Background: Medical abortion is a means of medical termination of pregnancy by drugs alone. This can be done upto 63 days using mifepristone and misoprostol tablets. For the conventional method, patient has to come to the hospital 3 times. In our study we reduced the interval between the drugs there by reducing the number of hospital visits. The objectives were to study the reduction of induction abortion interval following administration of evidence based regimen and to compare the proportion of patients developing complications in both the groups.Methods: It was a comparative study conducted at Department of Obstetrics and Gynecology, Govermnent MedicalCollege Kottayam, Kerala, India from May 2015 to November 2015.Results: The mean induction abortion interval in experimental group was 14.3 hours and in control group, it was 60.4 hours which was found to be statistically significant. Only 3 patients (7%) of experimental group had side effects whereas 12 patients. (17.4%) had side effects in the control group. Major side effects encountered were severe abdominal pain and severe bleeding per vaginum. Evidence based  regimen consist of administration of mifepristone 200mg and vaginal misoprostol 600µg 6 hours later for termination of pregnancy up to 63 days could reduce the induction abortion interval by 46 hours and had less side effects.Conclusions: As it reduces the induction abortion Interval and complications we feel that the evidence based regimen for Medical Termination of Pregnancy is superior to the FDA approved regimen.

The article describes the specific features of termination of progressive uterine pregnancy in the II trimester. It was found that the efficacy of medical abortion in the II trimester of pregnancy is 94–98%. High efficacy, low incidence of side effects or early and late complications and economic feasibility demonstrate that the method is promising and safe; therefore, it can be recommended as a priority when choosing a method for medical termination of pregnancy in the II trimester.

Termination of Pregnancy (TOP), in our country is carried out only when considerable danger to a fetus due to congenital abnormalities, intrauterine fetal loss or when maternal life is under threat. The principle concerns in providing second trimester termination include safety, efficacy, simplicity, low-cost, and fast acting with minimal side effects. Objectives: To compare the efficacy in term of complete abortion within 48 hours between protocol-I (200 µg misoprostol 4 hourly) versus protocol-II (200 µg misoprostol 6 hourly) for second trimester (14-24 weeks) medical termination of pregnancy. Study Design: Randomized controlled trial. Settings: Gynecology & obstetrics unit-II, Bahawal Victoria Hospital (BVH) Bahawalpur. Period: From 1st July 2017 to 30th June 2018. Material & Method: A total of 182 patients meeting the inclusion criteria were included in study. Ninety one patients were included in group-A and 91 patients were included in group-B.  Women allocated to group-A was given misoprostol 200µg every 4 hour. Similar drug and dose were administered to women of group-B intra-vaginally, 6 hourly. Patients in both groups were monitored up to 48. If abortion occurred within 48 hours of induction, it was labeled as effective abortion otherwise it was regarded as failed abortion. Results: The mean age in group-A was 26.71 years 25.49 years in Group-B. In group-A, 89 (97.8%) patients showed efficacy (abortion within 48 hours) and in group-B, 74 (81.32%) patients showed efficacy to misoprostol. Conclusion: The regimen of using 200μg of vaginal misoprostol 4 hourly is more efficacious and quick for 2nd trimester TOP with fewer side effects when compared to 200μg of vaginal misoprostol 6 hourly.

Background: A combination of antiprogesterone mifepristone and prostaglandin analogue misoprostol provides an effective non surgical method for termination of pregnancy up to gestational age of 49 days. Objective of present study was to assess the efficacy of this medical regimen for termination of pregnancy up to 49 days of pregnancy.Methods: A hospital based retrospective study was carried out in Department of Obstetrics and Gynecology in Sassoon General Hospital, Pune, India for a period of one year where 30 women requesting for medical abortion were studied. The medical regimen used was mifepristone 200 mg orally followed 48 hours later by misoprostol 800 microgram administered in posterior vaginal fornix.Results: The overall success rate of this regimen was 90%. Where success was defined as achieving complete abortion without needing surgical evacuation. Surgical evacuation was needed in 3 (10%) patients i.e. for incomplete abortion.Conclusions: Combination of oral mifepristone 200mg followed 48 hours later by vaginal misoprostol 800mcg is an effective method of medical termination of pregnancy.

