Predictors of quality of life and functioning in schizophrenia

Background: In order to pursue recovery, Quality of life (QoL) and general functioning in patients with schizophrenia are milestones that need to be addressed in the treatment of the disease. The major aim of the present study was to examine the relationship between QoL, functionality and symptoms, and clinical characteristics of the disease, and to identify whether the two domains of negative symptoms, avolition/apathy and diminished expression, are predictive factors for the quality of life and functionality in patients diagnosed with schizophrenia. In the present cross-sectional study, 87 patients diagnosed with schizophrenia completed a thorough battery of instruments evaluating: clinical symptoms, stigma, QoL, functionality, socio-demographic and clinical variables. Multiple regression analyses were performed to test the significance of predictor variables for QoL and functionality. Multiple regression analyses revealed that internalized stigma, depression, and excitement were significant predictors for the variability of QoL, and experiential deficit and cognitive symptoms were significant predictors of the variability of functioning. In patients with schizophrenia, functioning and quality of life are two related but different constructs. Even though they are strongly associated, they have different predictive factors. The clinical implication of these results is that both of these constructs should be assessed and that the strategies for improving them should be different.

Objective: The cross-sectional study evaluated the psychometric properties of the Functional Assessment of Multiple Sclerosis (FAMS) Spanish version and its use in measuring quality of life (QOL) of multiple sclerosis (MS) patients in Spain. Methods: The FAMS is a factorially derived self-report scale designed to assess six primary aspects of QOL of patients with MS: Mobility, Symptoms, Emotional Well-Being, General Contentment, Thinking and Fatigue, and Family/Social Well-Being. Its Spanish translated version was used to assess QOL of 625 MS patients recruited in an outpatient clinic setting from 58 hospitals in Spain. Internal consistency of the Spanish FAMS was evaluated. Multiple regression analyses were performed to identify significant predictors from demographic, clinical and treatment characteristics, and Kurtzke Expanded Disability Status Scale (EDSS) scores in predicting FAMS scale scores. Results: Most of the patients are females (66%), and 74% were of the relapsing-remitting (RR) clinical subtype. Cronbach’s alpha coefficients were high (range=0.78-0.96), indicating subscale homogeneity comparable to that of the original English version. Linear multivariate regression analyses revealed that the EDSS is a dominant variable in predicting all the FAMS subscales, especially mobility (R2=0.51) and the total scores. Conclusions: The Spanish FAMS is a psychometrically valid instrument that allows clinicians and clinical researchers the ability to measure the QOL concerns of MS patients in Spain.

Validity instrument european quality of life (EQ-5D-5L) Indonesian version to assess the quality of life patient with tuberculosis Background: Tuberculosis is an infectious disease that can cause death and easily contract droplets through the air. Tb sufferers will experience some clinical symptoms that indirectly improve their quality of life. Measuring the cauldron of living bags in patients with tuberculosis is still rare in Indonesia, especially by using the Instrument European Quality of Life  EQ-5D-5L Indonesian version.Purpose: To determine the validity and reliability of the EQ-5D-5L instrument with the Indonesian version of the utility set index as an alternative to measuring the quality of life of TB patients in Indonesia.Method: A cross-sectional study with an observational approach, conducted on patients with tuberculosis at Sentani Jayapura public health centre cover area I, using the Indonesian version of the EQ-5D-5L instrument.Results: Based on  Pearson Correlation tests on 5 EQ-5D-5L domains showed a significant value of 0.000 on each question item and was declared valid with a correlation value between ≥ 0.60-0.80, while the reliability test obtained a Cronbach Alpha value of 0.799. Overall the EQ-5D-5L instrument is accurate and reliable. Tool EQ-5D-5L Indonesian version can be used to assess the quality of  TB patients in Indonesia with a value of  Pearson Correlation r = > 0.50 and  Cronbach Alpha  0.799 (> 0.70).Conclusion: The EQ-5D-5L instrument with the Indonesian version of the utility set index can be used to assess the quality of life of TB patients in Indonesia.Keywords : Patient; Tuberculosis; Quality of Life; InstrumentPendahuluan: Tuberkulosis merupakan penyakit menular yang dapat menyebabkan kematian dan mudah tertular oleh droplet melalui udara. Mengukur kualitas hidup pada penderita tuberkulosis masih jarang dilakukan di Indonesia terutama dengan menggunakan instrumen European Quality of Life EQ-5D-5L versi Indonesia.Tujuan: Mengetahui validitas dan reliabilitas instrumen EQ-5D-5L dengan index utility set versi Indonesia sebagai alternatif mengukur kualitas hidup pasien TB di Indonesia.Metode: Menggunakan rancangan cross sectional dengan pendekatan observasi. Tempat penelitian di Puskesmas Sentani Kabupaten Jayapura, dilakukan padan pasien TB kategori I, menggunakan instrumen EQ-5D-5L versi Indonesia.Hasil: Berdasarkan uji Pearson Correlation pada 5 domain EQ-5D-5L menunjukkan nilai yang signifikan yaitu 0.000 pada tiap item pertanyaan dan dinyatakan valid dengan nilai korelasi antara ≥ 0.60-0.80, sedangkan uji reliabilitas diperoleh nilai Cronbach Alpha 0.799. Secara keseluruhan instrumen EQ-5D-5L adalah valid dan reliabel. Instrumen EQ-5D-5L versi Indonesia dapat di gunakan untuk menilai kualitas pasien TB di Indonesia dengan nilai Pearson Correlation r = > 0.60 dan Cronbach Alpha 0.799 (> 0.70).Simpulan: Instrumen EQ-5D-5L dengan index utility set versi Indonesia dapat digunakan untuk menilai kualitas hidup pasien TB di Indonesia.

