Cosmetic desires have improved with time and current trends speak volumes about gingival esthetics and smile designing. Gingival hyperpigmentation plays a negative role in an otherwise acceptable 'smile window'. In the present world, people are concerned more about their esthetics. Several techniques have been employed such as scalpel, electrosurgery, cryosurgery, chemical agents, abrasion, and laser method for the removal of melanin hyperpigmentation. The present case series were aimed to assess the efficacy of gingival soft tissue ceramic trimmer in gingival hyperpigmentation. Pain index (VAS), healing index (Landry, Turnbull & Howley index) and pigmentation index (Takashi et al. 2005) were evaluated to assess its efficacy. The results showed that ceramic trimmer has a lower pain index and a better healing index and delayed re-pigmentation index than rest all other procedures. Thus, it can be concluded that ceramic trimmer might prove to be a boon in achieving aesthetic satisfaction and could be used for depigmentation as it is very cost-effective, readily available and acceptable to the patient.

AbstractGingival melanin pigmentation occurs in all races in variable amount caused by melanin granules. The degree of pigmentation varies from person to person. Excessive gingival pigmentation may be a major esthetic concern for many patients. Methods of deepithelialization of the pigmented or discolored areas of gingiva using different methods such as scalpel method, bur method or laser method are well documented. The procedure for all three techniques has been described and evaluated here. Following presentation encloses a case series in which depigmentation of upper anterior gingival was carried out. The case presented with moderate to severe pigmented gingival (DOPI score ≥ 3) which were treated with one of the above mentioned techniques. The results of these cases suggested that ablation of the gingiva by a Diode laser, abrasion with a scalpel or rotary round bur is good enough to achieve esthetic satisfaction and fair wound healing without infection or severe pain.

Osteoradionecrosis (ORN) of the jaws and medication-related osteonecrosis of the jaws (MRONJ) are uncommon but serious diseases affecting the oral and maxillofacial region with clinically similar appearance but distinct pathophysiology. Management of ORN and MRONJ is inherently challenging and the treatment outcomes are unpredictable. The use of autologous platelet concentrates (APCs) to promote hard and soft tissue healing is well described in the literature, and the efficacy of leucocyte and platelet-rich fibrin (L-PRF) has been well documented in a number of clinical studies. The aim of this study was to present our treatment strategy and the outcomes of incorporating L-PRF as a surgical adjunct in management of ORN and MRONJ in our centre. Methods: eight cases of ORN and MRONJ were treated with a combination of sequestrectomy and L-PRF as a surgical adjunct. Results: the overall success was 87.5%. Using L-PRF as an adjunct, we were able to predictably manage ORN and MRONJ without causing significant morbidity. Conclusion: our experience shows that L-PRF may be used as a valuable and cost-effective adjunct to surgical management of ORN and MRONJ. However, due to a limited number of patients, and a short period of review, the true effectiveness of the method is yet to be demonstrated in a longer follow-up study including a greater number of patients, besides the inclusion of a control group.

Abstract Background Suicide is the leading cause of death among young Australians, accounting for one-third of all deaths in those under 25. Schools are a logical setting for youth suicide prevention activities, with universal, selective and indicated approaches all demonstrating efficacy. Given that international best practice recommends suicide prevention programs combine these approaches, and that to date this has not been done in school settings, this study aims to evaluate a suicide prevention program incorporating universal, selective and indicated components in schools.Methods This study is a trial of a multimodal suicide prevention program for young people. The program involves delivering universal psychoeducation (safeTALK) to all students, screening them for suicide risk, and delivering internet-based Cognitive Behavioural Therapy (Reframe IT) to those students identified as being at high risk for suicide. The program will be trialled in secondary schools in Melbourne, Australia, and target year 10 students (15 and 16 year-olds). safeTALK and screening will be evaluated using a single group pre-test/post-test case series, and Reframe IT will be evaluated in a Randomised Controlled Trial. The primary outcome is change in suicidal ideation; other outcomes include help-seeking behaviour and intentions, and suicide knowledge and stigma. The program’s cost-effectiveness will also be evaluated.Discussion This study is the first to evaluate a suicide prevention program comprising universal, selective and indicated components in Australian schools. If the program is found to be efficacious and cost-effective, it could be more widely disseminated in schools and may ultimately lead to reduced rates of suicide and suicidal behaviour in school students across the region.

