The article discusses the pathogenetic basis for the use of non-steroidal anti-inflammatory drugs (NSPVP) in obstetric practice for the treatment of pain syndrome in women with threatened abortion and pathological preliminary period. Provided with modern views on the mechanisms of analgesic clinical efficacy, side effects NSPVP. Provides information about the place of NSPVP during pregnancy, the risks to the fetus, the positive aspects in the conduct of women at risk of preterm birth, the pathological preliminary period. Key words: nonsteroidal anti-inflammatory drug, pain, premature birth, preliminary period.

Since indomethacin was first marketed, some 40 years ago, the class of nonsteroidal anti-inflammatory drugs has grown larger than any other drug class in history. At present, there are at least 25 such drugs being used in the US and abroad, both clinically and in research. Despite their widespread use, their implications to health care are just beginning to be understood. The authors review updated theories on the mechanism of action, side effects, and drug interactions of nonsteroidal anti-inflammatory drug therapy. Proposed guidelines for monitoring their use are given. A more thorough understanding of the risks-to-benefits ratio is provided in an effort to achieve maximum patient satisfaction and safety.

Chronic pain continues to remain one of the urgent problems of modern medicine. From 15 to 25% of the adult population suffers from chronic pain. Medical treatment includes the appointment of non-steroidal anti-inflammatory drugs (NSAIDs) and muscle relaxants. The greatestform for the appointment of NSAIDs is the topical form. According to the recommendations of International and National societies for the treatment of pain syndrome, osteoarthritis (OA) therapy are recommended to start with topical NSAIDs, as drugs with less systemic adverse side effects (NSAIDs).Topical NSAIDs have proven analgesic and antiinflammatory efficacy in the treatment of diseases of the musculoskeletal system, musculoskeletal pain, but have a low risk of developing systemic NSAIDs in comparison with oral forms, which expands the possibilities of their appointment in patients with comorbid pathology (diseases of the gastrointestinal tract, cardiovascular risks).Among NSAIDs, diclofenac is the "gold standard" of analgesia. After topical application, diclofenac penetrates through the skin and further into the deeper underlying tissues while maintaining sufficient concentration to provide a therapeutic effect. The topical form of diclofenac – diclofenac diethylamine 2%, when used correctly, can cause an analgesic effect comparable to the oral form. This topical form has a high clinical efficacy in the treatment of acute musculoskeletal pain (sprains), the course of therapy takes 1 week, for chronic pain syndromes (knee OA or hand) the course of therapy is from 2 to less than 6 weeks. The clinical efficacy of diclofenac diethylamine monotherapy is comparable to that for complex therapy in combination with oral forms of NSAIDs, while having good tolerability.

This study aims to determine the profile of self-medication sales of non-steroidal anti-inflammatory drugs at Mida Farma I Drugstore Gresik. It is conducted by using the observational method with data collection and observation on the Non-Steroid Anti-Inflammatory Drug (NSAID) stock cards in February 2020 at Mida Farma I Drugstore Gresik. The results show that the highest sales profile is the non-selective group of 87.68%, then the second is the COX-2 selective group of 12.32%. The most sold drug is ibuprofen of 20% and the lowest sold one is Aspirin of 14%. The pharmacokinetics of ibuprofen are it absorbs very quickly through the stomach; has a maximum plasma level that has reached 1 to 2 hours; and has low side effects

BACKGROUND: One of the harshest side effects following anticancer agent treatments is chemotherapy-induced neuropathic pain. After surgical staging, chemotherapy combination of paclitaxel carboplatin could be a choice of therapy for Stage II or more advanced stage of ovarian cancer. Different side effects may appear after the application of paclitaxel. CASE REPORT: Here, we show an uncommon case of paclitaxel-acute pain syndrome (P-APS), and how we deal with such cases according to our experiences. One uncommon side effect is P-APS, which can be treated effectively with the administration of non-steroidal anti-inflammatory drugs, corticosteroid, and supportive therapy. CONCLUSION: One uncommon side effect of Paclitaxel induced neuropathic can be treated effectively with the administration of non-steroidal anti inflammatory drugs, corticosteroid, and supportive therapy.

Transdermal nonsteroidal anti-inflammatory drugs (NSAIDs) are actively used for mild and moderate pain syndrome, muscle contusions and sprains, sports injuries, and the widest range of musculoskeletal diseases. The transdermal administration of NSAIDs aims to create sufficiently high drug concentrations in the lesion focus, provided that the side effects associated with its systemic action are maximally reduced.Objective: to comparatively simulate the effects of transdermal NSAIDs.Material and methods. Chemoreactome profiling of six NSAIDs (meloxicam, diclofenac, ibuprofen, ketoprofen, nimesulide, and dexketoprofen) was performed. The pharmacological capabilities of molecules were analyzed within the framework of a chemoreactome methodology, by comparing the chemical structure of the studied molecule with those of millions of other molecules, the pharmacological properties of which had already been studied in experimental and clinical studies. Training the artificial intelligence algorithms based on the big data available in in the databases PubChem/PHARMGKB, HMDB, STRING, and others was done with multi-level training quality control in the cross validation framework according to the combinatorial theory of solvability and the theory of feature value classification.Results and discussion. Meloxicam versus other NSAIDs accumulates primarily in the muscles and skin and, to a much lesser extent, in heart tissues, lymphocytes, gonads, and cartilage. This drug showed the greatest dose-dependent decongestant effect in the model of edema induced by croton oil. Analysis of the systemic effects of NSAIDs indicated that meloxicam might affect the metabolism of vitamins A, D, PP, and B6 to a lesser extent than other NSAIDs.Conclusion. The chemoreactomе analysis has demonstrated that meloxicam as a gel causing minimal side effects can be used effectively and long.

Presented classification and list registered in Ukraine representatives of this group of drugs. Posted current understanding of the mechanisms of clinical efficacy of NSAIDs and side effects. Presented the information about the place NSPVP during pregnancy, risks to the fetus, positive aspects in the management of women with threatened preterm labor, pathological preliminary period, as well as a basic component of postoperative pain management after obstetric and gynecological operations. Non-steroidal anti-inflammatory drugs (NSAIDs) are drugs that are difficult to replace in the treatment of inflammatory pain. Both the mechanisms involved in the development of inflammatory pain and the individual characteristics of each non-steroidal anti-inflammatory drugs currently allow for personalized drug selection in such a way as to escalate its effectiveness while minimizing the risk of side effects. Diclofenac is a non-steroidal anti-inflammatory drug with one of the longest clinical experience. Since the beginning of the 70s of the last century, it has become an important element of everyday rheumatological practice, used in both degenerative and autoimmune diseases.

The question of the prevention of pathologies of the labor is currently very important, the answer to them has become the favored formation of «dominants of delivery», choice of the optimal time to delivery, effective and safe method of induction, that promotes the promptness of the female organism and ripening of the cervix, as well as the timely identification and correction of pathological prelмmмnary period. In a review article describes the modern approaches and methods of preventing the pathology of labor activity, presented by the authors ‘ own experience regarding the use of prostaglandins, non-steroidal anti-inflammatory drugs, individual approach to the management of perinatal risk. Key words: childbirth, prevention, induction, prostaglandin, non-steroidal anti-inflammatory drugs.
