Background: Gastritis is one of the commonest clinical conditions encountered by a physician. The most common cause is said to be infection with Helicobacter pylori. The present study was aimed to diagnose the prevalence of H. pylori infection among the cases of gastritis and its correlation with histopathological findings and associated rapid urease test.Methods: A one year prospective study at a tertiary care hospital was conducted and all cases of gastritis were included and socio demographic data, clinical complaints and duration were noted. Endoscopy was performed on all cases. Biopsy was performed histopathological examination with staining and graded by Houston-updated Sydney system. Rapid urease test was performed and findings noted.Results: 325 cases with 215 male and 110 female cases were included. 26.15% were in age group of more than 60 years and number of cases increased with age. Mean age of study participants was 39.12±2.8 years and heart burn was the common complaint in the study cases. 81.54% of the cases revealed endoscopic gastritis on endoscopy and hyperaemia was commonest. 83.69% were positive by rapid urease test. Rapid urease test is more sensitive than histological staining in confirmation of H. pylori infection.Conclusions: To conclude on the present study, the prevalence of H. pylori infection is on a global rise and appropriate measures to reduce the prevalence is quite an urgent necessity. Histopathological interpretation of gastric biopsies is a reliable indicator of H. pylori infection as well as gastritis grading according to the Sydney grading system.

Background: The incidence of bacterial lesions has been on the rise over the decades. In our study, we assessed and studied the spectrum of bacterial lesions retrospectively for one year and their clinicopathological correlation which was diagnosed on histopathological examination in a tertiary health care center.Methods: The present study (retrospective) has been done to analyse the incidence of bacterial infections obtained in the histopathology laboratory and study its clinical correlation and significance. Clinical details of the cases were accessed from biopsy requisition forms and included tissues from various sites in the body. Special stains were done which included acid fast bacilli (AFB), Fite Farraco (FF) stain and gram stain for the identification of bacteria.Results: A total of 318 cases including 288 H. pylori infections, granulomatous inflammation 18, Hansen disease 9 and 3 actinomycosis infections.Conclusions: The histopathological diagnosis of bacterial lesions are of utmost importance as it is more reliable than culture in circumstances of reduced tissue availability. Early reporting is extremely necessary to reduce complications related to bacterial lesions.

Objectives: Recurrent abdominal pain (RAP) impacts quality of life of the children. RAP also hampers education and physical activity of the children. Current study was aimed to find out the frequency of Helicobacter pylori in children with RAP in our tertiary care hospital. Study Design: Descriptive, cross-sectional study. Setting:  Department of Pediatric Medicine, Nishtar Hospital, Multan, Period: From 27-12-2017 to 26-06-2018. Material & Methods: A total of 185 patients suffering from RAP, aged 2-12 years, with a disease duration > 3 months, were enrolled. Age of the children, gender, duration of illness, number of episodes of pain, maternal literacy, family income, residential status, source of drinking water and h.pylori infection were calculated in these children. Post stratification chi-square test was applied to see its effect on H. Pylori infection. Results: Of these 185 study cases, 101 (54.6 %) were male patients while 84 (45.4%) were female. Mean age of our study cases was 7.57 ± 1.93 years. Of A total of 95 (51.4%) children belonged to rural areas and 90 (48.6 %) to urban areas. Helicobacter pylori infection was noted in 103 (55.7%) of our study cases. When helicobacter pylori  was stratified with regards to study variables, male gender, age < 8 years, monthly family income <Rs. 35000, source of drinking water as Hand Pump and disease duration < 6 months turned out to be statistically significant (P value < 0.05). Conclusion: Frequency of H.pylori was high in children with RAP. Helicobacter pylori was significantly associated with male gender, younger age, poor socioeconomic status, source of drinking water and disease duration.

