Effect of non-removable orthodontic devices on the condition of solid dental tissues

We carried out the analysis of solid dental tissue among 50 patients at different stages of orthodontic treatment using brace systems made of materials, which are used most often in the process of bite correction. We determined the hygiene index (OHI-S), saliva buffer capacity and its immunoreactive properties, caries intensity to explain the development of demineralization. We conducted the study using ligature and self-ligating ceramic and metal brace systems. The examination revealed a clear tendency to imbalance in the local immune response of the body, deterioration of hygienic status in patients during the period of noted orthodontic treatment, as well as noticeable changes in the structure of solid tooth tissues. There is a noticeable development of pathological processes depending on the duration of treatment, and then a regressive effect in relation to hygienic deviations in tracking the retinal period. These studies have proved the pathogenetic effect of edgewise technology, mainly ligature representatives, on the general state of the oral cavity, including the body as a whole, which leads to the importance of the introduction of permanent preventive hygienic events and the implementation of treatment in conjunction with dental doctors of other directions.

The review of clinical and immunologic studies of periodontal inflammatory diseases is presented. At present, the most effective tools for treatment of malocclusions are fixed orthodontic appliances (dental braces), but the use of fixed appliances complicates the oral hygiene, predisposes to periodontal and gum inflammation. The rate of complications diagnosed during orthodontic treatment stays high. The principles of periodontal inflammatory diseases (gingivitis, periodontitis) treatment are closely related with the causes and mechanisms of periodontal inflammation. Patients who receive orthodontic treatment using braces should be treated as a group with a high risk for inflammatory periodontal diseases. The orthodontic devices stimulate the directed reorganization of periodontal structures achieving optimal aesthetic and functional standards. Physiology of periodontium restructuring is largely determined by the local and general health before, during and after the orthodontic treatment. This article surveys the mechanisms of periodontal damage, highlighting the important role of immune response. So far, only anecdotal descriptions of immune response on the stages of orthodontic treatment are published.

Relevance. Skeletal malocclusion stands at the head of all oral diseases and is encountered in 32-35% of children and adolescents in Russia [7;12;15]. The number of malocclusions has increase due to various reasons, one of which is early extraction of deciduous carious teeth resulting in impaired vertical dimension and occlusion of teeth [1;14]. Diagnosis with due regard to caries resistance degree and planning of respective operative and orthodontic treatment are indispensable in children with skeletal malocclusion. Purpose – to increase effectiveness of functional treatment of malocclusion in children with various degree of caries resistance.Materials and methods. There were examined 108 patients aged between 6 and 16 with Class I malocclusion according to Angle, abnormal arch-to-arch relationship and tooth position and various degree of caries resistance. 4 groups were formed: high, sufficient mean, decreased mean and low caries resistance of dental enamel. Intensity of carious process was detected in all patients before and after orthodontic treatment. The effectiveness of reminerlization administered by removable orthodontic appliances was evaluated by electrometrical testing of hard dental tissue. Surface EMG was used to assess normalization of tone of maxillofacial muscles in children by average amplitude of biopotentials of superficial masseter and temporalis muscles.Results. Сhanges in caries intensity in children after treatment with removable orthodontic aligners indicate the necessity for remineralization of hard dental tissues during orthodontic treatment and it is confirmed by decrease of electroconductivity of enamel in children with sufficient mean, decreased mean and low degree of dental enamel caries resistance. Increase of biopotential mean amplitude during «total mastication» for masseter and temporal muscles confirms effectiveness of preformed elastic positioner along with myodynamic exercises.Conclusions. The conducted study proves the necessity of comprehensive approach with procedures  increasing the degree of caries resistance of hard dental tissues during orthodontic treatment of skeletal malocclusion in children.

