Background. Every year the number of patients with chronic renal failure is steadily increasing. Allogeneic kidney transplantation from a post-mortem donor is a radical method to cope with chronic renal failure, improving the quality and life expectancy of patients. Currently available inhalation anesthetics make it easy to control the depth of anesthesia; they are excreted by the lungs unchanged, providing a quick emergence from anesthesia and easy waking up of the patient. An “ideal” inhalation anesthetic used for kidney transplantation should have a minimal amount of adverse effects.The aim was to compare the efficacy of inhaled anesthetics used for allogeneic kidney transplantation from a posthumous donor.Material and methods. A randomized, prospective, single-center study included 62 patients with end-stage chronic renal failure. The subjects were divided into three groups depending on the type of the inhalation anesthetic used. The first group included patients who underwent low-flow inhalation anesthesia with desflurane, the second and third groups were comparator groups where patients received sevoflurane or isoflurane, respectively, as an inhalation anesthetic. When assessing hemodynamic parameters, most episodes of hemodynamic instability were seen in the isoflurane group; the most stable statistically significant values were observed in the sevoflurane group, and desflurane took an intermediate position.Results. The use of desflurane as an inhalation anesthetic in a kidney transplant provided a quicker recovery of consciousness and early extubation of the patient after anesthesia compared to the sevoflurane or isoflurane use. So desflurane proved to be the most efficient of the three studied inhalation anesthetics. Conclusion. Desflurane is the optimal inhalation anesthetic used in kidney transplantation.

In the general structure of chronic renal failure, up to 40% is terminal chronic renal failure requiring replacement renal therapy, particularly hemodialysis, peritoneal dialysis, and kidney transplantation. Azerbaijan has an annual increase in the number of patients with chronic renal failure by 10–12 per cent. In 2006, the State Program of Measures for Chronic Renal Failure 2006–2011 was adopted to reduce the prevalence and improve the quality of treatment for chronic renal failure. The implementation of this program allowed to create a state register of patients with chronic renal failure, expand the network of hemodialysis centers, increase availability in medicines, use modern methods of early diagnosis and prevention of chronic renal failure. In order to further improve nephrological assistance, the second stage of the State Program for the Reduction of Chronic Renal Insufficiency in Azerbaijan for 2011–2016 years has been adopted. The effectiveness of Program to reduce chronic renal insufficiency in Azerbaijan has been demonstrated by the improvement of the quality of medical care, the increase in life expectancy and the reduction of mortality. It should be noted that thanks to this Program, the number of patients receiving hemodialysis per 21.0–25.0% increases annually. In 2010, the Republican Clinical Urological Hospital opened a kidney transplant center. The main objectives of the Program are to reduce mortality from chronic renal failure, Increasing hemodialysis centers according to the number of patients with chronic renal failure and kidney transplant operations. In 2016, the State Program of chronic renal failure for 2016–2020 was adopted. Some progress has been made during the implementation of the State Program. In many parts of the Azerbaijan, the population is provided with dialysis assistance free of charge from the State budget. By January 1, 2020, there are 3 kidney transplantation centers and 45 hemodialysis centers equipped with 716 dialysis devices.

