Systematic examination of the upper respiratory and digestive tracts (URDT) was performed in a group of 80 paracoccidioidomycosis (PCM) patients submitted to post-treatment follow-up ranging from 8 months to 17 years. Mucosae of the URDT had been involved prior to specific treatment in 74 patients, distributed as follows: oropharynx, 50 (41 alone, 7 in association with the larynx, and 2 with the nasal mucosa); larynx, 30 (23 alone and 7 in association); and nasal mucosa, 3(1 alone and 2 in association). Inactive lesions were observed in all the 50 patients with lesions of the oropharynx, 3 of whom with deforming scars (1 with retraction of the tongue and 2 with narrowing of the oral orifice). One case presented a destructive lesion, with perfuration of the palate. Of the other 46 cases, examination showed nacreous white striated scars which were nearly imperceptible in some cases and in others displayed partial retraction of anatomical structures without any alteration of their features. Patients presented a high rate of missing teeth. In 3 patients with involvement of the nasal mucosa, none of whom presented active PCM lesions, 2 still had nasal voices. In 30 patients with lesions of the larynx, 1 suffered a relapse of PCM and 2 developed epidermoid carcinoma. Of the other 27 cases, none of whom had active PCM lesions, 15 presented dysphonia, 3 were tracheotomized, and 9 were asymptomatic.

Purpose: To evaluate the incidence and reasons for secondary procedures in patients treated with iliac branch devices (IBDs) for isolated iliac aneurysm or aortoiliac aneurysms involving the iliac bifurcation. Methods: Between January 2005 and December 2015, 575 surgical-high-risk patients (mean age 72.0±8.4 years; 558 men) with isolated iliac aneurysms (n=79) or aortoiliac aneurysms involving the iliac bifurcation (n=496) were treated with placement of 650 ZBIS or Gore IBDs (75 bilateral) in 6 European centers. The primary outcome was procedure-related reinterventions for occlusion or high-grade (>70%) stenosis of the bridging device, occlusion of the ipsilateral common or external iliac artery (EIA), type I/III endoleak, rupture, or infection following IBD implantation. Clinical and radiological data were analyzed based on preset definitions of comorbidities, aneurysm morphology, intraoperative variables, and follow-up strategies. Results: Nine (1.6%) reinterventions were performed within 30 days for occlusion or endoleak. Among 10 (1.5%) occluded EIAs ipsilateral to a deployed IBD, 6 underwent a reintervention with additional stent placement after thrombolysis (n=4) or a femorofemoral or iliofemoral crossover bypass (n=2). Three of 14 patients with early type I endoleak had a reintervention for an insufficient proximal sealing zone (stent-grafts in 2 common iliac arteries and 1 bifurcated endograft). Mean clinical and radiological follow-up were 32.6±9.9 and 29.8±21.1 months, respectively. Forty-two (7.3%) patients underwent reinterventions in the follow-up period. The overall postoperative reintervention rate was 8.9%. Both external and common iliac segments occluded in 30 (4.6%) IBDs; 2 patients had a crossover bypass and 14 were treated with endovascular techniques. In the other 14 patients, no specific treatment was performed. Seven (1.2%) patients with isolated EIA occlusion were treated during follow-up. Nineteen of the overall 28 patients with type I endoleak underwent endovascular repair. The other 9 were under radiological surveillance due to less significant (<5 mm) sac increase. No reintervention was performed to recanalize 11 (1.6%) occluded internal iliac arteries. Conclusion: Midterm experience with placement of IBDs is associated with a low incidence of secondary procedures due to type I endoleaks and occlusions. The main reasons for reinterventions seem to be short proximal sealing zone and poor conformability of the ZBIS device in elongated EIAs.

