Background: Dysfunctional Uterine Bleeding (DUB) is a condition that affects nearly every woman at some point in her life. This study aims to compare the efficacy of Ormeloxifene and Norethisterone in reducing menorrhagia in such patients.Methods: This prospective study was done on 100 women presenting with dysfunctional uterine bleeding, of 20-50 years of age, who were ready for follow-up and were allocated into two equal groups, one was given Ormeloxifene and the other was given Norethisterone for a period of 3 months. Haemoglobin levels, endometrial thickness on ultrasound and Pictorial Blood loss Assessment Chart (PBAC) scores were assessed before and after the treatment.Results: It was found that both Ormeloxifene and Norethisterone reduced menorrhagia, with a significant difference in PBAC scores (p value <0.05). There was a notable reduction in PBAC scores in Ormeloxifene group (66.53% change from pretreatment mean value) as compared to Norethisterone group (31.38% change from pretreatment mean value); and same holds true for the change in haemoglobin levels as well as endometrial thickness. Ormeloxifene was found to have a greater effect on heavy menstrual bleeding in comparison to Norethisterone.Conclusions: Ormeloxifene is a new modality and is found to be a better option in reducing menorrhagia in DUB in respect to a greater success rate, better compliance and cost effectiveness.

Objective: To determine the mean change in central macular thickness after cataract surgery and to compare the mean change in central macular thickness after cataract surgery in non-diabetics and diabetics without diabetic retinopathy Study design: Case control   Study settings and duration: A case control study was carried out at Ophthalmology department, POF hospital, Wah Cantt. Study duration was 6 months (April 2019-September 2019)   Material and methods: A sample size of 60 patients was calculated by using Open Epi Software. We used non probability consecutive sampling. Patients were divided into two groups; Cases (Diabetic) and controls (non-Diabetic). All patients underwent phacoemulsification and observed after 4 weeks for macular thickness measurement using optical coherence tomography before and after surgery. Data analysis was done with SPSS version 20. Post stratification t test was applied. P value ≤0.05 was considered significant.   Results: Total 60 patients were included. Mean age of patients was 65.31 ±7. 63SD.There were 35 (58.3%) males and 25 (41.7%) female patients in the study. We found a significant increase in central macular thickness in cases and controls [(223.100±15.86SD vs 227.2667±17.9SD, p=0.000) and (221.200±12.16SD vs 226.289±16.7861SD, p =0.001)] before and after phacoemulsification in cases and controls respectively. However, no significant difference was found between the groups (p=0.486).   Conclusion: Central macular thickness was increased after uncomplicated phacoemulsification in both diabetics and non-diabetics without retinopathy for up to a follow-up period of 4 weeks but the thickness did not differ between the two groups.

This study assessed the effect of triweekly interdental brushing for a period of 6 months using the bleeding on full-mouth interdental brushing (BOFIB) index. All participants answered questionnaires and were instructed to clean all interdental areas using an interdental brush at least every second day. A dentist assessed the BOFIB index of 28 participants at baseline and at 3- and 6-month follow-up visits. We stratified the participants into three groups: IB-NN, those who used the interdental brush less than three times per week both before and after this program; IB-NY, those who used the interdental brush less than three times per week before the program but at least three times per week after the program; and IB-YY, those who used it at least three times per week both before and after the program. Owing to the weekly number of interdental brushings, the IB-YY and the other two groups showed a significant difference at baseline. At the 6-month follow-up, the weekly number of interdental brushings led to a significant difference between the IB-NN and the other two groups. The BOFIB index was lower among the compliant participants than among the noncompliant participants after 3 and 6 months of triweekly interdental brushing.

