Abstract The present article is a sequel of “Encounter with Death (I). An Interrupted Dialogue” (DOI:10.2478/rjp-2020-0013 Rom J Psychoanal 2020, 13(1):197-204) and it aims at exploring the possible psychoanalytical semantics of the double suicide committed by Arthur Koestler in 1983, together with his wife, Cynthia, at their home in London. It tries to relate the tragic event (in the context of Koestler’s previous life-threatening experience in the prison of Seville) to the Freudian death drive and to Imre Hermann’s clinging instinct, as approached in Philippe Van Haute and Tomas Geyskens’s book “From Death Instinct to Attachment Theory”.

The primary purpose of the present article is to provide an overview of three theories of mourning--The Dual Process Model of Coping with Bereavement, Meaning Reconstruction and Loss, and Attachment Theory and Loss: Revisited. These are linked both by their emphasis on the phenomenological and by ideas such as balance and flexibility. Connections are drawn between the mourning theories and counseling theories that are commonly employed by mental health counselors.

In 1956 Wilhelm Reich published ‘Re-emergence of Freud's “Death Instinct” as “DOR” Energy’. The scientist reverted to psychoanalysis in what proved to be his last theoretical article, in which he reassessed the Freudian theory of the death drive, the very thesis he had rejected in the 1930s, and whose rejection had made him unpopular with the psychoanalytic movement. In that article, not only did Reich intend to pay homage to his old master Sigmund Freud, but also to partly revise his theories on the Eros-Thanatos instinct duality. After summarizing his own theories on the OR orgone life energy and its DOR negative equivalent, and continuing to claim that there was no clinical evidence to support the death instinct, Reich cautiously stated that Freud had had his legitimate reasons to classify that duality: the libido and the death instinct would actually be the psychological correlates of physical phenomena that he was able to observe both in the neurotics’ character armour and in the weather phenomena and climatic events affecting the environment.

✓ Death from cerebrospinal fluid shunt malfunction is a rare but tragic event. The authors describe seven children who when admitted were lethargic but arousable because of shunt failure. Sudden deterioration prompted unscheduled emergency shunt revision in each case. Two children subsequently died. Although neither the history nor the physical findings predicted this life-threatening shunt malfunction, obliteration of the perimesencephalic cistern was apparent on all preoperative computerized tomography (CT) scans. For reference, the scans of 43 other hydrocephalic children were drawn randomly from the hospital files and analyzed. It is concluded that CT evidence of obliteration of the perimesencephalic cistern is a useful warning of life-threatening shunt failure.

Thromboembolism term is used to define a type of blood pathology which involves two terms basically i.e. Thrombo + Embolism. Thromboembolism is defined as the blocking or obstruction of a blood vessel by a clot (or a part of clot, debris) that has broken off from the place where it is formed and travelled to another organ. (Debris includes blood clots, cholesterol containing plaques, masses of bacteria, cancer cells, amniotic fluid, fat from the marrow of broken bones and injected substances). Thromboembolism is coagulation defect of blood which is responsible for certain life threatening conditions of human being. Ayurveda Samhita also has references regarding good as well as bad properties of the Rakta and Margavarana Janya pathological states that lead to defect of the Kapha, Rakta that produces features similar to Thromboembolism. Present article will help to understand Thromboembolism as per the Ayurveda perspective.

Necrotizing fasciitis is a serious and potentially life-threatening condition. Although bite wounds are common, they are not frequently reported as a cause of necrotizing fasciitis. In the present article, two cases of bite-associated necrotizing fasciitis caused by group A streptococcus are reported. Previously published cases are also reviewed.

