Introduction. Mediastinitis is a deadly surgical site infection (SSI) after cardiac surgery. Although rare, mortality is as high as 47%. Best practices for infection prevention to eliminate this deadly complication must be identified. Surgical dressings impregnated with silver have been shown to reduce SSIs in other surgical specialties. This aim of this study is to determine if the routine use of silver surgical dressings is beneficial to prevent mediastinitis after cardiac surgery. Methods. A single-center retrospective study was performed on patients who underwent sternotomy from 2016 to 2018 at the University of Kansas Medical Center. Prior to June 2017, all cardiac surgical patients were treated with gauze surgical dressings and is designated Group A. The routine use of silver-impregnated surgical dressings was implemented in June 2017, patients after this change in practice are designated Group B. Patient characteristics and rates of deep and superficial sternal wound infections (SWI) were compared.  Results. There were 464 patients in Group A and 505 in Group B. There were seven SWIs in Group A (7/464, 1.5%) and five in Group B (5/505, 1%; p = 0.57). Of these, there was one deep SWI per group (p = 0.61) and six superficial SWIs in Group A compared to four in Group B (p = 0.74). Severe COPD was higher in Group A (p = 0.04) and peak glucose was higher in Group B (p = 0.02).  Conclusions.The analysis conferred no benefit with silver-impregnated surgical dressings to prevent mediastinitis. Choice of gauze surgical dressings may be preferable to reduce cost.

Background and objective: Intrauterine growth restriction (IUGR) may be associated with significantly higher aortic intima–media thickening (aIMT) values. It is unknown if fetal aIMT is associated with glomerulosclerosis and amniotic albuminuria in utero. Design, setting, participants, and measurements: Fetal abdominal aIMT and amniotic albumin/creatinine ratio (ACR) were measured in 126 individual twin fetuses, recruited by the Obstetrics and Gynaecology Clinics of the University of Padua (Italy) Medical Center. The IUGR twin fetuses were classified into two groups: Group A were those fetuses whose estimated fetal weight (EFW) was <10th percentile with pulsatility index >2 SD and Group B were those fetuses whose EFW was <10th percentile and had no velocimetry abnormalities. Results: The median fetal aIMT was significantly different in the three groups (Group A = 0.9 mm; Group B = 0.7 mm; and appropriate for gestational age (AGA) = 0.5 mm; p < .0001). It was significantly higher in Group A than in the AGA group (p < .0001) and than in the Group B fetuses (p = .003), respectively. In addition, ACR was different in the three groups (Group A = 183,500 mg/g; Group B = 6,4720 mg/g; and AGA = 8,2750 mg/g; p = .0002). It was significantly higher in Group A than in the AGA group (p = .03) and than in Group B (p = .02), respectively. Conclusions: Growth-restricted twin fetuses with velocimetry abnormalities present are associated with aIMT and higher ACR levels in amniotic fluid, which could be possible markers in utero of preclinical atherosclerosis, and early glomerulosclerosis.

The concept of limit has a long history of being difficult for many students. This researcher attempted to find some better ways of helping the student acquire a clearer concept of limits. The problem was one of determining whether experience with limits in high school would help when future contacts with limits were encountered. Three groups of students were formed from freshmen enrolled in mathematics at the University of Kansas. Group A had studied in high school a given number of theorems on limits. Group B had had no work in limits but had had some analytic geometry in high school, while Group C had had neither of the above experiences.

The Rosenzweig P — F Study was administered to a group of South African Indian students (N = 403) from the University of Durban-Westville with slight modifications in administration. The subjects were divided into three groups and were instructed to react to Blacks in Group A, to Whites in Group B, and to Indians in Group C. The results indicated that the subjects differed in their responses to the three racial groups revealing predominantly intropunitive and impunitive responses to Blacks, and extrapunitive responses to Indians.

Background: The use of blood products is often necessary to prime the extracorporeal circuits for pediatric cardiac surgical patients. The use of blood products carries serious risks both in the acute and long-term aspects of patient care. However, excessive hemodilution during cardiopulmonary bypass (CPB) is associated with decreased oxygen carrying capacity, edema, and organ dysfunction. Especially, whether brain dysfunction after transfusion-free cardiac surgery on CPB occurred is unknown. Objective: This study aims to investigate whether excessive hemodilution affects child’s mental development. Methods: We studied 143 infants (24.2 +/− 15.8 months, 5 months ~ 3 years of age) had performed surgical closure of ventricular septal defect (VSD) in less than 12 months (4.8 +/− 2.6 months) of age in 7 children’s hospitals. Questionnaires mailed to the parents included the Tsumori-Inage Infant Developmental Scale estimating subjects’ behavioral developmental level as Developmental Quotients (DQ). The infants were divided into 3 groups; transfusion-free (Group A, n=52), transfusion during CPB (Group B, n=24) and transfusion after CPB (Group C, n=66). Results: Lower DQ was found for Group A (94.7 +/− 11.6) than Group B (103.5 +/− 16.0, p=0.008) and Group C (101.7 +/− 17.9, p=0.016). No correlation was found between DQ and minimum hemoglobin level during CPB (min-Hb) in Group A. The DQ was likely to be lower in the infants with below 6 g/dl (93.0 +/− 13.0) than with over 6 g/dl (98.7 +/− 8.3. p=0.10) of min-Hb. Conclusions: The results suggest that the infants after transfusion-free surgery of VSD on CPB delay in the mental development in their early childhood. Maintaining minimum hemoglobin level over 6 g/dl during CPB seems to prevent the mental development from delay.

