urban and rural areas were 77 hours and 836 hours per annum respectively. The multiple regression model for women participation in urban area found that the participation of women negatively correlated with status of employment, age and education level (p=0.000). In rural area, age and educational level were negatively influencing on women participation in agricultural activities (p=0.000). R-square values of fitted regression models were 72 % and 91% in urban and rural area respectively 72% and 91% variation in respective women participation were explained by these models. The obstacles for the women in participation in agricultural activities were reported as lack of knowledge and training in agriculture field, family burden, cultural and social barriers and physical constraints. 60% of women from rural areas and 90% of women from urban areas were involved in decision making especially in the selection of crops and varieties for planting and livestock rearing. Enhancing the awareness and the technical knowledge to the women in the field of agriculture would contribute to increase income from agriculture at household level, district level and finally at national level. Int. IntroductionUncontrolled hypertension is the leading risk factor for mortality globally, including low-income and middle-income countries (LMICs). However, pathways for seeking hypertension care and patients’ experience with the utilisation of health services for hypertension in LMICs are not well understood.ObjectivesThis study aimed to explore patients’ perspectives on different dimensions of accessibility and availability of healthcare for the management of uncontrolled hypertension in Sri Lanka.SettingPrimary care in rural areas in Sri Lanka.Participants20 patients with hypertension were purposively sampled from an ongoing study of Control of Blood Pressure and Risk Attenuation in rural Bangladesh, Pakistan, Sri Lanka.MethodWe conducted in-depth interviews with patients. Interviews were audio-recorded and transcribed into local language (Sinhala) and translated to English. Thematic analysis was used and patient pathways on their experiences accessing care from government and private clinics are mapped out.ResultsOverall, most patients alluded to the fact that their hypertension was diagnosed accidentally in an unrelated visit to a healthcare provider and revealed lack of adherence and consuming alternatives as barriers to control hypertension. Referring to the theme ‘Accessibility and availability of hypertension care’, patients complained of distance to the hospitals, long waiting time and shortage of medicine supplies at government clinics as the main barriers to accessing health services. They often resorted to private physicians and paid out of pocket when they experienced acute symptoms attributable to hypertension. Considering the theme ‘Approachability and ability to perceive’, the majority of patients mentioned increasing public awareness, training healthcare professionals for effective communication as areas of improvement. Under the theme ‘Appropriateness and ability to engage’, few patients were aware of the names or purpose of their medications and reportedly missed doses frequently. Reminders from family members were considered a major facilitator to adherence to antihypertensive medications. Patients welcomed the idea of outreach services for hypertension and health education closer to home in the theme ‘Things the patients reported to improve the system’.ConclusionPatients identified several barriers to accessing hypertension care in Sri Lanka. Measures recommended improving hypertension management in Sri Lanka including public education on hypertension, better communication between healthcare professionals and patients, and efforts to improve access and understanding of antihypertensive medications.Trial registration numberNCT02657746.

AbstractSri Lanka is an island country located in the Indian Ocean, characterized by tropical weather, with an estimated population of 21.4 million in 2017, one third of them being children. Advancing and rapidly changing technology, novel industries, consumer goods and equipment have introduced new health hazards of a chemical, biological and physical nature to future generations. The infant mortality rate has declined to around 8–9 per 100,000 since 2015. Congenital malformations were the most significant cause of neonatal death. Thus, there might be unproven environmental factors which affect infant mortality rates despite robust field and hospital services. Air pollution is a major environmental health hazard to children, largely due to hazardous industries, vehicular emissions and traditional cooking stoves. The health of children living in urban households and attending schools in cities is adversely affected by air pollution. Usage of polythene and plastic for toys, utensils, bags, lunch sheets and other daily essentials leads to overusage and improper waste management and open burning. This emits dioxins, which are carcinogens, hormone disruptors, and which can be transferred to the fetus via the placenta. In rural areas, the burning of agricultural waste and certain agro-forestry activities have also led to air pollution. Indoor air pollution (IAP) remains a large threat as 66% of the population use biomass for cooking. Use of inefficient stoves and biomass, poor ventilation and absence of chimneys, contribute to IAP. Other indoor air pollutants such as tobacco smoke, volatile organic compounds, asbestos, pesticides, kerosene, mercury, mosquito coils and biological pollutants also prevail. Biological pollutants, such as dust mites, droppings and urine from pets, insects and rodents, pollen from indoor plants and outdoor air, and fungi, including moulds in poorly maintained buildings, can trigger asthma or cause allergic reactions and infections among children inhabiting these environments. In rural areas, chemicals were accountable for 30.2% of acute poisonings, with kerosene oil being the most common cause, followed by paracetamol. More than 80% of agricultural workers in the country work in the informal sector with limited personal protective equipment use, while pesticide usage is increasing. Children who engage in agriculture-related work and hazardous industries are at risk of developing health consequences at early stages. Dengue remains a major environmental health problem as the reduction of mosquito breeding sites has been a challenging task for many years. It is essential to gain more evidence of existing environmental risk factors and their associations with the health of children. Information dissemination among local and international experts on Children’s Environmental Health (CEH) issues needs to be enhanced and properly established. Training programs should be conducted for healthcare workers to update their knowledge. Pediatricians are needed for remote hospitals and primary healthcare centers. Environmental parameters and biological parameters are not routinely monitored in most of the hazardous environments.

