Abstract Background: The US population is maturing. As of 2020, over 16% (52 million people) are age 65 or older. With a citizenry that is increasingly "gray," the nation is short of medical providers who specialize in geriatric medical care. For example, the number of geriatrician physicians per 10,000 adults 65 years and older has decreased since 2000, with approximately 5,300 in 2018. Nurse practitioners in geriatric medical care numbered 598 in 2018. Considering that the projected needs by 2030 will be over 30,000, the trajectory of geriatricians is becoming increasingly inadequate for the aging population. Physician assistants (PA) are another class of providers that are filling this geriatric medical care role, although little has been published. To address this role of PAs a study was undertaken. Methods: The National Commission on Certification of Physician Assistants databank provided the number and characteristics of PAs in geriatric medicine and compared them to all other certified PAs. Analyses included descriptive statistics, Chi-Square, and Wilcoxon Rank Sum tests for comparisons between PAs practicing in geriatric medical care vs. all other specialties. Where a comparison between PAs in geriatrics and other specialties was made, a P value of .05 or less was considered statistically significant. Results: As of 2018, there were 794 certified PAs, or 0.8% of the certified PA workforce, in geriatric medical care. This cadre has grown significantly since 2013, both in total number (increasing over 373%) and as a percentage of the PA workforce. The median age of certified PAs in geriatrics is 45 years, and 79% are female. Almost half (46%) of PAs in geriatric medicine work in extended care facilities or nursing homes, which differs from PAs in non-geriatrics care. Another 8% work in government facilities and 8% in rehabilitation facilities. In 2018, the mean annual income for this PA group was $106,680. Conclusions: As the American population continues to age, the relative growth of PAs in geriatric medicine makes them a natural part of the solution to the projected physician geriatrician deficit. The role of PAs in geriatric medical care remains to be explored.

Methods The National Commission on Certification of Physician Assistants databank provided the number and characteristics of PAs in geriatric medicine. PAs in geriatrics, their distribution, income, was compared to certified PAs. Analysis included descriptive statistics, Chi-Square and Wilcoxon Rank Sum tests for comparisons between PAs practicing in geriatrics vs. all other clinical specialties. For all analyses where a comparison between PAs in geriatrics and other specialties was made, a P value of .05 or less was considered statistically significant. Statistical analyses were conducted using R Results As of 2018, there were 794 certified PAs, or 0.8% of the PA workforce, specifically in geriatrics. This cadre has grown significantly 2010 both in total number (increasing over 400%) and as a percentage of the PA workforce. The median age of PAs in geriatrics is 45 years, and 79% are female. Almost half (46%) of the PA geriatric workforce are in extended care facilities or nursing homes - which differs from PAs in non-geriatric medicine specialties (0.3%). Another 8% work in federal government facilities with an additional 8% in rehabilitation facilities. In 2018, the median annual income for this PA cadre was $106,680. Conclusions As the American population continues to age, the relative growth of PAs in geriatric medicine makes them a natural part of the solution to the projected physician geriatrician deficit.

ABSTRAKLatar Belakang : Keberadaan kader di posyandu sebagai salah satu sistem penyelenggarakan pelayanan sangat dibutuhkan. Mereka adalah ujung tombak  pelayanan kesehatan yang merupakan kepanjangtanganan puskesmas Jawa Tengah tahun 2011 jumlah gizi kurang 5,35% dan gizi buruk 0,10%. Untuk Kabupaten Semarang dari 23.562 balita yang ditimbang pada tahun 2011 gizi lebih 1,13%, gizi baik 93,51%, gizi kurang 4,86% dan gizi buruk 0,49% (DepKes Prov Jateng, 2011). Tujuan Penelitian : Penelitian ini bertujuan untuk mengetahui hubungan peran kader posyandu dengan status gizi balita. Metode Penelitian : Desain penelitian ini adalah survey analitik dengan menggunakan pendekatan cross sectional. Populasi dalam penelitian ini adalah seluruh balita di Posyandu Mawar di Desa Gedangan sejumlah 40 responden, dengan teknik total sampling dan analisa data chi square. Hasil Penelitian : Hasil perhitungan chi square di peroleh X² hitung 10.644 pada df=4, P.value 0.031 dimana probabilitas lebih kecil dari level of significant 5 % (0,001 < 0,05) berarti Ha diterima dan Ho ditolak. The population in this study are all the toddlers at the Rose in the village of Posyandu Gedangan some 38 respondents, with total sample techniques and data analysis a chi square. The results :. The chi square calculation results in getting X ² count 10.644 on df = 4, P. value 0.031 where probability is smaller than the level of significant 5% (0.001 < 0.05) mean Ha Ho accepted and rejected. Conclusion : there is a connection between the role of cadres of posyandu with nutritional status on toddlers.Keywords : the role of cadres, toddler nutrition status

