ABSTRACT Objective: Reflect on the implications of COVID-19 for the nursing care of pediatric oncology patients. Methods: Theoretical and reflective essay, based on recent scientific publications and expert recommendations on the care of pediatric patients under cancer treatment in the context of the current pandemic. Results: Reflected on the involvement of the pediatric population in general by COVID-19, the vulnerability of pediatric oncology patients to the complications of the disease and the likely challenges for their care. The pandemic demanded a rapid adaptation of health services in terms of patient care flow, care protocols, infection prevention measures, and redefinition of cancer therapy, with repercussions for professionals, patients, and their families. Final considerations: It is not yet known how often or how severely children with cancer have been affected by COVID-19, but therapeutic and care adaptations for the maintenance of these patients’ treatment in the pandemic context are noted.

Abstract Background Central venous catheters (CVCs) are important for healthcare delivery in pediatric oncology patients. It is common to repair CVC breakage to prevent replacement. Existing evidence regarding the association between CVC repair and bloodstream infections (BSI) is limited in the general pediatric population and lacking in pediatric oncology patients. We aim at evaluating whether repairing broken CVCs is associated with an increased risk for subsequent BSI in a pediatric oncology center. Methods This is a retrospective case-crossover study of pediatric oncology patients with broken CVCs that underwent repair between July 2015 and June 2017. The incidence and characteristics of BSI in the 30-day pre-repair period were compared with those in the 30-day post-repair period. Wilcoxon-Mann–Whitney and Fisher’s Exact tests were used for comparison of continuous and categorical variables, respectively. Univariate logistic regression was used to identify potential risk factors for BSI post CVC repair. Multiple breakages of the same CVC, and BSIs in overlapping observation periods of consecutive breakages are assumed independent. Results Sixty-four patients had 99 episodes of CVC breakage/repair in 68 CVCs. Median age (range) at repair was 2.5 (0.15–17.6) years. 48% of CVC breakages occurred in patients with solid tumors, 24% in HSCT recipients, and 19% in patients with leukemia. Only 25% of patients had neutropenia at repair and 14% had CVC occlusion 72 hours prior to breakage. All CVCs were made of silicone and 88% were double lumen external tunneled. First CVC breakage occurred at a median (range) of 130 (2–718) days since insertion, and CVCs were removed at a median (range) of 72.5 (3–753) days from the last repair. End of treatment was the most common cause (43%) for removal. The post-repair incidence of BSI was 4.5 per 1000 line-days compared with a pre-repair incidence of 4.3 (RR= 0.95, 95% CI 0.44, 2.18). There is no statistical difference between the characteristics of the pre-repair and post-repair BSI (Table 1). Figure 1 shows the organisms causing BSI before and after CVC repair. None of the evaluated variables was identified as a significant risk factor for BSI 30 days after CVC repair (Table 2). Conclusion Repair of CVC in pediatric oncology patients was not associated with increased risk of BSI. Disclosures All authors: No reported disclosures.