Iron-deficiency anemia is a common reason for referral to a gastroenterologist. In adult men and postmenopausal women, gastrointestinal tract pathology is often the cause of iron-deficiency anemia, so patients are frequently referred for endoscopic evaluation. Endoscopy may be costly and at times difficult for the patient. Therefore, physicians need to know what lesions can be identified reliably and, more importantly, the importance of ruling out life-threatening conditions such as occult malignancy. Over the past decade, a number of prospective studies have been completed that examined the yield of endoscopy in the investigation of iron-deficiency anemia. The present article provides a broad overview of iron-deficiency anemia, with particular emphasis on hematological diagnosis, etiology, the use of endoscopy in identifying lesions and iron-repletion therapy. Other clinical scenarios, including assessment of patients on anti-inflammatory or anticoagulation therapy and patients with bleeding of obscure origin, are also addressed. The present article provides a diagnostic algorithm to iron-deficiency anemia, which describes a more systematic manner in which to approach iron-deficiency anemia.

Nel 2011 l’Associazione Medica Reale Olandese ha pubblicato un rapporto sul ruolo del medico nell’interruzione della vita su richiesta. In questo rapporto l’Associazione osserva che i medici olandesi hanno un’interpretazione troppo restrittiva della legge sull’eutanasia e li incita a far uso di tutto lo spazio che offre questa legge. Una condizione posta dalla legge affinché un medico che ha compiuto l’eutanasia o ha assistito a un suicidio non sia perseguito penalmente, è che la sofferenza del paziente coinvolto sia senza prospettive di miglioramento e insopportabile. Secondo l’Associazione la sofferenza può essere qualificata come tale anche se causata da un accumularsi di disturbi della vecchiaia e una combinazione di fattori che, presi in sé, non sono fatali o costituiscono una minaccia per la vita. Al contrario di un disegno di legge del gruppo d’iniziativa Uit Vrije Wil (2010), che sostiene che una persona che considera la sua vita come giunta a compimento, dopo aver raggiunto l’età di settanta anni, pur non soffrendo di una malattia somatica o psichica, può ricevere mezzi per suicidarsi, l’Associazione mantiene fermo il punto che per classificare la sofferenza come senza prospettive di miglioramento e insopportabile deve esserci in ogni caso un problema medico. L’Associazione suggerisce che un medico che non raccoglie una domanda di eutanasia, ha un obbligo, non giuridico ma morale, di indirizzare il paziente a un altro medico, forse pronto ad accogliere la richiesta. Inoltre secondo l’Associazione è permesso al medico che non raccoglie una tale richiesta, di accennare al paziente la possibilità di sospendere per proprio conto alimentazione e idratazione con lo scopo di affrettare la fine della vita. L’articolo presente è una discussione critica del rapporto dell’Associazione in base all’idea che non è lecito al medico di compiere l’eutanasia e di dare assistenza al suicidio. ---------- In 2011 the Royal Dutch Medical Association published a report on the role of the physician in terminating life at request. In this report the Association notes that Dutch physicians interpret the Dutch Law on euthanasia in a too restrictive way and urges them to use the entire scope of the Law. One of the conditions laid down by the Law in order that the physician, having performed euthanasia or assisted in suicide, will not be prosecuted, is that the suffering of the patient is hopeless and unsupportable. According to the Association the suffering can be qualified as such, also if caused by an accumulation of geriatric complaints and a combination of factors which in themselves are not fatal or life threatening. In contrast with the bill of the initiative group Uit Vrije Wil (2010), which implies that a person, considering his life as completed, after having reached the age of seventy years, though not suffering from a somatic or psychic disease, may receive means for terminating his life, the Association holds that for classifying suffering as hopeless and insupportable in any case a medical cause is required. The Association suggests that a physician who does not comply with a request of euthanasia, has an obligation, not juridical but moral, to refer the patient to another physician, ready to comply with the request. Moreover, it is permitted according to the Association for a physician who is not willing to agree to a request, to indicate the patient at the possibility of suspending to eat and to drink in order to hasten the end of life. The present article is a critical discussion on the report of the Association from the point of view that it is not allowed for a physician to perform euthanasia of to assist in suicide.

Abstract The majority of book-to-film adaptations operate more or less important adjustments on the initial text. In this respect, the present article attempts to investigate the psychoanalytical relevance of such a textual intervention in Roman Polanski’s 1992 film, “Bitter Moon”, based on Pascal Bruckner’s novel, “Lunes de fiel” (1981). The analysis takes the Freudian theories on sadomasochism and death instinct as a starting point.
