Background: Specialized female surgical clinics are the basic requirement in surgical department which should be addressed routinely. The management of surgical problems in female patients requires multidisciplinary approach mainly general surgery, obstetrics and gynaecology, general medicine, paediatrics, dermatology and psychiatry. If proper care is provided then this will uplift the follow up of same patients in such clinics. The objective of the study was to assess the success rate of female surgical clinics and to determine the follow up visits done by female patients once visited the female surgical clinics.Methods: Prospective educational interventional study was conducted. A total of 952 patients visiting the clinic were studied. Statistical analysis was done.Results: Out of 952 cases 944 (99.2%) cases were willing to continue visit to the clinic, while 809 (84.9%) cases revisited for follow up treatment in female surgical clinics within one year duration.Conclusions: It can be concluded that there is a requirement and need of a fully functional specialized female surgical clinic in our hospital. As the success rate for follow up visit by patients in this clinic was high.

Background: Specialised female surgical clinics are the need of the day and not addressed routinely in the Surgical Department. The management of surgical problems in female patients requires multidisciplinary approach mainly general surgery, obstetrics and gynaecology, general medicine, paediatrics, dermatology and psychiatry. The aim and objective was to introduce a new concept of subspecialty and super speciality in management of female surgical diseases.Methods: Prospective educational interventional study. 18 out of 60 sensitised interns were introduced and oriented in the specialised female surgical clinic. A total of 952 patients visiting the Clinic were studied. Statistical analysis was done.Results: Feedback of female patients were taken and recorded in a proformas and their analysis was done. There was a significant improvement in the knowledge and skill of the participating interns.  99.2% (944 cases) of the female patients were willing to continue their visit to such a specialized clinic in future.Conclusions: It can be concluded that there is a requirement and need of a fully functional specialized female surgical Clinic in our hospital.

Background: Hypertension is one of the leading causes of the global burden of disease. Hypertension has been associated with increased risk of cardiovascular diseases, including coronary heart disease (CHD), congestive heart failure (CHF), ischemic and hemorrhagic stroke, renal failure, and peripheral arterial disease. Majority of the hypertensive population are still either untreated or inadequately treated. Aims and objectives of the study were to compare ramipril alone and in combination with telmisartan as an antihypertensive in mild to moderate hypertension.Methods: This study was a hospital based prospective, comparative randomized, observational study conducted over a period of one year. The subjects of this study had mild to moderate hypertension selected from outpatient department of department of General Medicine of a tertiary care hospital. For the purpose of this study, equal numbers of subjects were randomly allocated equally between two groups: one group on ramipril alone and the other group on combination of ramipril and telmisartan. Patients were assessed for the blood pressure (BP) reduction during follow-up period of 6-months.Results: Ramipril alone and in combination with telmisartan, both were associated with significant reduction of systolic BP (SBP), diastolic BP (DBP), and mean BP (MBP) from beginning to the end of study. Combination of ramipril with telmisartan was more effective than ramipril in lowering SBP during 4 to 12 weeks but at the end of study both drug groups were found to be equally effective antihypertensive. Both ramipril alone and in combination with telmisartan were equally effective in lowering DBP and MBP from beginning to end of study.Conclusions: There was a significant reduction of SBP, DBP, and MBP from beginning to the end of study with both ramipril alone and in combination with telmisartan. Ramipril alone and in combination with telmisartan, both were equally effective antihypertensive for mild to moderate hypertension.

The main aim and objective of this study is to assess the effectiveness of training and orientation of interns by introduction of specialized female surgical clinic.Methods: Prospective educational interventional study was conducted. 60 sensitized interns were introduced and oriented in the specialized female surgical clinic. A total of 952 patients visiting the clinic were studied. Statistical analysis was done.Results: Feedback of interns with respect to female patients were taken and recorded. There was a significant improvement in the knowledge and skill of the participating interns. Most of the interns were willing to continue their duties to such a specialized clinic in future.Conclusions: It can be concluded that the interns sensitized and exposed to this innovative method of training have shown a positive approach to this new method of learning. Such fully functional specialized female surgical clinic is required in hospital, in which participation of interns is useful.

Background: Physicians working in casualty are often confronted with acute abdomen and get much more bothered as the diagnosis is not easy. This is due to the fact that the etiology of acute abdomen is always much diversified and the classical findings are masked making the diagnosis difficult. Objective was to study incidence of appendicitis and ovarian cyst among female patients presenting with acute abdomen.Methods: A hospital based follow up study was carried out among 64 cases presented with acute abdomen to the casualty from January 2018 to August 2018. All necessary investigations were done to confirm the etiology of acute abdomen. The cases belonged to surgery and gynecology departments where they were operated. The patients were followed from admission in the casualty to the final outcome. The data was analyzed using proportions.Results: During the study period a total of 15413 patients were admitted in the casualty out of them, 64 (0.42%) presented with acute abdomen. Of these 64 cases, majority i.e. 35 (54.7%) were due to acute appendicitis; 13 cases were due to renal colic and eight cases each were due to ectopic pregnancy and ovarian cyst. The most common age group affected was 21-30 years (62.5%) followed by less than 20 years age (21.9%). 25% of the 64 cases had delayed wound healing and no other complications were reported. No death was recorded.Conclusions: Authors achieved excellent results as there were no major complications and no death was recorded. Thus, meticulous diagnosis and prompt treatment can save patient life and at the same time rate of complications can be reduced.