Abstract: The patients’ attitudes towards kidney transplantation are significantly affected by the acceptance of the treatment to have a better quality of life. Exploration of the cognitive-emotional relation in patients with chronic renal failure. Our study included 285 kidney patients. We investigated three groups of patients: predialysis, dialysis and transplantation patients. In the framework of a pre-examination we have created an attitudinal questionnaire (85 items) that can be used to describe the cognitive and emotional attitude of kidney patients towards their illness and to measure scales. Negative attitudes towards transplantation were reported in dialysis and predialysis patients. In contrast, patients who had already undergone surgery were particularly positive about kidney transplantation. Many patients refuse transplantation which correlates with attitudes towards illness and healing. Our results showed that the exploration of the patients’ cognitive emotional relation along with the medical diagnosis would have a better adherence towards the kidney substitutional treatment which ensures a better quality of life. Introduction. All over the world and in Russia, the number of patients requiring dialysis therapy and kidney transplantation for chronic renal failure in the end-stage of the renal disease is increasing. In many countries of the world, the number of dialysis patients over 60 years of age accounts for 30 to 45% of all patients with chronic renal failure. Meantime, taking into account the improved methods for early diagnosis of chronic renal failure and the treatment methods for chronic kidney disease, including the renal replacement therapy, we can expect an increase in the number of elderly potential kidney transplant recipients. The likelihood of receiving a renal graft in elderly patients is significantly lower than in young recipients. Elderly patients are known to have a higher risk of death while waiting for a kidney transplant due to higher morbidity and lethality on dialysis. For this reason, the urgency of increasing the availability of kidney transplantation in elderly patients is growing over time. One of the solutions can be the use of kidneys from suboptimal donors with a far from ideal graft quality, but which could meet the needs for transplant care of the older age group of patients. The older age of a recipient entails a certain risk of developing a graft dysfunction due to the presence of concomitant diseases, and the potential risk increases even more with kidney transplants from expanded criteria donors. If a reduced functional reserve of kidneys removed from donors with extended criteria is identified, two-kidney transplantation is possible, which provides fairly good long-term results. To reduce the risk of a kidney graft loss, a careful selection of recipients is necessary, taking into account their co-morbidities, including the presence of urological diseases that impair the function of the upper and lower urinary tract. Their timely identification and correction makes it possible to raise the availability of kidney transplantation for elderly patients and improve its results. This review presents the results of the studies conducted in various world transplant centers, covers the mortality rates, kidney graft and recipient survival rates.The study purpose was to summarize the actual data and the results of the study on kidney transplantation in elderly patients with urological pathology.

ABSTRACTObjective: to describe the patient’s knowledge with chronic renal failure submitted to kidney transplantation on the importance of consultation with nursing for his treatment. Methodology: this is about an exploratory-descriptive study from qualitative approach. The data collection was from January to December 2007. The sample consisted of 36 patients undergoing kidney transplant from a total of 400. For the choice of the patients was a random draw simple daily. For data collection was used a questionnaire consisting of 11 questions concerning the information about the kidney transplant. For organization of the results was used thematic analysis proposed by Minayo. Results: as for nursing consultation for a significant number of patients claiming to be of importance to them, since they make them safer, take your questions and will be better targeted.  Conclusion: confirming the effectiveness of professional practice of nurses in the monitoring of renal patients who is subject to renal transplantation. Para a coleta de dados foi utilizado um questionário composto por 11 perguntas referentes as informações sobre o transplante renal. Para organização dos resultados foi utilizada a análise temática proposta por Minayo. Resultados: quanto à consulta de enfermagem um número significativo de pacientes afirmam ser de importância aos mesmos, uma vez que os tornam mais seguros, tiram suas dúvidas e  serão melhor orientados. Conclusão: estes dados vêm a confirmar a eficácia da atuação profissional do enfermeiro no acompanhamento dos pacientes renais que se submeterão ao transplante renal. La muestra consistió de 36 pacientes sometidos a trasplante de riñón de un total de 400. Para la elección de los pacientes fue de un sorteo aleatorio simple diario. Para la recogida de datos se utilizó un cuestionario de 11 preguntas relativas a la información sobre el trasplante de riñón. Para la organización de los resultados se utilizó el análisis temático propuesto por Minayo. Resultados: cuanto a consulta de enfermeria, un número significativo de pacientes, afirma ser de importância a los mismos, una vez que los hacen mas seguros, disipan sus dudas y serán mejor orientados. Conclusión: lo que confirma la eficácia de la actuación profesional del enfermero, en el acompañamiento de los pacientes renales que se sometieron al transplante renal. <p>A Insuficiência Renal Crônica (IRC) é uma doença crônica, progressiva, debilitante, que causa incapacidades e que apresenta alta taxa de mortalidade, sendo que a incidência e prevalência têm aumentado ainda mais na população mundial, relacionada a histórico familiar, sedentarismo, dieta com alto teor de sódio, assim como taxas alteradas de ureia e creatinina, bem como outros fatores que se relacionam com a história clínica e individual. O número de pacientes em tratamento hemodialítico vem aumentando gradualmente ao longo dos anos, sendo a hemodiálise o tratamento mais comum, os pacientes vivenciam mudanças bruscas na sua vida influenciando de forma considerável na qualidade de vida do portador de IRC. O presente trabalho tem como objetivo avaliar a repercussão do tratamento hemodialítico na vida dos pacientes com Insuficiência Renal Crônica. A pesquisa foi realizada no Centro de Hemodiálise do Hospital Regional de Cajazeiras. Os dados foram coletados após a aprovação do CEP, sob o protocolo nº1.515.907. Para a coleta de dados foi utilizado um roteiro de entrevista semiestruturado contendo informações a respeito de fatores sócios demográficos, bem como questões pertinentes ao tratamento hemodialítico. Os dados qualitativos foram organizados conforme a técnica e análise de dados de LEFEVRE; LEFEVRE e analisados mediante leitura pertinente. A pesquisa seguiu a Resolução 466/12 do Conselho Nacional de Saúde que trata de pesquisas desenvolvidas com seres humanos. A hemodiálise repercute de forma significativa na vida do portador de DRC, influenciando de várias maneiras desde aspectos clínicos, como social, financeiro e emocional.</p><p><strong><em>Impact of hemodialysis in the lives of patients with chronic renal failure in Paraiba Hinterland</em></strong></p><p><strong>Abstract </strong>Chronic Renal Failure (CRF) is a chronic, progressive, debilitating disease which causes disabilities and has a high mortality rate, and the incidence and prevalence have increased even more in the world population, related to family background, physical inactivity, diet high sodium content, as well as altered rates of urea and creatinine and other factors that are related to clinical and personal history. The number of patients in hemodialytic treatment has been increasing gradually over the years, in which hemodialysis is the most common treatment. Patients experience sudden changes in their lives influencing considerably in the quality of life of the IRC carrier. To assess the repercussion of hemodialytic treatment in the lives of patients with Chronic Renal Failure. The research was conducted in the Hemodialysis Center at Regional Hospital of Cajazeiras. For the data collection a semi-structured interview guide was used, containing information about social and economic factors, as well as issues related to hemodialytic treatment. The qualitative data were organized according to the technique and data analysis of LEFEVRE; LEFEVRE and analyzed through relevant reading. The research followed the Resolution 466/12 of the National Health Council, which deals with researches conducted on human subjects. A better understanding and characterization of the factors related to the repercussion of the hemodialytic treatment in the lives of the patients affected by Chronic Renal Failure.</p>

