Comparison of the Antibacterial Effect of Modified 3-mix Paste versus Ultrapex over Anaerobic Microorganisms from Infected Root Canals of Primary Teeth

Objective: The aim of this study was to evaluate in vitro the antimicrobial efficacy of a modified 3-mix paste and to compare it with an iodoform paste (Ultrapex) against anaerobic microorganisms isolated from root canals of infected or necrotic primary teeth. Study design: An in vitro experimental assay was performed over isolated and identified anaerobic microorganisms of 21 samples, in order to compare the antimicrobial ability of both root canal filling materials, using a disc-diffusion method. Results: A total of 21 microbial samples (15 polymicrobial and 6 monomicrobial) were obtained, from which 19 different strains were identified. Modified 3-mix paste showed an excellent antimicrobial effect against most of both kinds of microbial samples, although some of them exhibited resistance; on the other hand, Ultrapex showed only minimal antimicrobial ability (null or low categories). Clostridium ramosum exhibited the most resistance to both materials. Conclusion: The bactericidal effect of the modified 3-mix paste was superior to Ultrapex, with a statistically significant difference, against anaerobic microorganisms isolated from infected root canals of primary teeth.

The aim of this study was to investigate, in vitro, the percentage of filling of simulated lateral canals in teeth obturated with TP medium and standardized gutta-percha points. Twenty human mandibular canines were prepared with LA Axxess (SybronEndo) and K³ Endo rotary system (SybronEndo) up to a #50 file, according to the Free Tip Preparation Technique. During instrumentation, the root canals were alternately irrigated with 1% sodium hypochlorite and 17% EDTA. Six artificial lateral canals were prepared at the apical third of each tooth. Then, the teeth were assigned to two groups (n=10): Group 1 - filled with TP medium master gutta-percha points (Dentsply, Mailleffer); Group 2 - filled with standardized master gutta-percha points (Dentsply, Mailleffer). Root canal filling was complemented with AH Plus sealer (Dentsply, Mailleffer) and accessory gutta-percha points (Dentsply, Mailleffer), according to the classic technique. The teeth were radiographed and the images obtained were digitized. Linear measurements of the percentage of filling of the artificial lateral canals in each group were accomplished on the Image Tool 2.02 software. Statistical analysis of the data using Mann-Whitney U non-parametric test evidenced significant difference (p<0.01) between the experimental groups. The group obturated with TP medium points yielded higher percentage of filling of the lateral canals. It may be concluded that the use of master gutta-percha points with larger taper resulted in better filling of the simulated lateral canals, as compared to the use of standardized master gutta-percha points.

Recurrent endodontic infections of treated teeth are mainly linked to microbial persistency in the root canal system. Enterococcus faecalis is a bacterium that is often identified in these recurrent infections. Calcium hydroxide (Ca(OH)2) is a widely used root canal medicament and is now being developed to be produced from natural materials. Ca(OH)2, which is synthesized from natural limestone from Palimanan, Indonesia, has a structure similar to Ca(OH)2, which is often used as a root canal medicament. The purpose of this study was to determine the antimicrobial property of Ca(OH)2 paste synthesized from natural limestone compared to conventional Ca(OH)2 paste. This study is an in vitro experimental study consists of 3 groups (n=24, namely, conventional Ca(OH)2 paste, limestone synthesized Ca(OH)2 paste, and saline as the negative control, for 2 intervals of time and evaluated by Total Plate Counting (TPC) method. Samples were mandibular first premolar root canal prepared by root canal preparation and contaminated with E. faecalis (ATCC 29212). The infected root canals of each group were treated by the medicaments, and samples from the root canals were taken on day 7 (n=4) and 14 (n=4). Results showed that both Ca(OH)2 groups had antimicrobial activities against E. faecalis. The limestone Ca(OH)2 paste group had the least amount of bacterial colonies on the 7th day compared to other groups. In contrast, on the 14th day, the conventional Ca(OH)2 paste had the least bacterial colonies. It can be concluded that the Ca(OH)2 paste synthesized from natural limestone has an antimicrobial effect on E. faecalis, the antimicrobial effect was higher on the 7th day but decreases on the 14th day, whereas the conventional Ca(OH)2 had a higher antimicrobial effect on day 14 compared to day 7. Further studies are expected to improve the long-term antimicrobial effectiveness and sustainability of the natural limestone synthesized Ca(OH)2 paste.

ABSTRACT Objective: To evaluate the antibacterial activity of four formulations of calcium hydroxide paste against microorganisms commonly found in infected root canals. Methods: To evaluate antibacterial activity through the agar diffusion method, pastes of calcium hydroxide were made from its pro-analysis form, diffused into four separate vehicles: distilled water, camphorated p-monochlorophenol, propylene glycol and Otosporin®, testing the antimicrobial activity of these on strains of Staphylococcus aureus, Bacillus subtilis and Enterococcus faecalis. After the incubation period, the presence or otherwise of inhibition zones were observed and their sizes in three stages: 24h, 48h and 72h. With this data, the median between the four dishes was obtained and the consequent value was submitted to Kruskal-Wallis nonparametric statistical analysis, with post-tests of Mann-Whitney and Bonferroni correction, at a significance level of 5%. Results: Only pastes with camphorated p-monochlorophenol and Otosporin vehicles caused the formation of significant inhibition zones, with medians of 8.0 mm. Against the strains of Enterococcus faecalis, only pastes with the camphorated p-monochlorophenol vehicle resulted in the formation of significant inhibition zones, with a median of 3.0 mm. Conclusion : Otosporin and CMCP vehicles provide greater antimicrobial potential to calcium hydroxide against the studied bacteria. However, only the Ca(OH)2 and CMCP combination was effective against all the strains, and can thus be regarded as the paste formulation with the greatest antimicrobial effectiveness in this study.

