Preparasi kemomekanik pada saluran akar meliputi instrumentasi mekanis dan irigasi antibakteri yang secara prinsip dapat langsung mengeliminasi mikroorganisme pada sistem saluran akar. sejak diperkenalkan pada tahun 1988, instrumen rotary nikel-titanium (niti) telah digunakan secara umum dalam perawatan endodontik karena kemampuannya membentuk saluran akar dengan prosedur komplikasi yang minimal. Tujuan dari laporan kasus ini adalah untuk memaparkan perawatan saluran akar satu kunjungan menggunakan protaper rotary dan restorasi resin komposit gigi premolar. Penderita pria 21 tahun datang ke RSGM Prof. Soedomo UGM Yogyakarta mengeluhkan gigi belakang atas kiri yang berlubang tapi tidak sakit dan pasien ingin  dirawat.Gigi  Premolar satu atas  kiri terdapat kavitas disto oklusal dengan pulpa terbuka. Pemeriksan objektif pada gigi 24 sondasi, perkusi, palpasi, dan tes termal menunjukkan hasil negatif.Pemeriksaan radiografis tidak terdapat lesi, lamina dura tidak terputus dan saluran akar jelas dan lurus. Pada kasus ini dilakukan perawatan saluran akar dengan menggunakan ProTaper rotary ( X-Smart, Dentsply). Pasca perawatan saluran akar, gigi premolar satu atas kiri dilakukan tumpatan resin komposit kelas II.Hasil evaluasi klinik saat kontrol tidak ada keluhan rasa sakit, pemeriksaan objektif juga tidak ada rasa sakit, warna gigi serasi dengan warna gigi tetangga.prognosis pada kasus ini baik dan tidak ada keluhan. Kesimpulan paska perawatan saluran akar satu kunjungan dengan instrumenrotary, tidak terdapat keluhan.Penggunaan Instrumen rotary Nikel-Titanium (NiTi) sangat flexible dengan prosedur komplikasi yang minimal, dan hemat waktu.One Visit Treatment of Upper Premolar Tooth Using Rotary Protaper and Composite Resin Restoration. Chemomechanical preparation for root canal including mechanic instrumentation and anti-bacterial irrigation principally could eliminate microorganisms in root canal system. Many instruments and techniques have been described and developed for initial root canal treatment. Since first established in 1988, nikel-titanium (NiTi) rotary instrument has been used for endodontic treatment because of its capability in forming the root canal with minimal complication procedure. Clinically, safe use of NiTi instrument and understanding of the alloy basic metallurgy including fracture mechanism and its correlation with root canal anatomy in order to set the safe use of NiTi instrument are required. This research will explain about biological principal of root canal preparation with correct technique and instrumentation system using NiTi. A 21 year-old patient in RSGM Prof. Soedomo UGM Yogyakarta complained about his left side upper jaw, posterior teeth with cavity, painless and the patient wanted to treat the teeth. The left maxilla first premolar teeth has a distooclussal cavity, open pulp. Based on the radiograph test, there found no lession, lamina dura was fine, and root canal was clear and straight. In this case, the patient was treated with crown down rotary X-Smart (Dentsply) technique for root canal treatment. After the root canal treatment had been conducted, the teeth were restorated with class II composite resin. After the treatment, it is found that there is no pain in the teeth, the colour of teeth match with others, and the prognosis for this case is good.