Chronic Renal Failure (CRF) is a disease that has occurred after various diseases that damage the kidney nephrons period up to the point the two are not capable of functioning regulatory and ekstetoriknya to maintain homeostasis. Number of patients with kidney failure in Indonesia is estimated to reach 300.000 people. As many as 12.804 patients with renal failure undergoing hemodialysis. To determine correlation family support patient compliance with chronic renal failure undergo hemodialysis therapy in RS PMI Bogor. This type of research is descriptive quantitative analytical research with cross sectional design. How sampling in this study with purposive sampling with a sample of 152 respondents. The instrument used is questionnaire while data analysis techniques using univariate and bivariate analysis using statistical test Chi-Square. Of the 152 respondents with a family to support as many as 139 respondents (91.4%), adherent patients as many as 128 respondents (84.2%) and family support with adherent patients as many as 121 respondents (79.6%). Statistical test results using Chi-square test p value value 0,007 (p <0.05) and the value of OR 5.762. The conclusion of this research that, There is a correlation between family support patient compliance with chronic renal failure through hemodialysis therapy in PMI Hospital Bogor, 2016. The researchers recommend that increased family support by distributing leaflets to the patients and families about the importance of family support for compliance patients undergo hemodialysis therapy.

Kidney transplantation has become the preferred treatment option in end-stage chronic renal failure as it provides significant improvements over dialysis in terms of both quality and duration of life. Even after several randomized studies conducted in the last 20 years, the combination of CNI, MMF, and steroids continues to be considered the gold standard for kidney transplantation. However, novel molecules aimed at minimizing renal and cardiovascular toxicity, particularly with CNI sparing, are being identified. The present review assesses various such molecules available currently and briefly discusses the existing combination strategies and novel perspectives for the redesigning of protocols based on our novel therapeutic arsenal.