COVID-19 is an acute infectious disease of the respiratory system caused by infection with the SARS-CoV-2 virus (Severe Acute Respiratory Syndrome Coronavirus 2). Transmission of SARS-CoV-2 infections occurs through droplets and contaminated objects. A rapid and well-coordinated immune system response is the first line of defense in a viral infection. However, a disturbed and over-activated immune response may be counterproductive, causing damage to the body. Severely ill patients hospitalised with COVID-19 exhibit increased levels of many cytokines, including Interleukin (IL)-1β, IL-2, IL-6, IL-7, IL-8, IL-10, IL-17, granulocyte colony stimulating factor (G-CSF), monocyte chemoattractant protein 1 (MCP-1) and tumor necrosis factor (TNF). Increasing evidence suggests that Th17 cells play an important role in the pathogenesis of COVID-19, not only by activating cytokine cascade but also by inducing Th2 responses, inhibiting Th1 differentiation and suppressing Treg cells. This review focuses on a Th17 pathway in the course of the immune response in COVID-19, and explores plausible targets for therapeutic intervention.

Abstract Background In cancer cells, DNA methylation may be altered in two principle ways; global DNA hypomethylation and site-specific CpG island promoter hypermethylation. Since Long interspersed element-1 (LINE-1 or L1; a repetitive DNA retrotransposon) constitutes a substantial portion (approximately 17%) of the human genome, the extent of LINE-1 methylation is regarded as a surrogate marker of global DNA methylation. In previous studies, we demonstrated that LINE-1 hypomethylation was strongly associated with a poor prognosis in esophageal cancer, supporting its potential role as a prognostic marker (Ann Surg 2012). We also found that LINE-1-hypomethylated tumors showed highly frequent genomic gains at various loci containing candidate oncogenes such as CDK6 (Clin Cancer Res 2014). Given that immunotherapy, as represented by PD-1/PD-L1-targeting antibodies, has increasingly gained attention as a novel treatment strategy for esophageal cancer, better understanding of local immune response status in esophageal cancer is important. The aim of this study is to evaluate the relationship between LINE-1 methylation level and local immune response in esophageal cancer. Methods Using a non-biased database of 305 curatively resected esophageal cancers, we evaluated PD-L1 expression and TIL status (CD8 expression) by immunohistochemical analysis (Ann Surg 2017). Results TIL positivity was significantly correlated with longer overall survival (log-rank P < 0.0001). TIL-negative cases demonstrated significantly lower LINE-1 methylation level compared with TIL-positive cases (P = 0.012). This finding certainly supports that LINE-1 methylation level may influence the local immune response status. Conclusion PD-L1 expression was not related with LINE-1 methylation level. Further investigations in this field would provide deeper insights into esophageal tumor immunology and assist the development of new therapeutic strategies against esophageal cancer. Abstract Moringa oleifera (MO) is a widely used as the nutritious and non-traditional feed supplementation containing kinds of bioactive substances. However, the enhancement effect of Moringa oleifera leaf Polysaccharide (MOLP) as a feed additive in broilers growth performance and immunity remains unclear. In this study, MOLP was obtained by water extraction and alcohol precipitation method, then purified with Trichloroacetic acid (TCA) assay. Chickens were randomly divided into 4 groups, to receive different doses of MOLP (0, 0.1, 0.2, 0.4g/kg) in feed for 3 weeks. The body weight gain (BWG) and feed consumption were recorded for feed conversion ratio (FCR) and average daily feed intake (ADFI) calculation. Broiler chickens were sacrificed and sampled on day 14, 21, 28 (D 14, D 21, and D 28) respectively. Serological indicators, including total protein (TP), albumin (ALB), globulin (GLO), and creatinine (CREA) were detected. ELISA kits were applied for detecting the levels of immunoglobulin A (IgA), immunoglobulin G (IgG), interleukin-2 (IL-2), and tumor necrosis factor (TNF-α). From D 21 to D 28, the results showed that middle dose of MOLP significantly increased BWG and ADFI as well as liver and bursa indexes when compared with the control group. In addition, TP and GLO were also increased (P<0.05). All MOLP treatments enhanced the serum concentrations of IgG and IL-2 (P<0.01). Furthermore, results of quantitative RT-PCR showed that high dose of MOLP treatment significantly increased (P<0.001) the mRNA expression levels of IL-2 and TNF-α of chickens relative to the control group. In conclusion, the results showed that MOLP supplementation contributed to improve growth performance and immune response in broiler chickens, and MOLP could be considered as a promising feed additive.
