Food insecurity, food expenses and consumption of sugar and soft drinks in households with and without children and/or adolescents

ABSTRACT Objectives To estimate the frequency of food insecurity in households with and without children/adolescents; compare food expenses, sugar and soft drinks consumption in these households; and to analyze the relationship between food insecurity and demographic/socioeconomic variables with food expenses, and sugar and soft drinks consumption in households with and without children/adolescents. Methods Cross-sectional study with 628 households in Campinas, SP, Brazil. Food insecurity was estimated by the Brazilian Household Food Insecurity Measurement Scale. The dependent variables were the proportion of food expenses, and consumption of sugar and soft drinks; and the independent ones included food security/insecurity condition, monthly family income, gender, age and education of the household head. Results The frequency of food insecurity was higher in households with children/adolescents than in households without minors (41.4% vs. 27.9%). The proportion of food expenses was higher in households with children/adolescents and in all households it was associated with lower family income and, in households with minors, to the presence of a female householder. Soft drinks consumption was higher in households with children/adolescents; and was related to higher income in all households, and to the presence of male householder in households with minors. Sugar consumption in households with children/adolescents was associated with higher income, male gender and education level of the household head (<12 years). In households without children/adolescents, the higher sugar consumption was associated with food insecurity and the household head’s education (<8 years). Conclusion In households with children/adolescents there was a greater frequency of food insecurity and a greater commitment of income with food. Food insecurity was associated with increased sugar consumption in households without children/adolescents.

OBJECTIVE: To investigate whether food insecurity is associated with the demographic, socioeconomic, lifestyle, and health conditions of the elderly. METHODS: This cross-sectional study included 427 elderly (³60 years) from Campinas, São Paulo; half were users of a government-run soup kitchen and the others, their neighbors of the same sex. Food insecurity was measured by the Brazilian Food Insecurity Scale. Univariate multinomial logistic regression was used for calculating the odds ratio and 95% confidence interval to measure the association between the independent variables and food insecurity. Variables with p<0.20 were included in a multinomial model, and only those with p<0.05 remained. RESULTS: Most respondents (63.2%) were males; 15.2% and 6.6% were experiencing mild and moderate/severe food insecurity, respectively. The final model, adjusted for sex and age, showed that elderly with a total family income ≤2 minimum salaries (OR=3.41, 95%CI=1.27-9.14), who did not have a job (OR=2.95, 95%CI=1.23-7.06), and who were obese (OR=2.01, 95%CI=1.04-3.87) were more likely to be mildly food insecure. Elderly with cancer (OR=4.13, 95%CI=1.21-14.0) and those hospitalized in the past year (OR=3.16, 95%CI=1.23-8.11) were more likely to be moderately/severely food insecure. Finally, elderly living in unfinished houses (OR=2.71; and OR=2.92) and who did not consume fruits (OR=2.95 and OR=4.11) or meats daily (OR=2.04 and OR=3.83) were more likely to be mildly and moderately/severely food insecure. CONCLUSION: Food insecure elderly are more likely to have chronic diseases, poor nutritional status, and poor socioeconomic condition. Therefore, the welfare programs should expand the number of soup kitchens and develop other strategies to assure adequate nutrition to these elderly.

Abstract BACKGROUND Across all demographics, families without consistent access to adequate food place children at health risk. Consequences may persist beyond early life into adulthood. Public policy positions breastfeeding, the ideal nutrition for infants, as an important solution to reducing family food insecurity (FFI). However, few studies have investigated the association between breastfeeding duration and FFI. OBJECTIVES To evaluate the association between total breastfeeding duration and FFI in Canadian urban children. DESIGN/METHODS A cross-sectional study was conducted of children aged 0–2 years, from a practice-based child research network. Total breastfeeding duration was collected from parent-reported questionnaires. FFI was measured using 2-item food insecurity and validated 1-item NutriSTEP screens. Multivariable regression analysis was performed adjusting for pre-specified covariates. RESULTS Among 3838 children, the mean total breastfeeding duration was 10.6 months (SD=6.7). Families with food insecurity (14.7%) had increased odds of younger mothers, more males and older and more children. In adjusted model, breastfeeding duration was not associated with FFI (OR 0.99; 95% CI 0.98, 1.01). Low-income families were 9 times more likely to be family food insecure than high-income families (p=0.00). CONCLUSION Contrary to public policy, our study found no association between breastfeeding and family food insecurity (FFI). However, other factors may predominate, in particular family income and structure. Given the detrimental impact of FFI, further research is needed to understand the role of infant feeding practices within the larger political, policy and cultural framework.

