Background: Diabetes mellitus (DM) is a metabolic disorder that has the phenotype of hyperglycemia. According to World Health Organization (WHO) there were 65.1 million diabetics in India in 2013, International Diabetes Federation estimates this to increase to 190 million by 2035. Although a number of drugs are available for treatment of DM, their cost and safety profile are major concern. Medicinal plants are used by clinicians for treatment of diabetes. Gymnema sylvestre (GS) extract has been reported to increase insulin levels in diabetic rats. This study was designed to compare the antihyperglycemic effect of Gymnema sylvestre with metformin.Methods: Diabetes was induced in Sprague-Dawley rats using streptozotocin 45mg/kg. Methanolic extract of Gymnema sylvestre 120mg/kg p.o. prepared using Soxhlet apparatus.Results: GS extract reduced blood glucose levels but not statistically significant. GS extract increased HDL and triglycerides, reduced both serum ALT and AST but no statistical significance seen. Metformin significantly increased serum urea, which was not seen in GS extract group. GS extract showed regenerative changes in pancreas, liver and kidney.Conclusions: The study investigation demonstrates that methanolic extract of GS possesses antihyperglycemic and hypolipidaemic activity and so it can be considered as a promising natural remedy in a prediabetic state and in mild hyperlipidaemia to prevent its progression. Increase in β cell regeneration activity could be a probable mechanism of action. However, further long term clinical studies are recommended to define its possible role in diabetes mellitus and hyperlipidaemia. Role of GS as a potential hepatoprotective agent also needs further evaluation.

Background:The objective of this study was to explore the expression of urotensin II (UII), its receptor (GPR14), and vascular endothelial growth factor (VEGF), as well as their associations in the ischaemic brains of rats with focal cerebral ischaemia, under normal and diabetic conditions.Methods:Diabetes mellitus (DM) was induced by injection of streptozotocin (STZ) into Sprague—Dawley rats. Focal cerebral ischaemia was induced by middle cerebral artery occlusion (MCAO) four weeks after DM onset by STZ. Rats (n=80) were divided into four groups: normal control, DM, MCAO, and DM/MCAO. Immunohistochemistry and reverse-transcriptase-polymerase chain reaction (RT-PCR) were used to detect the expression of UII, GPR14 and VEGF in the diabetic and ischaemic brain.Results:Expression of UII and GPR14 was increased at mRNA and protein levels in the DM and MCAO group compared with controls. In the DM/MCAO group, expression of UII and GPR14 was increased significantly in the ischaemic brain, and was accompanied by a significantly increased VEGF expression.Conclusion:Diabetes mellitus was seen to aggravate brain lesions after ischaemia, and UII may have an important role.

Diabetes mellitus is a difficult metabolic disorder that has seriously impact the human health and quality of life. Medicinal plants are being used to control diabetes However, they are not entirely effective and no one has ever been reported to have fully recovered from diabetes. Many plants have been used for the management of diabetes mellitus in various traditional systems of medicine worldwide as they are a great source of biological constituents and many of them are known to be effective against diabetes. Medicinal plants with antihyperglycemic activities are being more desired, owing to lesser sideeffects and low cost. Streptozotocin was induced to all groups of rats at dosage of 35 -55mg/kg except for the normal. Streptozotocin induced diabetes in sprague dawly rats were used to study antidiabetic activity of methonolic extract of two medicinal plants Gymnema sylvestre,Andrographis paniculata methanolic leaf extract was administered orally in graded doses of 30 mg/kg,50mg /kg sprague dawly rats Gymnema sylvestre at a dose of 30mg/kg and Andrographis paniculata at a dose of 50mg/kg showed significant anti-hyperglycemic and anti-oxidative effect which was evident from the 1st week of treatment.

Abstract Background: Diabetic encephalopathy is a severe diabetes complication with cognitive dysfunction and neuropsychiatric disability. The mechanisms underlying diabetic encephalopathy is believed to be relevant with oxidative stress, vascular amylin deposition, immune receptors, inflammation, etc. This study wanted to evaluate the ability of curcumin and its analog A13 to alleviate oxidative stress and inflammation in diabetes-induced damages in brain.Methods: Sixty adult male Sprague-Dawley rats were divided into 5 groups: normal control (NC) group, diabetes mellitus (DM) group, curcumin-treated diabetes mellitus (CUR) group, high dose of A13-treated diabetes mellitus (HA) group, low dose of A13-treated diabetes mellitus (LA) group. Activation of the nuclear factor kappa-B (NF-κB p65) pathway was detected by RT-qPCR, immunohistochemical (IHC) staining and Western blot; oxidative stress was detected by biochemical detection kit; brain tissue sections were stained with hematoxylin–eosin (HE) staining and Myelin staining. Results: RT-qPCR, IHC staining and Western blot showed that curcumin and A13 treatment could inhibit the NF-κB p65 pathway. Curcumin and A13 increased the activity of superoxide dismutase and decreased the malondialdehyde level in the brain of diabetic rats. Furthermore, HE staining and Myelin staining demonstrated that the histological lesions of the brain in diabetic rats could be significantly ameliorated by curcumin and A13.Conclusion: Curcumin analog A13 could alleviate the damages in the brain of diabetes rats by regulating the pathways of inflammation and oxidative stress. A13 may be a new potential therapeutic agent for diabetic encephalopathy.

