Objective To assess the effectiveness of Problem-Solving Therapy (PST) on family caregivers through the use of scales to measure anxiety, depression and emotional distress; and to explore facilitating factors and obstacles for its use based on the narrative of nurses. Method A clinical trial and an exploratory focus group with the use of mixed analysis methodology. The study was conducted in a primary health care center in Tarragona, Spain, and the sample consisted of 122 family caregivers who were included in the home care service, and 10 nurses who participated in the intervention group. Family caregivers with evident symptoms of anxiety, depression and emotional distress received PST in the intervention group. The intervention group also consisted of a discussion with eight nurses, which was transcribed and submitted to content analysis. Conclusion Problem-Solving Therapy proved to be effective in reducing perceived anxiety, depression and emotional distress. We identified its strong points and obstacles as described by nurses.

AbstractThe prevalence of dementia in Chile is 1.06 %, meaning that over 200,000 people are affected. In 2017, the Ministry of Health launched the National Plan of Dementia, which proposed establishing a range of health-care services from primary care to Memory Units (MU).MUs have emerged as a new health care service composed of multidisciplinary teams with the goal of improving diagnosis and management of dementia patients.The creation and implementation of a MU should take into consideration the organization of a particular healthcare system. In this context, the evaluation of a Chilean MU might provide information for the standardization and replication of such a health service on a regional basis.The objective of this paper was to evaluate the implementation processes of a MU using the RE-AIM model, a multi-component model aimed to assist the evaluation of the implementation of ongoing programs.Regarding “R” (Reach): from March 2018 up to June 2019, a total of 510 patients were referred and assessed at the Hospital del Salvador. Most patients came from primary care (51.9 %) and from outpatient services at the Hospital (39.2 %), particularly from the Neurology (63.3%) and Psychiatry (16.0 %) departments. We estimated that the MU assessed 5.39% of dementia patients living in the area of referral.In relationship with “E” (Effectiveness): of patients evaluated by the MU, 60 (11%) were discharged. Of these, 41 (66%) were referred to primary health care, 9 (17%) to other outpatient services, 6 (10%) to a specialized mental health care center, and 4 (7%) to a daycare center.Due to the short lifespan of our MU, no other RE-AIM dimensions could be evaluated yet.This was the first evaluation of the implementation of a MU in Chile as part of the Chilean Dementia Plan. It showed that it is possible to implement a MU in a Latin American country and improve access to dementia diagnosis, management, and treatment. Ongoing challenges include continuing to collect clinical data, creating research projects as part of the MU, and developing a MU protocol that can be adopted elsewhere in Chile and other Latin American countries.

ABSTRACTBackground:The value of care provided by informal carers in Canada is estimated at $26 billion annually (Hollander et al., 2009). However, carers’ needs are often overlooked, limiting their capacity to provide care. Problem-solving therapy (PST), a structured approach to problem solving (PS) and a core principle of the Reitman Centre CARERS Program, has been shown to alleviate emotional distress and improve carers’ competence (Chiu et al., 2013). This study evaluated the effectiveness of problem-solving techniques-based intervention based on adapted PST methods, in enhancing carers’ physical and emotional capacity to care for relatives with dementia living in the community.Methods:56 carers were equally allocated to a problem-solving techniques-based intervention group or a control arm. Carers in the intervention group received three 1 hr visits by a care coordinator (CC) who had been given advanced training in PS techniques-based intervention. Coping, mastery, competence, burden, and perceived stress of the carers were evaluated at baseline and post-intervention using standardized assessment tools. An intention-to-treat analysis utilizing repeated measures ANOVA was performed on the data.Results:Post-intervention measures completion rate was 82% and 92% for the intervention and control groups, respectively. Carers in the intervention group showed significantly improved task-oriented coping, mastery, and competence and significantly reduced emotion-oriented coping, burden and stress (p < 0.01–0.001). Control carers showed no change.Conclusion:PS techniques, when learned and delivered by CCs as a tool to coach carers in their day-to-day caregiving, improves carers’ caregiving competence, coping, burden, and perceived stress. This may reduce dependence on primary, psychiatric, and institutional care. Results provide evidence that establishing effective partnerships between inter-professional clinicians in academic clinical health science centers, and community agencies can extend the reach of the expertise of specialized health care institutions.

Abstract Objectives Nonadherence to a long-term therapy, including diabetes mellitus, is one of the global problems that need to be overcome. This study aims to determine the effect of pillbox use and education by pharmacists toward medication adherence in patients with diabetes mellitus in a Primary Health Care Center in Mataram. Methods This research was an experimental research design with pretest-posttest with control group design. The study was conducted from October to December 2019 at Tanjung Karang Primary Health Care Center, Mataram. Measurement of adherence was done using the Adherence to Refill and Medication Scale questionnaire. The higher the score, the more nonadherence the patients. Patients were divided into three groups, which were the control group, educational intervention group, and pillbox and educational intervention group. Each group consisted of 11 patients. Results Patients’ medication adherence increased from 19.54 (SD 4.37) to 15.18 (SD 2.64) in the education and pillbox intervention group (p=0.004). Whereas, in the education and control group, the adherence did not provide a significant change (p>0.05). Based on the difference in adherence scores, it was known that what contributed to changes in compliance was refilling medicine and intentional nonadherence in taking medicine subscale (p=0.024). Conclusions Providing education and pillbox done by pharmacists at the Primary Health Care Center can increase adherence to the therapy of diabetes mellitus patients. Pharmacists at the Primary Health Care Center can use the intervention model to improve the level of adherence of patients with chronic illness.

