During long-term orthodontic treatment there are various complications associated with a reduction in the level of hygiene. The article is devoted to the study of the hygienic condition of the oral cavity and the level of dental education in dynamics in patients undergoing orthodontic treatment. The examination and questioning of 40 patients aged from 18 to 35 years with fixed orthodontic appliances. In patients after 12 months of use braces deteriorating hygienic condition of the oral cavity. Care should be taken to inform patients about the care products and hygiene items, and their rational use, depending on the timing of orthodontic treatment and dental status.

Abstract Background Hereditary gingival fibromatosis (HGF) is rare in clinical practice, and the long-term results of the combined orthodontic-periodontal treatment of HGF are rarely reported. Case presentation This study reports for the first time the results of seven years of follow-up in a seven-year-old girl with HGF. The diagnosis was confirmed by clinical signs, family history and histopathological examination. First, periodontal scaling and oral hygiene reinforcement were performed regularly in the mixed dentition stage. Next, gingivoplasty was performed on the permanent dentition. Two months after the surgery, treatment with fixed orthodontic appliances was conducted. The teeth were polished on a monthly basis, and oral hygiene was reinforced to control gingival enlargement. Gingival hypertrophy recurred slightly, and gingivectomies were performed in the months following the start of orthodontic treatment. Follow-up was performed for 24 months with orthodontic retention, and gingival enlargement remained stable after the combined treatment. Conclusions The risk of gingival hyperplasia recurrence during and after orthodontic treatment is high, but satisfying long-term outcomes can be achieved with gingivectomy, malocclusion correction, and regular follow-up maintenance.

BACKGROUND The placement of orthodontic apparatus in the oral cavity, according to the literature, should influence the alteration of oral flora, especially the subgingival one. The purpose of the study is to evaluate the subgingival flora of patients with fixed orthodontic appliances, regardless of placement time. OBJECTIVE The purpose of the study is to evaluate the subgingival flora of patients with fixed orthodontic appliances, regardless of placement time. METHODS In 3 cases of patients with fixed orthodontic appliances, a bacterial sample of the gingival sulcus was taken for laboratory examination. Patients were clinically evaluated for the presence or tendency, of having gingival hypertrophy. RESULTS Results from the 3 cases included in the study, 1 of them came up with Streptococcus Anginosus positive, Doxycilin-sensitive. The tendency for gingival hypertrophy was maximal 3% to 1.5% respectively in each patient. In the patient with different oral flora, daily topical treatment with tetracycline, placed in the gingival sulcus, was applied. CONCLUSIONS Alteration of the oral flora with the placement of fixed orthodontic appliances is not a fully verifiable fact, as it indicates the patient's follow-up, at the time of placement of the apparatus and until removal after orthodontic treatment, depending on the 2-3-year period of treatment. The tendency for gingival hypertrophy is apparently high, versus the presence of fixed orthodontic apparatus.

Background/Aim. Orthodontic treatment enables the establishment of functional occlusion and improvement of oral health, however, it increases the risk of periodontal disease development. The aim of this paper was to examine the efficiency of the applied programme for the prevention of gingivitis in children undergoing the fixed orthodontic appliance therapy and to determine the most efficient devices and techniques for maintaining oral hygiene during orthodontic treatment. Methods. The study included 80 patients of both genders - 60 patients comprised the experimental group and 20 patients comprised the control group. All of them were patients of the Clinic for Orthodontics at the School of Dentistry in Belgrade, aged between 13 and 18. The Silness-L?e Plaque Index (PI) was utilised for the assessment of oral hygiene quality and Silness-L?e Gingival Index (GI) and M?hlemann Papilla Bleeding Index (PBI) were utilised for the assessment of gingival state. Checkups were conducted as a single-blind study at the beginning and after the first, the third and the sixth month of the preventive and prophylactic programme. Results. During the observed period, a statistically significant change in PI, GI and PBI values was noticed (p < 0.005), as well as the difference in the dynamics of value changes during the periods between the observed groups. Conclusion. The preventive programme, applied to children undergoing the fixed orthodontic appliance therapy, had a positive effect both on oral hygiene quality and gingival state. The values of the examined parameters of the patients from the experimental group were significantly lower in comparison with those of the patients from the control group. The most efficient combination of devices for oral hygiene during orthodontic treatment was: a Curaprox CP5460 toothbrush, CD Ortho 60 orthodontic toothbrush and Curaprox CPS 14 interdental brush.