Abstract Background Susceptibility weighted imaging (SWI) is a 3D gradient-echo MR technique that is based on blood oxygen level dependent (BOLD) induced phase effects between the venous blood and the surrounding brain parenchyma. SW-MR imaging allows for noninvasive visualization of small veins at submillimeter resolution and, therefore, is used to depict venous architecture in brain lesions. The extreme sensitivity of SWI for the detection of neovascularity (venous blood), haemorrhage, and calcification has been an indispensable tool for characterization of the internal architecture of brain tumours. Objectives Is to evaluate the role of Susceptibility weighted imaging in assessment of adults Intra axial brain Neoplasms, and its ability to characterize them into high and low grade lesions in comparison to histopathology which will be used as gold standard. Methods A cross sectional study including 31 patients suspecting intracranial brain neoplasm radiologically and clinically, conducted at Private center, the patients were investigated using Siemens machine Magnetom Skyra 3T, the period was between January 2018 till the end of June 2019 . Results Our study included 31 patients. Including 15 female and 16 male patients, with the patient’s age ranging from 20 to 68 years old with median 48 years old ranging from 35.75 (25% percentile) to 58.75 (75% percentile). Among total cases, there were 8 patients with grade 2 glioma, 10 patients with grade 3 glioma and 6 patients with grade 4 glioma, 2 patients with lymphoma and 5 patients with brain metastasis (1 lung cancer and 4 breast cancer). All the patients were evaluated with MRI including SWI sequence with special comment on the number of the intratumoral susceptibility signal (ITSS), the size of the ITSS, its morphology as well as the ratio of the ITSS to the tumor size, which were then correlated with the patient histopathological results obtained later. The study revealed that the best parameter to accurately grade the tumor is the number of ITSS within the lesion with P value 0.001, followed by the size of the ITSS with P value 0.002 and Pearson Chi-Square value equals 20.6, while the lowest one was the ratio of the ITSS to the tumor size with P value 0.002 Pearson Chi-Square value equals 17.3. Our study showed that the morphology alone was not able to accurately grade the tumor with P value 0.007 ( Not significant) Conclusion SWI using 3T MR system provides quite useful information for preoperative tumor grading. There seems to be a strong correlation between pathological grading and that assessed with SWI.

Social media use: The role of parents' perceptions about social media impact in early marriageBackground: Early marriage is a marriage that is conducted at adolescence, the factors causing early marriage are socio-cultural factors, economic pressure, level of education, difficulty in getting a job, social media, religion and views and beliefsPurpose: To find out the influence of social media and the role of parents on the incidence of early marriageMethod: A quantitative research using descriptive analytical research method with cross sectional study design with variable use of social media with the criteria for the results "active and inactive". For the variable of the role of parents with 2 outcome criteria, namely "influential and not influential". And for the variable incidence of early marriage, there are 2 criteria, namely age 14-16 years and age 17-19 years, the questionnaire used has been through the validity of previous researchers. Then the results of the data obtained were analyzed in the SPSS program using the Chi Square testResults: From the Chi Square test results for social media variables obtained p value = 0.001, then the value of p = 0.001 <0.05 (α) while the role of parents variable Chi Square test results obtained p value = 0.022, therefore the value of p = 0.022 <0.05 (α).Conclusion: Based on the results of the study it can be concluded that, there is a significant influence between the use of social media and the role of parents in the event of early marriage Keywords: Social media; Parents, Early marriagePendahuluan: Pernikahan usia dini adalah perkawinan yang dilakukan pada usia remaja, faktor penyebab pernikahan usia dini adalah faktor sosial budaya, desakan ekonomi, tingkat pendidikan, sulit mendapat pekerjaan, media sosial, agama serta pandangan dan kepercayaan.Tujuan: Untuk mengetahui pengaruh media sosial dan peran orang tua terhadap kejadian pernikahan dini di Kecamatan Marioriawa Kabupaten Soppeng Metode: Penelitian kuantitatif dengan menggunakan metode penelitian deskriptif analitik dengan rancangan cross sectional study dengan variabel penggunaan media sosial dengan kriteria hasil “aktif dan tidak aktif”. Untuk variabel peran orang tua dengan 2 kriteria hasil yaitu “berpengaruh dan tidak berpengaruh”. Dan untuk variabel kejadian pernikahan dini ada 2 kriteria  yaitu umur 14-16 tahun dan umur 17-19 tahun,  kuesioner yang di gunakan sudah melalui uji validitas peneliti sebelumnya. Kemudian hasil data yang di dapatkan dianalisis di program SPSS dengan menggunakan uji Chi SquareHasil: Dari hasil uji Chi Square untuk variabel media sosial didapatkan nilai p=0,001, maka nilai p=0.001<0.05 (α) sedangkan variabel peran orang tua hasil uji Chi Square didapatkan nilai p=0,022, oleh karena itu nilai p=0.022<0.05 (α).Simpulan: Berdasarkan hasil penelitian dapat di simpulkan bahwa, Ada pengaruh yang signifikan antara penggunaan media sosial dan peran orang tua terhadap kejadian pernikahan dini

