Occupying a leading position in the structure of general injuries, burn injuries in children remain an urgent issue of combustiology in particular and medicine in general. At the same time, many issues related to the organization of first aid to such patients still remain unregulated. To this end, a study aimed at conducting a comparative analysis of the clinical effectiveness of emergency care for children with burn injuries, including the use of the suggested algorithm, was conducted. Materials and methods. 153 cases of burn injuries with boiling water of IIab–III degree on the area of 10–40% of the body surface in children aged from 8 months to 16 years, who were treated at the Clinical Center for Thermal Injury and Plastic Surgery of the Municipal Non-profit Enterprise «Vinnytsia Regional Pirogov Clinical Hospital Vinnytsya Regional Council» in the period of 2015–2020 were analyzed. All patients underwent comprehensive diagnosis and treatment including surgery. Depending on the nature of first aid, children were divided into two groups: patients of the main group (n=97) underwent prompt neutralization of the traumatic effects of exogenous hyperthermic factors on the injured areas from the moment the injury was sustained. Childrenin the control group (n=56) sought help on the 5th–14th day after the injury. The duration of prompt neutralization did not exceed 15 minutes or was not performed at all. It was often combined with the irrational use of pharmacological agents and folk remedies. Results. Comparative analysis of the course of the disease in children of the main group and the comparison group reliably confirmed the benefits of prompt neutralization of traumatic effects of hyperthermic exogenous and endogenous damage factors in burn injuries manifested by a more favorable course of the disease, faster improvement, lower local and systemic inflammatory response, shorter hospital stay with less occurrence of various complications, repeated surgical interventions, better aesthetic and functional results. Conclusions. The obtained results objectively confirmed the importance and effectiveness of the suggested system of organization of care for children with burn injuries, especially at the stages of self-help, mutual assistance and first aid. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of these Institutes. The informed consent of the patient was obtained for conducting the studies. The authors declare no conflicts of interests. Background. Acetabular fractures are severe intra-articular injuries that require anatomical reposition and early function, but they are often complicated by degenerative changes in both the acetabulum and the femoral head, leading to the development of post-traumatic coxarthrosis. The purpose was to study the clinical effectiveness of surgical treatment and postoperative management of patients using autologous platelet-rich plasma in patients with fractures of the acetabulum. Materials and methods. Twenty-eight patients with acetabular fractures of various degrees were operated at the City Emergency Hospital of Zaporizhzhia and the Municipal Clinical Hospital No 9 from 2017 to 2019. Gender composition: 22 men (78.57 %), 6 women (21.43 %), average age 46.64 ± 2.21 years, with a 95% confidence interval 42.31–50.96. Nineteen victims (67.86 %) had road traffic injuries, 7 (25 %) domestic injuries, and 2 (7.14 %) had industrial injuries. Results. In the main group of patients treated with platelet-rich plasma, radiologically visible adhesion of the injured area after 8 weeks occurred in 10 people (83.4 %). In one person (8.3 %), the adhesion occurred after 12, and in another (8.3 %) — 16 weeks after surgery. In the control group, adhesions at 8th week were registered in 14 patients (60.87 %), in 8 (34.78 %) — at 16th week. Given the general recovery of the body in the main group, the activation and social adaptation of patients occurred 2–3 weeks earlier. The assessment was performed radiologically and by the criteria of functional recovery.

