Aim. To evaluate the serum level of apelin-12 in patients with bronchial asthma. Methods. 11 healthy controls and 29 patients with bronchial asthma were examined (among them 19 with atopic asthma and 10 with non-atopic asthma). Serum apelin-12 levels were measured by enzyme immunoassay (ELISA, MyBioSource, USA). SPSS (Statistical Package for the Social Science, V. 13) for Windows was used for statistical analysis. Results. The phenomenon of low apelin-12 serum levels was revealed in patients with bronchial asthma. The most significant decrease of apelin-12 levels was noticed in atopic bronchial asthma. The apelin-12 levels were significantly correlated with a number of indices of bronchial obstruction. Apelin-12 levels correlated with a number of indicators characterizing the grade of bronchial obstruction. Moreover, low apelin-12 levels were suggesting bronchial obstruction. Besides, significant reverse correlation of apelin-12 levels and allergic sensitization was revealed. In addition, statistically significant relation between apelin-12 serum level and peripheral blood eosinophil and lymphocyte counts was revealed. There was an inverse association between apelin-12 serum level and peripheral blood neutrophil count. Conclusion. Literature analysis and our own results allow us to suppose the possible protective role of apelin-12 in bronchial asthma flare. Further studies of apelin-12 and of other isoforms of adipokines are needed for better understanding of the disease.

Background: Pulmonary hemodynamic disorders depend on the inflammatory phases and severity of the obstructive syndrome. However, the effect of asthma bronchial obstruction on the state of peripheral hemodynamics remains insufficiently known. Aims: To study the effects of airway obstruction on skin blood flow parameters and its regulatory systems in patients with persistent atopic bronchial asthma in the remission state.Materials and methods: A comparative study of the skin peripheral blood flow in patients with bronchial asthma with severe airway obstruction (1st group) and without obstruction (2nd group) was conducted. 20 patients with confirmed diagnosis of atopic asthma of 50–74 years old participated in the study. All patients received basic therapy in a constant dosing of high doses of inhaled glucocorticosteroids/long-acting beta-2-agonists. The control group included 20 healthy volunteers without evidence of bronchial obstruction. The study lasted for 3 months. The forced expiratory volume in 1 s (FEV1) was used to evaluate the bronchial obstruction by spirometry technique. Skin blood perfusion changes were recorded by laser Doppler flowmetry at rest and in response to short-term local ischemia. Registered peripheral blood flow signals were examined using the amplitude temporal filtering in five frequency intervals to identify the functional features of the peripheral blood flow regulation systems. Results: Consistent two-fold decrease of the oscillation amplitudes was found in the neurogenic interval at rest (p=0.031), as well as in the myogenic (p=0.043; p=0.031) and endothelial intervals (p=0.037; p≤0.001) both at rest and during the postocclusive reactive hyperemia respectively in the 1st group of patients with bronchial obstruction (FEV1 80%) compared with the control group. No significant changes were revealed for skin blood flow parameters in the 2nd patient group (without obstruction, FEV1 80%) in comparison to control subjects.Conclusions: The presence of bronchial obstruction has a significant impact on the changes of the amplitudes of skin blood flow oscillations in patients with bronchial asthma in the myogenic, neurogenic and endothelial intervals.

Background: Several studies have suggested that vitamin D (VD) deficiency (VDD) is associated with atopic dermatitis (AD). However, little is known about the relationship between AD and vitamin A (VA). The interaction between VA and VD on AD requires further study. Objective: We detected serum levels of VA and VD in children with AD to explore how VA deficiency (VAD) and VDD affect AD severity. Methods: We assessed the SCORing Atopic Dermatitis (SCORAD) index, total immunoglobin E levels and peripheral blood eosinophil counts. VA and VD levels were determined with high-performance liquid chromatography. Correlations among variables were investigated with Pearson's correlation analysis. Results: The VD and VA levels were significantly lower in children with AD than in normal children (p < 0.001, p = 0.0423). Both VD and VA levels were negatively correlated with SCORAD scores. The SCORAD scores were significantly higher in AD patients with both VDD and VAD (co-deficiency) than in other AD patients. Significant inverse correlations were observed between peripheral blood eosinophil counts and serum VA and VD levels. Conclusions: VA and VD co-deficiency may exacerbate AD symptoms in children, but the specific mechanism underlying this relationship requires further study. These findings may indicate the need for studies evaluating the use of VD and VA as potential treatments for AD patients.

