AbstractThe current HAART has modified natural history of HIV infection and it has practically turned the disease into a manageable chronic condition. Even though the benefits of HAART use are overwhelmingly greater than possible metabolic syndrome and cardiovascular disease risks, close management of those patients is called for, especially due to the fact that general population risk factors now overlap with specific ones in this population.Cardiovascular screening for HIV-infected patients who are receiving HAART must be done constantly with a collaboration between the infectious disease specialist and the internist.

Objective: To evaluate retinol and β-carotene serum levels and their relationship with risk factors for cardiovascular disease in individuals with morbid obesity, resident in Rio de Janeiro. Methodology: Blood serum concentrations of retinol and β-carotene of 189 morbidly obese individuals were assessed. The metabolic syndrome was identified according to the criteria of the National Cholesterol Education Program (NCEP) and World Health Organization (WHO). Lipid profile, insulin resistance, basal insulin, glycemia, blood pressure, and anthropometry and their correlation with retinol and β-carotene serum levels were evaluated. Results: Metabolic syndrome diagnosis was observed in 49.0% of the sample. Within this percentage the levels of β-carotene were significantly lower when body mass index increased. Serum retinol didn't show this behavior. Serum retinol inadequacy in patients with metabolic syndrome (61.3%), according to WHO criterion, was higher (15.8%) than when the whole sample was considered (12.7%). When metabolic syndrome was diagnosed by NCEP criterion, β-carotene inadequacy was higher (42.8%) when compared to the total sample (37.5%). There was a significant difference between average β-carotene values of patients with and without metabolic syndrome (p=0.048) according to the classification of the NCEP. Lower values were found in patients with metabolic syndrome. Conclusion: Considering the vitamin A contribution in antioxidant protection, especially when risk factors for cardiovascular disease are present, it is suggested that great attention be given to morbidly obese. This could aid in prevention and treatment of cardiovascular disease, which affects a significant part of the population.

Background: The increasing incidence of cardiovascular disease (CVD) threatens the Middle Eastern population. Several epidemiological studies have assessed CVD and its risk factors in terms of the primary prevention of CVD in the Middle East. Therefore, summarizing the information from these studies is essential. Aim: We conducted a systematic review to assess the prevalence of CVD and its major risk factors among Middle Eastern adults based on the literature published between January 1, 2012 and December 31, 2018 and carried out a meta-analysis. Methods: We searched electronic databases such as PubMed/Medline, ScienceDirect, Embase and Google Scholar to identify literature published from January 1, 2012 to December 31, 2018. All the original articles that investigated the prevalence of CVD and reported at least one of the following factors were included: hypertension, diabetes, dyslipidaemia, smoking and family history of CVD. To summarize CVD prevalence, we performed a random-effects meta-analysis. Results: A total of 41 potentially relevant articles were included, and 32 were included in the meta-analysis (n=191,979). The overall prevalence of CVD was 10.1% (95% confidence interval (CI): 7.1-14.3%, p<0.001) in the Middle East. A high prevalence of CVD risk factors, such as dyslipidaemia (43.3%; 95% CI: 21.5-68%), hypertension (26.2%; 95% CI: 19.6-34%) and diabetes (16%; 95% CI: 9.9-24.8%), was observed. The prevalence rates of other risk factors, such as smoking (12.4%; 95% CI: 7.7-19.4%) and family history of CVD (18.7%; 95% CI: 15.4-22.5%), were also high. Conclusion: The prevalence of CVD is high (10.1%) in the Middle East. The burden of dyslipidaemia (43.3%) in this region is twice as high as that of hypertension (26.2%) and diabetes mellitus (16%). Multifaceted interventions are urgently needed for the primary prevention of CVD in this region.