Purpose — to perform a comparative analysis of the clinical efficacy of intrastromal keratoplasty (ISKP) in patients with keratoconus during surgery with and without Verion digital marking device.Patients and methods. 37 patients (50 eyes) with corneal astigmatism up to -5.0 diopters were included in the study. These patients were operated with the femtolaser method of ISKP with implantation of corneal segments. The average age of the patients was 27.96 ± 3.40 years. The observation period is from 3 to 12 months. All patients were divided into two groups depending on the surgical technique: Verion-assisted, using a digital marking device and taking into account the angle of cyclotorsia, or standard, without taking into account the angle of cyclotorsia. The first main group included 28 eyes (56 %), the second control group included 22 eyes (44 %) accordingly.Results. The functional results after ISKP in both groups were quite good. Stabilization of functional parameters and keratometric data was noted in the postoperative period, by 3 months. Patients of the main group showed a statistically significant (p < 0.05) increase in average uncorrected visual acuity to 0.50 ± 0.16, and visual acuity with maximum correction to 0.70 ± 0.12 (p < 0.05). The average keratometry of the weak meridian decreased to 44.3 ± 1.4 diopters, the average keratometry of the strong meridian decreased to 50.8 ± 3.2 diopters. Patients in the control group showed a statistically significant (p < 0.05) increase in average uncorrected visual acuity to 0.45 ± 0.12, and visual acuity with maximum correction to 0.60 ± 0.17. The average values of keratometry of the weak meridian decreased to 45.3 ± 1.8 diopters, the average values of keratometry of the strong meridian decreased to 51.8 ± 2.2 diopters. The results of the vector analysis reliably indicate a significantly higher accuracy of the Verion-assisted ISKP. Which is confirmed by the achieved indicators of the “success” index, the calculated astigmatism vector close to the actual postoperative astigmatism vector, the smaller the magnitude of the astigmatism axis difference vector compared to the control.Conclusion. The use of the Verion-assisted intrastromal technique for keratoconus reduces the risk of erroneous positioning of the corneal segments in the tunnel during transferring diagnostic data to the operating room. Taking into account the angle of cyclotorsia during cutting into the intrastromal tunnel increases the effective correction of the cylindrical component of refraction, which is confirmed by the main coefficients of the vector analysis by N. Alpins.

The effectiveness of the immunomodulator Polymuramil (PM) was determined in the treatment of 40 patients with severe chronic generalized periodontitis based on the dynamics of the values of periodontal indices and the content of periodontal pathogens in the periodontal pockets. Patients of the main group (20 people) were treated with professional hygiene (PGO) in combination with daily intramuscular PM for 5 days at a dose of 200 mcg. Patients of the control group (20 people) were performed only PGO. The effectiveness of treatment was evaluated based on the dynamics of the clinical indices of Muehlemann and PMA and the presence or absence of major periodontal pathogens in the periodontal pockets. Results: in patients of the main group, the phenomena of periodontal inflammation and the main periodontal pathogens were eliminated in a shorter time and to a greater extent in comparison with patients of the control group

Results of randomized prospective study with participation of 94 patients aged from 20 to 70 years with monosegmental lumbar spine lesions are presented. Minimum invasive surgical interventions were performed in 55 patients from the main group. Control group consisted of 39 patients in whom decompressive-stabilizing operations via conventional posteromedian approach with skeletization of posterior segments of vertebral column were performed. Average size of operative wound in open interventions more than 10 times exceeded that size in minimum invasive interventions and made up 484 ± 56 and 36 ± 12 sq.cm, respectively. Mean blood loss was 326.6 ± 278.0 ml in the main group and 855.1 ± 512.0 ml in the comparative one. In the main group no one patient required substitution hemotransfusion, while in 13 patients from the comparative group donor erythrocytic mass and/or fresh-frozen plasma were used to eliminate the deficit of blood components. Intensity of pain syndrome in the zone of surgical intervention by visual analog scale in the main group was lower than in comparative group. In the main and comparative groups the duration of hospitalization made up 6.1 ± 2.7 and 9.7 ± 3.7 bed days, respectively. In no one patient from the main group complications in the zone of operative wound were noted. Three patients from the comparative group required secondary debridement and in 1 patient early deep operative wound suppuration was observed. Application of low invasive surgical techniques for the treatment of patients with degenerative lumbar spine lesions enabled to perform radical surgical treatment with minimal iatrogenic injury. The method possessed indubitable advantages over the conventional open operations especially intraoperatively and in early postoperative period.

Aim. Improving outcomes of diagnosis and treatment of patients with liver echinococcosis and its complications. Materials and methods. A comparative analysis of the results of surgical treatment of liver echinococcosis and its complications with traditional laparotomy access surgery (control group) and minimally invasive interventions (main group) was performed.Results. The study included 300 patients (170 in the control and 130 in the main group). In the main group, 37 (28.4%) cases performed open echinococcectomy from various mini-accesses, and 27 (20.7%) performed twostage operations using minimally invasive technology. Laparoscopic echinococcectomy was performed in 23 (17.7%) patients, laparoscopic pericystectomy 12 (9.2%) and laparoscopic liver resection in 10 (7.7%) patients. The frequency of postoperative complications in the main group was 17.7%, in the control 51.8%, postoperative mortality decreased from 2.3% to 0.8%.Conclusion. Minimally invasive technologies in the surgical treatment of liver echinococcosis show the better immediate results compared to traditional open surgical methods.