Abstract Objectives: to estimate the prevalence of Food Insecurity (FI) in pregnant women and to identify its association with demographic, socioeconomic, obstetric, and anthropometric variables in female users of Family Health Units (FHUs) in the city of Colombo, state of Paraná, South Region of Brazil. Methods: a cross-sectional study of a representative sample of pregnant women from 17 FHUs in Colombo. We used the short version of the Brazilian Household Food Insecurity Measurement Scale (EBIA). In addition, we used Poisson Regression Models with robust, crude, and adjusted variance to investigate the association between FI and exposure variables. Results: 316 pregnant women participated in the study. The prevalence of FI was 45.1% (CI95% = 39.6-50.6). Adjusted analysis showed higher prevalence of FI in pregnant women with 30 years of age or older (PR = 1.66; IC95% = 1.02-2.69), with black skin or indigenous background (PR= 1.39; CI95%= 1.08-1.79), with 7 years of education or less (PR = 1.58; CI95% = 1.14-2.19), and with lower income (PR = 2.07; CI95% = 1.36-3.14). Conclusions: There was a high prevalence of FI among pregnant women, particularly among those of older age and with worse socioeconomic conditions, a group that should be considered a priority for actions aimed at promoting food security.

Indigenous communities in Mexico are vulnerable to food insecurity (FI) due to a series of factors that prevent them from having access to sufficient and nutritious food. Therefore, this population group has the highest chronic malnutrition in the country. Objective: This study describes the FI conditions of an indigenous community in the Potosina Huastec. Methodology: FI was measured with the Household Food Insecurity Access Component Scale (HFIAS). Results: There is food insecurity ranging from concern about food lack, food reduction, and hunger. The 95.57% of households with children under 18 years were in some state of food insecurity, with severe food insecurity dominating. Limitations: Was a cross-sectional study carried out in a community of the Potosina Huastec. However, it is the municipality with the highest degree of marginalization in the area, but the community chosen is the largest in the municipality. Nonetheless, the results can be considered representative of the area. Conclusions: The food security community depends on climatic, socioeconomic, demographic, geographic, cultural factors, food preference conditions, among others. In addressing food insecurity, it is necessary to consider all factors to have a real diagnosis of food insecurity conditions in rural and indigenous communities.

Abstract Objective To investigate the relation of child dietary diversity and household food insecurity along with other socio-demographic with child anthropometric indices in north-central Ethiopia, an area with a high level of food insecurity and inadequate diet quality. Design A community-based cross-sectional study was used. Settings The study was conducted in Dessie and Combolcha towns of north-central Ethiopia from April to May 2018. Participants Randomly selected 512 mother-child pairs with child’s age range of 6–59 months. Results The mean (± SD) scores of weight-for-height/length, height/length-for-age, weight-for-age, and BMI-for-age Z-scores were 1.35 (± 2.03), − 1.89 (± 1.79), 0.05 (± 1.54), and 1.39 (± 2.06), respectively. From all anthropometric indicators, stunting and overweight/obesity remained the severe public issues hitting 43% and 42% of the children, respectively. In the model, mothers’ age and education and child’s age, sex, and dietary diversity were significantly related with child height-for-age Z-score while place of residence, sex of household head, child’s age, and dietary diversity score were the predictors of child BMI-for-age Z-score in the urban contexts of the study area. Nevertheless, food insecurity was not related to any of the child anthropometric indices. Conclusion The double burden of malnutrition epidemics (stunting and obesity) coexisted as severe public health concerns in urban settings. Anthropometric statuses of children were affected by multidimensional factors and seek strong integration and immediate intervention of multiple sectors.

AbstractConsidering food insecurity is a problem, even in developed countries, and the evidence gap regarding this issue in Italy, we aimed to assess the prevalence of food insecurity in a sample of Italian children and examine socio-economic and health correlates. This cross-sectional study was conducted in paediatric practices. Parents answered to the 18-items of the Household Food Security Module, 8 of which concern children. Paediatricians answered a questionnaire on children’s health. Socio-economic and health correlates of food insecurity in children were assessed using univariable and multivariable logistic regression. Among 573 households with children, 15·4% were food insecure, while 9·1% of children were food insecure. Socio-economic factors associated to food insecurity were living in south Italy, households with three or more children, lower household yearly income, worse economic situation description and younger parents. Food insecure children were less likely to have a normal relational [ aOR 0.31 (CI 0.11-0.85)] and physical development [aOR 0.32(CI 0.15-0.65) and had more school difficulties [aOR 3.1(CI 1.33-7.24)] compared to their food secure peers. Food insecure children had higher odds of a deterioration in their health since birth and of a worse perceived health status, as reported by their parents. Considering the results in this sample and the lack of research regarding this issues in Italy and Europe more broadly, we call for consistent, national monitoring to determine the magnitude of the problem of food insecurity in households with children in Italy and to examine the socio-economic variables and health implications in different contexts.

