Objective: The aim of the study was to monitor the magnitude and trends in antimicrobial resistance through surveillance programs and bacteriological diagnostic methods. Methods: The study research has performed a retrospective observational study for period of 6 months (Dec 2019–June 2020) to determine the antimicrobial susceptibility pattern on cultures of various specimens at Vivekanandha Medical Care Hospital. The resultant data were computed in Microsoft Excel. Analyses were expressed as the total number, frequencies, and percentage. Results: A total of 110 culture and sensitivity reports were collected from microbiology departments. Among the culture report, 84% accounted for gram-negative and 16% for Gram-positive organisms. Out of eight specimens, 15 different bacteria were identified and the most common bacteria were Klebsiella pneumonia (25%), E. coli (14%), Staphylococcus aureus (13%), and Pseudomonas aeruginosa (12%). Conclusion: The interferences from the present study can help in bridging the gap in future educations, training, and adequate decision-making.

Urinary tract infection (UTI), of varying severity is one of the most commonly encountered ailments in nephrology outpatient department (OPD). The commonest pathogens causing community acquired UTI (CAUTI) are E. Coli, Proteus mirabilis, Pseudomonas aeruginosa and Klebsiella pneumonia. There is a rapidly rising trend of antibiotic resistance among these uropathogens, may be due to indiscriminate antibiotic usage and poor patient compliance. We conducted a retrospective analysis of all culture proven UTI cases that attended the nephrology OPD of S. K. hospital, Trivandrum from August 2017 to July 2019 (24 months). 130 males and 121 females were included in the study. The mean age of the study group was 62.8 years with a male predominance in the age group above 50 years and a female predisposition in less than 50 years group. Commonest pathogen isolated in urine culture was E. coli (52.98%) followed by Klebsiella (39.09%), Citrobacter (8.76%), Pseudomonas (8.37%), Enterobacter (2.39%), Enterococci (1.99%), Candida (1.59%), Acinetobacter (1.2%), Proteus (1.2%), Streptococci (1.2%) and Serratia (0.4%).

E Coli is the most commonly encountered human pathogen which has been associated with various clinical manifestations.Analyzing resistance pattern of Escherichia coli to commonly prescribed antibiotics helps to decide empirical therapy.The study was conducted from January 2015 to December 2018.Total of 2135 E coli isolates were recovered from various clinical specimens .In our study antimicrobial susceptibility pattern of 1925 E coli isolates was studied Maximum no of E coli were isolated from urine 867 [40.60%] followed by skin & soft tissue specimens 691 [32.36%],stool specimens 179 [8.38%] , Respiratory Tract specimens 129[6.04%] , and blood 59 [2.76] .Out of 867 E coli isolates from urine specimens maximum susceptibility was found against Fosfomycin 806 [92.96 %] followed by Amikacin 753 [86.85 %], Ertapenem 702[80.96%], Gentamicin 563[64.93 %] and Nitrofurantoin 518[59.74% ]Most isolates of E coli from UTI, SSTI, RTI, BSI are susceptible to Carbapenems , Amikacin and CefoperazoneSulbactam..Increasing resistance to oral antimicrobial agents is a serious issue.Improved facilities for microbiology laboratories and judicious use of antimicrobial agents is need of the hour.

Urinary Tract Infection is alarming problem worldwide due to the intensity of antimicrobial resistance. Escherichia coli is the most predominant organism in UTI. This study was planned to evaluate demographic parameters, the prevalence of E. coli, and antimicrobial resistance patterns among E. coli isolates from UTI patients in Nishtar Hospital of Multan from January to June 2018. A total of 350 mid-stream urine samples were collected from different patients having age group from 25 to 60 years and processed by standard laboratory procedures. Out of 350 samples, 100 samples were observed as critical bacteremia. Escherichia coli and Klebsiella pneumonia were the most persistent (47 % and 19 % individually) among the Gram-negative pathogens followed by S. aureus (14 %), Enterobacter spp. (11 %) and Candida (9 %) respectively. The incidence of UTI was found higher in 25-35 age groups. The prevalence of UTI with E. coli as an infectious agent was 72 % in females, and 28 % in males. The prevalence of E. coli was more in rural areas (70 %) than in urban areas (26 %). The antimicrobial testing against E. coli showed the highest resistance to amoxicillin (65.9 %) and ciprofloxacin (38.2 %), whereas highly sensitive rate observed against Fosfomycin (FOS) (95.7 %), Gentamicin (GEN) (89.3 %), and Nitrofurantoin (NIT) (85 %) respectively. The increased resistance against ampicillin and ciprofloxacin was observed in Multan have a great emerging problem so there is a need for effective prevention strategies for the E. coli drug resistance and successful surveillance required to be improved.

