Introduction. Nasolabial folds are skin folds that run from both sides of the nose to the corners of the mouth. They are caused by the loss of fat in the dermis, followed by the loss of muscles in the middle part of the face.Aim. The aim of the work is to present contemporary knowledge of materials used for modeling nasolabial folds.Material and Methods. Medical databases PubMed, EBSCO Multiplier, were analyzed by entering the slogans: nasolabial folds, nasolabial folds, hyaluronic acid in nasolabial fold modeling, nasolabial fold correction. Analyzing the literature, data from eleven scientific articles from the last five years (2013–2018) were used.Results. In most articles for modeling nasolabial folds, crosslinked hyaluronic acid was used after prior photographic assessment of skin condition or after anamnesis examination — an interview. From the papers describing the cases, it appears that the applied materials for face aesthetic medicine treatments level the nasolabial folds.Conclusions. The obtained results are ambiguous and studies need to be continued. In the literature, the instrumental evaluation of facial skin was not used before modeling nasolabial folds.

In facial skin revitalization it is important to use preventive measures and introduce minimally invasive treatments as soon as the first signs of ageing appear. Such treatments can include the administration of stem cells and concentrated growth factors (CGF Harmony). This procedure was used in the case presented in this paper. Its effects were assessed on the basis of medical photographic documentation obtained using the FotoMedicus system, and instrumental examination of skin parameters, phototype, hydration and redness by means of a Cutometer Dual MPA 580. CGF Harmony was applied three times, and after the second and third application an improvement in the majority of the assessed parameters was observed. However, it cannot be unequivocally demonstrated that three CGF Harmony applications are sufficient for 40+ patients; it may be necessary to monitor skin condition over a longer period of time and introduce combined treatments in the area of facial aesthetic medicine.

Abstract Background Injection of hyaluronic acid filler, a common procedure for nasolabial fold correction, poses a risk of vascular compromise, which can result in skin necrosis and blindness. Objectives The aim of this study was to examine the facial arterial pathway in real time by Doppler ultrasound to avoid arterial complications during filler injections. Methods The facial artery pathway of 40 patients (80 nasolabial folds; 2 men and 38 women; mean age, 46.5 years; range, 28-72 years) was examined by Doppler ultrasound before hyaluronic acid filler injection. On the basis of this ultrasound evaluation, the filler injection targeted the subdermal, subcutaneous, and deep to submuscular layers by bypassing the facial artery. The efficacy of the filler injection was evaluated for each patient. Results The facial artery was detected lateral to the nasolabial fold in 31% of patients; it was detected beneath the nasolabial fold in the other patients as follows: subdermal layer, 13%; subcutaneous layer, 29%; muscular layer, 24%; and submuscular layer, 4%. The mean [standard deviation] Wrinkle Severity Rating Scale score was 3.68 [0.76] before injection and 2.28 [0.78] at 3 months after injection. Two patients experienced unilateral bruising of the nasolabial fold, which resolved after 2 weeks without treatment. Conclusions Doppler ultrasound can be considered as pretreatment tool for the prevention of vascular complications during filler injections to correct nasolabial folds. Level of Evidence: 4

The dermal filler Princess VOLUME is a highly cross-linked, viscoelastic hyaluronic acid injectable gel implant used for aesthetic treatment. To evaluate the efficacy and safety of Princess VOLUME in the treatment of nasolabial folds, an open-label uncontrolled, multicenter study was conducted. Forty-eight subjects were recruited who had moderate to deep wrinkles, according to the Modified Fitzpatrick Wrinkle Scale (MFWS). Subjects received Princess VOLUME in both nasolabial folds at Day 0. Nasolabial fold severity was evaluated at 30, 90, 180, and 270 days after treatment, using the MFWS and the Global Aesthetic Improvement Scale (GAIS). Adverse events and treatment site reactions were recorded. Among the 48 subjects, 93.8% were female with a median age of 52 years. There were significant improvements(P<0.0001)in the MFWS scores at 30, 180, and 270 days after treatment compared with those at baseline, with a mean decrease of 1.484 (±0.408), 1.309 (±0.373), and 1.223 (±0.401), respectively; hence the primary endpoint was achieved and clinical efficacy demonstrated. Princess VOLUME was well tolerated, and most adverse events were injection site reactions of mild to moderate severity. Subject satisfaction (97.9%), subject recommendation of the treatment (93.6%), and investigators GAIS scores (97.9% improvement) were high.

