Abstract OBJECTIVE To comprehend the psychosocial effects that Coeliac Disease diagnosis entails. METHOD Qualitative study, achieved through semi-structured interviews, analyzed in accordance to the Association of ideas map Technique. A total 12 recently-diagnosed patients from the Centro de diagnóstico, tratamento e apoio ao paciente com doença celíaca (Coeliac Disease Prevention, Support and Treatment Diagnose Centre) from the Hospital Universitário de Brasília (University Hospital of Brasilia) were enrolled for the study, between the years of 2013 and 2014. RESULTS The interviewed patients presented negative impacts in three categories: psychoaffective, family and social relationships, indicating issues with social readaptation once the treatment had started, as well as difficulty coping with a gluten free diet. CONCLUSION Coeliac Disease holds substantial impact on psychological functions, family and social relationships to diagnosed patients, requiring a clinical biopsychological assistance for better adherence to treatment and patients quality of life.

ABSTRACT Objective: to understand the challenges and strategies adopted by nurses for the exercise of Transformational Leadership in a university hospital. Methods: qualitative and exploratory study, in which 25 nurses working in a university hospital in the state of Bahia, Brazil participated. Data collection took place through semi-structured interviews and categorized according to Thematic Analysis, using Nvivo software. Results: The prevalent challenges involved: lack of encouragement from the institution for the training of leaders; professional inexperience and young age; resistance to leadership and insubordination. The strategies adopted by nurses consist of acting as team examples and establishing dialogic relationships. Final considerations: The practice of Transformational Leadership has been relevant in the daily lives of nurses and contributes to the quality of care.

Abstract Background Osteoporosis is a major health burden worldwide and require a multi-modal approach including exercise. Fractures have a substantial impact on quality of life (QoL) so exercise programs may to be conducted by trainers able to deal with this condition, because exercise alone, even if in the right dosage, seems to be not enough to improve QoL. A quasi-experimental pilot study performed to determine the feasibility and safety of an Adapted Physical Activity (APA) protocol specifically designed for osteoporotic women with vertebral fractures and its effect on QoL, with a specific focus on trainer's role. Methods The Experimental Group (EG) performed APA protocol 1hour twice a week for 6 months, while the Control Group (CG) received standard care. QoL was assessed at baseline and follow-up with ECOS-16 questionnaire and semi structured interviews to understand women perceptions regarding trainer's role. Using mixed-methods quantitative and qualitative data were analyzed using SPSS 5 and NVivo 12, respectively. Results 40 post-menopausal women (mean age: 67.6±4.6) divided in 2 groups EG = 22 and CG = 18, completed the study. At follow-up, EG improved significantly QoL (EG: -0.5±0.5 vs CG: 0.0±0.3 p = 0.02). The APA intervention had a significant effect on all the components of QoL, as measured by the disease-specific ECOS-16 questionnaire: Physical Component Summary (PCS) (EG: -0.4±0.5 vs CG: 0.0±0.4 p = 0.06) and Mental Component Summary (MCS) (EG: -0.5±0.6 vs CG: 0.1±0.5 p = 0.02). The interviews confirmed this result, emphasizing the key role of the trainer in improving their QoL. The adherence calculated as number of sessions performed compared to the sessions proposed was 75.8%. No injuries were observed. Conclusions The feasibility, the safety, and the positive effect of the proposed exercise protocol on QoL confirmed also by the interviews, show that principles of exercise education applied by trainers should be extended also to patients with other chronic condition. Key messages Trainers specifically educated, could be a means of achieving the goal improving people’s QoL, as they occupy the role of exercise experts and also an educational, relational and support role. The management of osteoporosis is multi-modal and includes exercise, often recommended as a beneficial non-pharmacological treatment to slow the rate of bone loss and improve quality of life.

Objective: Successive reorganizations of healthcare system around the globe have placed enormous pressure on the work of nurse managers (NMs) and this has eroded their quality of work life (QWL). However, little is known about the meaning of NMs’ QWL.Aim: Inspired by Watson’s Human Caring Science perspective, this study aimed to describe and understand the meaning of QWL among NMs working in a affiliated-university hospital.Methods: A descriptive phenomenological method the Relational Caring Inquiry (RCI) was conducted to describe and understand the experiential meaning of QWL. This qualitative method was used to collect and analyze data from two semi-structured interviews with 14 NMs in an affiliated-university hospital in Quebec, Canada.Results: The results have identified the following five Eidos themes to describe and understand the experiential meaning of QWL: (1) actualizing leadership and political skills to improve the quality of nursing care; (2) contextual elements conducive to humanization of the organization; (3) organizational support promoting socioprofessional and personal fulfillment; (4) learning culture within the organization to encourage the development of nursing management skills; and (5) personalized support addressing the specific needs of new NMs. For NMs, the essence of the QWL experience is defined as a socioprofessional emancipation of NMs in their clinical-administrative practice in humanist organizations.Conclusions: Taking an organizational humanization perspective, the results reveal sustainable and practical potential strategies aimed at optimizing QWL implementation programs.