Background: Risk factors can lead to clinical conditions, like metabolic syndrome, that predisposes the development of cardiovascular diseases. Objective: The goal of this population-based, prospective and non-randomised cohort study was to study the association between patients with metabolic syndrome and other various factors defining metabolic syndrome. Methods: All the patients referred to the department of Medicine, Al-Ameen Medical college hospital and District Hospital, Vijayapur, Karnataka, India over a period of twenty-two months extending from December 2013 to September 2015 were considered in this study. Results: In the current study, out of 100 patients, 62.9% patients had metabolic syndrome with positive family history of hypertension, diabetes mellitus. 70.8% patients had metabolic syndrome with positive history of smoking. 64.3% patients had metabolic syndrome with positive history of alcohol .73.9% patients had metabolic syndrome with positive history of IHD. 87% of the patients with metabolic syndrome had SBP> 130 mmHg, and 78.85% patients had DBP>85 mmHg. And metabolic syndrome was observed in 71.8% patients on anti hypertensive drugs. The mean level of total cholesterol, LDL cholesterol, triglyceride is increased whereas the mean level of anti-atherogenic HDL cholesterol is low in subjects with MS. At least one lipid abnormality was present in > 95 % of cases. Around 81% subjects with BMI <25 (out of 38) had metabolic syndrome and 58% subjects with BMI>25(out of 62) had metabolic syndrome. Conclusion: All the components defining the metabolic syndrome correlated positively with the abdominal obesity. Systolic blood pressure values were significantly higher than diastolic blood pressure in subjects with abdominal obesity. Metabolic syndrome has multiple risk factors determined by various aspects like the race, the life style, geographical factors larger study is needed to understand the correlation between various components defining it. A healthy lifestyle, that includes avoiding tobacco exposure and proper weight control, must be encouraged in this high-risk population. CHRONIC DISEASES AND LIFESTYLE IMPACT TO CATEGORY VERBAL FLUENCY PERFORMANCE IN ELDERLYABSTRACTIntroduction: In recent studies, chronic illness and sedentary lifestyle were found to bea risk factors for cognitive impairment. Verbal fluency (VF) is one of the most used instruments.Aim: To determine cognitive impairment using VF and its correlation with risk factors (chronic illness and lifestyle) in elderly.Method: This cross-sectional study was conducted toward 121 elderly respondents in August–September 2017 at Pusaka, West Jakarta. Cognitive impariment was assessesed using VF. VF otherwise disturbed if the value <16.03. Heart disease and stroke were determined by anamnesis. Blood pressure was determined using Riester tensimeter. that has been calibrated. Cholesterol and blood sugar were determined using laboratory result by CHOD-PAP method. Smoking was determined by asking the history of smoking. Physical activity was determined by 24-hour activity recall. Nutrition Intake was determined by food record.Results: There were 63.6% subjects wno had VF impairment. Most respondent were over 65 years old (71.9%), female (66.1%) and education level more than 6 years (72.7%). VF had a significant assosiation with education level (p=0.020; OR=3.792), and without cardiovascular disease (p=0.023; OR=0.111).Discussion: The mayority of respondents had VF impairment, lower education was consistenly as a risk factors while no history of cardiovascular disease as a protective factors of language impairment.Keywords: Chronic diseases, cognitive impairment, lifestyle, verbal fluencyABSTRAKPendahuluan: Pada beberapa penelitian terbaru ditemukan bahwa faktor risiko seperti penyakit kronis dan gaya hidup buruk meningkatkan risiko terkena gangguan fungsi kognitif. Salah satu instrumen yang sering digunakan adalah verbal fluency (VF).Tujuan: Mengetahui gambaran fungsi kognitif dengan instrumen VF dan hubungannya dengan faktor risiko (penyakit kronis dan gaya hidup) pada lansia.Metode: Penelitian potong lintang ini dilaksanakan pada 121 responden lansia pada bulan Agustus-September 2017 di Pusaka Jakarta Barat. Gangguan fungsi kognitif dinilai menggunakan instrumen VF. Verbal fluency dikatakan terganggu dengan nilai <16,03. Penyakit jantung dan stroke didapatkan dari anamnesis. Tekanan darah dari hasil pengukuran menggunakan tensimeter yang telah dikalibrasi sebelumnya. Pengukuran kolesterol dan gula darah didapatkan dari hasil laboratorium dengan metode CHOD-PAP. Merokok didapat dari anamnesis. Aktivitas fisik dinilai dari 24-hour activity recall. Asupan nutrisi dinilai dari kuisioner food record.Hasil: Hasil penelitian ini menunjukkan subjek dengan gangguan VF sebanyak 63,6%. Mayoritas responden berusia diatas 65 tahun (71,9%), dengan jenis kelamin wanita (66,1%) dan tingkat pendidikan diatas 6 tahun (72,7%). Hasil analisis menunjukan bahwa instrumen VF memiliki hubungan bermakna dengan tingkat pendidikan rendah (p=0,020; RO=3,792), dan tidak berpenyakit jantung (p=0,023; RO=0,111).Diskusi: Mayoritas responden memiliki gangguan bahasa, pendidikan rendah diketahui secara konsisten sebagai faktor risiko sementara tidak ada riwayat penyakit jantung sebagai faktor protektif untuk gangguan fungsi bahasa.Kata kunci: Gangguan fungsi kognitif, gaya hidup, penyakit kronis, verbal fluency

Non-communicable diseases (NSDs) are responsible for two-thirds of all deaths globally, whereas cardiovascular disease (CVD) alone counts for nearly half of them. To reduce the impact of CVD, targeting modifiable risk factors comprised in metabolic syndrome (e.g., waist circumference, lipid profile, blood pressure, and blood glucose) is of great importance. Beneficial effects of fish consumption on CVD has been revealed over the past decades, and some studies suggest that fish consumption may have a protective role in preventing metabolic syndrome. Fish contains a variety of nutrients that may contribute to health benefits. This review examines current recommendations for fish intake as a source of various nutrients (proteins, n-3 fatty acids, vitamin D, iodine, selenium, and taurine), and their effects on metabolic syndrome and the CVD risk factors. Fatty fish is recommended due to its high levels of n-3 fatty acids, however lean fish also contains nutrients that may be beneficial in the prevention of CVD.