AbstrakAsma merupakan penyakit gangguan inflamasi kronis saluran pernapasan dengan banyak sel serta elemen seluler yang berperan. Asma dihubungkan dengan hiperresponsif bronkus, keterbatasan aliran udara dan gejala pernapasan yang bersifat reversibel. Salah satu sel yang diketahui berperan besar dalam patogenesis asma adalah eosinofil. Eosinofil melepaskan berbagai mediator seperti Major Basic Protein, Eosinophil Cationic Protein, peroksidase eosinofil, leukotrien C4, serta Platelet-Activating Faktor yang akan merusak epitel saluran napas serta menyebabkan peradangan. Penelitian ini dilaksanakan pada bulan Januari sampai Desember 2012 di Bagian Rekam Medik RSUP Dr. M. Djamil Padang dengan tujuan untuk melihat gambaran jumlah eosinofil darah tepi pada penderita asma bronkial yang dirawat inap. Penelitian yang dilakukan bersifat deskriptif dengan tinjauan retrospektif. Penelitian dilakukan dengan melihat data rekam medis penderita asma bronkial yang dirawat inap di Bangsal Paru RSUP Dr. M. Djamil Padang periode Januari 2010-Desember 2012. Dalam periode dua tahun tersebut, diperoleh 18 sampel yang digunakan pada penelitian ini. Hasil yang diperoleh ialah 4 penderita memiliki jumlah eosinofil darah tepi normal (40-400/mm3) (22,22%) dan 14 penderita memiliki jumlah eosinofil darah tepi dibawah normal (<40/mm3) (77,78%). Tidak ditemukan penderita asma dengan eosinofilia (>400/mm3). Jumlah eosinofil darah tepi pada penderita asma bronkial lebih banyak dibawah normal.Kata kunci: Asma Bronkial, EosinofilAbstractAsthma is a chronic inflammatory disorder of the airways in which many cells and cellular elements play a role. Asthma is associated with hyperresponsif bronchi, airflow limitation, and respiratory symptomps that are reversible. One of cells which is known to play a major role in asthma pathogenesis is eosinophil. Eosinophil release a variety of mediators such as Major Basic Protein, Eosinophil Cationic Protein, Eosinophil Peroxidase, Leukotriene C4, and Platelet Activating Factor that will damage airway epithelium and cause inflammation. The research was conducted in January 2012 through December 2012 at Medical Records Department of RSUP Dr. M. Djamil Padang aiming to see the overview of peripheral blood eosinophil number in patient with bronchial asthma who are hospitalized. The research conducted is using descriptive retrospective review. The study is done by looking medical record data of bronchial asthma patient hospitalized in pulmonary ward of RSUP Dr. M. Djamil Padang in the period of January 2010-December 2012. In the two year period, there are 18 patients which are used in the study. The results are 4 patients had normal peripheral blood eosinophil number (40-400/mm3)(22,22%), 14 patients had low peripheral blood eosinophil number (<40/mm3)(77,78%). There is no patient with eosinophilia (>400/mm3). Peripheral blood eosinophil number in bronchial asthma patient is mostly low.Keywords: Bronchial Asthma, Eosinophil