Esthetics plays a crucial role in rehabilitation of removable dentures to achieve a pleasant smile. Denture esthetics is the effect produced, which improves the beauty and attractiveness of a person. With the advances of implants and ceramics, the conventional removable denture needs to be updated. denture characterization is a must in avoiding denture look and giving natural look. Giving the prosthesis as close as possible to the natural apparatus before the tooth loss is the key to a successful prosthetic, esthetic and functional rehabilitation. As a prosthodontist dealing in fulfilling the aesthetic challenges an economical alternative to high-end costly procedures is a must considering the Indian scenario, keeping in mind the biocompatibility and longevity of the treatment. As per literature light cure, gum staining is one of the techniques for masking denture base color and imparting natural color to mucosa but considering the cost involved many patients requiring aesthetic enhancement of prosthesis avoid going for such treatment thereby compromising their aesthetic. the present case series describe an innovative cost-effective alternative to the previously describe technique for masking the denture base color by using composite mixed with stains to replicate the natural apparatus.

Apples are among the most significant fruit crops in Russian horticulture. The wide variety, as well as the prominent economic potential of the crop, both enable its cultivation across many climate zones and bring orchard farming in general to the attention of investors in the agro-industrial sector. Breeders have met the rising challenges inherent in creating varieties that are superior in terms of productivity, abiotic- and biotic stress resistance, fruit quality and competitive fast-return capacity. In the present article, current research in apple breeding including methods for intensive selection is reviewed with a focus on breeding programmes for creating adaptive varieties having a high commercial and consumer value. Classical breeding can be complemented with modern techniques for an earlier selection of commercially valuable genotypes, identification of primary genotypes, as well as the creation of new donors and cultivars. The research achievements of leading national institutions in the development of apple varieties reflect additions to the Catalog of State-Permitted Cultivars of Agricultural Crops over the last decade. Most of the 422 permitted adapted apple cultivars are highly marketable due to having best-before-consumption dates in the winter. Despite current success in national orchard farming, further endeavours in crop breeding remain relevant today. Comprehensive research engaging genetics, physiology, phytopathology, virology, agrochemistry and nursery is essential for improving modern breeding programmes with the aim of supplying producers with high-quality planting material for a cost-effective, low pesticide, environmentallystable product.

Abstract Background Presence of right ventricular thrombus (RVT) is a rare but life-threatening condition, thus immediate diagnosis and therapy are mandatory. Unfortunately, detection and distinction from intraventricular tumor masses or vegetations represents a complex task. Furthermore, consecutive therapy is principally led by clinical presentation without considering morphological features of the thrombus. Current literature suggests a multimodal non-invasive imaging approach. In this paper, we discuss the role of cardiac magnetic resonance (CMR) for the detection of RVT in patients with pulmonary embolism (PE). We consider the relatively expensive and not broadly available imaging procedure and weigh it up to its assumed high sensitivity, specificity and importance for differential diagnosis and therapeutic decision making. Case Summary In this case series we report three cases of RVT with concomitant PE, whereof two were missed during routine cardiac workup by transthoracic echocardiography (TTE) and computer tomography (CT). CMR led to detection and further characterization of the thrombi in both cases. These patients were diagnosed and treated at the Cantonal Hospital of Baden in the division of General Medicine. Conclusions CMR reliably detects and characterizes RVT, even under unfavourable conditions for echocardiography such as arrhythmia, adiposity or in posterior position of RVT. Obtained information could facilitate the choice of therapeutic approach (anticoagulation vs. systemic lysis, vs. surgical thrombectomy). Future risk-stratification scores will promote cost-effective use of CMR.

Laparoscopic common bile duct (CBD) exploration can be performed following choledocotomy or via the trancystic approach. Laparoscopic CBD exploration is limited in some benign upper gastrointestinal units due to the cost of sterilization of the re-usable choledochoscope. We have recently published a case series confirming the safety and efficacy of the 5mm re-usable bronchoscope for CBD exploration. This case series evaluates a single-use bronchochoscope (Ambu aScopeTM) for laparoscopic CBD exploration. Data were collected from electronic records of the patients from January 2015 until December 2016; all the cases had confirmation of CBD stones. Twenty-one patients had CBD exploration using the disposable bronchochoscope. There were 7 male and 14 female (median age 42). 8 procedures were performed as emergencies and 13 were performed electively. All the cases were done laparoscopically apart from one planned as an open procedure due to previous extensive open surgery. Twenty cases had their CBD cleared using disposable bronchoscope and one needed subsequent special test follow up. Choledocotomy was performed in 15 and Tran’s cystic approach was performed in 6. No T-tube was used in the laparoscopic cases. 2 cases were performed as day case surgery. Median postoperative hospital stay was 2.5 days. In conclusion, the disposable bronchoscope is safe and effective for use in CBD exploration with results comparable to our previously published case series. It has guaranteed sterility and is cost effective compared to the re-usable bronchoscope especially when initial capital outlay, sterile processing and maintenance costs are considered.