In the present study we conduct a systematic review to evaluate the needs and experience of people with pediatric multiple sclerosis (MS) and their caregivers. The literature search was conducted across 10 academic databases, adhering to PRISMA-P guidelines. Quality appraisal was conducted using the mixed method appraisal test for individual studies, and GRADE-CERQual to establish overall confidence of findings. Results were analyzed using a process of narrative synthesis. We identified 26 studies which included 2253 children/adolescents with MS (CAMS) and 1608 caregivers. MS was reported to negatively impact experiences for CAMS in domains such as of school performance, social relationships, mental health, and overall physical functioning. Specifically, fatigue and social support were reported as the most important barriers and facilitators for CAMS, respectively. In terms of caregiver experience, negative impacts were reported on social functioning, mental health, and quality of life. Additionally, lack of awareness concerning MS was one of the biggest challenges reported. Caregivers expressed needs for psychological and social support. This study provides the first evidence regarding the needs and experiences of CAMS and their caregivers. Findings can be used to address policy gaps for supporting families affected by pediatric MS.

A incorporação de tecnologias duras pela equipe enfermagem em terapia é imprescindível em termos gerenciais,de controle, de vigilância e de cuidados prestados aos pacientes, necessitando, portanto, de suporte organizacionalpara a realização de um trabalho livre de riscos. Objetivos: Analisar a percepção dos trabalhadores de enfermagemsobre a incorporação das tecnologias duras ao trabalho em terapia intensiva e as implicações para a saúde dogrupo. Método: Qualitativo descritivo, tendo como campo uma unidade de tratamento intensivo de um hospitaluniversitário situado no município do Rio de Janeiro (Brasil). Utilizou-se a técnica de entrevista semiestruturada com25 trabalhadores de enfermagem em 2011. Aplicada a análise de conteúdo aos depoimentos. Resultados: Na visãodos trabalhadores a tecnologia dura é essencial na assistência ao paciente crítico por possibilitar maior controledo quadro clínico, prevenir complicações e facilitar o trabalho da equipe. Fatores como a ausência de manutençãopreventiva dos aparelhos acarretam estresse ocupacional devido à possibilidade de erros e efeitos adversos aopaciente. ABSTRACTThe incorporation of hard technologies by the nursing staff is imperative in terms of management, control,supervision and care to patients requiring, organizational support for conducting work free of risks. Objectives:Aimed both in this study was to analyze the perception about the incorporation of hard technologies by thenursing staff in the intensive care unit and to examine the repercussions to the health of that group. Method: Thequalitative descriptive method was applied at a intensive care unit in a university hospital in the municipalityof Niterói (Brazil). Semi-structured interviews of 25 nursing workers were conducted in 2011. Apply the contentanalysis to speaking. Results: Nursing workers view hard technology as essential to critical patient care by virtueof its affording greater control of their clinical condition, preventing complications and facilitating the nursingteam’s work. Factors such as lack of preventive maintenance of apparatus, result in occupational stress due to thepossibility of errors and adverse effects on patients. Conclusions: It was concluded that hospital risk managementis needed to safeguard quality of care, safety, wellbeing and team satisfaction.Keywords: Nursing; Intensive care units; Occupational risks; Workers’ health.

Ehlers-Danlos Syndrome (EDS) is a connective tissue disorder that results in negative health events. It also holds potential for periodic or permanent disability and psychosocial stress. While a diagnosis of EDS can occur at any age, the psychosocial effects of an EDS diagnosis can be especially challenging when symptoms present during postsecondary education. This qualitative study examined the psychosocial effects of EDS on the quality of life of Ontario postsecondary students (n=11). After a brief online eligibility survey, one-to-one semi-structured interviews were conducted. Thematic analysis of interview data revealed that emotional factors such as anxiety and depression, faculty and peer attitudes including issues of judgment and stigmatization, and feelings of frustration affect the quality of life of postsecondary students living with EDS.

The aim of this study was to evaluate agreement between nurses’ and physicians’ opinions regarding aggressiveness of treatment and to investigate and compare the rationales on which their opinions were based. Structured interviews regarding 714 patients were performed on seven general wards of a university hospital. The data gathered were then subjected to qualitative and quantitative analyses. There was 86% agreement between nurses’ and physicians’ opinions regarding full or limited treatment when the answers given as ‘uncertain’ were excluded. Agreement was less (77%) for patients with a life expectancy of less than one year. Disagreements were not associated with professional status because the physicians considered limiting life-sustaining treatment as often as the nurses. A broad spectrum of rationales was given but the results focus mostly on those for full treatment. The nurses and the physicians had similar bases for their opinions. For the majority of the patients, medical rationales were used, but age and quality of life were also expressed as important determinants. When considering full treatment, nurses used quality-of-life rationales for significantly more patients than the physicians. Respect for patients’ wishes had a minor influence.