Abstract Background The Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines define COPD as a disease state characterized by airflow limitation that is not fully reversible, is usually progressive, and is associated with an abnormal inflammatory response of the lungs to inhaled noxious particles or gases. Objective To identify outcomes of patients with eosinophilic COPD exacerbations requiring hospital admission. Patients and Methods This study is a prospective cohort study that was conducted on two groups of total 60 patients recruited from Ain Shams University hospitals between October 2019 and July 2020. Group 1: Eosinophilic COPD exacerbation if the peripheral blood eosinophil on admission is ≥ 200 cells/µL and/or ≥2% of the total leukocyte count Group 2: Non-eosinophilic COPD exacerbation if the peripheral blood eosinophil on admission is &lt; 200 cells/µL and/or &lt; 2% of the total leukocyte count. Results There was significant high diagnostic performance in predicting readmission at 6-month among eosinophilic group. Eosinophils count, percent (%) and NLR cutoff points had high characteristics (highest in NLR ≥3.1 at discharge) in predicting readmission at 6-month among eosinophilic group. Diagnostic performance of Eosinophils count, percent (%) and NLR were assessed. Eosinophils count, percent (%) and NLR had significant high diagnostic performance in predicting readmission at 6-month among eosinophilic group. Eosinophils count, % and NLR cutoff points had high characteristics (highest in NLR ≥2.1 at discharge) in predicting readmission at 6month among non-eosinophilic group. Conclusion Eosinophils can be used as a prognostic marker in non-infective COPD exacerbations. Validity of eosinophil count and percent as a prognostic parameter in COPD exacerbation can be increased by combining with other parameters for example NLR.

Aim: To observe the association of peripheral blood eosinophil percentage in patients with allergic nasal polyps. Design: Descriptive cross-sectional study Place and duration: Pathology Department of Bakhtawar Amin Medical & Dental Hospital, Multan from September 2020 to August 2021. Methodology: Blood samples of all the cases operated for nasal polyps in Bakhtawar Amin Trust Institute are drawn before surgery to look for eosinophils. Family and past history of allergy is recorded. Histopathology of all the operated specimen of nasal polyps is done and sample for eosinophil count is collected again in cases that proved to be allergic nasal polyps on microscopic examination to look for any alteration in eosinophil percentage in blood on excision of polyps. Results: Twenty nine out of forty (72.5) percent of patients with allergic nasal polyps reveal increase in peripheral blood eosinophil percentage that returned to normal in 26(65%) patient on excision of nasal polyps. Conclusion: The study disclosed a notable link between allergic nasal polyps and peripheral blood eosinophil percentage and this association is further enhanced by the fact that the blood eosinophil count returned to normal on removal of nasal polyps. Objective: This study aimed to explore the usefulness of the peripheral blood eosinophil count (PBEC) in assessing the level of fractional exhaled nitric oxide (FeNO) and predicting bronchodilation test results. Methods: We retrospectively analyzed the data of 384 outpatients who underwent FeNO measurement at our Department of Respiratory and Critical Care Medicine from March to June 2019. The FeNO level was compared among different PBECs to explore the association among them. Furthermore, the sensitivity and specificity of PBECs in predicting bronchodilation test results were assessed by using receiver operating characteristic (ROC) curve analysis. Results: There was a moderate correlation between PBECs and FeNO levels (r = 0.414; p < 0.05). In the subjects with PBECs ≥ 0.3 × 109/L, the median FeNO level was 39 ppb (interquartile range, 22.5‐65.5 ppb), significantly higher than in the subjects with PBECs < 0.3 × 109/L. The area under the ROC curve was 0.707 (p < 0.05). The maximum Youden index (0.348) was at PBECs = 0.205 × 109/L, which achieved sensitivity and specificity of 63% and 71.8%, respectively. Conclusion: PBECs ≥ 0.3 × 109/L can predict a positive bronchodilation test result and a high FeNO level, with a probability of 50% in the subjects with chronic cough and shortness of breath; in the absence of corresponding symptoms and a low PBEC, the predictive value was small. For hospitals not able to conduct FeNO measurements, for outpatients with poor economic conditions, and for patients with confirmed or suspected novel coronavirus disease 2019, the PBEC, in conjunction with a patient's clinical symptoms, can improve the diagnostic accuracy of allergic asthma and assessment of airway inflammation while reducing the risk of infection.