Background: Dysfunctional uterine bleeding is a common presentation to both general practitioner and gynaecologists which can have a significant effect on a woman’s quality of life. The aim of this study is to assess the effectiveness of treating dysfunctional uterine bleeding according to endometrial thickness.Methods: This study was a hospital based prospective study undertaken in gynecology outpatient clinic of Central Women’s Hospital, Mandalay, Myanmar for one-year period (2016). A total of 60 patients were recruited and divided into 3 groups based on endometrial thickness and offered targeted hormonal treatments. At the end of the one-month treatment, patients were asked to return for a follow-up visit and from their menstrual diaries, the number of bleeding days and bleeding scores were assessed and calculated.Results: Among sixty women with dysfunctional uterine bleeding, 55% of patients had endometrial thickness less than 6 mm, 25% had endometrial thickness 6-11 mm, with 20% of patients having endometrial thickness more than 11 mm. After one month of study period, treatment was found to be effective in 86.6% of the patients according to bleeding days and in 70% of the patients according to bleeding score.Conclusions: In women presenting with dysfunctional uterine bleeding, increased endometrial thickness was found to be associated with increased BMI. In the treatment of dysfunctional uterine bleeding, when the endometrial thickness of the patient was assessed and hormonal treatment was given according to the endometrial thickness, treatment was proven to be effective.

Background: Dysfunctional Uterine bleeding (DUB) is the most common cause of abnormal uterine bleeding (AUB). Medical therapy being the first line of management, an ideal drug should be able to block the estrogenic effects on the Endometrium without interfering with its beneficial effects on other tissues. Both Ormeloxifene - a selective estrogen receptor modulator (SERM) and Tranexamic acid-an anti fibrinolytic inhibitor is important in the treatment of DUB. The aim of this study is to assess the efficacy and safety of Ormeloxifene v/s Tranexamic acid in the treatment of DUB.Methods: This is a prospective double blind study conducted for 2 years in 200 women presenting with DUB attending to the Gynaecology outpatient department (OPD) of Narayana Medical College and Hospital, Nellore, AP, India. Two hundred women presenting with DUB were recruited into this study (after fulfilling inclusion and exclusion criteria) with informed consent and randomly divided into two equal groups (Group-A and Group-B) after obtaining college ethical committee approval.Results: The reduction in the mean pictorial blood loss assessment chart (PBAC) score with Ormeloxifene (from 278 to 97.6) was significantly more than that of Tranexamic acid (from 261 to 134.5) at six months (p <0.0001). The rise in Hemoglobin concentration (8.4g% to 11.15g% vs. 8.5 to 9.7gm%; p <0.008) and reduction in Endometrial thickness (from 10.57mm to 6.5mm v/s 9.96mm to 7.24mm p <0.0001) were also significantly more with Ormeloxifene than with Tranexamic acid. Relief from dysmenorrhoea and subjective improvement were also found to be considerable with Ormeloxifene.Conclusions: Both Ormeloxifene and Tranexamic acid can be considered for DUB treatment but Ormeloxifene is found to be superior to Tranexamic acid in various aspects.

Introduction Combination of chemotherapy and radiation are the treatment of choice in advance stage of nasopharyngeal cancer. Cisplatin-based chemotherapy is a regiment of choice in those cases. Several studies show that cisplatin-based chemotherapy affect renal function. Aims of the study to evaluate and compare the renal function before and after chemotherapy through comparing the blood urea nitrogen and creatinine serum level in patients with nasopharyngeal cancer who receive cisplatin and paclitaxel chemotherapy. Methods Design of this study is before and after using medical records data in West Nusa Tenggara General Hospital in Lombok. Result There were 33 patients who fulfill the inclusion and exclusion criteria in this study. Most of the patients are male with male and female ratio 3:1. The youngest patient age is eight years old, on the other hand, the oldest is 70 years old with the average age 46,6 years old. According to histopathology finding, on this study researcher didn't found WHO type I and most the patients were WHO type III (89,9 %). The mean blood urea nitrogen concentration before chemotherapy is 25,00 and after the treatment 33,33 with the p-value 0,01 with the paired t-test. On the other hand, creatinine serum level before and after chemotherapy consecutively 0,99 and 1,10. p-value 0,15, or there is no significant difference. Conclusion Blood urea nitrogen and creatinine serum are increase after cisplatin-based chemotherapy. However, only BUN has a significant difference.