Basic Health Research in 2013 recorded the prevalence of SEZ in pregnant women nationally reaching 24.2 percent, an increase from 2010 (18.8 %%) and 2007 (10.9%). The prevalence of SEZs in Central Sulawesi Province in 2013 was 32.6%, higher than the National average. Nutritional status monitoring in Central Sulawesi Province reported a prevalence of KEK Risk in pregnant women in 2015 of 32.6%. Districts with the highest prevalence were consecutively in Banggai Kepulauan District (67.8%), Tojo Una-Una District (51.9%), Sigi District (46.4%), Donggala District (36.7%), and Palu City (35.4%). Midwives in providing services to the community, especially pregnant women, always strive to prepare pregnant women since the first contact during prenatal care provides counseling or education to every pregnant mother about nutritional needs during pregnancy. This study aims to determine the role of midwives education in preventing Chronic Energy Deficiency (SEZ) in pregnant women in Central Sulawesi Province. Observational research with astudy design crosectional. Sampling using the proportional random sampling technique with a total sample of 98 midwives in the working area of 4 selected districts, namely Banggai Islands District (15 samples), Sigi District (18 samples), Donggala District (25 samples) and Palu City (40 samples). The results of the study showed that the midwives who carried out the role of education to pregnant women in their regions mostly did not have the incidence of pregnant women with SEZ (83.1%). The results of theanalysis Chi-Square obtained a p-Value value of 0.041, which means that there is a relationship between the role of midwife education and prevention of SEZ events in pregnant

Background: The measles rubella (MR) immunization coverage rate in Padang City is only 30.82%, still far below Indonesia's national target of 95%. The coverage of measles rubella immunization at the Rawang Health Center is around 62.1%. This study aims to determine the role of health workers in the success of measles rubella immunization in the Rawang Public Health Center, Padang City. Methods: This study used a cross sectional design. The study was conducted in October 2018-July 2019. The research population is mothers who have children aged 12-59 months (toddlers) as many as 1807 respondents. The sampling method is proportional random sampling with a sample of 87 respondent. Data was collected by interview using a questionnaire. Results: The results showed that 37.9% of children under five had not been immunized against measles rubella and 44.8% of health workers had an unfavorable role. The results of the chi-square test showed a significant relationship  between  the role of officers in  the success of measles rubella immunization  (p-value=0.020). Recommendation: It is hoped that health workers can increase health promotion activities regarding the risks due to children not being immunized against measles rubella and intensify door-to-door programs in the implementation of measles rubella immunization

The purpose of this chapter is to emphasize and describe the team nature of critical care medicine in the Cardiothoracic Intensive Care Unit. The chapter will review the importance of various team members and discuss various staffing models (open vs closed, high intensity vs low intensity, etc.) on patient outcomes and cost. The chapter will also examine the roles of nurse practitioners and physician assistants (NP/PAs) in critical care, and will briefly review the growing role of the tele-ICU. Most studies support the concept that a multi-disciplinary ICU team, led by an intensivist, improves patient outcomes and decreases overall cost of care. The role of the tele-ICU and 24 hour in-house intensivist staffing in improving outcomes is controversial, and more research is needed in this area. Finally, a brief discussion of billing for critical care will be discussed.