В работе рассматриваются результаты авторского социологического исследования, проведённого в мае 2021 г. Опрос был проведен среди студентов, обучающихся в медицинском институте ФГБОУ ВО «Майкопский государственный технологический университет» («МГТУ»), с целью изучения отношения студенческой молодежи к необходимости соблюдения мер профилактики коронавирусной инфекции, а также к вакцинации населения и отношения к ним студентов медицинских специальностей в период локдауна и после него. Результаты проведённого исследования показали высокую степень ответственности студентов - будущих медиков за своё здоровье и здоровье окружающих. В то же время часть опрошенных демонстрировала в этот период нигилизм по отношению к мерам профилактики COVID-19. Вместе с тем участники опроса продемонстрировали высокий уровень толерантности по отношению к студентам - гражданам других государств, обучающимся вместе с ними. Ответы студентов медицинских специальностей на вопросы анкеты показывают понимание параметрики, смысла и содержания ограничений, введённых в период пандемии, и необходимости соблюдения жителями региона мер, рекомендованных Роспотребнадзором для профилактики и нераспространения коронавирусной инфекции. The work examines the results of the author's sociological study conducted in May 2021. The survey was conducted among students learning at the Medical Institute of the Maykop State University of Technology in order to study the attitude of students to the need to comply with measures to prevent coronavirus infection, as well as to vaccinate the population and the attitude of medical students to them during the lockdown period and after it. The results of the study showed a high degree of responsibility of students - future medical experts for their health and that of others. At the same time, some respondents demonstrated nihilism towards COVID-19 prevention measures during this period. At the same time, the survey participants showed a high level of tolerance towards students - citizens of other states studying with them. The answers of medical students to the questions of the questionnaire show an understanding of the parameters, meaning and content of the restrictions introduced during the pandemic, and the need for residents of the region to comply with the measures recommended by Rospotrebnadzor for the prevention and non-proliferation of coronavirus infection.

Clinical trials dominate the therapeutic approaches used in pediatric oncology with the majority of pediatric oncology patients receiving treatment on medical research protocols. These protocols detail the medical treatment that enrolled patients will receive. Nursing care is not described in these documents. However, nursing care must complement the medical care as written in the medical research protocol. Administering treatments safely, assessing treatment responses, educating patients and families, and communicating with the entire health care team are some of the essential nursing responsibilities that must be carefully orchestrated. Nursing care guidelines, as described here, were created to provide the careful balance between the medical research protocol and associated nursing care. These guidelines describe the nursing actions and considerations involved in caring for pediatric oncology patients enrolled on a particular medical research protocol. The purpose of this article is to describe the process used to create and evaluate nursing care guidelines.

The search for effective means of preventing family (domestic) violence remains an urgent issue today. The problem requires efforts not only from law enforcement agencies. Prevention measures should be comprehensive and involve the resources of both law enforcement agencies and society as a whole.

Objective: To review all pre-selected publications on COVID-19 cases in the pediatric population and to formulate an organized synthesis addressing fundamental and specific points of the disease performance in children. Data sources: We used a crossover of keywords “COVID-19” and “child care", to search scientific publications in the Virtual Health Library in the data bases: LILACS, BVS, PubMed, and Scielo. Wethen selected publications that: 1) relevance, 2) addressed the theme of coronavirus infection mainly in childhood; 3) elucidated clinical particularities of COVID-19 in the child population; and 4) were published either in English or Portuguese. We excluded all publications that had conflicts of interest and those that were not available in full. Summary of the data: We identified 2,262 publications based on our keywords search which only 38 of them met the selection criteria for our analyses. Studies registered different patterns of COVID-19 infections in childhood when compared to adults, with less aggressive manifestations. However, studies report that infected children and young people symptoms can have serious complications with manifestation of multisystemic inflammatory syndrome, characterized by the late occurrence of vasculitis similar to Kawasaki's syndrome, incomplete Kawasaki's and / or toxic shock syndrome. Conclusions: Overall, epidemiology indicates a lower rate of contamination and a lower number of cases with severe outcomes in the pediatric population. However, the potential for transmissibility and the factors that determine serious outcomes in the pediatric population are still uncertain, which calls for greater investments so we can best protect and serve our upcoming generations.

The new coronavirus infection has posed a major public health challenge around the world, but new data on the disease raises more questions than answers. The lack of optimal therapy is a significant problem. The article examines the molecular mechanisms of SARS-CoV-2 infection and the pathogenesis of COVID-19, special attention is paid to features of pathological processes and immune responses in children. COVID-19 leads to a wide diversity of negative outcomes, many of which can persist for at least months. Many of the consequences have yet to be identified. SARS-CoV-2 may provoke autoimmune reactions. Reinfection, herd immunity, vaccines and other prevention measures are also discussed in this review.
