The article discusses the particular conditions of holding medical organizations accountable for causing harm to the health of patients. The conditions of civil liability, as well as the degree of responsibility of a medical organization in the provision of medical services, are analyzed. The authors are of the opinion that medical care should be organized in accordance with the procedures, conditions and standards for the provision of such care, however, the standards cannot cover all the options that may arise during the provision of medical care, therefore they are aimed at creating an average “sample”, to determine the approximate order of possible actions carried out by medical personnel. Attention is also paid to the consideration of the features of causing harm to the patient’s health, depending on whether the harm was caused in the provision of medical care or medical services. In conclusion, it was concluded that the conditions for bringing medical organizations to justice should include: unlawful action (inaction), harm, a causal relationship between unlawful action and harm, as well as the fault of the injurer.

The essence of the indicated categories is revealed and their interpretation is analyzed. The forms of abuse of the right in the field of medical care or medical services are considered. Attention is also paid to the consideration of civil liability without fault

Aim of the study. Analysis of medical tourism’s organizational features based on the example of the large medical organizations in the United Kingdom, South Korea, Italy and China. Materials and methods. The data were collected by the authors by interviewing the heads of medical organizations and their deputies in the United Kingdom, South Korea, Italy and China (3–4 respondents per medical organization) using the developed questionnaire to identify the main mechanisms and tools for organizing the export of medical services. SWOT-analysis (Strengths; Weaknesses; Opportunities; Threats) was performed in order to comprehensively evaluate the received information. Results. Along with weaknesses and threats that slow down the development of medical services exports, strengths (internal factors) and opportunities ( external factors) that contribute to the development of medical tourism were also identified: the widespread popularity of the brand of medical organizations abroad which is associated with the provision of premium medical services; versatility and ability to conduct high-tech surgical operations; the presence of a separate premium class building and an international department for working with foreign patients and promoting a medical organization in the world market; well-established business relationships with assistance companies; foreign medical personnel who speak foreign languages and possess necessary skills to treat foreign patients; developed electronic medical care system; developed system of quality control of medical care; the presence of branches in other countries; the presence of a medical visa in the system of legislation; established cooperation with many countries at the embassy level; state licensing and accreditation for the provision of medical services to foreign citzens; the availability of a state website on the provision of medical assistance to foreign citizens; the possibility of the age of value added tax. Conclusion. We identified main patterns in the organization of export of medical services that can be applied to develop this direction in medical organizations of the Russian Federation during the analysis the strengths and weaknesses of four large medical organizations abroad, as well as external factors that affect the work of these medical organizations.

Currently, work in any medical organization carries a risk of coronavirus infection, and, first of all, this applies to medical organizations dealing with the treatment of patients infected with coronavirus. Medical workers are a group at increased risk of infection with the SARS-CoV-2 virus in the provision of medical care in modern conditions, which determines their incidence of COVID-19 [1-3]. The aim of the study is to explore the prevalence of cases of COVID-19 infection in medical workers providing medical care on an outpatient basis. Materials and methods. The analysis of the incidence of COVID-19 medical workers in one of the large medical organizations of the Samara region during the period of the pandemic was carried out. The medical staff of the medical organization includes 207 doctors. There are 11 subdivisions in the structure of the medical and prophylactic institution. Research methods: content analysis of the modern regulatory framework (2020-2021), statistical, expert. For the period from May 2020 to March 2021 71 cases of infection of medical workers on an outpatient basis with SARS-CoV-2 were identified, of which 32 doctors (45%) and 39 people (55%) nurses. Based on the results of the expert assessment, the insurance nature of the infection case was established in 50 people (70%). Among the medical and nursing categories of medical workers, the largest number of cases was made by doctors and nurses of primary contact - specialists of the district service: general practitioners (40%), pediatricians (32%), doctors - obstetricians-gynecologists (12%), nurses adults (76%) and children (20%) polyclinic departments. A third of health workers received inpatient treatment for severe COVID-19, an average of 60 days. Conclusion. Medical workers are a risk group of COVID-19 who need support measures, since the possibility of occupational infection has been sufficiently proven, and the risk probably depends on the work performed and the conditions of direct contact of medical personnel, which requires further study in the current persisting epidemic conditions.