Objective: This study describes a surgical approach wherein a lobulated pedicled nasal mucosa flap technique was employed for endoscopic dacryocystorhinostomy (EDCR) as a means of treating nasolacrimal duct obstructions. This study also assessed the long-term outcomes of this EDCR approach when implemented without stenting. Methods: This was a retrospective study of a total of 63 patients (67 eyes) treated for nasolacrimal duct obstructions between January 2011 and November 2016. All patients had undergone ophthalmic diagnosis followed by EDCR treatment using a lobulated pedicled nasal mucosa flap without stenting. Patients were then monitored for both anatomical patency and sustained symptom relief during the follow-up period in order to assess objective and subjective study outcomes. Results: Patients were followed for a mean of 25.3 ± 1.2 months (range: 24-28 months), with a 100% anatomical patency success rate (67/67) and a 94.03% symptomatic cure rate (63/67). There were no instances of complications. Conclusions: The use of a lobulated pedicled nasal mucosa flap technique for EDCR without stenting is a straightforward, effective, and safe approach that keeps bone exposure to a minimum while offering a high rate of satisfactory outcomes, making it a procedure worthy of consideration as a means of treating patients suffering from nasolacrimal duct obstructions.

BackgroundHypochondriasis is generally considered difficult to manage. This study aimed to assess the effectiveness of cognitive therapy and to compare it with an equally credible, alternative treatment.MethodForty-eight patients with hypochondriasis were initially randomly assigned to either cognitive therapy, behavioural stress management or a no treatment waiting list control group. At the end of the waiting period, patients in the control group were randomly assigned to one of the two treatments. Assessments were at pre-, mid- and post-treatment or waiting list and at three-, six- and 12-month post-treatment follow-up.ResultsComparisons with the waiting list group showed both treatments were effective. Comparisons between the treatments showed that cognitive therapy was more effective than behavioural stress management on measures of hypochondriasis, but not general mood disturbance at mid-treatment and at post-treatment. One year after treatment patients who had received either treatment remained significantly better than before treatment, and on almost all measures the two therapies did not differ from each other.ConclusionsCognitive therapy is a specific treatment for hypochondriasis. Behavioural stress management is also effective but its specificity remains to be demonstrated.