BackgroundRepetitive laboratory testing in stable patients is low-value care. Electronic health record (EHR)-based interventions are easy to disseminate but can be restrictive.ObjectiveTo evaluate the effect of a minimally restrictive EHR-based intervention on utilisation.SettingOne year before and after intervention at a 600-bed tertiary care hospital. 18 000 patients admitted to General Medicine, General Surgery and the Intensive Care Unit (ICU).InterventionProviders were required to specify the number of times each test should occur instead of being able to order them indefinitely.MeasurementsFor eight tests, utilisation (number of labs performed per patient day) and number of associated orders were measured.ResultsUtilisation decreased for some tests on all services. Notably, complete blood count with differential decreased 9% (p<0.001) on General Medicine and 21% (p<0.001) in the ICU.ConclusionsRequiring providers to specify the number of occurrences of labs changes significantly reduces utilisation in some cases.

Objective: To determine causes behind cut throat injury, analyse the demographics, evaluate treatment, complications, and outcome of cut throat injuries. Methodology: This prospective observational type of study was conducted at a tertiary care hospital during June 2018 to December 2019. A total of 47 cases included in this study. The demographics were recorded, and evaluation of the patient was done as per ATLS protocol. Necessary investigations were performed, and management was done by a team comprised of anaesthetist, otolaryngologist, CTVS surgeon and psychiatrist. After discharge of the patients, follow up was done for 6 months. Results: In our study the M: F is 3.27:1 and 36-50 years age group were mostly affected. Most common cause of injury was suicidal (61.70%). Majority patient had zone II injury (76.6%). 68.09% patients discharged within 8-14 days. Alteration of voice was most common complications (14.9%).61.70% patients are living completely normal life without any morbidity. Mortality rate after admission was 8.51%. Conclusion: Cut throat injury is common in middle aged male and rural people. Suicidal injuries are most common cause behind it. Early planned interventions, multidisciplinary approach, and regular follow up can reduce the mortality and morbidity of the cut throat injury patient.

Objective: To evaluate the success rate of endoscopic endonasal dacryocystorhinostomy at tertiary care hospital. Material and methods: Between July 2020 to December 2020, total 50 patients having age between 20-60 years, both gender, cases of epiphora, cases with evidence of obstruction were recruited from Department of ENT, DG Khan Medical Collge, DG Khan. Success of DCR was evaluated. Results: Fifty cases were selected for this study. Age range was 20-40 years with mean age 38.80 ± 8.74 years. DCR was successful in 47 (94%) patients while in 3 (6%) patients surgery was not successful. Male patients were 35 (70%) while female patients were 15 (30%). Surgical success was noted in 33 (94.29%) male patients and 14 (93.33%) female patients. Insignificant (P = 1.00) association between surgical success and gender was noted. Conclusion: In this study high success rate of DCR was noted. Males were prominent. Most of the patients were between 20-40 years and no association of success of surgery with age group and gender found. Keywords: Dacryocystorhinostomy, Epiphora, Intubation, Chronic dacryocystitis

Purpose Haemorrhage, preeclampsia and sepsis are the leading causes renal dysfunction in women with a maternal nearmiss(MNM) complication. The study aimed to assess the prevalence, pattern and follow up of these women. Methods This was a hospital based prospective observational study, conducted over one year. All women with a MNM leading to acute kidney injury (AKI) were analysed for fetomaternal outcomes and renal function at 1 year of followup. Results Incidence of MNM was 43.04 per 1000 livebirths. 18.2% women developed AKI. 51.1% women developed AKI in the puerperal period. Most common cause of AKI was haemorrhage seen in 38.3% women. Majority of women had s.creatinine between 2.1 to 5 mg/dl and 44.68% required dialysis. 80.8% women recovered fully when the treatment was initiated within 24 h. One patient underwent renal transplant. Conclusion Early diagnosis and treatment of AKI results in full recovery.

Objective: To determine the frequency of new-onset hypertension in patients with Rheumatoid arthritis taking leflunomide, in comparison with methotrexate in Asian setting. Material and Methods: Perspective case-control study was conducted in 2014 in a tertiary care hospital located in Karachi, Pakistan. Adult patients, having rheumatoid arthritis were randomly prescribed leflunomide or methotrexate. Patients having chronic hypertension, proteinuria and chronic kidney disease were excluded. Patients were monthly followed for blood pressure and heart rate measurements. Hypertension was defined using JNC 7 criteria. Results: Out of 144 patients enrolled, 80 patients received Leflunomide while 64 were started on methotrexate. Mean systolic blood pressure in leflunomide group at the start and at the end of study was 108.5 and 135.4mmHg, respectively while in methotrexate group, mean systolic BP was 109.8 and 110.5 mmhg, respectively. After one year follow up, 33 out of 80 (41%) patients were receiving leflunomide had pre-hypertension or hypertension, while only 3 out of 64 patients (4.7%) were receiving methotrexate had hypertension. Conclusion: Risk of developing hypertension in patient receiving Leflunomide is much higher in Asian population like Pakistan as compared to western population.