Perawatan saluran akar (PSA) satu kunjungan merupakan perawatan saluran akar dengan prinsip triad endodontik (cleaning and shaping, medikasi dan obturasi saluran akar) diselesaikan dalam satu kali kunjungan. Keuntungan perawatan adalah memperkecil risiko kontaminasi mikroorganisme dalam saluran akar antar kunjungan, menghemat waktu perawatan karena tidak dilakukan penggantian medikasi intrakanal tetapi tanpa mengurangi kualitas hasil perawatan. Pulpitis ireversibel merupakan salah satu indikasi perawatan saluran akar satu kunjungan. Gigi posterior pasca PSA dengan kehilangan jaringan sehat yang tidak terlalu banyak dapat dilakukan restorasi menggunakan bahan resin komposit dengan penguat pasak parallel self-threading. Tujuan laporan kasus ini untuk menunjukkan keberhasilan perawatan saluran satu kunjungan pada kasus pulpitis ireversibel dan restorasi akhir menggunakan resin komposit yang diperkuat pasak parallel self-threading. Pasien laki-laki 47 tahun dilakukan perawatan saluran akar pada gigi molar kedua kanan mandibula dengan diagnosa pulpitis ireversibel. Pada pemeriksaan radiograf terlihat kavitas yang melibatkan pulpa dan tidak terdapat area radiolusen pada daerah periapikal. Kasus ini dilakukan PSA satu kunjungan dilanjutkan restorasi resin komposit dengan penguat pasak parallel self-threading pada kunjungan berikutnya. Perawatan saluran akar satu kunjungan disertai restorasi resin komposit dengan penguat pasak parallel self-threading berhasil dilakukan pada kasus pulpitis ireversibel pada gigi molar kedua kanan mandibula. Abstract: One Visit Endodontic Followed with Parallel Self Threading Dowel Reinforced Composite Resin Restoration on The Irreversible Pulpitis of Mandibular Right Second Molar. One visit endodontic root canal treatment (RCT) which endodontic triad (cleaning and shaping, medication, and obturation of the root canal) were completed in one visit. The advantages treatment is to minimize the risk of microorganisms contamination in the root canal, that saves time. In this treatment there is no intracanal medication replacement without reducing the quality of treatment. Irreversible pulpitis is one of one visit endodontic’s indications. Parallel self-threading dowel reinforced composite resin can be performed at minimal destruction post endodontically treated posterior teeth. The purpose of this case report is to demonstrate the irreversible pulpitis were treated by one visit root canal treatment successfully and its restorations with parallel self-threading dowel reinforced composite resin. Male patients 47years old who diagnosed irreversible pulpitis was treated by one visit root canal treatment on the mandibular right second molars. The radiographs image showed cavity involving to the pulp and there was no radiolucent area in the periapical region. Endodontic treatment was done by one visit root canal treatment and followed with composite resin restoration with parallel self-threading dowel. One visit endodontic followed with restored using composite resin material that reinforced by parallel self-threading dowel successfully performed on the mandibular right second molar with irreversible pulpitis diagnosis.

Gumboil is inflammation reaction in the gum caused by pulp infection. Gumboil is seen as an oral lesion characterized by a soft erythematous papule where a periapical abcess is draining into the oral cavity. An adequate root canal treatment can  cure gumboil and remove bacteria from the root canal. To support the success of root canal treatment, final restoration used in this case is composite resin restoration with reinforcing fiber (short fiber reinforced composite). The purpose of this treatment was to restore the functions of tooth in mastication and preserving the supporting tissue. A female patient age 23 years old came with complaints of pain in the lower right mandibular molar accompanied by swelling of the gums around the teeth since a week ago. Swelling is intermittent since last 3 months. The tooth have been restored for about 3 years ago. Clinical examination showed a positive percussion, positive palpation, negative vitality, and negative mobility. Radiographic examination showed bifurcation and periapical lesions. Root canal treatment is performed with crown down preparation technique and followed by direct composite resin restoration with short fiber reiforced composite. The success of root canal treatment followed by composite resin restoration with short fiber reinforced fiber is marked by the absence of complaints as well as the dissappearance of gumboil.

Morfologi saluran akar gigi molar mandibula ketiga memiliki variasi yang lebih kompleks dibandingkan gigi molar lainnya.   Pada gigi molar ketiga sering dilakukan pencabutan, namun dalam keadaan tertentu gigi molar ketiga dapat dipertahankan. Perawatan saluran akar satu kunjungan merupakan pilihan untuk terapi kasus ini. Tujuan laporan kasus ini untuk memperlihatkan keberhasilan dari perawatan saluran akar satu kunjungan pada gigi molar ketiga nekrosis pulpa disertai restorasi resin komposit dengan pasak tapered self theading. Pasien wanita 20 tahun datang ke RSGM Prof Soedomo FKG UGM dengan keluhan sakit saat pengunyahan pada gigi molar ketiga dan positif pada perkusi. Gambaran radiografis menunjukkan restorasi yang tidak sempurna, terdapat celah antara kavitas dengan restorasi. Rencana perawatan pada kasus ini, perawatan saluran akar satu kunjungan dan resin komposit dengan pasak tapered self threading sebagai restorasi akhir. Kesimpulan dari perawatan saluran akar satu kunjungan memiliki rekontaminasi mikroorganisme yang lebih kecil dibandingkan dengan multi kunjungan sehingga menjamin keberhasilan perawatannya. Restorasi resin komposit secara direkdengan pasak tapered self threadingmerupakan restorasi alternatif pasca perawatan endodontikkarena lebih cepat dan kuat.Composite Resin Restoration Using Tapered Self Threading Poston Left Mandibular Third Molar. Root canal morphology of mandibular third molar has more complex variation than the other molars. In third molar, the extraction teeth are often executed; however, the third molar can be maintained in other conditions. One visit root canal treatment is a therapy option for this case. The purpose of this case report is to show the success of one visit root canal treatment in third molar with pulp necrosis by restoring the composite resin through tapered self-treading post. A 20 year-old female patient who came to the Prof Soedomo RSGM, FKG UGM complained about the pain when chewing food on her third molar and positive in percussion. The radiographs showed that there was an incomplete restoration. There was a gap between cavities with restoration. The treatment plan for this case was one visit root canal treatment and composite resin with tapered self-threading post as final restoration. From the case, it can concluded that one visit root canal treatment results in a smaller chance for microorganism recontamination than the multi-visit in order to ensure the success of the treatment. Direct composite resin restoration with tapered self-threading dowel is an alternative restoration after endodontic treatment because it works out faster and more retentive.