Objective: The objective of this study was to compare and evaluate the efficacy, safety, adverse effects, and patient compliance of vaginal versus oral misoprostol in medical termination of pregnancy after tablet misoprostol up to 49 days of gestation. Methods: A comparative study of 100 patients divided randomly assigned to two control groups. Results: The study showed that 200 mg mifepristone followed by 800 mcg vaginal misoprostol is more effective for medical termination in gestational age up to 49 days as compared to 200 mg mifepristone followed by 400 mcg oral misoprostol. Conclusion: While both routes of administration are safe, gastrointestinal side effects are more with oral misoprostol. The vaginal route is more acceptable to the patients enrolled in the study.

Background: Unsafe and illegal abortions are one of the major problems in women health in India. Despite legal approval for medical termination of pregnancy in 1971, unsafe abortion still remains the third leading cause of maternal deaths in the country, contributes eight percent of such deaths annually. The objective of this study was to study efficacy of Mifepristone 200 mg orally followed 36-48 hours later by Misoprostol 800 microgms per vaginally in women undergoing medical termination of early pregnancy (up to 63 days of gestational age).Methods: The present study included 60 pregnant women requesting termination of pregnancy in the first trimester. Women who fulfilled the inclusion criteria were included in the study. Women without medical or surgical contraindications to Mifepristone and Misoprostol were included. Patients with previous caesarean sections were also included.Results: In our study 60 women were included, majority were in age group 20-29 years of age and majority of cases were primipara or multipara. The success rate in terms of complete abortion was 97%, 2% needed surgical evacuation and 1% lost to follow up. Side effects were nausea, vomiting, diarrhoea, abdominal cramps, pyrexia etc. The method proved to be safe, effective, cheap, non-invasive and has minimal or no complications.Conclusions: Medical termination of pregnancy with oral mifepristone and vaginal misoprostol is an effective method for first trimester abortion. The prerequisite for the method is patient counselling, patient participation and willingness for regular follow up and to report any complication. Hence this method comes out to be a safe alternative to surgical method which is invasive and costly.

Background: Combined mifepristone and misoprostol is the best method of medical termination of pregnancy. The study is undertaken to find out the success rate, acceptability, efficacy and patient satisfaction of medical abortion in pregnant women with 49 days of gestation. Methods: The pregnant women with amenorrhea of 49 days or less seeking termination of pregnancy received were studied. The 200 mg of oral mifepristone and followed 48 hours later with 600 microgram of oral misoprostol were given. Results: The success rate with the regime was 47 (94%) and 49 (98%) were satisfied. Weakness 24 (48%), Nausea 10 (20%), diarrhea 12 (25%), needing analgesic 4 (8%) were the side effects of the combined regime. Conclusions: Medical termination of pregnancy with combination of mifepristone and oral misoprostol has a good success rate. The side effect of the regimen in our setup is less and well accepted. Majority of the patient are satisfied with medical termination of pregnancy. Unsafe abortion is a major health issue. Therefore safe abortion services which is effective. acceptable and techniquely easy to perform should be available. Aim- To confirm effectiveness, side effects, safety and patient satisfaction with medical termination of early pregnancy. Methods- It was a prospective study 150 women who were less than 63 days pregnant and consented for medical termination of pregnancy were given! 200mg 0 Mefepristone orally and 48 hrs later 800 micro gram Misoprostol was inserted vaginally. They were followed up in 14 days. Bleeding, pain G.I symptoms any other side effects were recorded. Abortion was complete or not was confirmed clinically and by USG. Failure and success was documented depending upon need for surgical evacuation. Results- The success rate with this regime was 92.6% and failure rate was 7.3%. There were no major side effects with Mifepristone. The average bleeding after misoprostol insertion was 10 days and 80% women had more bleeding than normal period. All of them had pain. Only 13.3% needed analgesic. women (96%) were satisfied with the method. OBJECTIVE :- To assess the knowledge, attitude and practices about contraception among women seeking induced abortion. Material & Methods: This is a cross sectional study conducted in Dept.of OBGY,GMCH, Aurangabad between the period October 2019 to March 2020 using predesigned pretested questionnaire in women seeking medical termination of pregnancy. Observation:-Around 90% women were aware about contraception. Knowledge regarding tubectomy was more (77.66%) than other methods of contraception. Around 60% women were willing to use contraception and 70% women accepted contraception after counselling at the time of availing abortion services. Conclusion: Overall awareness of contraception was about 90%. Overall acceptance of contraception was 70% and utilization of tubectomy, as a permanent method of contraception was high due to government initiatives and incentives . Fear of side effects of contraception was an important factor for contraception denial . Husband’s support in the choice of contraception plays an important role.