Background: Diabetes mellitus is a class of metabolic disorder which results high level of sugar due to inadequacy in insulin secretion. High Sugar level in diabetes is linked with the impairment and dysfunction of eyes, kidneys, blood vessels, nerves and heart. In current research there is extended interest in herbal medicines because of the side effects noticed with oral hypoglycemic for therapy of hyperglycemia. Herbal medicines could play a dominant role in the control of diabetes mellitus. Methods: Methanolic extracts of seeds of Psoralea corylifolia L & Psoralea esculenta L has been screened for both in-vitro and in-vivo antihyperglycemic role using streptozotocin induced diabetic rats. The methanolic extract of both plants were evaluated for pancreatic tissue studies. Results: According to the outcomes, a stunning blood glucose level decrement was observed in the diabetic groups treated by methanolic extracts of Psoralea corylifolia L. seeds. Conclusion: In this study, the methanolic extract of Psoralea corylifolia L. seeds used to evaluate antihyperglycemic potential showed appreciably notable and astonishing results, when compared with the Glibenclamide. Methanolic extract of Psoralea corylifolia L. seeds have potential to regulate hyperglycemia in the In-vivo model. Hence Psoralea corylifolia L. methanolic seeds extract may be selected for its antihyperglycemic activity.

Objective: The aim of the present study was to examine the effect of a polyherbal formulation (PHF) on pharmacokinetics and pharmacodynamics of metformin in rats. Methods: The present study was conducted to determine the beneficial outcomes of PHF along with metformin by studying herb-drug interactions. PHF was prepared by five indigenous herbs, Those are being used traditionally as antidiabetic in India. PHF doses (100 mg/kg/day) were administered to Sprague-Dawley rats by an oral route of different groups for multiple weeks except for control. Metformin (100 mg/kg) was orally administered at 7th and 30th day to control and PHF pretreated rats for pharmacokinetics study while pharmacodynamics study was conducted in PHF treated and untreated diabetic rats. No more significant difference was found in the pharmacokinetic parameters in PHF treated at 7th day while a significant increase was found in AUC at the 30th day. Results: The hypoglycemic effect was observed with a combination of metformin and PHF, significantly more compared to control. Metformin decreased the blood glucose 1.51 fold at 7th day and 1.7 fold at 30th day respectively, compared to control. Conclusion: Thus, this finding indicates that PHF increased the AUC of metformin. It might increase bioavailability through drug-herb interaction thereby affecting the therapeutic effect. This formulation can be considered as an adjunct to metformin in the management of diabetes mellitus.

Due to substantial morbidity and high complications, diabetes mellitus is considered as the third “killer” in the world. A search for alternative antidiabetic drugs from herbs or fungi is highly demanded. Our present study aims to investigate the antidiabetic activities ofCordyceps militarison diet-streptozotocin-induced type 2 diabetes mellitus in rats. Diabetic rats were orally administered with water extract or alcohol extract at 0.05 g/kg and 2 g/kg for 3 weeks, and then, the factors levels related to blood glucose, lipid, free radicals, and even nephropathy were determined. Pathological alterations on liver and kidney were examined. Data showed that, similar to metformin,Cordyceps militarisextracts displayed a significant reduction in blood glucose levels by promoting glucose metabolism and strongly suppressed total cholesterol and triglycerides concentration in serum.Cordyceps militarisextracts exhibit antioxidative effects indicated by normalized superoxide dismutase and glutathione peroxidase levels. The inhibitory effects on blood urea nitrogen, creatinine, uric acid, and protein revealed the protection ofCordyceps militarisextracts against diabetic nephropathy, which was confirmed by pathological morphology reversion. Collectively,Cordyceps militarisextract, a safe pharmaceutical agent, presents excellent antidiabetic and antinephropathic activities and thus has great potential as a new source for diabetes treatment.