The purpose of the research is to improve the efficiency of restorative treatment of patients with chronic generalized periodontitis based on the inclusion in therapeutic schemes of innovative mineral bishophytic complexes ― gel "Bishophytic" and rinser "Polymineral." Materials and methods. The study involved 116 patients at the age of 35 to 73 years old suffering from periodontal pathology (mild and moderate chronic generalized periodontitis). They were distributed by simple randomization into 3 groups: the main group included 43 people who received standard treatment in accordance with Russian clinical recommendations for the treatment of periodontitis, 45 minutes later the patients were prescribed some applications with 10% dental gel "Bishophytic" on the gum, in addition, the patients were recommended to use a rinser "Polymineral" for 6 months; the group of comparison included39 people who received standard treatment and gum applications with 10% "Bishophytic" dental gel; the control group) included 38 people who received only standard treatment. Results. Inclusion of mineral bishophytic complexes in treatment programs of patients with CGP contributes to significant reduction of inflammatory activity of periodontal tissues: 61.3% (p 0.01) had the improvement of hygienic and plaque indices in comparison with initial data; 54.6% (р 0,01) had the improvement after additional use of only gum applications with "Bishophytic" gel; the improvement was 52.2% (р 0.01) after basic drug therapy. Thus, the improvement of enzymatic processes was by 30.0% (p 0.01), 26.9% (p 0.01) and 7.8%; reduction of cytokine activity by 62.8% (p0.01), 61.1% (p 0.01) and 40%; the improvement of dental health by 84.0% (p 0.01), 80.7% (p 0.01) and 74.5% (p 0.01) at рkg-ог 0,05 and рkg-ог0,01. Conclusion. Inclusion of innovative mineral bishophytic complexes (gel "Bishophytic" and rinser "Polymineral") in treatment programs of patients with chronic generalized periodontitis contributes to optimization of therapeutic measures: clinical effectiveness in the main group was 93.0%; after usage of dental gel "Bishophytic" (group of comparison) it was 89.7% and after basic therapy (control group) 81.6%.

The extreme manifestation of atherosclerotic lesion of the arteries of the lower extremities is the critical ischemia of the lower extremities. The number of high amputations in such patients ranges from 120 to 500 per 1 million population in the general population annually. In order to achieve the best results in the patency of the arterial bed in the near and distant periods, revascularization of the arteries of the proximal and distal blood flow is necessary. The aim of the work was to evaluate the possibilities and effectiveness of endovascular, open and hybrid arterial interventions on the lower extremities, particularly in patients with multilevel arterial disease, by conducting a retrospective analysis of treatment of critical ischemia. The results of the preoperative ultrasound duplex scan (UDS) of the arteries of 212 patients with critical ischemia of the lower limbs (CILL) shoved, that in 78 (36.8%) were multilevel arterial lesions (MLAL). Patients were divided into two groups. The first (main group) consisted of 50 patients (64%), who have been restored to the open-end and endovascular methods of MLAL, or only endovascular. The second (control) group included 28 patients (36%) — with restoration of permeability of the proximal segment without intervention on the arteries of the distal. Installed, during the period of 16 months, postoperative observation of 78 patients with MLAL, the primary frontal area of the femoral reconstruction in the main group was 92%, and limb preservation - 96%. In the control group, the permeability of the reconstruction zone was 75%, limb preservation — 82%. Thus, it has been established that the most optimal method of recovery of inflow and outflow pathways is one-time hybrid surgical interventions performed in 88% of these patients, which allow to achieve more effectively the recurrence of ischemia and maintain limb.