Cellulose-based aerogels hold the potential to become a cost-effective bio-based solution for thermal insulation in buildings. Low thermal conductivities (<0.025 W·m−1·K−1) are achieved through a decrease in gaseous phase contribution, exploiting the Knudsen effect. However, several challenges need to be overcome: production energy demand and cost, moisture sensitivity, flammability, and thermal stability. Herein, a description and discussion of current trends and challenges in cellulose aerogel research for thermal insulation are presented, gathered from studies reported within the last five years. The text is divided into three main sections: (i) an overview of thermal performance of cellulose aerogels, (ii) an identification of challenges and possible solutions for cellulose aerogel thermal insulation, and (iii) a brief description of cellulose/silica aerogels.

BACKGROUND: In order to offer the benefit of heart transplantation to a greater number of patients some centers match high-risk recipients (HR) with non-standard or ”marginal” donors (MD) in a strategy known as Alternate Listing Transplantation (ALT). However, pairing sicker recipients with lower quality donor organs remains both clinically and ethically controversial. The purpose of this study was to determine the cost-effectiveness of this strategy. METHODS: The United Network of Organ Sharing provided deidentified patient-level data. UNOS data was used to determine clinical outcomes, including waiting time, post-transplant survival, length of stay, re-hospitalizations, episodes of rejection, and infection, based on all adult heart transplant candidates (age [gt] 18 yo) and listed between 1995–2005 (n=35,049). Through a systematic literature review of randomized controlled trials and case series that estimated related costs, a cost model (see Table ) using an intention-to-treat assumption was developed to calculate the incremental cost-effectiveness ratio (ICER) for HR candidates who subsequently received marginal donor hearts (HTR) compared with candidates who were not transplanted (NTXP). High-risk recipient criteria included age [gt] 65yo, retransplantation, HepC+, HIV+, CrCl <30 ml/min, DM with PVD, DM with Crcl <40 ml/min. MD criteria included age [gt] 55yo, DM, HIV+, HepC+, EF<45%, and donor:recipient weight <0.7. RESULTS: Median survival from listing was 0.55 yrs and 5.1 yrs in the HTR and NTXP groups, respectively. The estimated ICER was $66,645($34,046–$127,491) ICER varied by HR recipient criteria. CONCLUSIONS: The ICER for ALT falls at the upper limits acceptability for being cost-effective. However, stratified analysis by HR group suggests that this strategy is not appropriate for all groups. Cost Model Assumptions

Objective: Maggot debridement therapy (MDT) has seen a resurgence in recent years in the treatment of hard-to-heal wounds, as a result of rising antibiotic resistance. The sterilised larvae of Lucilia cuprina have been used in MDT in Malaysia since 2003, with encouraging results for the treatment of hard-to-heal diabetic wounds. We report a case series of 30 patients selected from our clinic by convenient sampling with diabetic lower limb ulcers treated with MDT. The average age of patients receiving MDT was >50 years. Of the 30 patients in the study, nine were female and 21 were male. All patients had underlying diabetes, two patients had leg ulcers and 28 patients had diabetic foot ulcers. Sterilised Lucilia cuprina larvae were applied via a standard method of 10 maggots per square centimetre and dressed with sterile gauze. The study endpoint was defined as ≤5% coverage with slough or necrotic tissue following three successive applications of MDT. In this study, maximum debridement of wounds was achieved in 96.6% (29 patients) of our patients, with ≤5% coverage with slough or necrotic tissue, in addition to a reduction in wound-related pain, as assessed by a visual analogue scale. No adverse events were reported. The findings of this study support the use of MDT as a safe, efficacious, and cost-effective method of managing diabetic wounds.
