Effects of Ferula Assa-Foetida on Clinical, Hormonal and Sonography Parameters in Young Girls with Polycystic Ovary Syndrome: Pilot Randomized, Placebo Controlled, Triple-Blinded

Abstract Background: Polycystic ovarian syndrome is the most common endocrine disorder in reproductive aged women. As a result of side effect of pharmaceutical medications women are interested in using alternative medicines to treat. To determine the comparative effects of Ferula assa-foetida on androgenic hormone levels and ovarian features in patient with polycystic ovarian syndrome (PCOS). Methods: In this triple-blinded controlled clinical trial, 34 student participants were randomly divided in two groups. Intervention group received 100 mg of oleo-gum resin of Ferula assa- foetida, control group received oral paraffin (Placebo) twice daily for 3 months. The efficacy of this herbal medicines was measured after the 3-month intervention. Hormonal assay for evaluating Testosterone, DEHAS, Prolactin, TSH, FSH, LH levels and also abdominal sonography for evaluating ovarian volumes, number of follicles of both ovaries, and endometrial thickness. were measured before and after the study.Results: In this study, no significant hormonal changes were observed (p value>0.5). Although the greatest reduction in the number of ovarian follicles and ovarian volume was reported in the Ferula assa-foetide group (p value <0.01). Introduction: Radiation therapy before mastectomy increases the severity of stress and cortisol hormone. Because of the preference of patients and physicians for nonpharmacologic stress management methods, we conducted the present study with the aim of evaluating the effect of ear acupressure on anxiety and cortisol hormone levels in women receiving premastectomy radiotherapy. Methods: This randomized clinical trial was carried out on 66 patients (based on sample size formula) at Imam Reza Hospital in Tabriz during the years 2018-19. The intervention group was given acupressure, twice a day (8 minutes on each ear), for three days before surgery. Serum cortisol level and latent and manifest anxiety (the Spielberger State-Trait Anxiety Inventory) were measured before and after the intervention. Data were analyzed using SPSS 20 software. A P value less than 0.05 were considered significant. Results: There was no significant difference in latent anxiety, manifest anxiety, or cortisol levels between the groups at baseline. Ear acupressure caused a significant decrease in latent anxiety (from 43.22 ± 6.41 to 33.09 ± 4.91), explicit anxiety (from 49.50 ± 5.52 to 32.18 ± 5.01), and plasma cortisol levels (from 156.15 ± 18.95 to 115.59 ± 17.80) (P < 0.005), while no significant changes were observed in the control group (P > 0.05). Conclusion: Ear acupressure can reduce anxiety and stress (cortisol) levels in breast cancer patients undergoing radiotherapy before mastectomy.

OBJECTIVE: to assess the effectiveness of the chlorhexidine antimicrobial dressing in comparison to the gauze and tape dressing in the use of central venous catheters.METHOD: a randomized clinical trial was conducted in the intensive care and adult semi intensive care units of a university hospital in the south of Brazil. The subjects were patients using short-term central venous catheters, randomly assigned to the intervention (chlorhexidine antimicrobial dressing) or control (gauze and micro porous tape) groups.RESULTS: a total of 85 patients were included: 43 in the intervention group and 42 in the control group. No statistically significant differences were found between dressings in regard to the occurrence of: primary bloodstream infections (p-value = 0.5170); local reactions to the dressing (p-value = 0.3774); and dressing fixation (p-value = 0.2739).CONCLUSION: both technologies are effective in covering central venous catheters in regard to the investigated variables and can be used for this purpose. He narrated that woman who observes her fruitless menstruation in appropriate time, has corpulent and hairy cheeks, she is also (seized with) called as Pupaghni Revatī. The present clinical trial by trans cervical route by Śatāvhādi Tail based on the classical reference in Ashtang Samgraha The ingredients of Śatāvhādi Tail are viz, śatāvhā, Saŗśapa, Ajavāyan, Śigru,Hingu, Kustha, Mandanaphala, processed in Mustard oil. It has been given by Uttarbasti module. All the ingredients are mainly Katu Ras Usna Virya and Usna Tikshna Guna. The 30 married female recruited this clinical trial who received Śatāvhādi Tail 5-7 ml trancervical route at Bharati Vidyapeeth(D.U) College of Ayurved and Hospital, Katraj Dhanakawadi , Pune 43.The results assessed in terms of USG (Ovarian volume and no of follicles), LH:FSH Ratio and S.Testesterone and subjective parameter Sthula Lomash Ganda (Corupurulent hair on cheek) and Arthavkshaya. It showed effect on hormonal level {LH, FSH} but slight effect on serum