AbstractObjectiveFood insecurity (FI) is a major public health problem in Lebanon, a small middle-income country with the highest refugee per capita concentration worldwide and prolonged political and economic challenges. The present study aimed to measure the prevalence and sociodemographic correlates of household FI and to explore the association of household FI with anthropometric measures of children and their mothers.DesignCross-sectional survey (2014–2015).SettingLebanon.ParticipantsNationally representative sample of Lebanese households with 4–18-year-old-children and their mothers (n1204).ResultsFI prevalence (95 % CI), measured using the Arabic-translated, validated Household Food Insecurity Access Scale, was found to be 49·3 (44·0, 54·6) % in the study sample. Mild, moderate and severe FI were found in 7·0 (5·5, 9·2) %, 23·3 (20·1, 26·8) % and 18·9 (14·9, 23·5) % of households, respectively. Multiple regression analysis showed that low maternal and paternal education, unemployment and crowding were significant correlates of household FI (P<0·05). No significant associations were observed between FI and anthropometric measures of children and their mothers, after adjusting for other socio-economic correlates. Food-insecure households reported various mechanisms to cope with food shortage, such as reducing the number of meals/d (49·6 %), borrowing food (54·4 %), spending savings (34·5 %) and withdrawing children from schools (8·0 %).ConclusionsFI exists among a remarkable proportion of Lebanese households with children. Correlates of household FI should be considered when designing social welfare policies and public health programmes to promote more sustainable, resilient and healthier livelihoods among vulnerable individuals.

Abstract Objective: To examine the incidence of food insecurity and affecting factors in households with children in Turkey during the COVID-19 pandemic. Design: This is a cross-sectional study. Setting: The participants were recruited by the snowball sampling method and the data were collected via a link sent to their smart mobile phones through their social media accounts. Participants: This study included 211 households with at least one child. Results: The study revealed that 21.8% households had food insecurity that was not at the hunger threshold. The monthly income of 80.6% of the households was below the poverty line and monthly income decreased in more than half of the households during the pandemic. Food insecurity increased 2.5 times when the households comprised workers or self-employed individuals (OR=2.529, p=0.002), increased 3 times when the monthly income of the households decreased (OR=3.131, p=0.000), and increased 2 times when total monthly income of the household fell below poverty line during the pandemic (OR=2.001, p=0.049). Conclusion: It is determined that nearly half the households have food insecurity and that the pandemic poses a risk in terms of food security. We recommend that public health studies should be planned to ensure accessibility to healthy foods.

AbstractObjectiveTo identify the association of household food insecurity (HFI) with anthropometric status, the risk of vitamin A deficiency and anaemia, morbidities such as cough and fever, and hospitalizations for diarrhoea and pneumonia in children under 5 years old.DesignCross-sectional study using data from the 2006 Brazilian Demographic and Health Survey. HFI was measured with the Brazilian Food Insecurity Measurement Scale (EBIA). Vitamin A deficiency and anaemia were assessed in blood samples. Child morbidities were reported by the child’s mother and included cough, fever, and hospitalizations for diarrhoea and pneumonia. Regression results were expressed as unadjusted and adjusted OR and corresponding 95 % CI for severe food insecurity, with statistical significance set atP<0·05.SettingNationally representative survey.SubjectsChildren (n4064) under 5 years old.ResultsThere was no association between HFI and vitamin A deficiency, pneumonia, wasting or overweight. The prevalence of cough, fever, hospitalization for diarrhoea and stunting were associated with degree of HFI severity. There was a significant association of morbidities and stunting with severe food insecurity (v. food secure). After controlling for confounders, the association between severe food insecurity (v. food secure/rest of food insecurity categories) and the prevalence of common morbidities remained strong, showing that severely food-insecure children had a greater likelihood of experiencing cough (adjusted OR=1·79) and of being hospitalized for diarrhoea (adjusted OR=2·55).ConclusionsSevere HFI was associated with cough and severe diarrhoea among Brazilian children.