At present, the quality and availability of medical care are the aspects to which close attention is paid in the system of organizing medical care. One of the ways to achieve a high level of quality and accessibility of medical care is building up human resources, i.e. the presence of highly qualified general practitioners, sub-specialties and middle medical personnel in sufficient numbers in a medical organization. Currently, the oncological service of Russia has been assigned tasks to fulfill the instructions from the State Program for the Development of Health Care. The results of the work should lead to a decrease in mortality and an increase in the quality of life of the population. To address the issue of reducing mortality from malignant neoplasms, in particular from prostate cancer, a three-level system of oncological care has been created on the territory of the Nizhny Novgorod Region, including 1 regional center, 2 interdistrict cancer centers, 88 primary oncology rooms and 96 examination rooms. The tasks set to reduce morbidity and mortality from oncological diseases can be realized only with close interaction of the oncological service with the primary health care sector, in which the prevention should be the priority direction of work.

.Objective. To study the opinion of parents (legal representatives) on the problems of medical care for children with oncological diseases.Methods. The study was based on the Questionnaire for parents (legal representatives) on medical care children with cancer. The questionnaire consisted of 27 questions, the respondents were asked to choose one or several answers or to enter their own option.Results. This medical and social study demonstrated that 81.1±1.9% of respondents are not satisfied with the attitude of doctors and nurses towards them and their children, 15.9±1.8% of respondents are partially satisfied and only 3±0.8% of respondents are fully satisfied. The reasons for dissatisfaction were as follows: rude communication (35.8±2.4%), inadequate attention of medical personnel (21.3±2.0%) and lack of interest in the treatment success (19.7±2.0%). The combination of these options was noted by 23.2±2.0% of the respondents. 63.7±2.4% of respondents would prefer to treat their children at a medical organization of federal subordination, 33.9±2.4% – at medical organization outside the territory of the Russian Federation, of which 57.4±2.4% would like to receive a patient-oriented service and simplified routing between medical organizations during treatment. 12.7±1.6% mistrust in the qualifications of medical personnel in the Russian Federation, 11.8±1.6% of respondents indicated the lack of comfortable conditions for examination and treatment, 5.3±1.1% of respondents indicated the lack of necessary diagnostic and treatment methods and 12.8 1.6% of respondents indicated a combination of the above options.Conclusion. The results obtained indicate the need to implement patient-oriented approaches and to improve the routing of children with cancer.

Marketing in the health system contributes to increasing public satisfaction in the demand for medical care, providing the population with high-quality medical care and improving their standard of living. The method of identifying and analyzing contact points, based on the results of studying and taking into account the opinion of patients, for the implementation of patient-oriented and participative approaches to marketing medical services, provides for the active participation of the patient himself in discussing issues of his own health, which can be used to evaluate and qualitatively improve the activities of health organizations. In modern conditions, there are problems in the dental services market due to the lack of modern equipment and the provision of highly qualified quality dental care. The client orientation of medical personnel is a significant indicator of the quality of medical services, the patient's perception of the value of the service and is associated, both with a high level of medical service delivery and the qualification of medical personnel. The lack of managerial and financial autonomy of health-care organizations limits their ability to improve the use of financial resources. Marketing research is mainly used to optimize the activities of non-state health organizations and allows the development of a dental clinic by introducing economically justified forms of medical care and the possibility of improving the provision of medical services, which will contribute to improving dental care for the population.

Medical personnel have long been one of the weaknesses of our medical organization; their insufficient number, their uneven distribution throughout the country, and the inadequacy of their preparation to the requirements of life, were repeatedly discussed at congresses and in the press. But at the present time, when the cultural growth of the population and the socialist restructuring of the economy and everyday life have further increased the need for medical care and made a demand for a medical worker of a new formation, our old need has become especially acute. The December decree of the Central Committee of the All-Union Communist Party of Bolsheviks on medical care for workers and peasants, the decision of the Council of People's Commissars of the RSFSR dated January 28, 1930, the orders of the NKZ issued in development of these decisions, and finally, the decisions adopted in March this year. At the All-Russian Congress of Health Departments and at a methodological meeting under the Glavprofobra, the reorganization of health care was greatly advanced and at the same time not only put the issue of medical personnel on the order of the day, but also largely determined the ways of its solution.

