Background: Sickle cell disease (SCD) and its variants are genetic disorders resulting from the presence of a mutated form of hemoglobin, hemoglobin S (HbS). In this study we want to profile various types of haemoglobins and their relative percentage in sickle cell cases. Also, we will analyse RBC indices such as Hb, HCT, MCV, MCH, MCHC and RDW-CV.Methods: We analysed blood from 200 patients suspected to have Sickle cell hemoglobinopathies and subjected it to Sickling screening test. All positive cases will be subjected to HPLC to separate constituent haemoglobins and CBC analysis was done to check RBC indices.Results: In sickle cell trait (SCT) patients, there is a significantly higher level of HbA2 and HbS and significantly lower level of HbA. In sickle cell disease patients, there were significantly higher levels of HbA2, HbF and HbS and significantly lower levels of HbA. Both sickle cell trait and sickle cell disease patients had significantly lower levels of haematocrit, MCH and higher RDW CV.Conclusions: While analysing HPLC patterns, appearance of HbS, low levels of HbA and high levels of HbF and HbA2 should raise a suspicion for presence of Sickle cell hemoglobinopathy. There was statistical difference in levels of Hb, HCT, MCH and RDW-CV between cases and controls. High index of suspicion should be maintained when these parameters are on lower side, especially in population who is prone to have sickle cell disorders.

The highest frequency of sickle cell gene in India is reported in Odisha. The present study was taken up to assess the presence of sickle cell disease among febrile patients of a medical college of eastern Odisha. Patients referred from both pediatric and medicine department to the Hematology section of the department of Pathology, SCB Medical College, Cuttack were subjected to measurement of RBC indices, Sickling test, Haemoglobin Electrophoresis and Fetal Haemoglobin Estimation. Out of total 1000 referred patients 76(7.6%) were found to be positive for sickling. Two‐third of sicklingpositive patients had sickle cell trait with electrophoretic AS band. There was a significant association between age and positive sickling (χ2 = 24.357; df = 4, P = <0.0001). No significant association was observed between sickling and gender. Sickle cell positive cases are not uncommon in eastern Odisha. Our study demonstrated sickle cell trait to be more common among screened patients than other forms of sickle cell diseases.

Introduction: With the emergence of the Coronavirus Disease 2019 (COVID-19) as a global pandemic came the concern that it would adversely affect individuals with comorbid conditions. In sickle cell disease (SCD), the apprehension was largely driven by concerns that the disease causes chronic inflammation, organ dysfunction and a prothrombotic state, especially during a vaso-occlusive crisis. Our assumption and hypothesis was that SCD pathology would be exacerbated by SARS CoV-2 infection leading to higher rates of acute chest syndrome (ACS), venous thromboembolism (VTE), more robust inflammatory responses requiring ICU care and increased death rates. Sickle cell trait (SCT) is frequently perceived as a benign condition; however, mounting evidence has shown that severe hypoxia (such as that seen in COVID-19 infection) can trigger sickle-related complication such as VTE, papillary necrosis and splenic infarcts and, in rare cases, ACS. In this retrospective analysis, we aimed to determine if sickle cell disorders (SCD & SCT) would confer poorer outcomes and/or increased mortality due to COVID-19 infection. Methods: Within the MedStar health care system there are 7,551 adult patients with a diagnosis of sickle cell disorder. We performed a retrospective chart review of all patients in the MedStar system with hemoglobinopathy and a lab-verified diagnosis of COVID-19 infection between March 1, 2020 and June 30, 2020. Sixty-one patients met our search criteria with 21 patients admitted to the hospital and 40 patients managed in the outpatient setting. Of the 21 patients admitted: 9 patients had sickle cell disease (Hb SS, Hb SC, Hb SB thalassemia) and 11 patients had sickle cell trait. One patient had HbC disease. Results: Results were analyzed by SCD genotype (Hb SS, Hb SC, Hb SB Thalassemia) versus the presence of a SCT. Due to the small sample size, statistical significance was not met in the variables. In the 21-patient cohort: 76.2% were female (60% of SCD and 90.9% of SCT) and 100% were African American. Median age was 46.5 years in the SCD group and 38 years in the SCT group. Length of stay was 10 days in the SCD group and 6 days in the SCT group. Mean peak white blood cell count and platelet count were significantly higher in SCD group (WBC: 15.1 k/uL vs 7.5 k/uL; p value 0.099. Platelets: 523 k/uL vs 185 k/uL; p value 0.082). Inflammatory markers were more elevated in the SCD cohort: mean Ferritin (915 ng/mL vs 520 ng/mL), mean D Dimer (2.96 mg/L vs 1.07 mg/L) and mean LDH (543 units/L vs 392 units/L). However, mean peak C-reactive protein (CRP) was higher in the SCT group (117.5 mg/L vs 67.7 mg/L). Admission to the intensive care unit (ICU) occurred in 11.1% with SCD & 36.4% with SCT and renal replacement therapy was required in 11.1% with SCD & 28.6% with SCT. Nasal cannula was required in 75% of patients (88.9% of SCD & 57.1% of SCT; p value 0.262) with only one SCT patient requiring intubation and mechanical ventilation whom later died. Two patients with Hb SS developed ACS, one of whom died. One patient with HbSC disease developed ACS and an acute cerebrovascular accident. No VTE or pulmonary embolisms were documented in this cohort. Therapeutic drugs used in the cohort varied, with 43.8% receiving antibiotics (other than Azithromycin), 37.5% receiving Azithromycin and 25% receiving Hydroxychloroquine. None of the patients received convalescent plasma, steroids, tociluzumab, sarilumab or remdesivir. Anticoagulation was used in 93.8% of the patients (13.3% of which were in therapeutic doses). Conclusions: Sickle cell disease diagnosis did not seem to influence response to COVID19 illness when compared to age-matched individuals with SCT. The number of patients with sickle cell disease admitted to the hospital after contracting SAR CoV2 infection was also remarkably low. These findings were, in general, unexpected. Although the inflammatory biomarkers were generally higher in the patients with SCD, more patients with SCT were admitted to the ICU and required renal replacement therapy. The major drawbacks of the study are the small sample size and missing data points (lack of labs, lack of documentation, transfer to another facility) that did not allow us to draw statistical conclusions about the differences in the patients with SCD versus SCT. We were also unable to determine the influence of SCD severity and pre-existing organ complications on outcomes in COVID 19 disease. Additional studies with larger cohorts is warranted. Although sickle cell disease has long been viewed as a contraindication to free flap transfer, little data exist evaluating complications of microsurgical procedures in the sickle cell trait patient. Reported is the case of a 55-year-old woman with sickle cell trait who underwent a deep inferior epigastric perforator (DIEP) microvascular free flap following mastectomy. The flap developed signs of venous congestion on postoperative day two but was found to have patent arterial and venous anastomoses upon exploration in the operating room. On near-infrared indocyanine green angiography, poor vascular flow was noted despite patent anastomoses and strong cutaneous arterial Doppler signals. Intrinsic microvascular compromise or sickling remains a risk in the sickle cell trait population as it does for the sickle cell disease population. Just like in sickle cell disease patients, special care should be taken to optimize anticoagulation and minimize ischemia-induced sickling for patients with sickle cell trait undergoing microsurgery.