Background and Objectives: Symptomatic irreversible pulpitis in permanent mature teeth is a common indication for nonsurgical root canal treatment (NSRCT), but contemporary studies have reported on vital pulp therapy (VPT) applied in such teeth as a less invasive treatment. This systematic review assessed the outcomes of VPT, including partial and full pulpotomy performed with hydraulic calcium silicate cements (HCSCs) in permanent mature posterior teeth diagnosed with symptomatic irreversible pulpitis. Materials and Methods: The PRISMA guidelines were followed. The search strategy included PubMed®, EMBASE, Cochrane library and grey literature electronic databases. The quality assessment of the identified studies followed the Cochrane Collaboration Risk of Bias, ROBINS-I and Newcastle–Ottawa Scale tools. Results: The search of primary databases identified 142 articles, of which 9 randomized controlled trials and 3 prospective cohort studies were selected for review. The risk-of-bias was assessed as ‘high’ or ‘serious’, ‘fair’, and ‘low’ for three, seven and two articles, respectively. One to five years after VPT using HCSCs, the success rates mostly ranged from 78 to 90%. Based on two articles, the outcomes of the VPT and NSRCT were comparable at one and five years. Despite the necessity for the intra-operative pulp assessment in VPT procedures, the majority of the studies did not fully report on this step or on the time needed to achieve hemostasis. Small sample sizes, of under 23 teeth, were reported in three studies. Conclusions: The reviewed 12 articles reported favorable outcomes of the VPT performed with HCSCs in permanent mature posterior teeth with symptomatic irreversible pulpitis, with radiographic success in the range of 81 to 90%. Two articles suggested comparable outcomes of the VPT and root canal treatment. Universal case selection and outcome criteria needs to be established for VPT when considered as an alternative to NSRCT. This evidence supports the need for further research comparing longer-term outcomes of both of the treatment modalities.

ABSTRACTThis case report describes the repositioning of a laterally luxated primary central incisor with occlusal interference, using a composite inclined plane. The patient was a 4-year-old girl who applied to our clinic three days after the injury. Because of the time delay between injury and presentation, it was not possible to reposition the tooth with pressure. Following a root-canal treatment, an inclined plane was prepared on the lower primary incisors, using composite resin. The tooth was repositioned in two weeks, and the inclined plane was then removed. After 1 year of follow-up, the treatment was found to be successful, both clinically and radiographically. The use of a composite inclined plane, accompanied by careful follow-up, is an effective alternative to extraction for laterally luxated primary incisors with occlusal interference. The frequency of separation of NiTi rotary instruments has increased with the increasing use of these instruments, and this error may result from inadequate experience of using these instruments. This iatrogenic error can complicate root canal treatment, and special experience is required to manage this problem. This case report describes the management of the separation of an F3 instrument from a ProTaper NiTi rotary system. A 30-year-old man was referred to the College of Dentistry, Taibah University (Saudi Arabia) with an intracanal separated NiTi rotary instrument (F3 of the ProTaper system; Dentsply Maillefer, Ballaigues, Switzerland) in the canal of the maxillary right canine. The separated instrument was removed by using the modified hollow tube–based extractor system. The present case revealed that sometimes a simple method can be an effective alternative of a professional method; using NiTi rotary instruments with strict adherence is important for the prevention of instrument separation.