Abstract Background Although Jordan has made progress in meeting Family Planning (FP) needs in last decades, recently the use of FP methods has declined significantly. Women’s personal experiences, knowledge, and perceptions of how a FP method might impact their quality of life (QoL) can influence FP decisions. However, a lack of comprehensive understanding of the impact of modern FP methods on women’s QoL continues to exist among Jordanian couples. Therefore, this study aimed to investigate the relationship between the use of common modern FP methods and QoL among Jordanian women. Methods Using the WHOQOL-BREF questionnaire along with other questions, non-pregnant women of reproductive age were interviewed at their homes through face-to-face structured interviews. Women who visited the obstetrics and gynecology clinic of King Abdullah University Hospital for contraceptive advice and follow-up consultations were also included. Results A total of 548 women aged between 18 and 49 participated in the study. Based on the WHOQOL-BREF scale, the overall mean (SD) scores of the four domains were found to be average. Our findings show that women who used Intra Uterine Devices (IUDs) and women whose husbands used condoms had better QoL in the four domains (physical health, psychological health, social relationships, and environment) than those who used Oral Contraceptives (OCs). Women who used implant and injectable hormonal contraceptives had better QoL in terms of the physical health and social relationships domains. In contrast, women who had undergone permanent sterilization had lower QoL scores in all of the four domains. Further analysis revealed that women who had undergone tubal sterilization were less satisfied overall and more likely to experience side effects than women who used OCs. Conclusion The choice to use contraceptives and decide freely whether and when to have children is regarded as a fundamental reproductive health right and is strongly linked to women’s health and QoL. Women who use OCs and women who have undergone permanent sterilization are likely to have lower QoL than women who use IUDs or implant and injectable hormones and those whose husbands use condoms.

Abstract Purpose In Germany, numerous inpatient rehabilitation facilities employ migrant physicians. Yet the remote location of many such institutions makes it difficult for migrant physicians to establish social relationships and networks near their place of employment. This lack of social resources has implications for employee retention, quality of life, and quality of care. In this light, our study examines the following questions: what social relationships do migrant doctors have access to? What challenges and opportunities do these relationships represent? Our aim is to provide concrete recommendations for stakeholders to improve the social integration of migrant physicians. Methodology Semi-structured interviews with 58 migrant physicians and their colleagues. Braun & Clarke thematic analysis. Results Migrant physicians' social relationships fall into four broad categories: friends and acquaintances in general, family, community of origin, and working team. Migrant physicians in our sample face difficulties in each of these four categories. Implications Establishing social relationships in rural and semi-urban areas is more difficult for migrants in general, and migrant physicians in particular. We propose a number of recommendations to overcome these challenges on several levels: individual, work team, institution, regional, and trans-regional. Key messages The integration of migrant physicians in inpatient rehabilitation facilities is challenging. Specific measures can help the retention of migrant physicians.

Patient satisfaction is an important quality outcome indicator of health care in the hospital setting. Patients are nowadays care-partners with caregivers. This relation is practically important for patients presenting chronic diseases, especially cancer. The aim of this study is to evaluate cancer patient satisfaction, treated at the Department of Radiotherapy in Hassan II University Hospital, and to study the different components of this satisfaction. A prospective transversal study was conducted between December 2016 and January 2017. Data was collected by three investigators based on structured interviews, a validated, anonymous and a standardized questionnaire. During this period, we have included 230 patients: 159 women (69.1%), 71 men (30.9%) and the sex ratio (women/men) was 2.23. Half of the investigated patients have never been schooled (52.61%). The majority had urban origin (71.3%) and 90% of patients declared being satisfied with the care at Radiotherapy department. 93.48% of cases recommended Radiotherapy department to other patients and 95.65% will want to continue their treatment at this department. Reception conditions were judged as favorable in 92.14%. Satisfaction rates regarding the availability of medical and paramedics, health-care workers were 86.52% and 83.9%, respectively. The quality of medical and paramedical care was judged as excellent or good in 78% cases. However, 44.34% of patients complained about the complexity of administrative formalities. 60.87% of cases judged that the waiting time was too long, whereas 31.4% of patients claimed that care-quality of their pain was insufficient or bad. The majority of patients declared being very satisfied or at least satisfied with different care services. For items that were judged as less satisfactory, some recommendations will be taken especially at the level of pain’s and palliative care as well as the organization of patients’ circuit inside the department. The satisfaction’s variations can be attributed to personally patients factors as well as systemic ones at the level of the hospital. Assessing and understanding these factors are essential in developing appropriate measures to improve patient satisfaction.