Primary health care for children is the mainstay of the system of medical care for children and includes measures for the prevention, diagnosis, treatment of diseases, medical rehabilitation, the development of healthy lifestyles and sanitary and hygienic education for the children. Medical organizations located in rural areas have particularities in the system of providing medical care to the population. This concerns the type, capacity, disposition of medical organizations, their provision with qualified medical personnel, and the possibility of obtaining qualified medical care. In order to assess the state of outpatient care for children under the conditions of municipal health care, data was taken out of the federal statistical reporting “Information on medical organization” form 30 “Information about the number of diseases registered in patients living in the area of service of the medical organization” form 12 for 2015-2017. The research was based on the Toksovo District Hospital, a state budgetary health care institution in the Leningrad Region. In the course of the study, pediatric staff were evaluated for pediatric staff, indicators of outpatient care for the children, indicators of the burden of pediatricians and morbidity rates for the treatment of children. It was found that in the years under study there was a high level of provision of pediatricians with an attached pediatric population, an increase in the number of district pediatricians and a decrease in the part-time ratio. The increase in the workload of pediatricians conducting outpatient admissions, including district pediatricians, associated with an increase in visits for diseases in 2015-2017, indicates a deterioration in the health of the children’s population, as evidenced by an increase in the overall morbidity rate for outpatient using the child population.

To study the opinions of parents (legal representatives) on the problems of organizing medical care for children with oncological diseases, a medical and social study was conducted. This study aims to examine the opinions of parents on the problems of organizing medical care for children with oncological diseases. This study was conducted based on the developed questionnaire for recording the opinions of parents (legal representatives) on improving the organization of medical care for children with cancer. The questionnaire comprised 27 questions, in which the respondents were asked to choose one or more answer options for each of them. For objectivity, the questionnaires were filled in anonymously. This medical and social study showed that 81.1 1.9% of the respondents are not satisfied with the attitude of doctors and nursing staff toward them and their children, 15.9 1.8% are only partially satisfied, and 3 0.8% are fully satisfied (roughness in communication (35.8 2.4%), lack of sufficient attention from medical personnel (21.3 2.0%), and lack of interest in the success of treatment (19.7 2.0%)). The combination of these options was noted by 23.2 2.0% of respondents. As the preferred treatment site for their children, 63.7 2.4% of the respondents indicated a medical organization of federal subordination and 33.9 2.4% a medical organization located outside the territory of the Russian Federation, in which 57.4 2.4 % of the respondents mentioned the possibility of receiving a patient-oriented service and complex routing during treatment between the medical organizations in the Russian Federation as a reason for choosing. Furthermore, 12.7 1.6% of the respondents indicated a lack of confidence in the qualifications of medical personnel in the Russian Federation; 11.8 1.6%, the lack of comfortable conditions for examination and treatment; 5.3 1.1%, the lack of necessary diagnostic and treatment methods; and 12.8 1.6%, a combination of the listed options. Overall, the findings of this study indicate the need to further improve the organization of medical care for children with cancer: introducing patient-oriented approaches and improving patient routing to create a patient-centric health care system for organizing medical care for children with cancer are necessary.

In 1993, Taiwan has transformed into an aged society. Compared with general patients, older patients differ considerably in body functionality, mentally, and mobility-wise. The primary customers of medical institutions are adult patients in Taiwan, and the needs of older patients are often ignored; however, older patients visiting the case hospital in this study accounted for nearly 30% of the total number of outpatients. This indicates that the needs of older patients are a matter requiring the serious attention of medical personnel. This is a common issue for all countries that were entered an aging structure of the population. This study investigated the attention paid by supervisors and medical personnel involved with medical decisions to the quality of medical services received by older outpatients. An analytic hierarchy process (AHP) was employed to interview 10 experts in older medical care providing age-friendly medical care at the case hospital, medical staff including senior physicians, nursing department supervisors, administration department supervisors, and certified managers. The results indicate that the supervisors and medical personnel considered the “communication and services” provided to older patients during their medical treatments to be the most crucial factor in their treatment process, rating it twice as important as the next most important criteria, the care process and physical environment. Medical personnel paid great attention to communicate with and servicing older patients; listening to their needs and concerns was deemed the most critical, followed by being able to empathize with them to solve problems. Asking patients about their problems when they visited the hospital was considered the most crucial aspect of the care process; in the physical environment, it was spatial planning and traffic flow design. The results of this study enable other countries' medical institution managers and relevant competent authorities to gain insight into the attention paid by front-line employees in promoting age-friendly medical care. If supervisors and medical personnel involved with medical decisions share a similar attitude toward promoting medical service quality, government authorities and medical institutions can implement and enhance an age-friendly medical treatment environment for older patients, as well as increase quality of patient-centered medical services.