Objectives: The intent of this study was to evaluate and compare the preferences and treatment choices between dentists and dental interns with regard to the following different treatment modalities: Root Canal Treatment (RCT) with restoration versus extraction with Implant-Supported Crown (ISC) or surgical treatment in relation to the given case scenarios. Methods: The questionnaire was presented as an online survey with a case scenario. The total number of the respondents were 165. The four clinical case scenarios included an anterior and posterior tooth having apical periodontitis, with and without previous RCT. The treatment options were as follows: RCT with restoration, extraction then implant, and surgical treatment. Results: A total of 165 dentists and dental interns were included in this study. A hundred and three 62.4% respondents were dentists, 60; 36.3% were males and 105; 63.7% were females. Most of the respondents graduated from the College of Dentistry, Jazan University (93.9%). The highest percentages and numbers for Anterior Teeth (AT) were selected in related to the RCT and restorations in the four scenarios among gender, dentists, and interns, with no considerable differences. A high percentage of RCT and restoration option was recorded for Posterior Teeth (PT) with no previous restoration and around 50% for the same treatment modality to posterior teeth with previous restorations. For Future Planning Postgraduate Studies (FPPS), it was obvious that most of the selected specialties agreed with the RCT and restorations choice. Conclusion: All dentists and interns in both genders preferred RCT with restorations over extraction, and then ISC in the AT with and without previous RCTs. In the PT with no previous RCT, the participants agreed that RCT with restorations is superior to other choices. Among the FPPS, the respondents demonstrated an absolute agreement to RCT and restorations as a treatment of choice for different scenarios. Clinical Significance: Dentists should preserve the natural teeth by RCT with restoration as the first treatment choice followed by other choices. The nonsurgical approach should always be adopted as a routine measure in PA lesions of endodontic origin. Conservative orthograde endodontic therapy demonstrates favorable outcomes with a regular periodic review and assessment of the healing process of PA lesions.

Latar belakang. Masalah pulpa dan periodontal menyebabkan lebih dari 50% kehilangan gigi. Kasus pada laporan kasus ini merupakan lesi primer endodontik dan lesi sekunder periodontal dan secara klinis terdapat sinus tract. Kondisi ini merupakan kasus yang secara kolaborasi dikerjakan oleh bidang konservasi gigi dan bidang periodonsia untukmemdapatkan hasil yang maksimal. Kasus. Terdapat tiga kasus lesi endodonti-periodontal, dua merupakan kasus (gigi 21 dan 37) yang setelah beberapa tahun dilakukan perawatan saluran akar (PSA) mengalami pembentukan sinus tract, dan satu kasus (gigi 47) yang setelah PSA namun tidak memeperlihatkan perbaikan sinus tract yang terbentuk di gingiva. Tindakan bedah flap periodontal dilakukan untuk mencari penyebab, menghilangkan jaringan granulasi, dan memperbaiki kerusakan tulang alveolar yang terjadi. Seluruh kasus terlihat terdapat kerusakan tulang anguler di daerah furkasi pada gigi posterior (37 dan 47), dan daerah interdental pada gigi anterior (21); satu kasus (gigi 21) mengalami fenestrasi di fasial. Defek tulang anguler dan daerah fenestrasi setelah dibersihkan dari jaringan granulasi yang terinfeksi, diisi dengan graf tulang dan ditutup oleh membran pericardium sebagai guided tissue regeneration (GTR), kemudian dijahit. Kontrol 14 hari setelah tindakan bedah, sinus tract  telah hilang dan warna gingiva normal. Kesimpulan. Lesi endodontik-periodontal yang memperlihatkan sinus tract yang persisten setelah perawatan saluran akar merupakan indikasi adanya kerusakan periodontal yang kompleks. Defek periodontal kompleks bisa diperbaiki dengan tindakan bedah regeneratif.Kata kunci. Lesi endodontik-periodontal, sinus tract, defek tulang anguler, fenestrasi Abstract Background. Dental pulp and periodontal problems account for more than 50% of tooth loss. The cases in this case report were primary endodontic lesions and secondary periodontal lesions and clinically contained a sinus tract. This report is a collaborative carried out by the conservative dentistry and periodontics to obtain maximum results. Case. There were three cases of endodontic-periodontal lesions, two were cases (teeth 21 and 37) where after several years of root canal treatment had sinus tract formation, and one case (tooth 47) after endodontic treatment but did not show any improvement in the sinus tract in the gingiva. Periodontal flap surgery is performed to find the cause, remove the granulation tissue, and repair the alveolar bone damage that has occurred. All cases showed angular bone defects in the furcation areas of the posterior teeth (37 and 47), and the interdental areas of the anterior teeth (21); one case (tooth 21) had facial fenestration. Angular bone defects and areas of fenestration after cleaning of infected granulation tissue, filled with bone graft and covered by pericardial membrane as guided tissue regeneration (GTR), then sutured. Control 14 days after surgery, the sinus tract was gone and the gingival color was normal. Conclusion. Endodontic-periodontal lesions showing persistent sinus tracts after root canal treatment are indicated of complex periodontal damage. Complex periodontal defects can be corrected with regenerative surgery.Keywords. endodontic-periodontal lesions, sinus tract, angular bone defect, fenestration