Background: Thyroid nodules are a common finding in general practice. These nodules are either solitary or multinodular. In the present study thorough evaluation of all the cases presenting with a solitary thyroid nodule (STN) is done. The clear overview of prevalence of STN, associated risk factors, its distribution and its percentage of malignancy, clinic-pathological correlation and findings on ultra-sonogram.Methods: A one year cross sectional study at a tertiary care hospital was done after ethical committee approval. All cases of thyroid with solitary thyroid nodule were included and socio demographic data, clinical examination and USG data was noted. Thyroid hormone profile, FNAC and HPE was performed for every case enrolled and data was noted. The data was analyzed using SPSS version 22.Results: 350 cases were enrolled with 44.42% prevalence and 61.43% were females. Maximum age group was 31-40 years with swelling as the most common sign. 81.14% were euthyroid, 6.3% of cases had lymph node involvement. Micro calcification in 69.7% of nodules and 78.65 were solid. 40.3% of the STN were of colloid on FNAC and 6% were malignant. Malignancy on HPE was 14.6% and papillary carcinoma was the most commonest and follicular least.Conclusions: All cases of STN require a thorough clinical approach supported by ultra-sonogram, FNAC and detailed HPE after surgery for evaluation of benign and malignant lesions. Fine needle aspiration cytology has become an invaluable, minimally invasive and reliable tool in the preoperative assessment of patients with suspicion of malignancy.

Objectives: To determine the frequency of Helicobacter pylori infection in the local population presenting with dyspeptic symptom report to a tertiary care hospital of Nawabshah. Methodology: This cross-sectional study took place at department of medicine, Peoples Medical College and Hospital Nawabshah; during six months, form August 2015 to February 2016. A total of 115 patients with dyspeptic symptoms with upper abdominal disorders such as discomfort and pain, nausea, fullness, bloating, heartburn, anorexia, belching and regurgitation were included. After conducting detailed clinical examination and history, patients were subjected to relevant examinations, i.e. Blood antibody test (IgA antibody test), for H. pylori and venous blood was drawn. All the information was collected in the proforma. Results: The patients’ average age was 30.92±6.10 years. There were 66(57.4%) male cases and 49(42.6%) female cases. Helicobacter pylori (H. pylori) infection was observed to be frequent (60% cases) among both genders. Conclusion: In conclusion, the present results confirm that the frequency of patients suffering from helicobacter pylori infection was quite high in local population at tertiary care hospital, Nawabshah. Strategies could be developed at an earlier stage and prompt treatment may reduce the morbidity. Key Words: Dyspepsia, Helicobacter pylori, IgA antibody test

Background: Prevalence of Helicobacter pylori (H. pylori) emerges throughout the world and instigates peptic ulcer disease (PUD). The study was conducted with the aim to determine the prevalence of H. pylori in patients with PUD undergoing upper gastrointestinal endoscopy.Methods: This prospective study was conducted on 150 cases of PUD from August 2009 to February 2011. Endoscopy was done in all cases. Biopsy was done and sent for histopathological examination and rapid urease test for confirmation of presence of H. pylori.Results: Out of 150 patients with mean age of 45.76 years, 109 patients were diagnosed to have been infected with Helicobacter pylori (72.66%). Out of 89 patients with gastric ulcer, 61 patients were infected with Helicobacter pylori (68.53%). Forty two out of 51 patients (82.35%) with duodenal ulcers and 06 of 10 patients (60%) with carcinoma of stomach were positive for H. pylori. The remaining patients were found to be negative for the H. pylori infection.Conclusions: The findings of the study conclude that H. pylori was consistently associated with PUD.

Background: Helicobacter pylori (H. pylori), a curved rod shaped bacteria, is found to be associated with acid peptic disease patients, more in ulcer than non-ulcer, leading to a belief that H. pylori plays a role in its etiopathogenesis. In this study we examine the prevalence of H. pylori infection among dyspepsia patients with mucosal lesion.Methods: 300 patients of dyspepsia were enrolled in the study as per inclusion/exclusion criteria for a period of eight months. Enrolled patients were subjected to upper gastro-intestinal endoscopy and findings were recorded. Biopsy specimen taken from mucosal lesions were immediately inoculated on ‘RUT dry kit’ to test for H. pylori infection.Results: 234 out of 300 patients (78%) were found to be positive for H. pylori infection. The incidence of H. pylori infection was higher in patients with ulcer dyspepsia (88.46%) when compared to the patients with non-ulcer dyspepsia (72.44%).Conclusions: Prevalence of H. pylori infection was high and was found to have an association with ulcer dyspepsia allowing us to conclude that H. pylori may have a role in its etiopathogenesis.