Introduction. During orthodontic treatment, there is increased risk of periodontal disease and caries. Therefore these patients must be trained to maintain proper oral hygiene to minimize risks. The aim of this study was to examine the effects of oral hygiene training with different devices as well as a motivation and remotivation in oral hygiene using brochures, verbal methods and tablets for plaque identification. Material and Methods. The study included 80 subjects, both genders, divided in control (20) and experimental groups (60). All examinees in the experimental groups used toothbrush Curaprox CP5460 for oral hygiene. Subjects in experimental group 1 used mouthwash Curasept ADS 205, in the experimental group 2 orthodontic toothbrush CD Ortho 60 and proximal toothbrush Curaprox CPS14 while subjects in experimental group 3 used proximal toothbrush Curaprox CPS14 and mouthwash Curasept ADS 205. All participants were given tablets erythrosine and brochure in which was explained and illustrated the use of the aforementioned means for oral hygiene. Control check ups were conducted at the beginning, after the first, third and sixth month of the preventive program. Results. The values of the examined parameters, plaque index (PI), gingival index (GI) and gingival bleeding index (GBI), in experimental groups were significantly lower compared to control group (p<0.05). Conclusion. Motivation, compliance and implementation of all preventive procedures proposed by this program, as well as regular controls, contributed in maintaining gingival health during orthodontic treatment.

Background: During the course of fixed orthodontic therapy, it is difficult to maintain high standard oral hygiene care by mechanical tooth cleaning only. Therefore, mouthwashes are commonly used as adjunct for mechanical tooth cleaning. Most of the mouthwashes are chemicals and have a wide range of side effects. Plant extracts have stood the test of time as safe inexpensive and available remedy for providing general health care. Objective: The present study attempted to evaluate the efficacy of gargling with aqueous extract of Dandelion (Taraxacum officinale) on the oral hygiene status of patients wearing fixed orthodontic appliances. Methods: Eleven male patients undergoing orthodontic treatment with fixed appliances were recruited in the study. All patients were given the same oral hygiene regimen during the study period. Clinical evaluation of OPI (orthodontic plaque index) and salivary streptococcus mutans count were recorded at the first activation visit (three weeks after placement of the orthodontic appliances) which considered the baseline visit and after one week of gargling with the extract. The data were statistically analyzed with SPSS (version 21) using Paired Samples t-test. Result: The results of the statistical analysis revealed that, there were highly significant reductions in both OPI and salivary streptococcus mutans count after one week gargling with aqueous extract of Dandelion (Taraxacum officinale). Conclusion: The present finding concluded that, aqueous extract of Dandelion (Taraxacum officinale) could be use as a promising ingredient in manufacturing mouthwashs and other dental supplies that contribute in maintaining good oral hygiene during the course of orthodontic treatment with fixed appliances.

The prevalence of malocclusion and a need for orthodontic treatment is high. Orthodontic appliances increase biofilm accumulation by expanding plaque retention sites. The aim of this study was to investigate the self-reported changes in oral hygiene habits among adolescents receiving orthodontic treatment. A cross-sectional study of 291 patients aged 10–17 years (mean (M) = 12.98; standard deviation (SD) = 2.36) was conducted in the Department of Orthodontics, Lithuanian University of Medical Sciences (LSMU) Hospital (Kaunas, Lithuania) during the fall semester (October–January) of the 2017/2018 study year. An anonymous self-administered questionnaire covered background information, experience of orthodontic treatment, oral hygiene habits and the seeking of professional dental care. Statistical data analysis was performed using SPSS version 22. To establish relationships between categorical variables, Chi-squared tests (χ2) were used. A p-value ≤ 0.05 was set to indicate statistically significant difference. The univariate logistic regression analysis evaluated the probability of an event given a certain risk indicator, including odds ratio (OR) and its confidence interval (95% CI). Associations were found between the usage of auxiliary measures (OR = 1.797 (1.118–2.887), p = 0.015), tongue cleaning (OR = 1.712 (1.059–2.767), p = 0.028), mouth rinsing after meals (OR = 1.707 (1.048–2.781), p = 0.032) and experience of orthodontic treatment, respectively. More orthodontic patients underwent professional oral hygiene regularly than non-orthodontic patients (p = 0.024). More patients with fixed orthodontic appliances reported significantly changed oral hygiene habits, while more orthodontic patients with removable appliances did not change their oral hygiene habits.