ObjectivesTo investigate the relation between resilience, health-related quality of life (HRQOL) and depression in multiple myeloma (MM) and its premalignant stages. MM is one of the most frequent haematological disorders. It is regularly preceded by asymptomatic stages of the disease namely monoclonal gammopathy of undetermined significance (MGUS) and smouldering multiple myeloma (SMM). Survivors have to cope with mental and physical impairment in terms of HRQOL and depression. The concept of resilience refers to a person’s ability to adapt to adversity.DesignCross-sectional study.SettingMM outpatient department at a University Hospital in Germany (tertiary care).Participants292 consecutive patients from our MM outpatient department.Outcome measuresHRQOL, depression and psychological resilience were assessed with validated questionnaires.ResultsRegression analyses were performed to determine associations between resilience, HRQOL and depression. 98 patients (33.6%) had a new diagnosis of active MM, 106 patients (36.3%) were already treated for MM and 88 patients had the diagnosis of a precursor (MGUS or SMM; 30.1%) of MM. Multivariate linear regression analyses revealed a strong positive impact of resilience on physical (b 7.20; 95% CI 4.43 to 9.98; p<0.001) and mental (b 12.12; 95% CI 9.36 to 14.87; p<0.001) HRQOL. Ordered logistic regression analysis showed that the odds for higher depression severity were lowered for individuals with a high level of resilience in comparison to the individuals with a low level of resilience (OR 0.11; 95% CI 0.06 to 0.19; p<0.001).ConclusionsResilience may be a protective factor in the disease trajectory of MM and its precursors. As a next step, future research should focus on longitudinal assessments at various time points to elucidate the role of resilience in one of the most frequent haematological malignancies.

Background: The patients on hemodialysis have a significantly decreased quality of life. One of many problems which reduce the quality of life and increase the mortality in these patients is osteoporosis and osteoporosis associated fractures. Objectives: To assess the bone density of those on hemodialysis by dual energy X ray absorptiometry and to examine the risk factors of bone density reduction in these patients. Patients and Method: This is a cross-sectional study, including 93 patients on chronic hemodialysis at the department of Hemodialysis at Cho Ray Hospital. Results: Mean bone densities at the region of interest (ROI) neck, trochanter, Ward triangle, intertrochanter and total neck are 0.603 ± 0.105; 0.583 ± 0.121; 0.811 ± 0.166; 0.489 ± 0.146; 0.723 ± 0.138 g/cm2 respectively. The prevalences of osteoporosis at those ROI are 39.8%, 15.1%; 28%; 38.7%; and 26.9% respectively. The prevalences of osteopenia at those ROI are 54.8%; 46.3%; 60.2%; 45.2% and 62.7% respectively. The prevalence of osteopososis in at least one ROI is 52.7% and the prevalence of osteopenia in at least one ROI is 47.3%. There are relations between the bone density at the neck and the gender of the patient and the albuminemia. Bone density at the trochanter is influenced by gender, albuminemia, calcemia and phosphoremia. Bone density at the intertrochanter is affected by the gender. Bone density at the Ward triangle is influenced by age and albuminemia. Total neck bone density is influenced by gender, albuminemia and phosphoremia. Conclusion: Osteoporosis in patients on chronic hemodialysis is an issue that requires our attention. There are many interventionable risk factors of bone density decrease in these patients. Background: Vietnam is entering the stage of aging population; as a result, the quality of life (QOL) of the elderly is really a matter of concern. There is a great deal of research on the quality of life of the elderly while Vietnam witnesses a lack of research on this topic, especially on how physical and social activities have impact on QOL of the elderly. Objectives: (1) To describe the situation of participation in physical, social activities and QOL of the elderly residing in Truong An Ward, Hue city. (2) To examine the association between physical, social activities and QOL among participants. Methodology: This study used cross-sectional study design. A total sample of 420 people aged 60 and above in Truong An Ward, Hue city were interviewd. The SF36 was used to measure the QOL of participants. Results: Percentage of the elderly taking part in physical activity were 66%. 49% reported having the habit of taking part in social activities. The majority of the elderly get their QOL level in three health fields, namely physical, mental and general health at above average. (56.0, 60.7 and 60.8 respectively). There were significant associations between physical activities, social activities and QOL among participants. Conclusion: The percentage of the elderly taking part in physical and social activities were still low. Besides, QOL of the elderly at the research location was not really high. It’s important to encourage the elderly taking part in physical and social activities to improve their quality of life. Key words: Physical activities, social activities, QOL, the elderly
