Prevalence of CYP2C19 Polymorphisms in Clopidogrel Treated Turkish Patients: Preliminary Results, 2017

Background: Clopidogrel is one of the most frequently prescribed antiplatelet agents to reduce the risk of atherosclerotic symptoms. CYP2C19 enzyme is involved in clopidogrel metabolism, and several genetic variations of CYP2C19gene are able to affect the clinical response of clopidogrel. Despite the lack of a fully accepted guideline for CYP2C19 pharmacogenetic testing before clopidogrel treatment by relevant communities, we believe that determination of the variant frequencies is important to predict the efficiency and possible clopidogrel related risks before the initiation of treatment on the basis of populations. Our aim was to determine the distribution of gene polymorphisms affecting the enzyme activity in Turkish cardiac patients prescribed clopidogrel. Methods: 54 clopidogrel prescribed patients were included in the study. The presence of CYP2C19*2, *3, *4, *5, *6, *7, *8, *9, *10 and *17 polymorphisms were investigated using a microarray platform. Results : No variant allele was detected for *4, *5, *6, *7, *8, *9 and *10 polymorphisms. The genotype frequencies were detected as 38.89% for *1/*1, 16.67% for *1/*2, 11.11% for *2/*17, 1.85% for *1/*3, 1.85% for *2/*3, 27.78% for *1/*17 and 1.85% for *17/*17. According to genotype analysis, 1.85% of the patients were recorded as poor and 29.63% intermediate; whereas 27.78% as rapid and 1.85% ultra-rapid metabolizers. Conclusion: Although our study population does not consist of a high number of patients, since the high frequency of intermediate, rapid and ultra-rapid metabolizer patients were detected in relatively high frequencies, CYP2C19 polymorphisms should be taken into account for efficiency and possible clopidogrel related risks in Turkish cardiac patients.

Abstract: Organizational Citizenship Behavior (OCB) is a variable that can be influenced by job satisfaction. This study aims to determine the effect of job satisfaction variables on Organizational Citizenship Behavior OCB alumni level IV leadership training in Banten Province. This research uses a quantitative approach through survey methods. Data analysis technique used is simple linear regression. The population in this study were 40 respondents and the sample used was 40 respondents. Determination of the sample using total sampling techniques or samples taken from the entire study population. The results of the analysis and interpretation of research data show that the value of the regulatory coefficient of influence on job satisfaction on Organizational Citizenship Behavior (OCB) alumni of leadership training level IV is 0.740 thus it can be concluded that job satisfaction has a direct positive effect on Organizational Citizenship Behavior alumni training, meaning that improvement of satisfaction in IV level leadership training is 0.740. work has an impact on improving Organizational Citizenship Behavior for training alumni, So Organizational Citizenship Behavior for training alumni can be achieved through job satisfaction.Keywords: Job satisfaction, Organizational Citizenship Behavior, Leadership Training Alumni Level IV

Abstract Background Sex determination is one of the leading criterion in identification and verification of an individual. However, the potential roles of differences in adjacent fingerprint white line count (FWLC) in sex inference are not well elucidated in the literature especially among Hausa population. The study was conducted to determine sexual dimorphism and predict sex using adjacent digit FWLC difference (adj. DFWLCD) among Hausa population of Kano state, Nigeria. Methods The study population involved 300 participants. FWLC was determined from a plain fingerprint captured using live scanner. The formula for adj. DFWLCD of thumb and fifth digit is dR15 for right hand. The same applied for possible combination in cephalocaudal direction. Mann-Whitney and t tests were used for comparison of variables between sexes. Binary logistic regression analyses were employed for determination of sex. Results We observed a significantly larger adj. DFWLCD in males compared with females in most of the digit combination. A significant sexual dimorphism was observed in most of the adj. DFWLCD involving ring digit in both right (dR14, dR24, and dR34) and left (dL14, dL24, and dL34). The best discrimination was observed in adjacent FWLC difference of second and fourth digits in both right and left digits (dR24 and dL24). This was further supported by stepwise logistic regression analyses. Conclusion The adj. DFWLCD exhibits sexual dimorphism. The best prediction potentials were found to be dR24 and dL24 for right and left hands respectively.