According to the literature, women with a history of ovarian surgery have reproductive health problems not only during the planning stage of pregnancy, but also during the gestational period. The aim of the study was to reduce the frequency of obstetric complications in pregnant women with operated ovaries with hyperhomocysteinemia by developing a scheme of therapeutic and preventive measures. The study included 117 pregnant women with surgical interventions for benign ovarian tumors/ovarian tumors in the presence of hyperhomocysteinemia, of which 60-the developed scheme was used (group E1) and 57-observations and treatment were carried out according to the orders of the Ministry of health of Ukraine and the Ministry of health of the DPR (group E2). The control group (group K) consisted of 30 conditionally healthy pregnant women. The analysis of the obtained results showed the effectiveness of the proposed complex of therapeutic and preventive measures in pregnant women with operated ovaries with hyperhomocysteinemia.

Widespread use of surgical interventions in the abdomen of dogs determines the relevance of the study. The research tested the influence of therapeutic epidural blockade with 0.2% bupivacaine after surgical interventions on the background of potentiated local anaesthesia on the body of clinically healthy dogs undergoing ovariohysterectomy. In the control group postoperative analgesia was performed with the nonsteroidal anti-inflammatory drug rimadyl on the background of potentiated thiopental anaesthesia. Wound healing rates were determined in 10 animals of both control and experimental groups; blood counts and the level of inflammatory response were indicated in 5 animals of each group. The dynamics of reparative processes and complete wound healing in dogs of the experimental group occurred earlier than in animals of the control group, which confirmed the fact that the use of bupivacaine had a beneficial effect on tissue regeneration. The results of morphological and biochemical parameters of blood in animals of the experimental group manifested only an increase of leukocytes by 1.58 times, fibrinogen by 2.02 times, while in dogs of the control group the level of erythrocytes, haemoglobin and hematocrit decreased, leukocytes increased by 1.74 times, platelets decreased by 2.87 times and fibrinogen increased by 1.72 times. Among the stress response markers in the control group there was a 1.94 times increase in glucose level after surgery. In the experimental group there was a decrease in the intensity of the inflammatory reaction in dogs, accompanied by an increase of anti-inflammatory interleukin 4 (IL-4) only on the seventh day – 1.86 times, while in the control group the level of anti-inflammatory interleukin 1 receptor antagonist (IL-1RA) after three days increased by 2.30 times, after seven days – by 2.48 times, and the level of proinflammatory interleukin 6 (IL-6) after surgery increased by 2.57 times. It will be promising to conduct further research on the influence of therapeutic blockades with bupivacaine in animals in cases of various surgical interventions and pathological conditions, which will lead to faster recovery of animals and alleviate pathological processes.

Aim. To evaluate the effectiveness of the developed special sports complex in the prevention of hypodynamia and obesity among firefighters-rescuers.Materials and methods. A survey of 490 men with no history of chronic infectious and non-infectious diseases aged 20-59 was conducted. 328 people were the main group, 162 people were the control group. The baseline data included anthropometric development indicators, bioimpedance test results, the extraction of biochemical blood test results; the body mass index (BMI) and the ratio of waist and hip circumference were determined. A questionnaire was conducted to identify hypodynamia (according to the IPAQ questionnaire) and the nutrition structure (according to the questionnaire on the food label literacy questionnaire, past). Both questionnaires were adapted to the Russian Federation. Statistica for Windows 8.0 was used for statistical processing. We used a comparison of the means of the Student method. To compare the values expressed in percent, the method of inverse trigonometric Fisher transformations was used.Results. We have developed and proposed for firefighters-rescuers, included in the main group, special sports complexes, which were used for 6 months. These complexes represent additional physical training by the developed method. There are two of them: one with an emphasis on the prevention and rehabilitation of people with osteochondrosis of the lumbar spine, and the other for cervical and thoracic localization, including with a syndrome of shoulder-scapular periarthritis.Classes were held 3 times a week, the duration of 1 session was 45-50 minutes. The choice of the complex was carried out at the request of firefighters-rescuers. The first complex was selected by 52 people. Based on the results of the study, against the background of the use of sports complexes, there was a 2-fold decrease in the incidence of obesity. Such a high efficiency is due to the fact that obesity of the I degree prevailed among the firefighters-rescuers. In addition, the frequency of occurrence of dyslipidemia, hyperglycemia, and hypodynamia decreased.Conclusion. The use of this sports complex can reduce the risk factors for development of disability among rescue firefighters by preventing hypodynamia and reducing the incidence of hyperglycemia and dyslipidemia, as factors in the development of obesity.