We examined the effect of occupation-based intervention supported with problem-solving therapy of individuals with type 2 diabetes mellitus on participation and satisfaction of meaningful occupations, diabetes-related psychosocial self-efficacy, preferred coping strategies and individual well-being. Methods: This study was planned as a single-blind, randomized controlled study with 3-months’ follow-up involving sixty-seven adults with type 2 diabetes. The Canadian Occupational Performance Measure, the Diabetes Empowerment Scale, the Brief COPE and the Five-item World Health Organization Well-Being Index were used. The intervention was conducted in 6 modules, and each implemented about 60 minutes and for 1 session per week. The six modules included evaluations, diabetes education, and problem-solving therapy. The most important feature of the program is its focus on meaningful occupations for the person and provided an opportunity for the participants to apply problem-solving therapy through valued occupation. Differences between groups were analysed with the Mann Whitney U test, and the Friedman test was used to calculate group-time interaction differences (i.e. baseline, after six modules and after three months). Results: Individuals with type 2 diabetes mellitus in both groups identified the most significant meaningful activity problems in self-care as personal care. Significant improvement was reported in the intervention group when compared to the control group regarding the participation in meaningful occupation, the satisfaction of performance, psychological self-efficacy and well-being results (p<0.001) after the six modules and 3 months follow-up. The participants' use of the effective coping strategies, active coping and acceptance strategies, self-efficacy as revealed results showed improvement in favor of the intervention group (p<0.05). The intervention group showed significantly improving between measurements at baseline and after three months of modules except for ineffective coping style (p<0.001). Conclusions: The person-centred occupation-based problem-solving therapy encourages participation in meaningful occupations, positive effects on psychological self-efficacy, and improves effective coping styles and well-being of type 2 diabetes mellitus. Problem-solving therapies that incorporate individuals’ priorities via meaningful occupation could be used to supported to lead a meaningful life of individuals of type 2 diabetes mellitus. Aloe is a medicinal plant in Indonesia, which is often used as traditional medicine. The purpose of this study was to find out the influence of Aloe juice administration on changes in lipid profile (HDL, LDL, Triglycerides) in East Pontianak Primary Health Care Center. This study used quasi-experimental. This study used sampling on 36 people in 2 groups. In the treatment group, there was Aloe juice administration of 250 ml/day for 15 days while in the control group there was no Aloe juice administration. HDL level in the intervention group had an increase of 14.89 mg/ whereas in the control group had an increase of 1.22 mg/, where there was no significant difference between the intervention group and control group (p&gt; 0.05), LDL level in the intervention group had a decrease of 10.56 mg/ while the control group had a decrease of 5.94 mg/ where there was no significant difference between the intervention group and the control group (p&gt; 0.05) and triglyceride level in the intervention group had a decrease of 8.78 mg/ whereas in the control group had a decrease of 3.50 mg/ where there was no significant difference between the intervention group and the control group (p&gt; 0.05) which means intervention group and control group had no significant differences. The mean HDL level had an increase while the LDL level and triglyceride level had a decrease.

Stress stimulates the sympathetic nervous encouraging the heart and cause an increasing blood pressure. Efforts to reduce hypertension pharmacological and non-pharmacological. Non-pharmacological experiment of which is hypnosis. The research objective was to analyze the decrease of stress to elderly hypertensive therapy with hypnotherapy. The research design used experimental pretest - Posttest with Control Group. The population were all the elderly with hypertension in the elderly health care center PacarKeling , and Kemayoran Surabaya with 35 elderlies. Number of sample 15 intervention group, 15 control group. The sampling technique used Simple Random Sampling. Variable intervention was therapeutic applications of hypnosis to elderly hypertensive. The dependent variable was the stress in elderly hypertensive. The statistical test used paired t-test. It showed changes in stress to the control group -7.33, while the experminent group amounted to 14.60. Results can reduce stress in the group treated with hypnotherapy an average decrease of 14.6. Results showed that there were differences in the level of stress control and experiment groups with significant value of p = 0.000 (p <0.05), The effect of hypnotherapy was also shown in the systolic blood pressure change between control and experiment groups showed a mean difference of ranks. In the control group 20.8, while in the experimental group 10.2. Mann Whitney test results obtained by the significant value of p = 0.000 (p <0.05) in mean systolic blood pressure differences between control and experiment groups. Hypnotherapy can reduce stress and blood pressure in the experiment group. Causes of stress on the individual such as physical pain, and the outside factor could be due to family problems, economic. Suggestions that hypnosis can be used as a new experiment method stress problem in the elderly