Sickle cell trait was included because, at that time, a great deal of speculation and new information was forthcoming regarding sudden death in military recruits who had sickle cell trait. The members of the Sports Medicine Committee believed that it was important to indicate that, in spite of these new concerns, there were no data to indicate that anyone with sickle cell trait should not be included in any athletic activities. Sickle cell disease was excluded because it is a disease with variable expression and one which is characterized by numerous exacerbations and periods of quiescence.

Hyposthenuria was investigated in subjects with sickle cell trait and in patients with sickle cell anemia. The following were observed: 1) in subjects with sickle cell trait both normal and reduced maxima of urinary concentration are found, whereas all untreated patients with sickle cell anemia over 6 months of age have hyposthenuria; 2) hyposthenuria becomes increasingly more severe with advancing age in both sickle cell anemia and sickle cell trait; 3) in a 6-month-old patient with sickle cell anemia and hyposthenuria, the maxima of urinary concentration returned to normal after two transfusions of normal erythrocytes. Reasons are presented for favoring the hypothesis that hyposthenuria in sickle cell disease is due to renal damage, possibly from intravascular sickling of erythrocytes in renal vessels or from the presence of "free" circulating S-hemoglobin.

A patient who presented with sickle retinopathy and hemoglobin electrophoresis results compatible with sickle cell trait was found, on further investigation, to be a compound heterozygote with hemoglobin S and hemoglobin New York disease. This recently reported form of sickle cell disease was not previously known to cause retinopathy and surprisingly was observed in a non-Asian individual. The ophthalmological findings, the laboratory diagnosis, and possible pathophysiology of this disorder are discussed. Persons diagnosed with sickle cell trait who present with symptoms of sickle cell disease may benefit from specific screening for this variant.

Abstract People with sickle cell disease have a chronically activated coagulation system and display hemostatic perturbations, but it is unknown whether they experience an increased risk of venous thromboembolism. We conducted a case–control study of venous thromboembolism that included 515 hospitalized black patients and 555 black controls obtained from medical clinics. All subjects were assayed for hemoglobin S and hemoglobin C genotypes. The prevalence of the S allele was 0.070 and 0.032 for case patients and controls, respectively (P < .001). The odds that a patient had sickle cell trait were approximately twice that of a control, indicating that the risk of venous thromboembolism is increased approximately 2-fold among blacks with sickle cell trait compared with those with the wild-type genotype (odds ratio = 1.8 with 95% confidence interval, 1.2-2.9). The odds ratio for pulmonary embolism and sickle cell trait was higher, 3.9 (2.2-6.9). The prevalence of sickle cell disease was also increased among case patients compared with controls. We conclude that sickle cell trait is a risk factor for venous thromboembolism and that the proportion of venous thromboembolism among blacks attributable to the mutation is approximately 7%.
