Analysis of quality control group of departments on improving effect of PICC maintenance quality in outpatient children with tumor

To explore the clinical effect of quality control group of departments on improving PICC maintenance quality in outpatient children with tumor. Methods: A total of 4100 children with tumor who were treated in our outpatient department of intravenous therapy from January to December 2020 were divided into observation group (July to December 2020: after the establishment of quality control department) and control group (January to June 2020: before the establishment of quality control group of departments). In the control group, conventional PICC treatment management and maintenance measures were used. In the observation group, quality control group was used to manage, and moreover, the PICC maintenance quality, the incidence rate of PICC-related risk events and the level of PICC maintenance knowledge and satisfaction evaluation were compared between the two groups. Results: (1) Comparing the PICC maintenance quality between the two groups, the rate of absence of PICC maintenance during the course of tumor management in the observation group (1.14%) was significantly lower than that in the control group (4.00%), and there was significant difference in the PICC maintenance quality between the two groups (P<0.05). (2) Comparing the relative indexes of children with tumor: Before intervention, there was no significant difference in the relative indexes of children with tumor (PICC maintenance knowledge level) between the two groups (P>0.05), but after intervention, the relative indexes of children with tumor (PICC maintenance knowledge level, and service evaluation recognition degree) in the observation group were significantly higher than that in the control group (P<0.05). (3) Comparing the probability of PICC-related risk events between the two groups: In the course of tumor management, PICC-related complications were observed in the observation group (3.59%, 3.18%), which were significantly lower than those in the control group (11.95%, 11.37%) (P<0.05). Conclusion: The application of quality control group of departments in the management of PICC in outpatient children with tumor has obvious clinical effect. It can effectively improve the level of maintenance knowledge and service satisfaction of family members, optimize the quality of maintenance, reduce the rate of lack of maintenance and risk related to PICC, and is worthy of being popularized in clinical practice.

Abstract Context Pain is one of the most common and intolerable symptoms in cancer patients. But cancer pain control is still negative in China. Objectives This paper explores the application of quality control circle in the treatment of cancer pain in inpatients to improve the quality of life of patients with cancer pain. Method Established a quality control circle group to analyze the current status of cancer pain control in inpatients with moderate cancer pain, set goals, formulate corresponding countermeasures and implement and review them in stages. The plan-do-check-act method was cyclically applied. The Brief Pain Inventory was used to evaluate the cancer pain status of patients with cancer pain hospitalized before (January to April 2016) and after (September to December 2016) the implementation of quality control circle activities. Results The pain control effect of the observation group was significantly better than that of the control group. The mean (standard deviation) of pain severity and pain interference in the observation group were significantly lower than those in the control group (6.21 [2.86] vs 4.31 [2.25], 10.54 [4.10] vs 7.25 [3.77]). There was significant difference after the implementation of quality control circle (P &lt; 0.01). Conclusion Quality control circle activities are used to manage patients with cancer pain, to improve the situation of pain control and to improve the quality of life of patients. This management tool and method is worthy of clinical promotion and application.

Abstract Objective:The purpose of this study is to retrospectively observe the clinical efficacy and adverse reactions of docetaxel hyperthermic intraperitoneal chemotherapy (HIPEC) combined with endostar in treatment of malignant ascites. Methods:56 cases of malignant ascites admitted to Zhejiang Hospital from July 2019 and October 2020 had received no less than second-line chemotherapy. The observation group (n = 29) was treated with endostar 60mg d1,4,7 Q3W intraperitoneal injection combined with docetaxel 60mg/m2 d4 HIPEC Q3W for 2 cycles. The control group (n = 27) was just treated with docetaxel 60mg/m2 d1 HIPEC Q3W for 2 cycles. The general clinical data and qualitative data of the treatment results were processed by SPSS26.0 using χ2 test, and quantitative data were processed by t test. When P < 0.05, statistical data can be considered statistically significant.Results:There were significant differences between the observation group and the control group in objective response rate (ORR) (65.5% vs 37.0%,P=0.033),in the improvement rate of KPS (48.3% vs 22.2%, P=0.042),also in the median control time (57dvs 45d,P=0.027). The incidence of III to IV adverse reactions was low, and no treatment-related death was observed. There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Endostar combined with docetaxel HIPEC has good clinical effect on malignant ascites and can improve patients’ quality of life, and the adverse reactions can be tolerated.