Abstract BACKGROUND AND AIMS: Hairy cell leukemia (HCL) is very sensitive to purine analogs (PAs), but ~40% of patients relapse and become progressively less responsive to these myelotoxic and immune-suppressive drugs. Having discovered the BRAF-V600E kinase-activating mutation as the genetic lesion underlying HCL (Tiacci et al, NEJM 2011;364:2305), we performed the first clinical trial of a BRAF inhibitor (vemurafenib) in refractory/relapsed HCL. In particular, this is a phase-2, academic, single-arm, Italian, multi-center (n=8) study (HCL-PG01; EudraCT 2011-005487-13). METHODS: In 11 months we enrolled 28 BRAF-V600E+ HCL patients, needing therapy due to cytopenias and including: i) 6 patients primary refractory to a PA; ii) 21 patients who relapsed early and/or repeatedly after PAs and had received a median of 4 previous therapies; and iii) a 81-year old patient showing severe myelotoxicicity after a PA (discouraging its further use). Previous treatments other than PAs included interferon, rituximab and splenectomy in 12, 14 and 8 patients, respectively. Complete remission (CR) required resolution of cytopenias (N≥1500/mmc, PLT≥100000/mmc, Hb≥11 g/dl), no morphological evidence of HCL cells in the bone marrow biopsy and blood smear, and no splenomegaly. Partial remission (PR) required resolution of cytopenias, and a ≥50% reduction of splenomegaly and of marrow and blood HCL involvement by immunophenotyping. Two patients were not evaluable as they went off-study after ≤1 week of treatment (due to drug-unrelated acute myocardial infarction and consent withdrawal after grade-3 drug-related reversible pancreatitis). RESULTS: Vemurafenib, given orally at the dose of 960 mg twice daily on an outpatient basis for a median of 16 weeks, was generally well tolerated. Drug-related adverse events (mainly arthralgias, skin toxicities, pancreatitis; no myelosuppression) were frequent, but reversible in all patients, and were typically grade 1-2. Only 7 patients developed grade 3 events, and none grade 4 events. Although we did not observe any cutaneous squamous cell carcinomas/keratoachantomas (as reported in BRAF-V600E+ melanoma patients treated with vemurafenib), 3 patients developed 2 basaliomas and 1 superficial melanoma, all treated with a simple excision. Notably, overall response rate was 96% (25/26 patients): 9/26 (34.6%) CRs and 16/26 (61.4%) PRs, obtained after a median of 8 and 9 weeks respectively. CR and PR patients included 1 and 5 primary refractory ones, respectively, as well as 4 and 10 not responding to the last prior treatment, respectively. In all CR patients immunohistochemistry showed minimal residual disease (≤10%) at the end of treatment. Six of 9 (67%) CR patients enjoyed normal blood counts at a median of 13 (range 12-15) months from the end of treatment (see Figure): 3 of these 6 patients showed no morphological evidence of HCL in the bone marrow biopsy (complying with a continuous CR) at 12, 13 and 15 months, respectively, whereas the other 3 lost the bone marrow CR status, all at 12 months. The remaining 3/9 CR patients (33%) developed a mild cytopenia (N ~1000/mmc or PLT ~80000/mmc) 5, 9 and 12 months post-treatment, respectively: in the 2nd patient the cytopenia remained stable until the last follow-up at 15 months, whereas in the other two cases it worsened requiring therapy 9 and 18 months post-treatment, respectively (see Figure). These two latter patients were recently retreated with vemurafenib for 12 and 4 weeks, and obtained a PR and a second CR. Among the 16 PR patients, 5 (31%) mantain normal blood counts at a median of 12 (range 8-17) months post-treatment (see Figure). The other 11 PR patients developed cytopenia(s) after 3 months of median follow-up (range 5-10): in 6 patients (38%) no anti-leukemic therapy was started at a median of 9 (range 6-12) months post-treatment, whereas in the remaining 5 cases (31%) cytopenia(s) worsened requiring therapy at a median of 8 (range 5-11) months of follow-up (see Figure). Four of these latter 5 patients were retreated with vemurafenib for 12 weeks: 3 cases had a minor response and the last one witnessed a second PR that lasted less than the first PR (3 versus 9 months). CONCLUSIONS: In heavily pre-treated HCL patients, a short oral course of vemurafenib was safe, and proved quickly and highly active. Retreatment with vemurafenib was able to reinduce remissions in patients relapsing after a CR, but was less effective in patients relapsing after a PR. Figure 1 Figure 1. Adherence is key to improving quality of life of patients with allergopathology on allergen-specific immunotherapy (ASI). Treatment adherence is influenced by a variety of parameters: socio-economic, medical, patient-related factors, health condition and therapy specifics. The purpose of the study was to evaluate adherence of patients with allergic and non-allergic diseases to specific and non-specific treatment. A study of adherence of patients (n=270) residing in the Samara region was conducted using a standardized questionnaire. The result was calculated according to the formulas and was rated as unsatisfactory, satisfactory and high. The adherence rate of patients with allergic and non-allergic diseases of the upper respiratory tract added up to 61.84 and 52.07, respectively, both corresponding to the average indicators. High adherence rates were more common in the group of patients with allergic diseases rather than in the comparison group: 73.6% versus 51% (ꭓ2 =13.9, p=0.001). Male and female adherence indicators hardly differed. In patients under 18, high adherence rates were significantly more frequent – 85.2% in children versus 56% in adults (ꭓ2=22.37, p=0.0001). In general, among all patients, readiness for follow-up was higher compared to the adherence to lifestyle changes (U=54164.5, p=0) and to drug treatment (U=40701, p=0.019) among all patients. Regular medical follow-up does not eliminate the need for lifestyle changes and drug therapy. There is a need to improve measures aimed at improving adherence.