The flipped classroom, as an active learning model, has given remarkable results in several areas in the university teaching; however, its execution is still able to improve. This research shows the implementation and improvement of the flipped classroom model in the course of Pavements. It evaluates their influence on the students’ final grades and their learning experience. Three groups of students participated in this study, who enrolled in the course of Pavements in the Civil Engineering. Group A took the course with the traditional model, while Group B took it with a flipped classroom, and Group C experienced it with a reinforced flipped model. Groups did the course the subject in 2017, 2018 and 2019, respectively. Results show that even though with the flipped classroom models, the finals grades did not increase compared to the scores of the traditional model; however, it improved their learning experience. The students were more satisfied with the method; they even asked for fewer modifications than they did in the traditional model. This research shows that adding little academic things to the course, it would greatly influence their students' opinion.

COVID-19 respiratory failure is a life-threatening condition. Oxygenation targets were evaluated in a non-ICU setting. In this retrospective, observational study, we enrolled all patients admitted to the University Hospital of Genoa, Italy, between 1 February and 31 May 2020 with an RT-PCR positive for SARS-CoV-2. PaO2, PaO2/FiO2 and SatO2% were collected and analyzed at time 0 and in case of admission, patients who required or not C-PAP (groups A and B) were categorized. Each measurement was correlated to adverse outcome. A total of 483 patients were enrolled, and 369 were admitted to hospital. Of these, 153 required C-PAP and 266 had an adverse outcome. Patients with PaO2 <60 and >100 had a higher rate of adverse outcome at time 0, in groups A and B (OR 2.52, 3.45, 2.01, respectively). About the PaO2/FiO2 ratio, the OR for < 300 was 3.10 at time 0, 4.01 in group A and 4.79 in group B. Similar odds were found for < 200 in any groups and < 100 except for group B (OR 11.57). SatO2 < 94% showed OR 1.34, 3.52 and 19.12 at time 0, in groups A and B, respectively. PaO2 < 60 and >100, SatO2 < 94% and PaO2/FiO2 ratio < 300 showed at least two- to three-fold correlation to adverse outcome. This may provide simple but clear targets for clinicians facing COVID-19 respiratory failure in a non ICU-setting.

<p>Research on how to counter phishing from a user behavior perspective has been explored for over a decade, yet the prevalence of such threats is increasing. This thesis aims to provide a modern perspective by considering if there is a difference in how susceptible an individual is on a mobile device versus a desktop email client. Currently very few studies consider phishing on mobile devices and the research is unclear as to the potential difference in susceptibility rates between the two device types. Initially a review of 60 phishing emails received by the university that had passed mail filtering were used to assist in the design of the messages to be used in the second stage of the study. Following this a simulated phishing attack on two groups in one unit of professional administrative staff in the university (141 in total with 71 in Group A and 70 in Group B) within the university was undertaken. The defining characteristic between the groups was how they responded to a message with a ‘loss versus gain’ appeal. This area has received limited exploration in the research and findings remain unclear. This study found that people were statistically far more susceptible to the ‘gain’ message of a free coffee at 28.2% than the ‘loss’ message of Office365 account suspension 7.1% . For device type there appears to be no statistically significant difference, even between the groups. This study highlights the complexities of device usage around phishing, that have not been clearly highlighted in previous studies, such as people viewing emails with one device and falling victim on another device.</p>

Compressed/interactive video is a relatively new format for providing preservice and inservice courses to rural and/or remote areas (Willis, 1993). At the University of Kansas Medical Center only two programs, those of Nursing and the Deaf Education, have engaged in this format of distance education. While the methodology was expected to assist graduate students in areas of the state who could not travel to participate in courses on one of the three urban campuses, it was also expected to challenge instructors to teach effectively while still encouraging active learning activities. Surveys of questions to rural and urban students were given to 13 graduate students in two courses in the fall of 1992, and to three additional students in the fall of 1993. Analyzed results documented the success of the compressed/interactive (ITV) video teaching format for this small group of master level students in Kansas.

This is the fifth of a six-part series on diversion of controlled substances in an acute health care setting. The six articles are meant to accompany the recommendations in the Hospital Pharmacy article “Controlled Substance Diversion Detection: Go the Extra Mile” for a comprehensive analysis of all aspects of controlled substance handling. 1 The series focuses on preventing diversion from automated dispensing cabinets (ADCs), the operating room, and at all handling points within a pharmacy. The objective of the series is to provide practical recommendations to aid pharmacy managers as they seek to prevent or detect diversion. Experience is key to diversion detection; however, a pharmacy should not have to experience its own diversion in order to learn from it. The collective experience of pharmacy leaders at the Ohio State University Medical Center and the University of Kansas Hospital are gathered to assist other pharmacy leaders in the hopes that they do not have to gain this experience firsthand. This article will describe the features of one popular reporting software package along with the experience that the University of Kansas Hospital has had in using this software. This description is an attempt to demonstrate the value of an add-on software package and to encourage its use by more hospital pharmacies.