Aim. To evaluate the effectiveness of the Cochrane Russia Initiative to improve the articles of the Russian-language Wikipedia by including information from Cochrane Systematic Reviews (CSR) to ensure the accuracy and impartiality of their content as an information basis for the quality use of medicines by doctors and the public. Methods. Wikipedia articles on selected drugs were improved by introducing evidence from Cochrane Systematic Reviews Cochrenized. A parallel open-label non-randomized controlled intervention study was conducted. We assigned 2 groups of drugs and Wikipedia articles about them: the intervention group (Cochrenization) and the control group (36 articles each). Control group articles were not edited. The change in the number of visits to Wikipedia pages for the year (20182019) was measured, the statistical significance of the differences was assessed by using the Wilcoxon signed-rank test. Results. We edited 36 Wikipedia articles, including 13 articles on migraine treatment, 9 diabetes mellitus, 14 pain and inflammation articles. These articles constituted the intervention group. The control group consisted of articles on cardiovascular (11), gastrointestinal (14) and dermatological agents (11). We used the Cochrane Russia Initiative dashboard on Wikipedia to compare the number of article views before and after the intervention and Cochrane analytics on demand for translations of Cochrane systematic reviews summaries. After Cochrenization, the number of Wikipedia article views (20182019) increased in total/average article views for treatments: migraines by 18%/47%, pain and inflammation 16%/43%, diabetes mellitus 18%/0%. Analysis of Cochrane reports showed an increase in the number of views of Cochrane systematic reviews summaries on the Cochrane.org website in general by 9 times and from Russian-speaking browsers by 11 times. Improvement of medicine-related articles of the Russian-language Wikipedia by the introduction of information from Cochrane systematic reviews was accompanied by an increase in their demand in terms of the number of views in general by 34%, with a similar increase in control (without intervention) (p-value of the experimental group 0.002, control 0.000). The Wikipedia articles on medicines, which belonged to the Russian Vital and Essential Drugs List or the World Health Organization Model List of Essential Medicines, got more views. Conclusion. The role of confounding factors justifies the feasibility of developing a methodology for studying the usefulness of improving Wikipedia articles, different from controlled trial research methodology; further inclusion of Cochrane knowledge into Wikipedia is needed.

Dear Colleagues! We are glad to present the 9th Edition (revised) of Standards of Diabetes Care. These evidence-based guidelines were designed to standardize and facilitate diabetes care in all regions of the Russian Federation. The Standards are updated on the regular basis to incorporate new data and relevant recommendations from national and international clinical societies, including World Health Organization Guidelines (WHO, 2011, 2013), International Diabetes Federation (IDF, 2011, 2012, 2013), American Diabetes Association (ADA, 2018, 2019), American Association of Clinical Endocrinologists (AACE, 2019), International Society for Pediatric and Adolescent Diabetes (ISPAD, 2014, 2018) and Russian Association of Endocrinologists (RAE, 2011, 2012, 2015). Current edition of the Standards also integrates results of completed randomized clinical trials (ADVANCE, ACCORD, VADT, UKPDS, SAVOR, TECOS, LEADER, EXAMINE, ELIXA, SUSTAIN, DEVOTE, EMPA-REG OUTCOME, CANVAS, DECLARE, CARMELINA, REWIND, etc.), as well as findings from the national studies of diabetes mellitus (DM), conducted in close partnership with a number of Russian hospitals. Latest data indicates that prevalence of DM in the world increased during the last decade more than two-fold, reaching some 463 million patients by the end of 2019. According to the current estimation by the International Diabetes Federation, 578 million patients will be suffering from diabetes mellitus by by 2030 and 700 million by 2045. Like many other countries, Russian Federation experiences a sharp rise in the prevalence of DM. According to Russian Federal Diabetes Register, there are at least 4 584 575 patients with DM in this country by the end of 2018 (3,1% of population) with 92% (4 238 503) Type 2 DM, 6% (256 202) Type 1 DM and 2% (89 870) other types of DM, including 8 006 women with gestational DM. However, these results underestimates real quantity of patients, because they consider only registered cases. Results of Russian epidemiological study (NATION) con- firmed that only 54% of Type 2 DM are diagnosed. So real number of patients with DM in Russia is 9 million patients (about 6% of population). This is a great long-term problem, because a lot of patients are not diagnosed, so they dont receive any treatment ant have high risk of vascular complications. Severe consequences of the global pandemics of DM include its vascular complications: nephropathy, retinopathy, coronary, cerebral, coronary and peripheral vascular disease. These conditions are responsible for the majority of cases of diabetes-related disability and death. In сurrent edition of the Standards: New goals of glycemic control for the elderly, based on the presence of functional dependence, as well as for pregnant women, children and adolescents, are given. Added a snippet that describes the continuous glucose monitoring. Only low-density lipoprotein cholesterol level is used as a target for lipid metabolism. Proposes more stringent target levels of blood pressure. It also features updated guidelines on stratification of treatment in newly diagnosed Type 2 diabetes: the excess of the initial level of HbA1c over the target level was used as a criterion. In the recommendations for the personalization of the choice of antidiabetic agents, it is taken into account that in certain clinical situations (the presence of atherosclerotic cardiovascular diseases and their risk factors, chronic heart failure, chronic kidney disease, obesity, the risk of hypoglycemia) certain classes of hypoglycemic agents (or individual drugs) have proven advantages. Recommendations for psychosocial support are added. The position of metabolic surgery as a method of treatment of DM with morbid obesity is updated. Recommendations for diagnostic and treatment of hypogonadism syndrome in men with DM are added. For the first time, evidence levels of confidence and credibility levels of recommendations for diagnostic, therapeutic, rehabilitative and preventive interventions based on a systematic review of the literature are given in accordance with the recommendations of the Center for Healthcare Quality Assessment and Control of the Ministry of Health of the Russian Federation. This text represents a consensus by the absolute majority of national experts, achieved through a number of fruitful discus- sions held at national meetings and forums. These guidelines are intended for endocrinologists, primary care physicians and other medical professionals involved in the treatment of DM. On behalf of the Working Group