Current international guidelines recommend Trauma-Focused Therapies (TFT) to treat Post-Traumatic Stress Disorder (PTSD). However, non-response and dropout rates have shown to be substantial. TFT, delivered in a highly intensive dose may lead to a more efficacious symptom reduction and lower dropout rates. This article describes an inpatient treatment program with Narrative Exposure Therapy (NET), called High-Intensive NET (HI-NET). The program consists of sixteen NET sessions during two weeks, combined with art therapy and yoga. After describing the treatment program, we present two case reports of patients who received HI-NET. We measured amongst others symptoms of PTSD and depression, sleep quality and general psychopathology at pre-treatment, post-treatment and at four months' follow-up. The two patients showed that applying HI-NET is a promising approach to append to the existing way of conducting NET. One patient reported a slight increase in symptoms at post-treatment, but was fully recovered of PTSD at follow-up. The other patient recovered gradually, with full recovery at follow-up. The other measures also showed important improvements at the end of treatment and at follow-up. Concluding: NET in a high intensive form is applicable and feasible. Our two cases showed an important improvement in PTSD symptoms and in general psychopathology. More research is needed with larger patient groups to determine efficacy and efficiency more properly.

Abstract Background: ABVD (Doxorrubicin, Bleomycin, Vinblastine, Dacarbazine) can be considered as first line treatment for Hodgkin Lymphoma (HL). Our cooperative group has an experience of 584 patients treated with 3 or 6 cycles plus involved field radiotherapy (IFRT) with a complete remission (CR) of 91% and an event free survival and overall survival at 60 months of 79% and 95%. The new recommendations by B. Cheson define CR for HL as the lack of signs and symptoms of lymphoma with a negative PET-CT. (B Cheson et al, JCO: 25-579-586, 2007). A negative PET-CT either early during treatment of ABVD or after completion of it, has also shown to be a powerful prognostic tool (Hutchings: Blood 2006, Gallamini: Haematologica 2006). In an attempt to maintain high rate of CR and to reduce toxicity due to chemotherapy and radiotherapy we have conducted a trial with PET–CT adapted therapy. Aims: Test the feasibility and efficacy of treatment to all stages of HL adjusted to PET-CT results after 3 cycles of ABVD. Evaluate the outcome of patients who have a negative PET scan after three cycles of ABVD and receive no further treatment. Offer more intense therapy to patients who have persistent hypermetabolic lesions in PET-CT after 3 cycles of ABVD. Method: Since October 2005, 123 newly diagnosed patients with HL have been included in a prospective multicenter trial. One hundred and two have already finished treatment. Initially all patients received 3 cycles of ABVD. After the third cycle, patients were evaluated with a PET-CT. Those patients who achieved CR with a negative PET-CT received no further treatment. Patients with partial response (PR) completed 6 cycles of ABVD and IFRT on PET-CT positive areas. Those patients with less than PR after 3 cycles received high doses of chemotherapy and an autologous stem cell transplant (ASCT). All patients were reevaluated at the end of treatment. The median age at diagnosis was 33 years. Eighty-two patients (80%) had localized stage at diagnosis (I–II) and 20 (20%) presented with advanced stage (III–IV). Fifty-one (55%) patients had IPI 0–1, 37 (40%) had IPI 2–3 and 5 (5%) patients had IPI 4–5. Sixteen (16%) patients had bulky disease at diagnosis. Results. All patients completed treatment as planned. Eighty-seven (85%) achieved CR with negative PET-TC after the first 3 cycles of ABVD. Fifteen were PET positive, one with PD who achieved CR after ESHAP and ASCT. The other 14 patients completed 6 cycles of ABVD + RT. Twelve achieved CR and 2 PR. One died of progressive disease and the other one is in CR after third line treatment. Six patients relapsed, 4 are in second CR, and 2 are currently under treatment. Five of these 6 patients had achieved CR after 3 cycles of ABVD and 1 had progressive disease in PET–TC after the third cycle of treatment and received ESHAP and ASCT. With a median follow up of 17 months, 95 (93%) are in first CR, 4 (4%) in second CR and 2 in treatment. The event free survival and overall survival at 18 months are 92% and 100%. Conclusion: Treating patients with ABVD, evaluating response after 3 cycles with PET-CT, and adapting further therapy, leads to a high rate of CR avoiding potential long-term toxicity due to more aggressive chemotherapy and radiotherapy. Three courses of ABVD without radiation are adequate in patients with early CR defined by negative PET-CT. In early positive PET-CT, it is possible to intensify therapy improving the otherwise bad prognosis. These results need to be confirmed by a larger group of patients and a longer follow-up.