Background: Banana counted as nearly perfect food since it contains six nutrients : water , sugar , proteins , fats , vitamins , and minerals . Because of that , bananas are often used as a staple food for most people because of its nutritional value. However, sometimes people do not realize the benefits of bananas as a whole , from leaves , fruits , flowers and stems of the bananas . Penggaron Lor society , still minimal in using the banana plant . Usually the community will sell fruit and leaves to the market , so that the results obtained from the banana plant community less than optimal. Method : This study is an observational analytic study . The research was done in the Village of Penggaron Lor Semarang , and was conducted in March-June 2014. The instrument in this research was primary data obtained directly from respondents through interviews using questionnaires distributed before and after counseling . To determine whether there are differences in knowledge before and after counseling on the benefits of banana plants as a medium for maintaining oral hygiene, statistical test Wilcoxon Sign Ranks test was conducted, and processed with SPSS. Result: The results of SPSS calculation because the data is not normal ( p value < 0.05), then the data was processed using the non-parametric test for two sample pairs , ie Wilcoxon Sign Ranks test , and obtained p value of 0.008 . This means that there was a significant difference between the mean value of the pre-test and post-test. Conclusion : there are differences in knowledge before and after counseling , counseling means that a significant impact on respondents' knowledge of the benefits of banana plants for maintain oral health.

ABSTRACT Aim This is a prospective study to find the effect of ormeloxifene on menstrual blood loss and its side effects in patients with abnormal uterine bleeding (AUB). Materials and methods Fifty women in the age group of 30 to 52 years with AUB were enrolled in the study. After baseline assessment, each patient was given 60 mg of ormeloxifene orally twice a week for first 3 months, then once a week for the next 3 months. The patients were followed up after 1, 3, and 6 months. The effect of ormeloxifene was evaluated by a change in the pictorial blood loss assessment chart (PBAC) score, passage of clots during menstruation, hemoglobin level, endometrial thickness (ET), and dysmenorrhea. Side effects of the drug were also noted at each visit; p-value < 0.05 was taken as significant. Results There was statistically significant reduction in median PBAC score from baseline 360 to 113, 73, and 41.5 after 1, 3, and 6 months. There was significant reduction in ET, passage of clots, and dysmenorrhea at each visit. Mean hemoglobin level increased significantly by 1.56 gm% after 6 months treatment. Adverse effects included amenorrhea (28%), ovarian cyst (11.3%), nausea (4.5%), headache (2%), and weight gain (4.5%). There was no major side effect requiring termination of treatment. Conclusion Ormeloxifene is a safe and efficacious alternative for medical therapy in the management of AUB. Clinical significance Ormeloxifene can be used in women with AUB to avoid the side effects of hormonal treatment and unnecessary hysterectomies. How to cite this article Sharma R, Singhal SR. Background: Abnormal uterine bleeding (AUB) is a diagnosis that does not apply to menorrhagia only, but also includes (menometrorrhagia) excessively prolonged and frequent bleeding. Ormeloxifene (also known as centchroman) is a selective estrogen receptor modulator. This study was conducted to assess the efficacy, safety and compliance of ormeloxifene in the medical management of AUB.Methods: This quasi experimental, one group pre and posttest research design study was conducted among 30 participants in a tertiary care institute in a metropolitan city during January 2016 to June 2017. Patients with AUB were administered with ormeloxifene were advised to follow up one monthly for the first three months and then three monthly follow up for 6 months.Results: The mean age of all the patients was 35.96±5.62 years. There was statistically significant difference of mean hemoglobin, mean endometrial thickness and mean pregnancy-associated breast cancer (PABC) score observed between pre-treatment and 3 months and 6 months of post treatment. Mean hemoglobin concentration was increased while mean endometrial thickness and mean PABC score was decreased as the duration of treatment was increased from 3 months to 6 months.Conclusions: Ormeloxifene is quick acting, effective and appears to be a promising option for the medical management of dysfunctional uterine bleeding. It leads to a significant reduction in menstrual blood loss, a significant rise in hemoglobin concentration and a significant decrease in endometrial thickness with very minimal side effect.