The primary problems concerning water resources in Sri Lanka are the depletion and degradation of the resource caused by various anthropogenic activities. Surface inland waters in urban areas are polluted heavily with domestic sewage and industrial effluents, and in rural areas with agricultural runoff. With regard to ground water in certain areas of the dry zone, there is a high fluoride content and in hard, rocky, alluvial areas, there is a high concentration of iron. In urban over-crowded cities, there is biological contamination of ground water. Over-utilization, particularly through tube wells, is another major problem affecting ground water resources in Sri Lanka. Oil spills, dumping of waste from ships, coral and sand mining, and industrial activities are the main causes of marine pollution in the country. Except for pipe-borne water supply, irrigation and hydropower schemes, in general water resources in Sri Lanka are managed very poorly. Regulations are available to control most water related problems but enforcement of these regulations is lacking. The ultimate result of degradation and depletion of water resources is the increasing health hazards. Water-borne and vector-borne diseases are prevalent, particularly amongst urban low-income communities with poor sanitary facilities and drainage. Despite government initiatives and legislation, very slow progress has been made towards combating water pollution. This paper examines the most significant water and wastewater related issues in Sri Lanka and their controlling mechanisms.

Sri Lanka now has a population nearing 13.5 million, the majority of whom are Sinhalese (72 per cent). The other major ethnic groups are the Tamils (27 per cent) and the Muslims (7 per cent). The religion of most of the Sinhalese people is Buddhism and that of the Tamils is Hinduism (67 per cent and 17 per cent of the total population respectively). Christianity (8 per cent) is the religion of a minority of Sinhalese and Tamil people. Free and compulsory education has resulted in a high literacy rate (84 per cent of those below 30 years and 80 per cent of those below 60 years of age). Eighty-seven per cent of the people are classified as living in rural areas (Census of Population, 1975).

In Sri Lanka, firewood is regarded as a common conventional fuel, mainly in food preparation purposes. Essentially due to economically availability, firewood has still been using as the main source to lit fire in rural areas. Other than for household food preparation, firewood is still the main source of generating heat in food related rural industries, such as tea factories, bakeries, hotels etc. In tea factories and in some of the bakeries, firewood is used in mass scales. Even tons of fire wood is used in each of them, per day. To make easy burning, the wood logs need to be cut in to small pieces. In almost all such places, wood splitting is done manually. It finds very difficult to split wood logs by using an axe. This needs time and more manpower. On the other hand, an additional cost is involved for workers’ payment. If the wood splitting can be mechanized, it may be huge beneficial for such industries as tea factories and bakeries etc. Therefore, this project is aimed to introduce a wood splitting machine, which has the specific objectives of efficient supply of usable level firewood at a shorter time with lower cost and without much manpower to split firewood logs. Lower maintenance requirement together with lower failure frequencies may be added advantages, when compared to the hydraulic devices [4].

We report on a field experiment using several methods for collecting deposits made in formal bank accounts in rural areas in Sri Lanka. We find that only frequent, face-to-face collection increases aggregate household savings. Collection involving community lock boxes increases balances at the collecting bank, but not overall household savings. Only community box collection appears to have the possibility of being financially viable. The various collection methods allow us to unbundle the role of frequency, salience and habit formation in deposit decisions. We find that frequency and salience affect the number of transactions, but not the level of savings.