One of the tools to determine the recommended order of the courses to be taught is to set the prerequisites, that is, the conditions that have to be fulfilled before commencing the study of the course. The recommended sequence of courses is to follow logical links between their logical units, as the basic aim is to provide students with a coherent system according to the Comenius' principle of continuity. Declared continuity may, on the other hand, create organizational complications when passing through the study, as failure to complete one course may result in a whole sequence of forced deviations from the recommended curriculum and ultimately in the extension of the study period. This empirical study deals with the quantitative evaluation of the influence of the level of initial knowledge given by the previous study on the overall results in a certain follow-up course. In this evaluation, data were obtained that may slightly change the approach to determining prerequisites for higher education courses.

AbstractDesign research as a field has been studied from diverse perspectives starting from product inception to their disposal. The product of these studies includes knowledge, tools, methods, processes, frameworks, approaches, and theories. The contexts of these studies are innumerable. The unit of these studies varies from individuals to organizations, using a variety of theoretical tools and methods that have fragmented the field, making it difficult to understand the map of this corpus of knowledge across this diversity.In this paper, we propose a model-based approach that on the one hand, does not delve into the details of the design object itself, but on the other hand, unifies the description of design problem at another abstraction level. The use of this abstract framework allows for describing and comparing underlying models of published design studies using the same language to place them in the right context in which design takes place and to enable to inter-relate them, to understand the wholes and the parts of design studies.Patterns of successful studies could be generated and used by researchers to improve the design of new studies, understand the outcome of existing studies, and plan follow-up studies.

Abstract Background/Purpose The Nuss procedure is the most common surgical repair for pectus excavatum (PE). Surgical steel wires are used in some modifications of the Nuss procedure to attach one or both ends of a support bar to the ribs. During follow-up, wire breakage was found in some cases. Patients with wire breakage may undergo prolonged bar removal surgery and may be exposed to excessive radiation.In this study, we had a series of patients who received polydioxanone suture (PDS) fixations instead of steel wires. This retrospective study was conducted to explore the differences between these two fixation materials in the incidence of related complications and efficacies. Furthermore, we attempted to observe whether the two materials lead to similar surgical efficacy in the Nuss procedure, whether they have divergent effects on the bar removal surgery, and whether PDS can reduce the risks due to steel wire breakage as expected. Methods We retrospectively studied PDS and surgical steel wires as fixation materials for the Nuss procedure in children with congenital PE and reviewed the outcomes and complications. A total of 75 children who had undergone Nuss procedure repairs and bar removals from January 2013 to December 2019 were recruited to participate in this study. They were divided into three groups: the PDS group, the unbroken wire (UBW) group, and the broken wire (BW) group, according to the fixation materials and whether the wires had broken or not. Moreover, we selected the duration of operation (DO), intraoperative blood loss (BL), bar displacement (BD), postoperative pain score (PPS), and incision infection as the risk indicators and the postrepair Haller index (HI) as the effectiveness indicator. These indicators were statistically compared to determine whether there were differences among the three groups. Results One BD occurred in the PDS and BW groups while none took place in the UBW group. No incision infection was found in any of the groups. The PDS group had the shortest DO, while the DO in the UBW group was shorter than that in the BW group (p < 0.05). BL in the PDS group was less than that in the other two groups (p < 0.05). Additionally, no difference was observed in BL between the BW and UBW groups (p > 0.05). The PPS of the PDS group was less than that of the BW group (p < 0.05), whereas no differences were found between the other two groups. No statistical difference emerged in HI among the groups (p > 0.05). Conclusion PDS fixation results in a similar repair outcome and shows certain advantages in the DO, BL, and PPS; also, PDSs are safe and effective in the Nuss procedure.