Determination of high-speed crafts’ hydrodynamic coefficients will help to analyze the dynamics of these kinds of vessels and the factors affecting their dynamic stabilities. Also, it can be useful and effective in controlling the vessel instabilities. The main purpose of this study is to determine the coefficients of longitudinal motions of a planing catamaran with and without a hydrofoil using Reynolds-averaged Navier–Stokes method to evaluate the foil effects on them. Determination of hydrodynamic coefficients by experimental approach is costly and requires meticulous laboratory equipment; therefore, utilizing the numerical methods and developing a virtual laboratory seem highly efficient. In this study, the numerical results for hydrodynamic coefficients of a high-speed craft are verified against Troesch’s experimental results. In the following, after determination of hydrodynamic coefficients of a planing catamaran with and without foil, the foil effects on its hydrodynamic coefficients are evaluated. The results indicate that most of the coefficients are frequency-independent especially at high frequencies.

Abstract: Employee performance is very important for an organization to achieve its goals. This study aims to analyze the influence of work culture, on employee performance, leadership on employee performance, work discipline on employee performance, compensation on employee performance and work culture, leadership, work discipline, compensation on employee performance at PT. KAI Daop 5 Purwokerto. The study population is all employees of PT. KAI Daop 5 Purwokerto as many as 623 employees. Determination of sample size with Slovin formula obtained by 86 respondents. The data used is the primary data obtained from the questionnaires distributed to respondents.Tehnik data analysis used is a quantitative approach. The data of the research were analyzed using multiple regression analysis. The results showed that: Work culture affect the performance of employees means that if the work culture increases then the performance of employees will increase. Leadership affects the performance of employees means that if the existing leadership is conducive to work then the performance of employees will increase, work discipline does not affect the performance of employees means employees who discipline work high and employees who work discipline less have the same performance height, Compensation effect on the performance of meaning if the compensation increases then employee performance will increase, work culture, leadership, work discipline and compensation together affect the employee's performance, meaning that if work culture, leadership, work discipline and compensation together increase the employee performance will increase. Penelitian ini bertujuan untuk menganalisis pengaruh budaya kerja, terhadap kinerja pegawai, kepemimpinan terhadap kinerja pegawai, disiplin kerja terhadap kinerja pegawai, kompensasi terhadap kinerja pegawai dan budaya kerja, kepemimpinan, disiplin kerja, kompensasi terhadap kinerja pegawai di PT. KAI Daop 5 Purwokerto. Populasi penelitian adalah semua karyawan PT. KAI Daop 5 Purwokerto sebanyak 623 orang karyawan. Penentuan ukuran sampel dengan rumus Slovin diperoleh sampel sebanyak 86 responden. Data yang digunakan adalah data primer yang diperoleh dari kuisioner yang dibagikan kepada responden.Tehnik analisis data yang digunakan adalah pendekatan kuantitatif. Data hasil penelitian dianalisis menggunakan analisis regresi berganda. Hasil penelitian menunjukkan bahwa: Budaya kerja berpengaruh terhadap kinerja pegawai artinya apabila budaya kerja meningkat maka kinerja karyawan akan meningkat. Kepemimpinan berpengaruh terhadap kinerja pegawai artinya apabila kepemimpinan yang ada sangat kondusif untuk bekerja maka kinerja karyawan akan meningkat, Disiplin kerja tidak berpengaruh terhadap kinerja pegawai artinya pegawai yang disiplin kerjanya tinggi dan pegawai yang disiplin kerjanya kurang memiliki kinerja yang sama tingginya, Kompensasi berpengaruh terhadap kinerja artinya apabila kompensasi meningkat maka kinerja karyawan akan meningkat, Budaya kerja, kepemimpinan, disiplin kerja dan kompensasi secara bersama-sama berpengaruh terhadap kinerja pegawai, artinya apabila budaya kerja, kepemimpinan, disiplin kerja dan kompensasi  secara bersama-sama mengalami peningkatan maka kinerja karyawan akan meningkat. The purpose of the study is to assess the health of patients in the activity of tertiary prevention dedicated to preventing blindness caused by POAG (primary glaucoma with open angle and high tension) and NTG (primary glaucoma with open-angle and statistically normal tension—particular form of glaucoma with open angle) and preservation of the remaining visual function. The design of the study is epidemiological, observational, descriptive and retrospective, and uses only the data recorded in the existing records in the archives of the Ophthalmology office within the Integrated Outpatient Clinic of the Emergency Clinical Hospital of Oradea (IOCECHO) during the years 1999–2019 (anamnestic data; objective examination and paraclinical examination: intraocular pressure—IOP and visual field—VF). The methods of the study included the standardized protocol: anamnesis, physical ophthalmological examination, IOP determination, and computerized perimetry with the “Fast Threshold” strategy performed with the “Opto AP-300” perimeter. The obtained results were statistically processed with a specialized software (S.P.S.S.—I.B.M. Statistics version 22). The study examined the available data of 522 patients of which 140 were men (26.8%) and 382 were women (73.2%). The gender ratio was 0.37. In the period 1999–2019, 150,844 people with ophthalmic pathology were consulted in the Ophthalmology office of IOCECHO out of which 522 patients (0.35%) were diagnosed with primitive open-angle glaucoma, 184 people (35.2%) presented high IOP (POAG), and 338 people (64.8%) had statistically normal IOP (NTG). The annual proportion of cases diagnosed with glaucoma in the total number of patients examined was between 0.1% (2005; 2008; 2010) and 2.4% in 2012, when 101 people were detected. In the studied records, no cases of uni- and/or bilateral blindness were mentioned. The mean age of glaucoma patients at the first consultation was 60.81 ± 12.14 years with high frequencies in the 55–69 age groups and at the last consultation it was 66.10 ± 12.47 years with high frequencies in the age groups between 60–74 years. Monitoring and treatment of glaucoma patients was beneficial; IOP decreased statistically significantly: in patients with POAG by 46.16%, from 30.50 ± 7.98 mmHg to 16.42 ± 3.01 mmHg (p = 0.000) and in those with NTG by 17.44%, at 16.39 ± 3.66 mmHg at 13.53 ± 1.92 mmHG (p = 0.000). The duration of treatment and monitoring was on average 5.1 ± 3.4 years, for 184 patients (35.2%) with POAG and 5.1 ± 3.8 years for 338 patients (64.8%) with NTG. Tertiary prevention of glaucoma, by providing specialized care, ensures effective control of IOP and implicitly of the long-term evolution of the disease. IOP is the only modifiable risk factor in patients with POAG and NTG and its decrease prevents the progression of the disease and emphasizes the importance of early diagnosis and treatment. The management of the glaucoma patient consisted of: complete ophthalmological examination (subjective and objective), paraclinical examination with IOP, and VF measurement (valuable ophthalmological diagnostic tool) for disease detection and effective assessment of disease progression in order to improve the process of therapeutic decision making.