Helicobacter pylori (H. pylori) plays a crucial role in the pathogenesis of gastritis, peptic ulcer, and gastric cancer. The presence of pathogenicity islands (PAI) genes contributes to the pathogenesis of many gastrointestinal disorders. Cytotoxin-associated gene A (cagA) and vacuolating cytotoxin gene (vacA) are the most known virulence genes in H. pylori. So, our aim was to study H. pylori virulence genes’ role in gastric disorders pathogenesis. Our study included 150 adult patients who suffered dyspeptic symptoms and were referred to the GIT endoscopy unit. Gastric biopsies were attained for rapid urease test (RUT) and histopathological examination, and multiplex PCR technique for detection of virulence genes was performed. It was found that 100 specimens were (RUT) positive, of which sixty samples (60%) were PCR positive for H. pylori ureC gene. The vacA and cagA genes were identified in 61.6% and 53% of H. pylori strains, respectively. Only 5 cases were vacA-positive and cagA-negative. The most virulent vacA s1 allele existed in 56.6% of cases. Out of the 60 H. pylori strains, 66% had at least one virulence gene and 34% did not show any virulence gene. H. pylori infection showed significant increase with age. H. pylori are prevalent amid dyspeptic patients in our region. The main genotype combinations were vacA+/cagA+ of s1m1 genotype and they were frequently associated with peptic ulcer diseases, gastritis, and gastroesophageal reflux disease.

Introduction: Helicobacter pylori (H. Pylori) as a primary etiological factor in carcinoma stomach. Associationof H. Pylori in gastric cancer has been documented to be in more than 50% of cases. In underdevelopedcountries, this association is shown to be much higher according to different studies.Methods: A prospective observational study of 40 consecutive cases of carcinoma stomach was under takenin surgical department of Shree Birendra hospital and Bir hospital, for a period of two years 2009 to 2011.Location and pathological types of the lesion were noted and all specimens were investigated to see presenceof helicobacter pylori by rapid urease test (RUT) and histological examination.Results: Out of 40 patients, helicobacter pylori positivity was seen in 27 (67.5%) cases by both rapid ureasetest and Histopathological examination (HPE). Regarding the pathological types, out of 26 intestinal type of castomach, 20 (76.92%) cases were positive for H. pylori infection, whereas out of 14 diffuse type of ca stomach,7(50%) cases were positive for H. pylori. In 29 cases of distal ca stomach, H. pylori positivity was seen in 22(75.86%) cases, whereas in rest of the 11 cases the lesion involved other part of the stomach, H. pylori positivitywas seen in 5(45.45%) cases.Conclusion: Helicobacter pylori infection is higher in prevalence in cases of stomach cancer. Its associationwith intestinal histological type of stomach cancer is more common than diffuse type. Objectives: Determination of serum ferritin and iron profile in Helicobacter pylori infected young adult male reporting at a tertiary care hospital of Sindh. Study Design: Case control study. Study Setting & Duration: Department of Medicine, Layari General Hospital Shaheed Muhtrama Benazir Bhutto Medical College from January 2015 to February 2016. Study Subjects and Methods: 100 cases (H. pylori stool specific antigen positive) and 100 controls (H pylori negative) were selected and studied. 5 ml blood was collected in a disposable syringe by venesection. 2 ml was put in EDTA tubes and 3 ml was centrifuged (3000 x rpm for 10 minutes). Sera were used for the estimation of iron profile. Elisa assay method (Fortress diagnostics) detected the H. pylori stool specific antigen (HpSA). Data was analyzed on SPSS 22.0 (USA) at 95% CI (P≤ 0.05). Results: Age of control and cases was noted 35.48±4.79 and 33.60±3.96 years (P=0.053). Hb, Hct and RBC counts were low in cases (P=0.0001) significantly. Serum iron (Fe++), ferritin and TIBC in controls and cases were noted 152.72±6.08 and 118.79±43.30 μg/dl, 394.34±136.50 and 529.87±101.0 ng/dl, & 140.80±19.99 and 130.88±28.46 μg/dl respectively (P=0.0001) (Table-I). Conclusion: Helicobacter pylori infection causes iron malabsorption as detected by serum iron and ferritin and total iron binding capacity in young adult male.