Abstract The most hazardous adverse reactions following hyaluronic acid injections in aesthetic medicine involve vascular complications, known as the Nicolau Syndrome. This article presents a vascular complication in the area of the upper part of the nasolabial fold following subcutaneous administration of 0.5 ml of hyaluronic acid. At the time of the injection, paling occurred, which was followed by livedo racemosa appearing an hour later. Upon the lapse of a week, an ulceration appeared. It was not until the tenth day after the hyaluronic acid injection that hyaluronidase was administered. After 15 hyperbaric oxygen exposures, the ulcer was completely healed

Nowadays, dermal fillers are used in aesthetic medicine very often, hence, a large number of complications after hyaluronic acid products application may occur. Upon early diagnosis and adequate treatment, almost any adverse reaction can be avoided. Proper injection technique, approved products and detailed anatomical education decrease the risk of long-term sequelae. Herein, a clinical case of foreign body granuloma formation in the area of nasolabial fold after injection of hyaluronic filler is described. A short overview of the pharmacodynamics and possible side effects of the injectable hyaluronic fillers is also presented.

The microneedle therapy system (MTS), a mechanical method involving making minute multiple holes in the skin, reportedly improves skin condition, such as by reducing flushing and melanin. A newly attempted bloodletting therapy, Jae-Seng Acupuncture, has several advantages over traditional mechanical punching methods because it allows the practitioner to regulate the depth and direction of needle stimulations and to choose whether to stimulate the muscle layers. This study was conducted to determine the efficacy of Jae-Seng Acupuncture in the treatment of nasolabial folds and eye wrinkles. The nasolabial folds and eye wrinkles of 107 patients ranging in age from their 20s to their 70s were subjected to DermaVision, a digital skin image analyzer, before the treatment and one to six months after treatment. Additionally, stimulation of the meridians, such as Taeyang, Tongjaryo, Chongmyong, Sungup, Sabaek, Yonghyang, Chichang, Taeyong, was performed to improve the function of the stomach, large intestine. Analyses of the images indicate that Jae-Seng Acupuncture improved nasolabial folds and eye wrinkles, suggesting that this technique is a safe and effective method for the improvement of facial skin conditions.

Local and system influence the Repleri gel implants on the base of hyaluronic acid used to correct age-related changes in skin has been studied. Human stromal stem cells with CD44 membrane receptor to hyaluronic acid migrates in vitro into gel structure and maturates morphologically in fibroblast-like forms stained positively with acid phosphatase. Repleri gels implantation in the group of women with local and system age-related changes in homeostasis has positive local effect connected with full deletion of nasolabial fold in facial skin. Repleri gels injection has been decreasing during 1 year the system indices of real biological age as compared with chronological (passport) one. Lipoproteins and aspartate aminotransferase concentrations diminished statistically in women blood. An effect revealed may be considered as potential way of anti-age therapy of not only skin but parenchymal organs. At the same time, a long-term control of homeostasis distant indices in women with serious changes in parenchymal organs is necessary after local Repleri gels injection because of organism’s hormonal reactions.

This study proposes a review on hyaluronic acid (HA) known as hyaluronan or hyaluronate and its derivates and their application in cosmetic formulations. HA is a glycosaminoglycan constituted from two disaccharides (N-acetylglucosamine and D-glucuronic acid), isolated initially from the vitreous humour of the eye, and subsequently discovered in different tissues or fluids (especially in the articular cartilage and the synovial fluid). It is ubiquitous in vertebrates, including humans, and it is involved in diverse biological processes, such as cell differentiation, embryological development, inflammation, wound healing, etc. HA has many qualities that recommend it over other substances used in skin regeneration, with moisturizing and anti-ageing effects. HA molecular weight influences its penetration into the skin and its biological activity. Considering that, nowadays, hyaluronic acid has a wide use and a multitude of applications (in ophthalmology, arthrology, pneumology, rhinology, aesthetic medicine, oncology, nutrition, and cosmetics), the present study describes the main aspects related to its use in cosmetology. The biological effect of HA on the skin level and its potential adverse effects are discussed. Some available cosmetic products containing HA have been identified from the brand portfolio of most known manufacturers and their composition was evaluated. Further, additional biological effects due to the other active ingredients (plant extracts, vitamins, amino acids, peptides, proteins, saccharides, probiotics, etc.) are presented, as well as a description of their possible toxic effects.

Abstract Background Verrucous carcinoma of the esophagus is a rare disease leading to dysphagia, chest pain, and weight loss. The diagnosis is difficult because even repeated biopsies are often without tumor evidence. We present a patient with verrucous carcinoma of the esophagus and a literature review. Case report A 64-year-old patient with dysphagia and sore throat received esophagogastroduodenoscopy illustrating segmental circumferential verrucous inflammation and Candida esophagitis in the middle part of the esophagus. Repeated mucosal biopsies revealed reactive hyperkeratosis of the squamous epithelium with minimal atypia but without ulcera, eosinophilic esophagitis, or suspicion of cancer. Mucosal infection with adenovirus, herpes simplex virus 1, human papilloma virus types, and cytomegaly virus was ruled out. Veruccous carcinoma was detected finally by endoscopic mucosal resection. The patient was successfully treated by esophageal resection. Tumor stage was G1, pT1b, pN0, L0, V0, Pn0, R0. Conclusion The results suggest that macroscopic suspicion of verrucous esophageal carcinoma should lead to resections of larger tissue specimens by EMR to confirm the diagnosis.