Introduction: Oligohydramnios is an extremely rare condition where the liquor amnii is deficient in amount to the extent of less than 200 mL at term. Methods and Materials: This study is a retrospective study conducted in Saveetha Medical college from JAN2021- MAR2021 done by random sampling method and 50 patients with oligohydramnios were taken as study population after satisfying inclusion and exclusion criteria. Results: In the current study, 78 percent of patients received emergency LSCS, while only 22 percent had a normal delivery, in this study, approximately 20% weighed less than 2 kg, The most common cause of Oligohydramnios in this study was idiopathic. Conclusion: Oligohydraminos is now being identified more frequently in regular USG examinations. Oligohydraminos is linked to a higher likelihood of pregnancy problems, as well as a higher rate of death and morbidity.

Background: Gastroesophageal Reflux (GER) is the regurgitation of gastric contents and acid into the esophagus. Frequent and abnormal amounts of reflux leads to Gastroesophageal reflux disease (GERD) which causes symptoms like heartburn, regurgitation, and /or other complications. In view of absence of any data on the prevalence of GERD from this part of India, the current population based study was conducted to study the prevalence of GERD and its association with various risk factors.Methods: In this community based prospective cross-sectional observational study, 500 patients from rural and urban areas of West Bengal were included during the one year period from July 2014 to June 2015.Results: Out of a total of 500 patients studied, the percentage of patients with GERD in our study population was found to be 31.3%. Out of 292 males enrolled for the study, number of patients with GERD was 66 (22.6%) as compare to females where the same was found to be 43.26% (90/208). The majority (68.2%) of patients had mild GERD, 18.5% had moderate GERD whereas only 13.3% of patients had severe GERD.Conclusions: The percentage of patients with GERD in our study population was found to be 31.3%. It was observed to be significantly associated with increasing Body Mass Index (B.M.I), smoking, the female gender, a sedentary lifestyle, dinner to bed-time interval of ≤2 hours, chronic NSAID use, and a past history of abdominal surgery.