Abstract Background and Aims There is a strong evidence of importance of HLA matching in kidney transplantation and associations between HLA-DR mismatches and rejection, transplant glomerulopathy, graft failure, and death with functioning graft following kidney transplantation. Our aim is to evaluate the effect of degree of HLA mismatch on transplant outcome among live donor kidney transplant recepients. Method The current study is a retrospective cohort study which was performed in Urology and Nephrology Center, Mansoura University, Egypt. The study included 2200 kidney transplant recipients who underwent renal transplantation between March 1976 and August 2019. The patients were divided into 3 main groups according to the degree of HLA mismatch: Group I: 0, 1, 2 HLA mismatch (568 patients). Group II: 3 , 4 HLA mismatch (1462 patients). Group III: 5 HLA mismatch (170 patients Results Demographic and medical characteristics were comparable among the 3 groups. Transplantation from unrelated donors was more frequent among group III (p value: 0.001). Ischemia time was comparable and over 90% of the patients had immediate diuresis.The degree of HLA mismatch affected the choice of induction therapy as lymphocyte depleting agent (ATG) was used more frequently in group III while basliximab was used more frequently in group I (p value: 0.001). Patients in group I were maintained on dual immunosuppressive protocols more frequently than the other 2 groups where triple immunosuppressive protocols were commonly used (p value: 0.02).Tacrolimus-based protocol was used more among group I while cyclosporine-based regimen was frequently used among group III (p value: 0.002, 0.001 respectively). acute rejection episodes were more frequent with group II and III. (p value: 0.001). Chronic rejection was revealed in graft biopsies of group II and III more than group I (p value: 0.046). Incidence of post-transplant Hypertension and Diabetes mellitus was higher in group III (p value: 0.004, 0.016 respectively). Median serum creatinine after 1 year follow up didn't differ significantly between the studied groups. However, serum creatinine was higher in group III after 2, 3, 4 and 5 years post transplantation (p value: 0.006) with subsequent lower creatinine clearance (p value: 0.036). The majority of patients were alive with functioning graft at last follow-up especially in group I with statistically significant difference among the 3 groups (p value: 0.001). More patients were alive with failed graft at last follow up in group III than in the other 2 groups with statistically significant difference (p value: 0.003). There was comparable percent of patients among the 3 groups were died either with functioning or with failed graft. On the other hand the 5, 10 and 15 years graft and patient survival showed statistical significant difference among the 3 groups with better survival for group I (p value: 0.04, 0.001) Conclusion The degree of mismatch affected the choice of immunosuppressive regimen. Higher HLA mismatch was associated with higher incidence of diabetes and hypertension and lower patient and graft survival.

Premarital period is the best time to prepare physical, psychological, and social condition. Health check-up program and reproductive health promotion for prospective wives are ways to form the quality of family’s health. The aim of this study is to figure out the difference of knowledge before and after reproductive health promotion for prospective wives. This study was conducted in September 2017 at Puskesmas Pucang Sewu. This study is a quntitative research with cross sectional method with paired t-test. The sample of this study was 32 respondents and were given some questionnaires which provided questions about prospective wives’ knowledge of reproductive health before and after the promotion. The promotion of reproductive health was done to increase their knowledge in terms of nutrition level, reproductive health to prepare the precious generation. This research found thatbefore counseling 62.5% of respondents had less knowledge. It then decreased to 12.5% after counseling. The average value of respondents before counseling was 50.62 and then increased to 66.25 after counseling. The mean value is 15.625 and the p value is 0.031 or α less than 0.05. Thus, there was a significant difference of prospective wives’ knowledge before and after promotion was given. Puskesmas Pucang Sewu is recommended to keep giving the premarital counseling with lectures and frequently asked questions method and using slide show media because it has been proven to improve wives’ knowledge. Beside, it is necessary to implement collaboration of wives’ program holder with nutrition and psycology officer related about promotion materials in order to improve knowledge about nutrition and psycology management at wives’.