ABSTRACT Aim To compare the proximal contacts and contours in class II composite restorations in premolars achieved with Automatrix band with separation ring vs Automatrix band alone. Materials and methods A total of 188 premolar teeth with proximal cavity were assigned on alternate basis in two groups. Intervention group teeth received Automatrix band with separation ring using clamp forceps, while the control group received Automatrix band alone. All preparations were restored with P-60 composite (3M-ESPE Dental, USA). The outcome (proximal tightness) was determined by a blinded assessor. Chi-square test was applied to compare the contact tightness between the two groups. Spearman's rank correlation coefficient was applied to determine any correlation between the contact tightness with proximal contours. p-value of 0.05 was taken as significant. Results The intervention group exhibited better proximal contacts than the control group (p-value 0.040). There was no correlation between the contact tightness and the proximal contours. Conclusion The use of separation ring along with Automatrix matrix band in class II composite restorations resulted in significantly better proximal contacts than Automatrix band alone. However, the separation ring does not contribute in producing an additional improvement in the proximal contours. The proximal contour has no relationship with the proximal contact tightness. Clinical significance Several techniques have been advocated to get tight contacts in composites including interdental separation ring. Our study focuses on using separation ring with circumferential matrix band instead of a sectional band. Background and Aims: Fear of delivery in females can lead to negative side effects, such as the experience of long and hard labor. Reduction of delivery fear is one of the goals of natural delivery program, and hypnosis has been considered a way to reduce this fear. With regard to the contradictory results of the studies in this field, this study aimed to determine the effect of self-hypnosis on the experience of delivery fear and duration of labor in primiparity. Materials and Methods: This clinical trial was conducted on 63 pregnant women who were divided into self-hypnosis (n=30) and control groups (n=33) using a blocking method in Mashhad, Iran, during 2019. The intervention group received two sessions of hypnosis at 37th and 38th weeks of pregnancy which consisted of listening to an audio file until delivery time. On the other hand, the control group received only the routine care. Fear was evaluated at 37th week of pregnancy and 24 h after delivery. Data were analyzed using SPSS software (version 16), and a p-value less than 0.05 was considered statistically significant. Results: The mean scores of delivery fear experience (P<0.001), as well as the duration of the second stage (P=0.013) and third stage of labor (P=0.009) were significantly lower in the self-hypnosis group, compared to the controls. Conclusion: Self-hypnosis was effective in reducing the experience of delivery fear and duration of the second and third stages of labor in primiparity. Therefore, self-hypnosis is recommended to reduce the experience of delivery fear and duration of the second and third stages of labor in primiparity.

Abstract Background: vitamin D is being increasingly recognized for its important non-skeletal functions including endocrine actions. This study investigated if a single, large, intramuscular post‐artum injection of vitamin D improve adiponectin levels among women with gestational diabetes mellitus (GDM). Methods: A total of 45 pregnant women with GDM participated in this randomized clinical trial. They were randomly divided into intervention and control group. The intervention group received an intramuscular injection of 300,000 IU of vitamin D during 3 to 10 days after their child delivery, but controls did not. Serum 25‐hydroxyvitamin D, fasting blood glucose, HbA1c and serum adiponectin were measured at baseline and after 3 months of intervention. Results: Serum 25 OH vitamin D increase significantly in the intervention but not in the control group from 24.25 to 62.10 (nmol/l) (p-value< 0.01). Comparison in within group showed that adiponectin level increased significantly only among intervention group after the vitamin D injection from 7.45 to 8.98 (ngr/dl) (P-value=0.01), while between group comparisons showed no significant differences in adiponectin concentration after the intervention (P-value<0.05). Between and within group comparisons reported no significant alterations in the levels of glycated hemoglobin (HbA1c) and FPG (fasting plasma glucose), as well. Conclusions: The 300,000 IU single dose of intramuscular injection of vitamin D is regarded as an effective procedure to improve vitamin D status which significantly increased the adiponectin levels among mothers with gestational diabetes after delivery. Introdution: Cataract is a common surgery in older people. Nausea is an unpleasant feeling after general anesthesia that may cause serious effects on such patients. This article aimed to study the effect of pressure pericardium 6 on nausea after cataract surgery in elderly patients in the Khalili Hospital, Shiraz, Iran, in 1395. Methods: This randomized double-blind clinical trial was done on 50 elderly patients in the intervention group and 50 in the control group. An hour before the surgery, the patients closed a 30-band wristband containing a push-button at the point (P6), and in the control group, the same wristband was closed without a button. Five hours after the induction of general anesthesia, the wristband was removed from both groups and nausea level of the patients were recorded immediately after their recovery at 8 and 12 hours after the surgery. After 5 weeks, the data were collected and analyzed using SPSS version 16 and independent T-test (P-value < 0.05) . Results: The results showed the meaningful difference in average severity and frequency of nausea 8 hours after cataract surgery in two groups (p<0.01). But the severity of nausea immediately and 12 hours after cataract surgery was not significantly different in both groups (p>0.159). Conclusion: The finding showed that acupressure is effective in reducing the severity of nausea after cataract surgery. Due to low cost, low complication and simplicity of acupressure, it can be recommended as a way of caring by nurses and other medical staff in reducing nausea.

AbstractBackgroundConsidering the increase in drug resistance over time to Helicobacter pylori treatment relying on the anti-inflammatory and antibacterial effects of atorvastatin to increase the success rate of H. pylori eradication, we examined the effect of adding atorvastatin to standard treatment of H. pylori eradication.Materials and MethodsA total of 186 symptomatic patients who had been diagnosed with Helicobacter pylori infection and tested for H. pylori eradication were examined by a pathological response or positive urea breath test. Patients who received atorvastatin in addition to standard treatment were also identified based on a table of random numbers. Standard treatment included a 240mg bismuth subcitrate tablet, a 40mg pantoprazole tablet, a 500mg metronidazole tablet, and 2 capsules of 500mg amoxicillin, all taken BID for 14 days. After 4 weeks of treatment, all patients underwent stool testing for H. pylori fecal antigen. If the test was positive, the request was considered a failure of treatment, and if the test was negative, it was considered a successful eradication of H. pylori. The clinical trial registration code for this study is IRCT20190823044589N1.ResultsThe eradication rate of H. pylori was 80% in the control group and 80.9% in the intervention group, which did not show a statistically significant difference between the two groups (P-value = 0.971).ConclusionAdding atorvastatin to 4-drug regimen of PPI, bismuth subcitrate, amoxicillin, and metronidazole as the first line of treatment for H. pylori eradication is ineffective.