Abstract Aim The aim of this study was to evaluate the oral hygiene status of patients with fixed orthodontic appliances. Methods and Materials The following indices were used to evaluate the oral hygiene status of patients in orthodontic treatment: gingival bleeding index (GBI), plaque index (PI), and ortho-plaque index (OPI). A self-administrated questionnaire was prepared covering oral hygiene practice, oral hygiene cleaning aids, and number of visits to a dental hygienist. Results Fifty patients (15–30 years old) were selected for the study from among the orthodontic patients treated at the King Saud University College of Dentistry, Riyadh, Saudi Arabia. Results showed that the PI and OPI were high with mean scores of 65.24 (SD 16.43) and 53.56 (SD 8.74) respectively, while the average GBI was a much lower value at 19.14 (SD 7.95). No significant difference was observed between male and female patients for the PI (p=0.925) and for the OPI (p=0.072), but a significant difference was observed for the GBI at the 5 percent significance level (p=0.033). The result of OPI showed that 20 (40 percent) of the patients had fair oral hygiene, whereas 30 (60 percent) had poor oral hygiene. Only 16 (32 percent) of the participants reported visiting the dental hygienist during their orthodontic treatment, while the remaining 34 (68 percent) did not. Conclusion The oral home care of the orthodontic patients surveyed was not at an optimal level, which indicated the need to establish an oral hygiene maintenance program. Clinical Significance Inadequate oral home care among orthodontic patients may make them more prone to develop gingivitis during orthodontic treatment. It is, therefore, essential that oral hygiene instructions and a hygiene maintenance program not be overlooked during orthodontic treatment. Citation Atassi F, Awartani F. Introduction. The theme of a healthy lifestyle remains relevant at the present time. The function of the dentofacial system is an integral and important component of human health in general, is responsible for speech formation, respiration and nutrition. The goal of orthodontic treatment is to preserve the health of ones own teeth, to establish the correct function of the masticatory apparatus, to create a smile aesthetics, to exclude or facilitate other dental procedures without resorting to more invasive methods of treatment. Orthodontic treatment directly affects the quality of human life, so it is important to understand the need for its timely implementation. A large number of people seek orthodontic care in connection with raising awareness among the population and its availability.Results. The fast pace of development of dentistry and modern technologies allow us to correct both congenital and acquired defects of the dentition. For the adult population, one of the main means of orthodontic treatment is a fixed orthodontic technique, in the form of a bracket system. The average duration of treatment with this method is 1.5-2 years. But already in the first month, the orthodontist dentist observes the presence of plaque in his patients, which occurs due to insufficient oral hygiene.Conclusion. Studies have shown that a low level of hygiene entails the growth of carious and periodontopathogenic microflora, as a result of which irreversible changes occur on the surface of the teeth, leading to enamel demineralization and hypersensitivity, the occurrence of gingivitis and periodontitis. These changes, or the risk of their occurrence, force the dentist to prematurely remove orthodontic equipment without achieving the desired result. An analysis of the causes of this problem and the coverage of research results indicate the need to improve the methodology of individual oral hygiene in order to avoid the side effects of orthodontic treatment.

Abstract Objectives: Fixed orthodontic appliances make daily application of oral hygiene standard procedures more difficult and in time may lead to accumulation of oral biofilms and development of gingivitis and hyperplasia. The aim of the study is to evaluate the periodontal health expressed by clinical indices in patients under orthodontic treatment with fixed appliances, according to different oral hygene maintenance programs. Material and method: We performed a randomized prospective study on 60 patients with fixed orthodontic appliances (17-25 years of age) devided in three study groups. The clnical indices recorded were: modified gingival index, plaque index and sulcular bleeding index. Statistical analysis or the results were carried out using Student t test. Results: The patients were randomly divided into three groups: group A-patients were instructed to use electric brush, water flosser and interdental brush, group B- electric brush, interdental brush and fluoride and group C- manual brushing and fluoride. Statistical comparison of the values of the indexes with the Student t test for independent samples showed statistically significant differences in all three groups of patients studied between initial and final values of all recorded clinical parameters. Conclusions: Fixed orthodontics do not induce periodontal disease if basic principles of oral hygiene are followed in compliant patients, which are correctly instructed to deal with real challenge, represented by complete elimination of debris and bacterial accumulation.

ABSTRACT Objective:  To analyze the initial changes in salivary levels of periodontal pathogens after orthodontic treatment with fixed appliances. Materials and Methods:  The subjects consisted of 54 adult patients. The Simplified Oral Hygiene Index, Plaque Index, and Gingival Index were measured as periodontal parameters. Both the plaque and gingival indexes were obtained from the central and lateral incisors and first molars of both arches. Whole saliva and periodontal parameters were obtained at the following four time points: immediately before debonding (T1), 1 week after debonding (T2), 5 weeks after debonding (T3), and 13 weeks after debonding (T4). Repeated measures analysis of variance was used to determine salivary bacterial levels and periodontal parameters among the four time points after quantifying salivary levels of Aggregatibacter actinomycetemcomitans (Aa), Fusobacterium nucleatum (Fn), Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi), Tannerella forsythia (Tf), and total bacteria using the real-time polymerase chain reaction. Results:  All periodontal parameters were significantly decreased immediately after debonding (T2). The salivary levels of total bacteria and Pg were decreased at T3, while Pi and Tf levels were decreased at T4. However, the amount of Aa and Fn remained at similar levels in saliva during the experimental period. Interestingly, Aa and Fn were present in saliva at higher levels than were Pg, Pi, and Tf. Conclusion:  The higher salivary levels of Aa and Fn after debonding suggests that the risk of periodontal problems cannot be completely eliminated by the removal of fixed orthodontic appliances during the initial retention period, despite improved oral hygiene.