Objective: To investigate the clinical efficacy of zoledronic acid in the treatment of senile osteoporosis. Methods: One hundred and six cases of senile osteoporosis who visited to our hospital from August 2017 to December 2018 for treatment were selected and randomly divided into a control group and an observation group. The control group was treated with conventional therapy, while the observation group was treated with zoledronic acid in addition to the treatment of the control group. Bone mineral density, pain degree, therapeutic effect and adverse reactions of the two groups were compared. Results: The total effective rate of the observation group was 96.67%, higher than 80.00% of the control group (P<0.05); the bone mineral density of lumbar vertebrae, femoral neck and Ward’ area in the two groups increased after 6 months of treatment, and the bone mineral density of the observation group increased more than that of the control group (P<0.05); the pain degree of the observation group was lower than that of the control group after 6 months of treatment, and the difference was significant (P<0.05). There was no significant difference in the occurrence of adverse reactions between the two groups (P>0.05). Objective: To explore the clinical effect of moxifen tablets combined with medroxyprogesterone acetate injection on patients with endometriosis. Methods: 90 patients with endometriosis from August 2018 to March 2019 were randomly divided into control group and observation group, with 45 cases in each group. The control group was treated with moxifene tablets and the observation group with medroxyprogesterone acetate. The changes of serum sex hormone, P, CA125 levels, adverse reactions and therapeutic effects were compared between the two groups. Result: Before treatment, there was no significant difference in serum sex hormone and P, CA125 levels between the two groups (P > 0.05); after treatment, the indexes of the two groups were reduced, and the LH, E2, FSH, CA125 and P water in the observation group were lower than those in the control group (P < 0.05); the curative effect of the observation group was significantly better than that in the control group (P < 0.05); the incidence of adverse reactions in the observation group was lower than that in the control group (P < 0.05). Conclusion: Moxifene combined with medroxyprogesterone acetate has a significant therapeutic effect on endometriosis. It can significantly reduce the level of serum sex hormone. The incidence of adverse reactions is low and the treatment safety is high. It is worth popularizing.

Objectives: To explore the clinical value of minimally invasive aspiration and drainage of intracranial hematoma in the treatment of cerebral hemorrhage. Methods: Seventy-eight patients with cerebral hemorrhage who were treated in the Taian City Central Hospital and the Second Affiliated Hospital of Shandong First Medical University between June 2018 and December 2019 were selected. The patients were randomly numbered and divided into two groups by drawing lots, 39 in each group. The control group was treated with the traditional internal medicine conservative therapy, and the observation group was treated with minimally invasive intracranial hematoma aspiration and drainage. The indexes of the two groups were compared. Results: The efficacy rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). The National Institutes of Health Stroke Scale (NIHSS) score of the observation group was lower than that of the control group after treatment, and the difference was statistically significant (P<0.05). After treatment, the good recovery rate of the observation group was higher compared to the control group, and the difference had statistical significance (P<0.05). The incidence of complications in the observation group was lower than that of the control group, with a statistically significant difference (P<0.05). Objective. To explore the clinical effect and aesthetics of All-on-4 immediate loading implant denture in severe periodontitis. Methods. Totally, 60 patients with dentition loss caused by severe periodontitis who were admitted to our hospital from February 2017 to February 2019 were selected. The patients were randomly divided into the observation group (n = 30) and the control group (n = 30) according to different restoration methods. The control group was used in traditional implant restoration treatment, and the observation group was used in All-on-4 immediate loading implant denture restoration. The clinical effect, periodontal index, aesthetic effect, and complications of the two groups were compared, and the levels of serum leptin (LEP), tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), and interleukin-6 (IL-6) were measured. Results. The total effective rate of the observation group (83.33%) was higher than that of the control group (60.00%) ( P < 0.05 ). After treatment, the plaque index, gingival sulcus bleeding index, and depth of exploration of the two groups were lower than those before treatment ( P < 0.05 ), and there was no significant difference between the two groups ( P > 0.05 ). After treatment, the serum LEP, TNF-α, CRP, and IL-6 in both groups were lower than before treatment, and the serum LEP, TNF-α, CRP, and IL-6 in the observation group were lower than that in the control group ( P < 0.05 ). The aesthetic rate of teeth in the observation group (90.00%) was higher than that in the control group (66.67%) ( P < 0.05 ). There was no significant difference in the total incidence of complications between the two groups ( P > 0.05 ). Conclusion. All-on-4 immediate loading implant denture has a good effect in the treatment of severe periodontitis, which is conducive to maintaining periodontal health, reducing the level of inflammation, improving aesthetics, and has high safety.