Aim: This experimental study in vitro aimed to compare the disinfection of the primary teeth root canals system using the rotary and reciprocating file system. Methodology: Forty-eight root canals from twenty-four primary molar teeth were contaminated with the standard strain of Enterococcus faecalis and randomically splitted into 4 groups (n=12). The Group PN comprised the root canals prepared with the ProTaperTM  Next rotary system. The Group WOG was defined by the usage of WaveOneTM Gold reciprocating system. The files from both systems were thermo mechanical processed. The Group PU used the ProTaperTM Universal system, with no thermic treatment, and the Group C (negative control) received no treatment. Samples from the root canals were collected with sterile paper points before and after its preparation, diluted and distributed in plates with Brain Heart Infusion (BHI) agar. The bacterial colonies were counted and the results were statistic analyzed by Wilcoxon and Kruskal-Wallis (Dunn) tests. Results: There was significant bacterial reduction using the systems (p>0,01). However, there was no significant difference among them (p<0,05) (PN=3.38; WOG=3.85; PU=3.26). Conclusion: Both systems (rotary and reciprocating) provide disinfectionon the primary molar teeth root canals system.

ABSTRACT Aim To compare the accuracy of apex locators with conventional techniques in determining the root canal length in primary teeth. Materials and Methods This in vitro study was conducted on twenty extracted primary incisors. After endodontic access preparation, root canals were irrigated with physiological saline solution. The access cavities were dried with cotton pellets and, the roots were dried with paper points before performing the electronic measurement. One operator determined the actual working length (AWL) of the canal and another operator measured working length using conventional F-speed radiographs, digital radiography (Dr suni plus digital radiography system, model no 900-0540) and electronic apex locator- Raypex 5 (VDW GmbH Postfach) 5th generation apex locator. The mean value of differences was obtained and statistical analysis was performed using analysis of variance (ANOVA) and the paired t-test. Results There was no statistically significant difference between tooth lengths estimate on conventional, digital and apex locators. 0.5 to 1 mm of clinically acceptable discrepancy was found between three techniques. Conclusion Although there was no significant statistical difference, electronic apex locator (EAL) has more advantage over other techniques, it is considered accurate in determining the working length in primary teeth. Aim. The purpose of this in vitro study was to assess the antimicrobial efficacy of Biopure MTAD against E. faecalis in contaminated root canals. Materials and Methods. Forty-two single rooted extracted human teeth were inoculated with E. faecalis and incubated for four weeks. The samples were divided in two control and five experimental groups irrigated with 1.5% sodium hypochlorite solution (NaOCl); 3% NaOCl; BioPure MTAD; 1.5% NaOCl/17% EDTA; or 3% NaOCl/17% EDTA. After a one-week incubation, complete disinfection was confirmed by the absence of turbidity in the incubation media. Dentin shavings were taken from samples with no turbidity to verify whether E. faecalis was present in dentin tubules. Results were analyzed statistically using Fisher's exact test, with the level of significance set at . Results. Statistical analysis of the data obtained at Day 7 and after dentin shaving analysis showed that BioPure MTAD had significantly greater antibacterial activity than 1.5% NaOCl, 1.5% NaOCl/17% EDTA and 3% NaOCl/17% EDTA. No significant difference was detected between MTAD and 3% NaOCl. Conclusions. These findings suggest that BioPure MTAD possesses superior bactericidal activity compared with NaOCl and EDTA against E. faecalis.

The aim of the study was to investigate the leukocyte- and platelet-rich plasma (L-PRP) antimicrobial activity. The studied sample comprised 20 healthy males. The L-PRP gel, liquid L-PRP, and thrombin samples were testedin vitrofor their antibacterial properties against selected bacterial strains using the Kirby-Bauer disc diffusion method. Two types of thrombin were used (autologous and bovine). Zones of inhibition produced by L-PRP ranged between 6 and 18 mm in diameter. L-PRP inhibited the growth ofStaphylococcus aureus(MRSA and MSSA strains) and was also active againstEnterococcus faecalisandPseudomonas aeruginosa. There was no activity againstEscherichia coliandKlebsiella pneumoniae. The statistically significant increase of L-PRP antimicrobial effect was noted with the use of major volume of thrombin as an activator. Additionally, in groups where a bovine thrombin mixture was added to L-PRP the zones of inhibition concerning MRSA,Enterococcus faecalis, andPseudomonas aeruginosawere larger than in the groups with autologous thrombin. Based on the conducted studies, it can be determined that L-PRP can evokein vitroantimicrobial effects and might be used to treat selected infections in the clinical field. The major volume of thrombin as an activator increases the strength of the L-PRP antimicrobial effect.