Antimicrobial resistance (AMR) is an emerging public health problem in modern times and the current COVID-19 pandemic has further exaggerated this problem. Due to bacterial co-infection in COVID-19 cases, an irrational consumption of antibiotics has occurred during the pandemic. This study aimed to observe the COVID-19 patients hospitalized from 1 March 2019 to 31 December 2020 and to evaluate the AMR pattern of bacterial agents isolated. This was a single-center study comprising 494 bacterial isolates (blood and urine) that were obtained from patients with SARS-CoV-2 admitted to the ICU and investigated in the Department of Microbiology of a tertiary care hospital in Delhi, India. Out of the total bacterial isolates, 55.46% were gram negative and 44.53% were gram positive pathogens. Of the blood samples processed, the most common isolates were CoNS (Coagulase Negative Staphylococcus) and Staphylococcus aureus. Amongst the urinary isolates, most common pathogens were Escherichia coli and Staphylococcus aureus. A total of 60% MRSA was observed in urine and blood isolates. Up to 40% increase in AMR was observed amongst these isolates obtained during COVID-19 period compared to pre-COVID-19 times. The overuse of antibiotics gave abundant opportunity for the bacterial pathogens to gradually develop mechanisms and to acquire resistance. Since the dynamics of SARS-COV-2 are unpredictable, a compromise on hospital antibiotic policy may ultimately escalate the burden of drug resistant pathogens in hospitals. A shortage of trained staff during COVID-19 pandemic renders it impossible to maintain these records in places where the entire hospital staff is struggling to save lives. This study highlights the extensive rise in the use of antibiotics for respiratory illness due to COVID-19 compared to antibiotic use prior to COVID-19 in ICUs. The regular prescription audit followed by a constant surveillance of hospital infection control practices by the dedicated teams and training of clinicians can improve the quality of medications in the long run and help to fight the menace of AMR.

Background: Indiscriminate and inappropriate use of antimicrobial agents (AMA) resulted in rapid emergence of antimicrobial resistance. Institutional level surveillance program to be carried out to track AMA use. The study was conducted to evaluate the prevalence of uropathogens and their susceptibility and resistance pattern in a tertiary care hospital to revise empirical therapy.Methods: Urine samples received from the inpatients and outpatients Departments of Mahatma Gandhi memorial hospital for culture sensitivity between January 2018 to December 2018 were included in this study. Data collected from the Department of Microbiology register by using WHONET software. After identification, isolates were tested for antimicrobial susceptibility by the standard Kirby Bauers diffusion method. Descriptive analysis done and results were expressed as percentage.Results: Out of 3425 samples 68.5% showed no growth, 15.5% normal flora and only 15.9% reported as culture positive. In this study the highest isolate was Escherichia coli (59%) followed by Klebsiella pneumoniae (10.6%), Enterococcus sp. (7%), Staphylococcus aureus (5%), Candid (3.6%), Acinetobactor (3%) and Pseudomonas (2.9%). Uropathogens developed resistance against penicillins, cephalosporins, macrolides and cotrimaxazole.Conclusions: This study confirms, the frequently prescribed empirical therapy drugs were less susceptible and developed resistance than less frequently prescribed and costly drugs. The current antimicrobial resistance pattern alarms the irrational and excessive use of antimicrobial agents. Hence the treating physicians should revise empirical therapy periodically based on the institutional antibiogram and resistance pattern reported from the laboratory to preserve antimicrobial source for the future generation.

Background: The study was designed to provide account of etiological agents of urinary tract infection in pediatric patients and the antimicrobial resistance pattern plus biofilm producing profile of the isolates.Methods: The prospective study was conducted in Alka Hospital, Nepal with 353 clean catch urine samples from children. It was obtained during July 2014 to January 2015 which were first cultured by semi-quantitative method, followed by antimicrobial susceptibility testing and biofilm production assay on Congo red agar. Multidrug- resistance, extensively drug- resistance and pandrug- resistance among isolates were considered as per international consensus.Results: Out of 353 samples, 64 (18.13%) showed positive growth in culture, confirming urinary tract infection. E. coli, 44 (68.8%) was the predominant organism followed by Klebsiella spp. 6 (14.1%). Most E. coli were sensitive to amikacin (93.2%) followed by nitrofurantoin (86.4%), and highly resistant to ampicillin (95.5%). Of 64 isolates, 23 (35.93%) were found to be multidrug- resistant strains. Biofilm was produced by 36 (56.25%) isolates.Conclusions: This study showed higher biofilm production and resistance to in-use antibiotics rendering ineffective for empirical use. Regular surveillance of resistance patterns should be done to regulate multidrug- resistant bugs and to ensure effective management of urinary tract infection in children in a tertiary care setups.Keywords: AMR; antimicrobial resistance; biofilm; urinary tract infection; UTI.