Objective: To observe the clinical efficacy of sequential butylphthalide therapy combined with dual antiplatelet therapy in the treatment of elderly patients with acute cerebral infarction (ACI). Methods: One hundred and twenty-two elderly patients with ACI who were admitted to the department of neurology of our hospital at May 2016-August 2018 were selected grouped into a control group and an observation group by random number table method, 61 in each group. On the basis of conventional treatment, the patients in the control group were given dual antiplatelet therapy (aspirin enteric-coated tablets + clopidogrel bisulfate tablets), while the patients in the observation group were given sequential butylphthalide therapy on the basis of the control group. The clinical effects of the two groups were compared after four weeks of treatment, and the changes of National Institutes of Health Stroke Scale (NIHSS), ADL score, plasma 3-mercaptopyruvate sulphurtransferase (3-MST) and Amyloid β42 (Aβ42) levels and the occurrence of adverse reactions during treatment were recorded. Results: The clinical efficacy of the observation group was better than that of the control group (P<0.05). There was no significant difference in NIHSS and ADL scores between the two groups before treatment (P>0.05). After treatment, the NIHSS and ADL scores of the observation group were better than those of the control group (P<0.05). There was no significant difference in plasma levels of 3-MST and AB42 between the two groups before treatment (P>0.05). The level of plasma 3-MST in the observation group was higher than that in the control group, and the level of plasma Aβ42 was lower than that in the control group (P<0.05). No serious adverse reactions occurred during the treatment period in both groups. Conclusion: Butylphthalide sequential therapy combined with dual antiplatelet therapy is effective in the treatment of elderly ACI.  Objective: Investigate the effectiveness of nursing risk management in the care of critically ill patients in the respiratory unit. Methods: Among the critically ill respiratory patients admitted to our hospital between May 2019 and April 2020, 78 patients were randomly selected and divided into an observation group and a control group, each consisting of 39 patients. In the observation group, a nursing risk management model was implemented, i.e., patients' clinical symptoms were observed at any time to monitor their treatment satisfaction and the effectiveness of their care and routine care was implemented for the control group. Results: The heart rate, respiratory rate, and pH of patients in the observation group were more stable than those in the control group, and their respiratory status was better, with differences in data. There was also significant statistical significance (P<0.05). The incidence of patient-provider disputes, unplanned extubation, and unplanned events were lower in the observation group compared to the control group, and their data difference was statistically significant (P<0.05). The treatment satisfaction as well as the total effective rate of patients in the observation group was also much higher than that of the control group, and there was also a statistically significant difference in the data (P<0.05). Conclusion: The nursing risk management model has a significant therapeutic effect in the care of critically ill respiratory patients. Therefore, it is worth popularizing to use in the clinical nursing of respiratory critical patients.

Objective: To investigate the clinical effect of combination treatment of Chinese medicine Baogan Lishui decoction and western medicine on hepatitis B cirrhosis related refactory ascites.Method: Control group was given conventional western medicine therapy while the observation group was given same conventional western medicine therapy with additional of Chinese medicine Baogan Lishui decoction.Result: The total effective of clinical treatment on hepatitis B cirrhosis related refractory ascites in observation group was higher than that of the control group which was 88.89% compared to 71.11%, respectively and it was significant different (p<0.05).Conclusion: The combination treatment of Chinese medicine Baogao Lishui decoction and conventional western medicine can significantly increased the treatment effect on hepatitis B cirrhosis refractory ascites, and thus increases the quality and safety of life.

【Abstract】Objective: To investigate the clinical efficacy of carboprost tromethamine combined with mifepristone in the treatment of uterine fibroids. METHODS: A total of 66 patients with uterine fibroids admitted to our hospital between April 2018 and January 2019 were selected as subjects. According to the two different treatment methods, patients were divided equally. The observation group and the control group, each group of 33 people. The oxytocin drug treatment was medicated to the control group, and the prostaglandin tromethamine combined with mifepristone was medicated to the observation group. The treatment effect, adverse reaction, operation, and uterine muscle before and after surgery were observed in these two groups. Tumor tissue progesterone receptor and estrogen receptor levels. Results: The clinical treatment effect of the observation group was 93.94%, and the clinical treatment effect of the control group was 60.61%. The clinical treatment effect of the observation group was significantly higher than that of the control group; also the incidence of adverse reactions in the observation group in terms of rash, fatigue, nausea and vomiting was much lower than the control group, and could observe a significant difference. Finally in observation group, the amount of intraoperative blood loss, operation time and hospitalization time were better than those of the control group. The progesterone receptor and estrogen receptor levels in the uterine fibroid tissue after surgery should also be better than the control group, it is worth to make further comparison. Conclusion: Carprostol tromethamine combined with mifepristone is effective in the treatment of uterine fibroids and can be further developed.
